ObjectiveThis study aims to explore the potential mechanism of Danggui Niantongtang in treating knee osteoarthritis (KOA) by regulating the intestinal flora through 16S rDNA analysis. MethodThirty-six C57BL/6J mice were subjected to anterior cruciate ligament transection (ACLT) to establish a KOA model and were randomly divided into the sham surgery group, model group, low-dose Danggui Niantongtang group (0.819 g·kg-1), medium-dose Danggui Niantongtang group (1.638 g·kg-1), high-dose Danggui Niantongtang group (3.276 g·kg-1), and Meloxicam group (0.975 mg·kg-1), with 6 mice in each group. Except for the treatment groups, the sham surgery group and model group were given normal saline by gavage. After 4 weeks of continuous intervention, feces and intact knee joints of the mice were collected. Hematoxylin-eosin (HE) staining and Safranin O-Fast Green staining were performed to observe the pathological changes in knee joint tissue morphology. The 16S rDNA sequencing was used to analyze changes in the abundance and diversity of intestinal microorganisms before and after treatment, along with corresponding functional predictions. ResultHigh-dose Danggui Niantongtang and Meloxicam significantly relieved pain symptoms in KOA mice, improved the disorder of joint structure, maintained the integrity of knee articular cartilage, increased the expression of type Ⅱ collagen alpha 1 (Col2a1) in articular cartilage, and decreased the expression of matrix metalloproteinase-13 (MMP-13). The results of 16S rDNA sequencing showed that high-dose Danggui Niantongtang could adjust the abundance and structure of intestinal microbial species. Compared with the sham surgery group, the abundance of Proteobacteria, Actinobacteria, Ruminococcus, and Bacteroides was significantly increased in the model group (P<0.05), while in the Danggui Niantongtang group, the abundance of these four flora was significantly reduced compared with the model group. Compared with the sham surgery group, the abundance of Verrucomicrobia, Oscillospira, and Akkermansia was significantly decreased in the model group (P<0.05), while in the Danggui Niantongtang groups, the abundance of these three flora was significantly increased compared with the model group (P<0.05). Functional pathway prediction of differential genera revealed that species differences among groups mainly involved metabolic pathways with high abundance associated with biosynthesis and precursors, as well as energy production, including amino acid biosynthesis, nucleotide and nucleoside biosynthesis, cofactors, prosthetic groups, electron carriers, and vitamin biosynthesis. ConclusionDanggui Niantongtang can effectively protect articular cartilage and delay the progression of KOA, possibly by regulating the structure of the intestinal flora, promoting probiotics, and inhibiting the growth of harmful pathogenic bacteria.

BACKGROUND:Bone tissue defects are one of the most common diseases in orthopedics, and the current treatments for this disease are inadequate. The development of tissue engineering brings new hope for bone defect repair:by regulating the release of bioactive substances and the process of vascularization and neurogenesis at the defect site,it can effectively improve the microenvironment of bone tissue and promote osseointegration, which is the most promising research idea for large-size bone defect repair. OBJECTIVE:To explore the research progress of regulating bone microenvironment changes in bone defect repair in recent years from the effects of bioactive substances, vascularization and neurotization on three aspects of bone microenvironment changes, and to provide new ideas and strategies for the treatment of large-size bone defects. METHODS:The search terms"bone tissue engineering, angiogenesis, neurotization, cytokines, bone morphogenetic protein, vascular endothelial growth factor, neuropeptides, bone microenvironment"in Chinese and English were used to search for articles on the influence of changes in the bone microenvironment and their application in bone tissue engineering published from January 1,2001 to December 31,2022 on CNKI,WanFang,Web of Science,Science Direct, and PubMed.Finally,109 articles were included for review. RESULTS AND CONCLUSION:(1)The bone microenvironment is essential for the induction of bone tissue stem cell growth and differentiation, and mainly consists of the extracellular matrix of the bone tissue seeds and the biochemical factors required for intercellular interactions, the local blood circulation network and the surrounding nerve tissue.(2)Bone defect repair is a continuous process divided into multiple phases that overlap and are mediated by multiple cytokines, and the same cytokine can have mutually synergistic or antagonistic effects in one or more healing phases.(3)Neovascular regeneration is key to initiating bone repair,as neovascularisation not only provides essential nutrients, osteoblasts and growth factors for bone repair, but is also a gateway for repair cells to enter the injury zone.(4)In addition to regulating the type, dose and timeliness of vascular-inducing factor release to achieve blood transport reconstruction. The study of differential release delivery systems of multiple factors and the application of gene transfer technology will be the future research direction to solve large bone defects.(5)Neuropeptides can bind to relevant receptors and act on specific signaling pathways to guide vascular growth and influence bone healing, bone regeneration and the balance between osteogenesis and osteolysis through a variety of pathways.(6)In the establishment of neuralized tissue-engineered bone, the role of changes in the bone tissue microenvironment and neuromodulation is bidirectional.Cytokines in the bone matrix can participate in neuronal signaling pathways through the blood-nerve barrier.Neuropeptides secreted by glial cells act on the bone microenvironment, affecting bone healing, bone regeneration and the balance between osteogenesis and osteolysis.(7)There are still many questions regarding the regulation of the bone microenvironment by bioactive substances and the processes of vascularization and neurogenesis, such as the rapid diffusion and degradation of cytokines in the body and their loss of activity, the temporal and spatial distribution of angiogenesis-related growth factors, and the establishment of neurogenesis through the body's feedback regulatory mechanism, which need to be improved by subsequent studies.

