Objective To study the correlation between small intestinal bacterial overgrowth (SIBO) and plasma endotoxin,platelet-derived growth factor(PDGF)level,and levels of serum hyaluronic acid (HA),laminin(LN),N-terminal procollagen Ⅲ peptide(PⅢNP)and type Ⅳ collagen(Ⅳ.C). Methods Lactulose hydrogen breath test (LHBT) was performed in 64 patients with chronic liver disease. Plasma endotoxin was measured with Limulus test, plasma PDGF was tested with ABC-enzyme-linked immunosorbent assay,serum HA,LN,PⅢNP and Ⅳ.C were tested by radioimmunoassay(RIA) in the patients. The relationship between SIBO and plasma endotoxin, PDGF and HA,LN,PⅢNP and Ⅳ.C levels were analyzed. Results Positive LHBT suggesting the existence of SIBO were observed in 22 patients(34.4%). Plasma endotoxin[(62?19) pg/ml vs.(31?13) pg/ml] and PDGF [(211?77) pg/ml vs.(136?57) pg/ml] levels were significantly higher in patients with SIBO than those in patients without SIBO (P

OBJECTIVE:To investigate the characteristics and regulations of antibacterials-induced adverse drug reactions (ADR) in our hospital,and to provide reference for promoting rational drug use. METHODS:233 patients with antibacterials-in-duced ADR in our hospital from Oct. 2013 to Jun. 2015 were selected and analyzed statistically according to patient's age and gen-der,route of administration,the type of antibacterials,occurrence time,organs/systems involved in ADR,etc. RESULTS:There were a large number of ADR in patients age ≤18 and ≥70 years,accounting for 19.3% and 18.5%;the male was more than the female;the incidence of ADR induced by intravenous route was the highest,accounting for 94.0%. Most of ADR was caused by cephalosporins,accounting for 33.9%;ADR often occurred within 1 d after medication,accounting for 63.9%;lesion of skin and its appendants injury was main ADR,accounting for 39.3%. CONCLUSIONS:Adhere to theclear indications,a detailed inqui-ry about allergy history before drug use,right dose,suitable route of administration and course,and timely ADR disposal can re-duce the damage caused by ADR.

Objective To analyze the ischemia degree of initial electrocardiogram in the patients of acute ST-segment elevation myocardial infarction (STEMI) and investigate its value in the predicting cardiogenic shock (CS) after percutaneous coronary intervention (PCI).Methods Three hundred and ninety patients with STEMI from the onset of symptoms to admission within 12 h were divided into two groups based on grade Ⅱ ischemia (group A,248 cases) or grade Ⅲ ischemia (group B,142 cases) in the initial electrocardiogram.Clinical data,TIMI risk score,ST-segment resolution (STR),CS and cardiovascular events (hospital mortality,ventricular arrhythmias,reinfarction) were recorded in all patients.Results The gender,time from onset to balloon opening,smoking,hypertension,type 2 diabetes,hyperlipidemia,stroke,postoperative TIMI flow 3 grade,coronary artery lesions and lesions in the left main stem between two groups was not statistically significant (P ＞0.05).In group B,the rate of ST segment resolution ＞ 50％ was significantly lower than that in group A [53.2％ (132/248) vs.29.6％ (42/142)] (P ＜ 0.01).The incidence of CS,in-hospital death,malignant ventricular arrhythmias in group B was higher than that in group A,and the difference was statistically significant (P ＜ 0.05).The age,left ventricular ejection fraction,TIMI risk score ＞ 3 points,Killip grade ＞ 1 grade,anterior myocardial infarction between two groups was statistically significant (P ＜ 0.05 or ＜ 0.01).The indicators that were statistically significant in the univariate analysis were included into Logistic regression model and analyzed,with CS-related factors as independent variables and CS as the dependent variable and found that age (P =0.008),Killip class ＞ 1 grade (P =0.049),ST segment resolution rate (P =0.008) and grade Ⅲ ischemia (P =0.001) as independent predictors of CS after PCI.Conclusions Grade Ⅲ ischemia is an independent predictor of CS after PCI in STEMI patients.And it has predictive value for hospital mortality and ventricular arrhythmias.

Objective To study the clinical efficacy of recombinant human epidermal growth factor (rhEGF) in the treatment of sacrococcygeal decubitus ulcers grade Ⅲ with direct polymerizing suture after appling rhEGF to reinforce wound bed preparation (WBP). Methods From January 2007 to October 2009, 60 patients with sacrococcygeal decubitus ulcers Grade Ⅲ, were divided into control group and treatment group. The ulcer size was 3 cm × 4 cm to 10 cm × 12 cm and all ulcers were infected for 15-70 d. Treatment group received traditional dressing change and appling rhEGF to reinforce wound bed preparation. Control group received traditional dressing change only. The operative technique that we used in two groups was direct polymerizing suture. Cure rate of stage Ⅰ and complication morbidities were analyzed. Results Cure rate of stage Ⅰ was 87% in treatment group and 70% in control group. Complication morbidities were 13% in treatment group and 30% in control group. The difference between these two groups was statistically significant (P ＜ 0.05). Conclusion Appling rhEGF to reinforce wound bed preparation before operation could make subsequent treatment more effective and improve the cure ratio of operation with decreasing complications and morbidities. And more, dissecting under fascia possesses the advantages of easiness to perform and rich blood supply.The method of appling rhEGF with direct polymerizing suture is a simple, high efficient approach for the first repairment of sacrococcygeal decubitus ulcers grade Ⅲ, especially desirable for the elderly.

