1.Perioperative nursing of patients with post-thoracotomy atelectasis treated with bronchoalveolar lavage combined with pulmonary insufflation

Objective To summarize the perioperative nursing of patients with post-thoracotomy atelectasis treated with bronchoalveolar lavage combined with pulmonary insufflation.Methods Before operation,the nursing measures included the full preoperative preparation,full anesthesia and mental care.During the operation,the nursing focused on intensified monitoring,close cooperation with doctors in manipulation of bronchoalveolar lavage and pulmonary insufflation.After operation,the postoperative nursing was performed.Results The lung recruitments were successful in all the 12 patients.Compared to pre-operation,the arterial oxygen saturation(SaO2)and arterial oxygen partial pressure(PaO2)6 hours after treatment were significantly improved(P<0.05) while the CO2 partial pressure(PaCO2)was significantly decreased(P<0.05).There were no severe complications and no recurrence of pulmonary atelectasis.All the patients were successfully discharged.Conclusions The bronchoalveolar lavage combined with pulmonary insufflation is effective in treating post-thoracotomy atelectasis.To ensure the smooth treatment,perioperative nursing is also contributive.
2.Investigation on Characteristics and the Yield of the Secondary Metabolites of a Myxobacterium Isolated from Beach Soil

A myxobacterial strain, So ce cpu-1, was isolated from a soil sample near the Huanghai beach. So ce cpu-1 had broad antimicrobial activity, active component has maxima] absorption at 210 nm. The effects of different culture conditions on the yield of the secondary metabolites were investigated. The results showed that, when cultivating the strain in the M, medium (containing 10% w/v D312 neutral absorber resin ), the air up to 70% of the whole flask volume, adding 5?L the secondary metabolites as the revulsant, at 30℃, 200r/min, for 6 days, the yield of the secondary metabolites achieved the maximum.
3. Clinical efficacy of CT-guided implantation of iodine 125 seeds combined with intra-arterial perfusion of gemcitabine plus 5-fluorouracil on pancreatic cancer

Objectives: To evaluate the safety and efficacy of CT-guided iodine 125 seed implantation combined with gemcitabine and 5-fluorouracil (5-FU)-based intra-arterial perfusion in the treatment of patients with advanced pancreatic cancer. Methods: A total of 16 patients with advanced unresectable pancreatic cancer underwent CT-guided iodine implantation combined with gemcitabine plus 5-FU-based intra-arterial perfusion. One week after implantation of iodine 125 seeds, all the patients received concurrent intra-arterial infusion of gemcitabine and 5-FU, which was repeated for 3 to 4 cycles. Three patients complicated with liver metastasis were given additional arterial embolization concurrently. Survival period, pain-relieving period, and adverse reaction of patients were observed. Results: All the patients were followed up for 2-25 months. CT scan found that there were 2 cases with complete response, 9 cases with partial response, 3 cases with stable disease, and 2 cases with progressive disease after 2 months of follow-up. The overall effective rate (CR + PR) was 68.8%. The median survival period for all enrolled patients was 11 months. It was 15 months for patients at stage II + III and 9 months for those at stage IV. No serious complications including gastrointestinal bleeding, pancreatitis, pancreatic fistula and radiation colitis were observed. Conclusion: CT-guided iodine 125 seed implantation combined with gemcitabine and 5-fluorouracil (5-FU)-based intra-arterial perfusion has better palliative pain-relieving effects. It has safe and effective effects in short terms and causes less complications in the treatment of advanced pancreatic cancer patients.
4. Expressions of P-gp, ToPo II α, GST-π and c-erbB2 in endometrial cancer and their significance

Objective: To investigate the expressions of P-glycoprotein (P-gp), topoisomerase II alpha (ToPo II α), glutathione-S-transferase-pi (GST-π), and oncogene c-erbB2 in endometrial cancer and their significance. Methods: Immunohistochemistry SP method was used to determine the expressions of P-gp, ToPo II α, GST-π and c-erbB2 in 127 tumor samples from patients with endometrial cancer. Results: Among 127 tumor samples from patients with endometrial cancer, the positive expression rates of P-gp, ToPo II α, GST-π and c-erbB2 were 63.0%, 44.9%, 70.1%, and 47.2%, respectively. The co-expression rate of P-gp and GST-π was 50.7%. The expression levels of P-gp, GST-π and c-erbB2 in poor differentiation endometrial cancer were significantly higher than those in well/moderate differentiation endometrial cancer (P < 0.05). While the expression level of ToPo II α in well/moderate differentiation endometrial cancer was significantly higher than that in poor differentiation endometrial cancer (P < 0.05). Univariate analysis indicated that the expression of P-gp, GST-π and c-erbB2 were associated with the survival time and poor prognosis of endometrial cancer patients. COX regression analysis suggested that the expression of P-gp and co-expression of P-gp and GST-π correlated with the survival rate of patients. Over-expression of ToPo II α tended to have protective effects on patients. Conclusion: Multidrug resistance-associated protein (MRP), GST-π, Topo II α, and lung resistance protein (LRP) plays important roles in the multidrug resistance(MDR) of tumor chemotherapy. Combined effects of the four multiple factors, P-gp, ToPo II α, GST-π and c-erbB2 were the main action mechanism for the MDR of endometrial cancer. It has an important guiding significance for predicting the prognosis and making rational chemotherapeutic regimens for patients with endometrial cancer.
5.Detection of specific proteins in serum of acute stage patients with SARS and its clinical application

