1.Therapeutic effects of tension-free vaginal tape and tension-free vaginal tape obturator in women with stress urinary incontinence:meta analysis

Objective To assess the effectiveness of tension-free vaginal tape obturator tape(TVT-O)as treatment of SUI by means of a systematic review and meta analysis.Methods Using the search terms"TVT,TVT-O,SUI,RCT,TOT",the literature in Chinese and English from January 2001 tO March 2007 on the difference of TVT and TVT-O was searched from MEDLINE,PUBMED,EMBASE,Google Scholar,CNKI,WAN FANG DATA,and criteria randomized controlled trials(RCTs)were studied by Meta-analysis in RevMan 4.2.At the same time,Ors of randomized model and fixed model were calculated tO evaluate the sensitivity.Results There were six RCTs that compared TVT-O with TVT.When compared by Subjective cure,TVT-O at 1-17 months were no better than TVT(OR 0.67;95% CI 0.40,1.13).Adverse events such as bladder injuries (OR 0.15;95% CI 0.03,0.66)was less common,where as groin/thigh pain(OR 8.61;95% CI3.03,24.52)was more common;but there were rio significant difference in de novo urgency(OR=1.16;95% CI 0.54,2.47),urinary retention(OR=0.54;95% CI 0.24,1.20)or urinary tract infection(OR=1.07;95% CI 0.61,1.87)between the tWO groups.Conclusions There is no significant difference between TVT and TVT-O.TVT-O group had no bladder injuries complications,but groin/thigh pain was more common.
2.Effect of Tongxinluo capsule on the plasma C-reactive protein and endothelin-1 in acute coronary syndrome patients

Objective To study the effect of Tongxinluo on the plasma C-reactive protein(CRP)and endothelin-1(ET-1)in acute coronary syndrome(ACS) patients.Methods 100 patients with ACS were randomly divided into conventional therapy group and treatment group(conventional therapy+Tongxinluo gelatin capsule).The changes of CRP and ET-1 in the first day,7th and 14th day were observed.Results In the treatment group,CRP and ET-1 were significantly decreased in the 7th and 14th day(P<0.05,P<0.01),and there was significant decrease only in the 14th day(P<0.05)in the conventional therapy group.CRP and ET-1 levels in the treatment group were significantly different as compared with conventional thereapy group(P<0.01).Conclusion Tongxinluo capsule may protect blood vessel endothelium through inhibiting CRP and ET-1 to decrease the inflammatory response of endangium.
4.Observation of the early tear film changes after trabeculectomy with Keratograph 5 M

AlM: To observe the early tear film changes after trabeculectomy using Keratograph 5M.METHODS:Fourty-one patients (46 eyes) of glaucoma who underwent trabeculectomy were involved. First tear break up time ( FTBUT ) , average tear break up time (ATBUT), non-invasive tear meniscus height (NlTMH), lipid layer thickness and meibomian gland scores were measured at 1d preoperatively and at 1d, 1wk, 1mo postoperatively.RESULTS:At 1d, 1wk and 1mo postoperatively, FTBUT and ATBUT decreased greatly (P<0. 05), as well as lipid layer thickness (P<0. 017). NlTMH at 1d postoperatively increased significantly, but at 1wk and 1mo postoperatively decreased greatly (P<0. 05). There was no significantly difference in meibomian gland scores between preoperation and postoperation (P>0. 05). CONCLUSlON: Keratograph 5M could be used to evaluate tear film function rapidly and accurately. Trabeculectomy significantly alters tear film stability and tear secretion in the short term after operation.
5.Endovascular treatment for closed articular artery injuries

Objective To explore the efficacy of endovascular treatment for closed articular artery injuries.Methods The clinical data of 13 patients from Jan 2010 to Dec 2014 treated for closed articular artery injuries were analyzed retrospectively.The location,severity and extent of arterial injury were confirmed by intraoperative arteriography.The diameter and length of the injured arteries were measured.The guidewire was sent to the lesion site and working wire pathway was established.When the guidewire was difficult to pass through the lesion site,femoral and posterior tibial or radial artery may be used to establish working wire pathway.The stent or stent-graft was implanted through the contralateral femoral artery or distal end of ipsilateral artery.Results Intraoperative angiography confirmed intimal injury (n =2),partial transection (n =5),complete transection (n =5) and arteriovenous fistula (n =1).Eighteen stents or stent-grafts were implanted.Treatment was successful in all patients without peiropeartive death and procedure-related complications.All were followed up for 15 to 48 months and the mean follow-up was (30 ± 11) months.Three patients with stent lumen stenosis less than 50% as showed by angiography.There was no stent fracture,displacement,or deformation.Conclusions Endovascular treatment for closed articular artery injuries is of less invasion,shorter operative time and quick postoperative recovery.
6.Research of neuropsychological development in twin premature infants