ObjectiveTo construct a quantitative prediction model of three indicators(moisture absorption rate, film thickness and coating weight gain) during the coating process of Vitamin C Yinqiao tablets(VCYT) by near-infrared spectroscopy(NIRS), and to realize the endpoint judgment. MethodReal-time NIRS data of 4 batches of VCYT during the coating process were collected by diffuse reflection method. The coating method employed was the rolling coating method, and the samples were obtained at the spray stage from the coater's sampling port every 10 minutes, and 57 batches of samples(about 1 800 tablets) were collected at various coating times, the tablets were embedded in molten paraffin, cut longitudinally, and observed by stereomicroscope. The film thickness, with a target value of 38 μm, was then measured using Motic Images Advanced 3.2 software. Furthermore, the mositure absorption rate of samples, aiming for a target value of 3%, was determined in accordance with guiding principles for drug hygroscopicity testing in the 2020 edition of Chinese Pharmacopoeia, and 3 samples were randomly selected from each batch(10 tablets per batch), and the coating weight gain was calculated(target value of 4%). Partial least squares regression(PLSR) was used to construct a quantitative model of the 3 coating indicators, and the predicted values of the coating indicators were smoothed using the moving average method and used to determine the coating endpoints. ResultThe prediction determination coefficients(Rp2) for moisture absorption rate, film thickness and coating weight gain were 0.933 4, 0.932 6 and 0.965 9, the root mean square errors of prediction(RMSEP) were 0.163 5%, 1.870 9 μm and 0.240 3%, the relative percent deviations(RPD) were 3.711 0, 2.760 7 and 5.415 8, respectively. The results of the external validation set demonstrated that the real-time predicted values obtained by the models exhibited the same trend as the measured values, and the coating endpoint could be accurately predicted(with a prediction error of less than 7.32 min and a relative error of less than 5.63%). ConclusionThe established NIRS model exhibits excellent predictive performance and can be used for quality control of VCYT during the coating process.

Hypertriglyceridemia-associated acute pancreatitis is an inflammatory disorder of exocrine pancreas caused by metabolism disturbances of triglyceride-rich lipoproteins.Currently,hypertriglyceridemia-associated acute pancreatitis is characterized by an escalating incidence rate,a tendency for more severe cases, and a lack of therapeutic drugs.Traditional Chinese medicine has distinct advantages in treating this disease, but its theoretical framework has not yet been established.Hypertriglyceridemia-associated acute pancreatitis manifests itself as a febrile disease, aberrant accumulation of fat and turbidity may stem from dietary imbalances and visceral dysfunction in ordinary individuals. The prolonged accumulation of fat and turbidity can transform into turbid pathogen, subsequently engendering heat, constituting a pivotal pathogenic factor.Throughout the progression of the disease, the fiery pathogen consumes the fat and turbidity, resulting in the generation of toxic heat, which is a crucial mechanism in the exacerbation of the disease severity. Thus, this article posits therapeutic principles aimed at averting the transformation of fat and turbidity into turbid pathogen and counteracting toxic heat in this disease. This article reviews two key theories from traditional Chinese medicine classics relevant to hypertriglyceridemia-associated acute pancreatitis:the theory of fat-turbidity associated with hypertriglyceridemia and the febrile disease related to acute pancreatitis.Combining these traditional theories with modern research on the mechanisms that intensify hypertriglyceridemia-associated acute pancreatitis and the corresponding targets of traditional Chinese medicine,it suggests that the pathogenesis of"fat-turbidity-toxic heat"serves as the theoretical basis of traditional Chinese medicine for the aggravated severity of hypertriglyceridemia-associated acute pancreatitis. The article aims to offer new insights for the treatment of hypertriglyceridemia-associated acute pancreatitis.

