1.Change and significance of serum HO-1/CO system level in patients with coronary heart disease complicating heart failure

Objective To compare the difference of serum heme oxygenase-1 (HO-1) and carbon monoxide(CO) levels between the patients with coronary heart disease(CHD) caused chronic heart failure(CHF) and CHD patients with normal cardiac function,and further to explore the protective mechanism of HO-1/CO system during the pathogenesis process of CHF.Methods Ninety-one patients with CHF were selected as the observation group and 72 CHD cases with normal cardiac function were taken as the control group.The concentration of HO-1 was determined by ELISA arid the Chalmer S method was used to detect serum CO concentration.The general clinical data of the two groups were recorded by the using the heart failure questionnaire.And the liver and kidney functions,blood lipids,NT-proBNP,BNP and cardiac echocardiography examination were performed.Results The serum HO-1 level in the observation group was (8.13±0.27)ng/mL,which was higher than (2.80±0.52)ng/mL in the control group;the CO level in the observation group was (0.35±0.06)mg/L,which was lower than(0.59±0.07)mg/L in the control group,the difference was statistically significant(P<0.01);the HO-1 level in the observation group was gradually increased with the increase of cardiac function grade (P<0.01);while the CO level was decreased with the increase of cardiac function grade (P<0.01).Conclusion The serum HO-11evel in the patients with CHF is highly expressed with the heart failure aggravation;endogenous CO is gradually decreased due to consumption after cardiac failure aggravation.
3.Perioperative nursing of patients with post-thoracotomy atelectasis treated with bronchoalveolar lavage combined with pulmonary insufflation

Objective To summarize the perioperative nursing of patients with post-thoracotomy atelectasis treated with bronchoalveolar lavage combined with pulmonary insufflation.Methods Before operation,the nursing measures included the full preoperative preparation,full anesthesia and mental care.During the operation,the nursing focused on intensified monitoring,close cooperation with doctors in manipulation of bronchoalveolar lavage and pulmonary insufflation.After operation,the postoperative nursing was performed.Results The lung recruitments were successful in all the 12 patients.Compared to pre-operation,the arterial oxygen saturation(SaO2)and arterial oxygen partial pressure(PaO2)6 hours after treatment were significantly improved(P<0.05) while the CO2 partial pressure(PaCO2)was significantly decreased(P<0.05).There were no severe complications and no recurrence of pulmonary atelectasis.All the patients were successfully discharged.Conclusions The bronchoalveolar lavage combined with pulmonary insufflation is effective in treating post-thoracotomy atelectasis.To ensure the smooth treatment,perioperative nursing is also contributive.
4.A control study on the curative effect and reliability of escitalopram with paroxetine for treatment of depression

Objective To evaluate efficacy and safety of escitalopram in treating depression. Methods In this randomized, double-blind, double-mimicry, paroxetine parallel controlled study,the study group was treated with escitalopram and the control group with paroxetine. Hamilton Rating Scale for Depression was taken to evaluate efficacy score. Results At the end of six weeks,compared with the total scores of HAMD before and after treatment, escitalopram (17.5±7.8) and paroxetine (16.0±7.9) group both showed the significant difference, while the difference compared between two groups ,the efficacy (87.0% and 81.7%) for depression and side effects were no significance. Conclusion Escitalopram is an efficient and safety antidepressant.
6.Value of radiotherapy following breast-conserving surgery in early stage breast cancer

