1.The study of totally transumbilical single -port laparoscopic herniorrhaphy and traditional surgery in treat-ment with pediatric indirect inguinal hernia

Objective To compare the effect and safety of totally transumbilical single -port laparoscopic herniorrhaphy and traditional surgery in treatment with pediatric indirect inguinal hernia.Methods 100 children with indirect inguinal hernia were divided into control group(n =50)and observation group(n =50).The control group was given to traditional surgery.The observation group was given to totally transumbilical single -port laparoscopic herniorrhaphy.The operation related index,postoperative complications and hernia recurrence rate were compared between the two groups.Results The intraoperatve bleeding,incision length,operation time and hospital stays of the observation group respectively were (18.1 ±5.9)mL,(1.3 ±0.3)cm,(27.4 ±7.9)min and (4.5 ±1.2)d,those of the control group respectively were (38.7 ±8.1)mL,(2.2 ±0.4)cm,(46.7 ±9.2)min and (7.4 ±1.3)d,the difference was statistically significant (t =3.439,3.811,2.872,2.878,P =0.026,0.019,0.045,0.049).The postoperative complications rate of observation the group and the control group were respectively 8.0%(4 /50)and 24.0%(12 /50),the difference was statistically significant(χ2 =4.762,P =0.029).Postoperative follow -up of 1 year,the hernia recurrence rate of the observation group and the control group respectively was 0(0 /50)and 12%(6 /50),the difference was statistically significant (χ2 =6.383,P =0.012).Conclusion Using totally transumbili-cal single -port laparoscopic herniorrhaphy treat with pediatric indirect inguinal hernia is safe and effective.It is ben-eficial to prevent recurrence,and is worth clinical promotion.

Clinical diagnosis and treatment of constipation lags behind relatively with unsatisfactory efficacy. Pathogeneses and molecular mechanisms for different types of constipation are waiting to be further clarified. New biomarkers and therapeutic targets for clinical diagnosis of constipation are so urgent. As for current problems in diagnosis and treatment of constipation, it is necessary to use the concept of translational medicine to break existing imprisonment of thinking, and find out new thinking ways of research methods, diagnosis and treatment approaches, thereby improving diagnosis and treatment levels.

3.THE DISTRIBUTION AND CLINICAL SIGNIFICANCE OF SENTINEL LYMPH NODE DEMONSTRATED BY GASTRIC CARCINOMAS WITH SOLITARY LYMPH NODE METASTASIS

To investigate the distribution and clinical significance of sentinel lymph node(SLN) in gastric cancer, and to provide novel methods for further prospective study, the location of the first node with metastasis, and the correlation between it and primary tumor site were studied in this paper by retrospectively examining 75 cases of gastric cancer with only one positive node demonstrated during operation and pathologic examination. This positive node was regarded as SLN. The results revealed that the solitary metastatic nodes were found in the nearest perigastric nodal area in 55% of cases, in a fairly remote perigastric area in 28% of them, and in the N2-N3 area without N1 involvement in 17%. Among 13 cases of gastric carcinoma with “leaping” metartasis, the depth of invasion was mucosal in one patient, SM, SS or Se in other 12 cases. We conclude that the location of SLN in gastric cancer is random and multidirectional, and the rate of “leaping” metastasis is much higher than expected ,which indicates that the examination of the nodal area close to the primary tumor may not be a reliable method to detect SLN in gastric carcinoma. Although the feasibility of SLN concept in gastric cancer cannot be questioned with these findings, novel techniques combining the dye directed and radioguided methods need to be developed.
4.Primary culture of human vascular smooth muscle cells and their calcification mode

Objective To establish a calcification mode in vitro of human vascular smooth muscle cells (HVSMCs) induced by β-GP. Methods Primary HVSMCs were obtained from human embryo by plant method and confirmed by stain with α-sm-actin antibody. The cells after 4-6 passages were divided into two groups.The control group was incubated with normal DMEM medium while the calcification group was incubated with the medium containing 10 mmol/L β-glycerophosphate for 10 days.Calcification was confirmed by alizarin red S stain and alkaline phosphatase(ALP) assays. Results The primary cells observed by S-P stain were positive and the cells after being stained were pale yellow. After being induced with β-GP, the cells of calcification group began concentric growth and formed vesicles. Alizarin red S staining showed that the reaction of calcifying nodules was red,ALP activity was higher than that of controls at various time points(4 d,6 d,8 d and 10 d,P<0.01 ).Conclusion The HVSMCs could be induced into calcification in vitro by β-GP, and this model contribates to further studies of vascular diseases.
5.Cemented artificial femoral head arthroplasty treats elderly unstable intertrochanteric fracture

BACKGROUND: Many studies have suggested that artificial joint replacement has a greater advantage in the treatment of cases of elderly osteoporosis, comminuted fractures and unable to internal fixation or failure of internal fixation. OBJECTIVE:To analyze the curative effect of cemented artificial femoral head arthroplasty for treatment of elderly unstable intertrochanteric fracture. METHODS: Totaly 18 patients with unstable intertrochanteric fracture, aged>75 years, were enroled and underwent cemented artificial femoral head arthroplasty. At 3 months after replacement, the curative effect was evaluated according to the Harris hip joint function score method. The early complications after replacement and long-term complications of prosthesis were observed by folow-up. RESULTS AND CONCLUSION:Al these 18 patients were folowed up for 13 to 34 months, and had no infections, pressure sores, femoral shaft fractures and other complications. X-ray films showed that the frauture healed wel, without prosthesis infection, dislocation, loosening, sinking and breaking. After 3 months of replacement, al patients returned to pre-injury level of walking function. The curative effect was excelent for eight cases, good for seven cases, fair for two cases, poor for one case. These results demonstrate that cemented artificial femoral head arthroplasty for treatment of elderly unstable intertrochanteric fracture has good biocompatibility and stability, and can restore the limb function of patients.