Clavicular crook plate is designed based on anatomic features of acromioclavicular joint,which matches clavicle appearance,and well attaches the shaft.Not passing through the joint,the internal fixation exhibits many advantages,such as convenient usage,reliable fixation and micromovement of fixed acromioclavicular joint.So the patients could move the joint early and avoid osteoarthritis.Allogenous tendon has abundant sources,simple sampling process,and short repairing duration and can reduce injury to patients.Cryopreserved allogenous tendon has low antigenicity and immunological refection,good histocompatibility.Reconstructed coracoclavicular ligament by allogenous tendon has good material intensity,and can meet the requirements of local stress,better substitute integrity and function of coracoclavicular ligament.Moreover,it has precise effect and simple operation.It acts as long-term fixator following plate removal,thus is important supplement of clavicular crook plate fixation.Clavicular crook plate fixation in combination with allogenous tendon transplantation reconstructs acromioclavicular joint stability,effectively provides early mechanical stability and subsequent biological stability,and maximizes restoration of acromioclavicular joint structure and function.

Objective To investigate the clinical efficacy of S-1 combined with cisplatin in the treatment of old patients with advanced gastric cancer.Methods 85 patients were randomly divided into two groups.Experimental group was treated by S-1 combined with cisplatin,while control group was treated by 5-FU and cisplatin.Short-term efficacy and adverse reactions were evaluated after four cycles.Results All patients had succcssfully accomplished chemotherapy.Compared with the control group ( RR =52.4％,DCR =81.0％ ),RR and DCR in experimental group ( RR =67.4％,DCR =93.0％ ) had statistically significant difference ( P ＜ 0.05 ).The top three of adverse reactions and complications in patients of the two groups were:nausea and vomiting,low WBC,low hemoglobin concentration.Compared with the control group,the toxicity in experimental group had no statistically significant difference ( P ＞0.05).All patients were followed up with a meau of( 15.5 - 2.7 ) months.The 1 year survival rate was 60.5％(26/43) in the experimental group; the 1 year survival rate was 52.4％ (21/42) in the control group.There was no statistically significant difference( P ＞ 0.05 ).Conclusion The S-1 combined with cisplatin had shown better efficacy in the treatment of old patients with advanced gastric carcinoma,and the toxicity was relatively mild and tolerable.

Objective To evaluate the effects of propofol on the proliferation of hippocampal neurons in fetal rats in vitro.Methods Pregnant Sprague-Dawley rats at 16-18 days of gestation,were sacrificed and the fetal rats were taken out from the abdominal cavity.The hippocampal neurons of the fetal rats were isolated and seeded in culture plates.After being cultured for 9 days,the neurons were divided into 7 groups using a random number table(n =36 each):control group (C group),intralipid group (I group) and propofol 0.1,1.0,10.0,100.0,1 000.0 μmol/L groups (P1-5 groups).In group I,10％ intralipid was added to the culture media until the final concentration reached 100 μmol/L.In P1-5 groups,propofol was added to the culture media until the final concentration reached 0.1,1.0,10.0,100.0 and 1 000.0 μmol/L,respectively.The neurons were then incubated for 3 h.The proliferation of hippocampal neurons was determined by MTT assay at 12,24,36,48,60 and 72 h after the end of incubation with propofol.Results Compared with group C,the proliferation of hippocampal neurons was significantly decreased in P1-5 groups (P ＜ 0.05),while no significant change was found in group I (P ＞ 0.05).Compared with group P1,the proliferation of hippocampal neurons was significantly decreased in group P5 (P ＜ 0.05),while no significant change was found in P2-4 groups (P ＞ 0.05).Conclusion Propofol can decrease the proliferation of hippocampal neurons in fetal rats in vitro.

Objective:To obtain recombinant Profilin of silkworm,identify its immunogenicity,predict its B cell epitopes and construct the evolutionary trees. Methods: The nucleotide sequence of Profilin was acquired from NCBI,synthesized it and cloned it into pET-28 vector. Then,the recombinant plasimids were transformed to E. coli BL21. After induced by IPTG,recombinant protein was purified by Affinity chromatography. Furtherly,its allergenicity was identified by Western blot,the potential B cell epitopes was analyzed through DNAStar and build the evolutionary trees by MEGA5. 05. Results: The recombinant protein of Profilin was successfully expressed and purified by affinity chromatography. Besides,the protein contains a high IgE-binding activity with IgE existing in serum of patients allergic to silkworm. Conclusion: The recombinant proflilin has IgE-binding activity, and it is meaningful for fundamental research and specific diagnosis studies of allergic diseases caused by silkworm.

Objective To investigate the therapeutic effect of trans-TrkC gene neural stem cells (NSCs)on the recovery of neural function after spinal cord injury.Methods Sixty SD rats were ran- domly divided into six groups:normal control group(A),hemisection group(B),NSCs transplantation grnup(C),NSCs transplantation with the regional application of NT-3 group(D),trans-TrkC gene NSCs transplantation group(E)and trans-TrkC genc NSCs transplantation with the regional application of NT- 3 group(F),10 rats in each group.Nine days after the set up of animal models,cell transplantation into the injured spinal cord was performed.The BBB locomotor score was calculated,and MEP(motor evoked potential)and SEP(somatesensory evoked potential)were pedormed two months after cell transplanta- tion.Results Two months after cell transplantation,the BBB locomotor score was partly recovered, and the MEP and SEP(somatosensory evoked potential)results were also markedly improved in Group F, which indicated the restoration of the upward and downward nerve conduction function of the injured spinal cord.But it seemed that the restoration of the downward nerve conduction was better than that of the up- want,and the extent of the improvement of MEP and SEP results was larger than that of motion function recovery.The onset latency,peak to peak amplitude of MEP and SEP,and the BBB score of Group F re- stored the best compared with the other groups,and the differences were statistically significant(P