Objective To explore the neuropsychological development in twin premature infants,and to analyze the risk factors for the retardation of development.Methods The premature infants hospitalized in the Neonatal Ward First Affiliated Hospital of Kunming Medical University from Jun.2010 to Jun.2012 were divided into study groups of twin premature infants and a control group of singleton premature infants.The neuropsychological development of 88 premature infants was evaluated at corrected gestational age of 1 year old by Bayley scales of infant development.According to normal lab findings of the Bayley scales of infant development,the premature infants were then divided into abnormal neuropsychological development group [mental development index(MDI) ≤79 scores] and normal neuropsychological development group (MDI > 79 scores),and the data of 2 groups were statistically analyzed.Results The frequency distribution of MDI score in study group showed a skewed distribution,of which 59% (27/46 cases) had abnormal neuropsychological development (MDI ≤ 79 scores); However,the frequency distribution of MDI score in control group showed an approximate normal distribution,of which only 12% (5/42 cases) for abnormal neuropsychological development.Head circumference,body length,body weight,MDI,psycho-motor development index of twin premature infants of one-year old were lower than those of the singleton premature infants of the same age,and the differences were statistically significant (all P <0.05).The single factor analysis demonstrated that the risk factors for abnormal neuropsychological development (MDI ≤ 79 scores) of twin premature infants were gestational age,birth weight,mother' s gcstational age,the cultural degree of parents,mode of feeding,neonatal hyperbilirubinemia,neonatal hypoglycemia and neonatal sepsis.Multiple regression analysis showed that neonatal hyperbilirubinemia was the independent risk factor for abnormal neuropsychological development of twin premature infants.Conclusions At same gestational age,neuropsychological development of twin premature infants lagged behind singleton premature infants.Neonatal hyperbilirubinemia may be the risk factors for neuropsychological development of twin premature infants,so the earlier management of neonatal hyperbilirubinemia may be beneficial for the neuropsychological development of twin premature infants.

OBJECTIVE To observe the effects of RNA interference mediated PIK3CA gene silencing on the proliferation and invasiveness of laryngeal squamous cell carcinoma(LSCC) cells, and investigate the feasibility of PIK3CA gene as a potential therapeutic target in the treatment of LSCC. METHODS The lentiviral vector system expressing short hairpin RNA targeting PIK3CA gene(PIK3C-shRNA)was constructed and transfected subsequently into Hep-2 cells mediated by liposome in vitro. The expression of PIK3CA gene was detected by real-time RT-PCR and Western blot respectively. The proliferation of Hep-2 cells was measured by MTT, colony formation, and cell growth curve. The invasive power was determined by Boyden chamber model in vitro. RESULTS The lentiviral vector system expressing short hairpin PIK3CA- shRNA was constructed successfully. OBJECTIVE To reverse multidrug resistance(MDR)to chemotherapeutic agents in human laryngeal cancer cells(LSC-1/TAX)by knockdown the expression of P-glycoprotein(P-GP), the MDR1 gene product, using RNA interference(RNAi)technique.METHODS LSC-1/TAX cells were transfected with lentivirus vector that contains the shRNA construct targeting MDR1 mRNA.The drug resistance was measured by MTT assay in vitro, and sensitivity of the laryngeal cancer cells to various anti-neoplasm agents was quantified in vivo.The knockdown of MDR1 gene expression was assessed by immunocytochemistry in vitro and vivo.RESULTS Multidrug resistance phenotype in human laryngeal cancer cell line (LSC-1/TAX)was reversed in vitro.Tumor growth assay in vivo revealed a reverse of MDR in the laryngeal cancer cells.Immunocytochemistry showed that P-GP expression was significantly inhibited.CONCLUSION MDR1 shRNA lentivirus vectors can significantly inhibited MDR1 expression.Inhibition of MDR1 gene expression conferred an increased sensitivity of drug-resistant in laryngeal cancer cells to conventional chemotherapeutic agents.
9.Emergency orthotopic liver transplantation for acute hepatic failure:a report of 8 cases

Objective To study the efficacy of emergency orthotopic liver transplantation(EOLT) for acute(hepatic) failure(ALT).Methods A retrospective review was undertaken on the clinical data of 8 patients undergoing emergency liver transplantation for ALT.Results The 8 patients completely regained consciousness in 12 to 72 hours after operation.No case developed central nervous complications.One case of severe(hepatitis) complicated by acute renal failure died of respiratory infection and ARDS on postoperative day 7.One case who refused to take medication died from chronic rejection 12 months after operation.One case was(complicated) by bile duct stricture and biliary sludge at 14 months postoperatively and survived for 18 months.Four of the other 5 cases were followed up for 17 months and 1 cases for 14 months,and thir quality of life was excellent.3 of them have returned to work.Conclusions Emergency orthotopic liver thansplantation is an effective means to treat ALF.Intensive care and effective treatment preoperatively are pre-requisite(conditions) to ensure the success of EOLT.
10.Construction and identification of the lentiviral vector system expressing MDR1 small interference RNA

OBJECTIVE To construct the lentiviral vector system expressing MDR1 small interference RNA, and identify reversal efficiency of multidrug resistant phenotype in the human laryngeal cancer multidrug resistance cell lines (LSC-1/TAX). METHODS Three target sequences of oligonucleotides were selected according to MDR-1 gene sequence, the complementary DNA contained both sense and antisense strands were designed and synthesized. After the oligonucleotides were inserted into the plasmid expression system pLVTHM, the plasmid was cotransfected along with pCMV-dR8.74 and pMD2G into 293T cell lines to package lentiviral particles. Interference efficiency of the lentiviral vector system expressing MDR1 small interference RNA was determined by RT-PCR, real time PCR and Western Blot in the human laryngeal cancer multidrug resistance cell lines (LSC-1/TAX), drug resistance was measured by MTT assay after interference. RESULTS It was confirmed by digestion and sequencing that lentiviral vector had the correct structure and could express the GFP and siRNA. The functional titer of concentrated virus was more than 1?108TU/ml. The vectors expressing 3 target sequences can infect LSC-1/TAX, and the third vector has the best interference efficiency. CONCLUSION The lentiviral vector system expres-sing MDR1 siRNA has been constructed, which is necessary to reverse multidrug resistance phenotype in the human laryngeal cancer multidrug resistance cell lines