7.Surgical treatment of ulcerative colitis, report of 16 cases

Objective To explore surgical treatment of ulcerative colitis (UC). Methods Clinical data of 16 patients of UC undergoing surgery were retrospectively analyzed. Results In this series,16 out of 78 UC patients undergoing surgery included failing to control symptom by conservative therapy in 10, bowel obstruction in 3,suspected malignance in 3. Eight cases underwent total proctocolectomy and ileostomy,4 with total proctocolectomy and ileal pouch-anal anastomosis (IPAA),2 with total colectomy and ileorectal anastomosis,2 with partial colectomy. Conclusions The main indications for surgery in UC patients were failure of medical treatment, complicated bowel obstruction and suspected malignance. Total proctocolectomy and ileostomy is a cure for UC, total proctocolectomy and ileal pouch-anal anastomosis benifits defecation control but suffering from high incidence of stomal ulcer.
8.Study on Pharmacokinetics and Bioavailability of Ranitidine Hydrochloride Capsule in Human Body

OBJECTIVE:To study the pharmacokinetics and bioequivalence of both domestic ranitidine hydrochloride capsules and imported ranitidine hydrochloride tablets.METHODS:20healthy volunteers were randomized into groups,whose plasma concentrations of ranitidine were determined at different time after single oral dose of300mg ranitidine hydrochloride capsule or ranitidine hydrochloride tablets300mg by own control by a RP-HPLC method,the pharmacokinetic parameters were computed and which were experienced variance analysis and two-way t-tests and one-way t-tests.RESULTS:The respective pharmacokinetic parameters of ranitidine hydrochloride tablets and ranitidine hydrochloride capsuless were as fol?lows,the C max were(1247.1?547.5)?g/L and(1294.8?613.2)?g/L;t max were(2.98?0.73)h and(2.73?0.80)h;t 1/2 were(3.17?0.36)h and(3.33?0.42)h;AUC 0~t were(5805.9?1403.5)(?g?h)/L and(5941.2?1526.3)(?g?h)/L;AUC 0~∞ were(6163.8?1456.4)(?g?h)/L and(6351.8?1652.7)(?g?h)/L;The relative bioavailability of the ranitidine hydrochloride capsules to ranitidine hydrochloride tablets was(104.3?24.3)%.CONCLUSION:Ranitidine hydrochloride capsules and the ranitidine hydrochloride tablets were bioequivalent.
9.Treating neuropathic pain with low level infrasound

Objective To observe the effect of low level infrasound on neuropathic pain and explore its underlying mechanism. Methods Rats were divided into experimental and control groups after their L5 spinal nerves had been ligated to create a neuropathic pain model. The experimental group was subjected to 40 to 80 dB infra sound. The control group received no infrasound treatment. Paw withdrawal latency in response to heat radiation was measured and the average gray scale of the microglia in a slice of the L5 spinal cord was compared. Results The experimental group expressed significantly lengthened paw withdrawal latency on the 12th and 14th day. The average gray scale showed significantly weakened activation of spinal microglia in the 2nd week of infrasound treatment compared with the control group. Conclusion Low level infrasound can ameliorate neuropathic pain to a certain extent,which might be related with inhibition of spinal microglia.
10.Survival,Proliferation and Differentiation of Human Mesenchymal Stem Cells Derived from Umbilical Cord Transplanted to Hypoxic-Ischemic Encephalopathy Rat Models

Objective To transplant the umbilical cord mesenchymal stem cells(UCMSCs) derived from human umbilical cord into cisterna magna of hypoxic-ischemic encephalopathy(HIE) rat model,and to observe their survival,proliferation and differentiation in the rat brain.Methods UCMSCs were isolated from human umbilical cord of babies delivered after full-term normal cesarean section,and labeled by bromodeoxyuridine(BrdU).Pregnant rats were randomly divided into experimental group(n=6) and control group(n=1).HIE models were built by ligating both sides of the uterine arteries of full-pregnant rats(21 days) in experimental group rats for 15 minutes.The neonatal rats in experimental group were divided into stem cells group(n=24) and PBS group(n=19) at random.The labeled UCMSCs were injected into cisterna magna of the rats in stem cells group,while PBS was injected into the rats of PBS group.In 1,2,3 and 4 weeks after transplantation,the brain tissue section slides were immunohistochemically stained with antibodies against BrdU,Nestin,neuron specific enolase(NSE) and glial fibrillary acidic protein(GFAP),and thionin.Control group with normal delivery was tested as concurrent control.Results At 1 week after transplantation,BrdU,Nestin,NSE and GFAP positive cells were found in the hippocampal dentate gyrus of the rats in stem cells group rats.The number of BrdU-positive and Nestin-positive cells increased(Pa0.05).The NSE-positive and GFAP-positive cells gradually increased from 1-4 weeks post transplantation and comparisons between groups had statistical significance(Pa