Purpose::In patients with heatstroke, disseminated intravascular coagulation (DIC) is associated with greater risk of in-hospital mortality. However, time-consuming assays or a complex diagnostic system may delay immediate treatment. Therefore, the present study proposes a new heatstroke-induced coagulopathy (HIC) score in patients with heat illness as an early warning indicator for DIC.Methods::This retrospective study enrolled patients with heat illness in 24 Chinese hospitals from March 2021 to May 2022. Patients under 18 years old, with a congenital clotting disorder or liver disease, or using anticoagulants were excluded. Data were collected on demographic characteristics, routine blood tests, conventional coagulation assays and biochemical indexes. The risk factors related to coagulation function in heatstroke were identified by regression analysis, and used to construct a scoring system for HIC. The data of patients who met the diagnostic criteria for HIC and International Society on Thrombosis and Haemostasis defined-DIC were analyzed. All statistical analyses were performed using SPSS 26.0.Results::The final analysis included 302 patients with heat illness, of whom 131 (43.4%) suffered from heatstroke, including 7 death (5.3%). Core temperature ( OR = 1.681, 95% CI 1.291 - 2.189, p < 0.001), prothrombin time ( OR = 1.427, 95% CI 1.175 - 1.733, p < 0.001) and D-dimer ( OR = 1.242, 95% CI 1.049 - 1.471, p = 0.012) were independent risk factors for heatstroke, and therefore used to construct an HIC scoring system because of their close relation with abnormal coagulation. A total score ≥ 3 indicated HIC, and HIC scores correlated with the score for International Society of Thrombosis and Hemostasis-DIC ( r = 0.8848, p < 0.001). The incidence of HIC (27.5%) was higher than that of DIC (11.2%) in all of 131 heatstroke patients. Meanwhile, the mortality rate of HIC (19.4%) was lower than that of DIC (46.7%). When HIC developed into DIC, parameters of coagulation dysfunction changed significantly: platelet count decreased, D-dimer level rose, and prothrombin time and activated partial thromboplastin time prolonged ( p < 0.05). Conclusions::The newly proposed HIC score may provide a valuable tool for early detection of HIC and prompt initiation of treatment.

Objective To explore the value of serum microRNAs-145(miR-145)and P53 antibody detection in the evaluation of therapeutic effect and prognosis in the treatment of breast cancer with Long Bei Xiaoyao Powder. Methods Ninety cases of breast cancer patients admitted to Beijing Aerospace General Hospital from January 2020 to January 2021 were selected as the study subjects. The patients were randomly divided into an observation group and a control group by random number table method, with 45 cases in each group. All of the patients in the two groups received individualized comprehensive treatment, and additionally the observation group was treated with Long Bei Xiaoyao Powder. The course of treatment covered three months.Real-time fluorescence quantitative polymerase chain reaction(PCR)method was used to detect serum miR-145 expression level, and enzyme-linked immunosorbent assay(ELISA)was used to detect serum P53 antibody expression level. The differences in the pre-and post-treatment expression levels of serum miR-145 and P53 antibodies in the two groups were compared, the relationship between the serum miR-145 and P53 antibody expression level and the poor prognosis of the patients in the observation group was explored by logistic regression analysis, and receiver operating characteristic curve(ROC)was drawn to analyze the value of the serum miR-145 and P53 antibody detection in the prediction of poor prognosis in the observation group.Results(1)After treatment, the detection value of serum miR-145 in the two groups of patients showed an increasing trend(P<0.05),while that of the P53 antibody showed a decreasing trend(P<0.05).The intergroup comparison showed that the detection value of serum miR-145 in the observation group was significantly higher than that of the control group, while the detection value of P53 antibody was significantly lower than that of the control group, the differences being all statistically significant(P<0.05).(2)In the observation group, the detection value of serum miR-145 in the patients with good prognosis was significantly higher than that in the patients with poor prognosis, and the detection value of serum P53 antibody in the patients with good prognosis was significantly lower than that in the patients with poor prognosis, the differences being all statistically significant(P<0.05).(3)Logistic regression analysis showed that high expression of miR-145 was a protective factor for the patients with poor prognosis in the observation group, and high expression of P53 antibody was a risk factor for the patients with poor prognosis in the observation group(P<0.05).(4)The area under the ROC curve(AUC)for the prediction of prognosis of the patients in the observation group by the combination of serum miR-145 and P53 antibody was the largest(0.877,95%CI:0.769-0.985),and its sensitivity and specificity were 93.75%and 93.10%,respectively,with statistically significant differences(P<0.05).Conclusion Low expression of serum miR-145 and high expression of serum P53 antibody are presented in the breast cancer patients with poor prognosis, and Long Bei Xiaoyao Powder exerts satisfactory therapeutic effect in treating breast cancer, which is conducive to enhancing the expression level of serum miR-145 and reducing the expression level of serum P53 antibody.