Purpose:To validate further the importance of r adiotherapy following breast-conserving surgery in early breast cancer. Methods:From 1994.12 to 2001.12,237patients with early stage breast can cer were treated Of these patients,eighty-three received breast-conserving surgery followed by radiotherapy,and one hundred and fifty-four received modified radical mastectom y.All patients with breast-conserving surgery underwent postoperative radiother apy.The range of radiation dose depends on the size,location of tumor,and involv ement of axillaty nodes.Tangential chest wall fields delivered 5000cGy with 8MV -X ray beam in 5 weeks .An additional 1500cGy was delivered in 8 days to tumor bed with electron beam.Among the patients with modified radical mastectomy.only 3 received postoperative radiotherapy.Results:In both the breast-conserving surgery group and modified radical mastectomy gro up,one patient recurred regionally,and one died respectively .In the modified ra dical mastectomy group,there were no regional lymph nodes recurrence,but two of them had distant metastasis.The 5year survival of the two groups were 96.49%and 98.61%,respectively.The breasts are soft and resilient in the breast-conserving surgery group,and breast shape whole after surgery.Conclusions :Breast-conserving surgery plus postoperative radiotherapy in early breast can cer can obtain treatment results similar to modified radical mastectomy,with goo d cosmetic
7.Titration of isoflurane using BIS index improves early recovery of elderly patients undergoing abdominal surgery

90 min, isoflurane concentration was increased or a bolus dose of fentanyl (25-50 ?g) was given i.v.. The BIS values before and during anesthesia and at extubation, emergence time, extubation time, the time between the end of operation and Aldrete anesthesia recovery score≥9, MMSE score and isoflurane consumption were recorded and compared between the two groups. Results The total amount of isoflurane consumed was 34% lower in BIS group than in control group. The average BIS values were higher and end-tidal isoflurane concentration was lower in BIS group than in control group. The time from the end of surgery to eye-opening, to extubation and Aldrete score≥9 were significantly shorter in BIS group than in control group. The MMSE score was significantly decreased at 1 h after operation as compared to the baseline score before induction of anesthesia while in control group MMSE score was still significantly decreased at 2 h after operation. Conclusion Titration of isoflurane using BIS monitoring can reduce the dose of isoflurane during operation and contribute to faster recovery from anesthesia in elderly patients undergoing intra-abdominal surgery.
8.Determination of N-ethyl morpholine in Air of Workplace by Gas Chromatography

Objective To establish a GC analytical method for the determination of N-ethyl morpholine in the air of workplace.Methods N-ethyl morpholine in the air of workplace was collected by silica gel tube, desorbed by ethanol, and according to The Criterion of Detection Methods for the Air of Workplace, the linearity range, precision, accuracy and limit of detection were determined by the GC under a certain experimental condition and the experiments of sampling efficiency , the desorption efficiency, penetrating capability and the stability of sample were carried.Results The linear range of N-ethyl morpholine was 0.025-20 mg/ml(r=0.999 8);Relative standard deviations were 3.2%-3.9%;the rates of recovery were 101.00%-102.43%(n=18), the limit of detection was 0.49 ?g/ml, the desorption efficiency was 96.10%-98.73%, the penetrating capability was 14.37 mg(100 g silica gel), and the content of samples were reduced less than 10% percent at the ninth day.Conclusion The method has higher sensitivity, better precision, accuracy, simple sampling, accurate and stable results.It can be used as an effective monitoring method to detect N-ethyl morpholine in the air of workplace.
9.THE INHIBITORY EFFECTS OF NIMODIPINE IN ISOLATED GUINEA PIG LEFT ATRIUM AND THIER MECHANISM OF ACTIONS

Fifteen min after adding nimodipine 50 ?mol/L, the contractility of the left atrial muscle decreased from 100 % of control to 55.5 +4.2 o/o and its inhibitory effect was equal to the effect of verapamil (25 ?mol/L ) . Nim at the same concentration inhibited strongly the positive staircase phenomenon but could not reverse it into the negative staircase phenomenon as verapamil 25 ?mol/L did. The same concentration of Nim suppressed significantly the post rest contraction. On the contrary, the post rest contraction was not affected by verapamil at 25 ?mol/L. In addition, Nim 50 ?mol/L reversed or prevented significantly the ouabain-induced arrhythmias in isolated left atria. These results suggest that the negative ino-trbpism of Nim may be related not only to the inhibition of Ca2 + influx to the cells and also to the decrease of the intercellular Ca2+ release and that Nim might present the possibility in treating arrhythmias or other heart disease