Objective To observe the changes of specific proteins in serum of acute stage patients with SARS and its clinical application. Methods The acute stage patients with SARS group included 42 cases who had clinical symptoms less than five days. In normal group: 30 healthy adults were as controls. Tests for specific proteins were immunoglobin G ( IgG ) , immunoglobin A ( IgA) , immunoglobin M ( IgM) , complement C3( C3),complement C4(C4),C-reaction protein (CRP), al acid glycoprotein( a1-AG) , a1 antitrypsin(a1-AT) ,transferrin(TRF), ceruloplasmin( CER) in sera by Ratenephelometer. Results The level of CRP, a1-AG, a1-AT in sera of patients with SARS were higher than that in control group ( P 0. 05). Conclusion It suggested that specific proteins in serum of acute stage patients with SARS change in a particular way. There were some help for the diagnosis of the phlogistic degree of acute stage patients with SARS by detecting CRP, a1-AG and TRF simultaneously.

Objective To compare the clinical value of uterine submucosal myoma classification by two dimensional and three-dimensional contrast-enhanced ultrasonography and surgical pathologic results.Methods The imaging data of ultrasonographic hysterography including 2D and 3D of 124 patients with uterine submucosal myoma were retrospectively analyzed,and the results were compared with the surgical pathologic results.The diagnostic accuracy of uterine submucosal myoma classification and the operation success rate of uterine submucosal myoma for Ⅰ grade by ultrasonographic hysterography including 2D and 3D were compared.Results The patients were diagnosed pathologically with 0,Ⅰ and Ⅱ grade of uterine submucosal myoma in 26 cases,52 cases,68 cases,respectively.The patients were diagnosed by 2D ultrasonic sonohysterography with 0 grade,Ⅰ grade and Ⅱ grade of uterine submucosal myoma in 26 cases,62 cases,58 cases,respectively.The patients were diagnosed by 3D ultrasonic sonohysterography with 0,Ⅰ and Ⅱ grade of uterine submucous myoma in 26 cases,52 cases,68 cases,respectively.For pathological results as thegold standard,the diagnostic sensitivity,specificity and accuracy of uterine submucosal myoma for 0 grade by 2D and 3D ultrasonic sonohysterography were all 100.00%.The diagnostic sensitivity,specificity and accuracy of uterine submucosal myoma for Ⅰ and Ⅱ grade by 2D ultrasonic sonohysterography were 92.32%,79.46%,85.00%,respectively.The diagnostic sensitivity,specificity and accuracy of uterine submucosal myoma for Ⅰ and Ⅱ grade by 3D ultrasonic sonohysterography were 96.24%,88.24%,91.76%,respectively.There were significant differences in the diagnostic sensitivity,specificity and accuracy of submucosal myoma of uterus for Ⅰ and Ⅱ grade by 2 D and 3 D ultrasonic sonohysterography (x2 =3.21,2.78,2.17,2.33,all P < 0.05).The patients diagnosed as uterine submucous myoma for 0 grade all underwent the hysteroscopic surgery for successful resection,while the patients with uterine submucous myoma for Ⅱ grade underwent laparoscopic surgery or open surgery.The operation success rates of uterine submucous myoma for Ⅰ grade by hysteroscopic surgery diagnosed by 2D and 3D ultrasonic sonohysterography were 75.81%,98.07%,respectively.The operation success rate of uterine submucous myoma for Ⅰ grade by hysteroscopic surgery diagnosed by 3D ultrasonic sonohysterography was significantly higher than that diagnosed by 2D ultrasonic sonohysterography (x2 =7.15,P < 0.05).Conclusion The accuracy of uterine submucosal myoma classification by 3D ultrasonographic hysterography is better than 2D ultrasonographic hysterography.
9.The application of array comparative genomic hybridization for the detection of chromosomal defects in newborns

Objective To study the application of the array comparative genomic hybridization (Array-CGH) for the detection of chromosomal disorders in newborns.Method The Array-CGH technique was used to analyze the whole genome of the patients who were suspected of chromosomal disease in neonatal ward of our hospital from January to December in 2014,and further verification in genomic unbalanced ectopia was carried out by FISH (fluorescence in situ hybridization,FISH).Result Among 514 patients,104 were found carrying chromosomal abnormalities with a detection rate of 20.2%.The most common chromosomal disease is the Down syndrome syndrome (24 cases),followed by the chubby Willy and Angel syndrome(17 cases),while the Wolf-Hirschhorn syndrome in 5 cases,Williams syndrome in 5 cases and the Criduchat syndrone in 5 cases.The results of FISH were consistent with Array-CGH.Conclusion The technique of Array-CGH can be used to scan the whole genome of children with unknown disease.As a high-throughput and rapid research method,this technique has important clinical significance in the screening of chromosomal diseases.
