Objective:To explore the influence of infusion drop speed on the efficacy of tiopronin for injection combined with sol-vents in the treatment of abnormal liver function. Methods:Totally 74 patients were randomly divided into the observation group and the control group with 37 ones in each. The infusion drop speed was 60-65 drops per minute in the observation group, and that was 30-35 drops per minute in the control group, and the infusion time was controlled in 1h in the observation group and 2h in the control group. The clinical effect and the improvement of liver function were compared between the two groups after a treatment course of 4 weeks. Results:The total effective rate of the observation group(83. 78%) was obviously higher than that in the control group(56. 76%) (P<0. 05) after the treatment, and ALT, AST and TBIL of liver function indices were improved obviously except for ALB in the two group, and the difference was statistically significant between the two groups(P<0. 05). Conclusion:Tiopronin for injection can effectively improve the clinical effect by strictly controlling the time and drop speed of infusion.

Objective To investigate the role of survivin and P53 in apoptosis of gastric adeno- carcinoma,as well as its relationship with the clinicopathologic features and the prognosis. Methods The gas- tric tissue microarrays were composed of those from 100 cases of gastric cancer and 30 controls. At these tissue microarrays, expressions of survivin and P53 were investigated immunohistochemically, and tumor cell apoptosis index was examined by TUNEL method.Of the 100 cases,47 cases were followed-up from 14 months to 13 years,in which the survival was analyzed. Results Two paraffin-embedded gastric carcinoma tissue micro- arrays were successfully constructed, including 114 and 116 tissue spots, respectively.Immunohistochemical analysis showed that survivin was expressed in 78 cases (78%).No expression of survivin was detected in control tissue (P0.05).In the 47 cases with followed-up data, univariant analysis revealed that the survival was correlated with invading of vessel and nerve,TNM stages, and expression of survivin. The histological grades and expression of P53 were not related to prognosis. However,Cox stepwise proportional hazards analysis showed that only TNM staging and survivin status retained significant independently in prospecting prognosis. Conclusions The expression of survivin was associated with the pathologic features,TNM stages and prognosis in gastric carcinoma, indicating that overex- pression of survivin may be a poor prognosis factor for gastric carcinoma.

Objective To explore the neuropsychological development in twin premature infants, and to analyze the risk factors for the retardation of development. Methods The premature infants hospitalized in the Neonatal Ward First Affiliated Hospital of Kunming Medical University from Jun.2010 to Jun.2012 were divided into study groups of twin premature infants and a control group of singleton premature infants. The neuropsychological development of 88 premature infants was evaluated at corrected gestational age of 1 year old by Bayley scales of infant development.According to normal lab findings of the Bayley scales of infant development, the premature infants were then divided into abnormal neuropsychological development group [mental development index(MDI) ≤79 scores] and normal neuropsychological development group (MDI > 79 scores),and the data of 2 groups were statistically analyzed. Results The frequency distribution of MDI score in study group showed a skewed distribution,of which 59% (27/46 cases) had abnormal neuropsychological development (MDI ≤ 79 scores); However, the frequency distribution of MDI score in control group showed an approximate normal distribution,of which only 12% (5/42 cases) for abnormal neuropsychological development.Head circumference, body length, body weight,MDI,psycho-motor development index of twin premature infants of one-year old were lower than those of the singleton premature infants of the same age,and the differences were statistically significant (all P <0.05).The single factor analysis demonstrated that the risk factors for abnormal neuropsychological development (MDI ≤ 79 scores) of twin premature infants were gestational age, birth weight, mother' s gcstational age,the cultural degree of parents, mode of feeding, neonatal hyperbilirubinemia, neonatal hypoglycemia and neonatal sepsis.Multiple regression analysis showed that neonatal hyperbilirubinemia was the independent risk factor for abnormal neuropsychological development of twin premature infants. Conclusions At same gestational age, neuropsychological development of twin premature infants lagged behind singleton premature infants.Neonatal hyperbilirubinemia may be the risk factors for neuropsychological development of twin premature infants,so the earlier management of neonatal hyperbilirubinemia may be beneficial for the neuropsychological development of twin premature infants.

In order to find new cephalosporin with more and more potent antibacterial activity, nine new fourth-generation cephalosporins (N1-N9) were synthesized from ethyl 2-(2-aminothiazol-4-yl)-(Z)-2-methoxyiminoacetate (1) via acylation, substitution, hydrolysis, active esterification, condensation and salt formation. The structures of compounds (N1-N9) were confirmed by IR, MS, 1H NMR and elemental analysis. The target compounds possess different antimicrobial activities against Gram-positive and Gram-negative bacteria. The preliminary results of antibacterial activities revealed that they showed better antibacterial activities against Gram-positive bacteria than cefpirome sulfate. In particular, their activities against Staphylococcus aureus and Streptococcus albus are better.

Chloroplast-based expression system is promising for the hyper-expression of plant-derived recombinant therapeutic proteins and vaccines. To verify the feasibility of obtaining high-level expression of the SARS subunit vaccine and to provide a suitable plant-derived vaccine production platform against the severe acute respiratory syndrome coronavirus (SARS-CoV), a 193-amino acid fragment of SARS CoV spike protein receptor-binding domain (RBD), fused with the peptide vector cholera toxin B subunit (CTB), was expressed in tobacco chloroplasts. Codon-optimized CTB-RBD sequence was integrated into the chloroplast genome and homoplasmy was obtained, as confirmed by PCR and Southern blot analysis. Western blot showed expression of the recombinant fusion protein mostly in soluble monomeric form. Quantification of the recombinant fusion protein CTB-RBD was conducted by ELISA analysis from the transplastomic leaves at different developmental stages, attachment positions and time points in a day and the different expression levels of the CTB-RBD were observed with the highest expression of 10.2% total soluble protein obtained from mature transplastomic leaves. Taken together, our results demonstrate the feasibility of highly expressing SARS subunit vaccine RBD, indicating its potential in subsequent development of a plant-derived recombinant subunit vaccine and reagents production for antibody detection in SARS serological tests.

OBJECTIVE To explore the strategy of diagnosis and therapy of lingual ectopic thyroid(LET). METHODS We retrospectively reviewed the clinical data of6 cases of LET, and figured out the clinical features, imaging characteristics, therapy and prognosis.RESULTS 6 lingual ectopic thyroids were diagnosed by the Tc-99m thyroid static imaging; 3 cases accepted surgery, one only took oral levothyroxine, and one selected watch and see. 3 cases of lingual ectopic thyroid accepted the ectopic thyroid transposition and took oral levothyroxine postoperatively. Pharynx foreign body sensation and sleep snoring disappeared one month later. The lingual ectopic thyroid carcinoma resection was performed by direct laryngoscope in one case which was lost to follow up.CONCLUSION The thyroid static imaging is specific to the diagnosis of the LET. The indications of the surgery were obvious symptoms such as airway obstruction, snoring and pharyngeal foreign body sensation, and mass hemorrhage, cystic degeneration and carcinogenesis. The principle of therapy is to retain thyroid function as far as possible and comprehensive use of surgery, thyroid hormone replacement and iodine radiotherapy to improve symptoms, prevention and treatment of hypothyroidism.
