3.Research on radioimmunotherapy of lung cancer in nude mice using lung cancer monoclonal antibody LC-1 combined with ⁹⁰Y.

BACKGROUNDTo evaluate potential clinical roles of monoclonal antibody (MoAb)-based radioimmunotherapy in the treatment of lung cancer.
METHODSAnti-lung cancer monoclonal antibody LC-l IgM was combined with ⁹⁰Y to produce radioimmunological targeting drug. Human lung cancer tissue was inoculated subcutaneously in 35 nude mice. They were randomized into seven groups while the tumor was 5 mm in diameter. The groups were divided by the dose of the drug injected to the tail vein of the mice: blank group, LC-1 IgM alone, ⁹⁰Y 50 μCi alone, 50 μCi, 150 μCi, 300 μCi, and 400 μCi combined therapy groups. The size of the tumor was measured weekly and the mice were killed in four weeks after treatment. The tumors were resected and weighed.
RESULTSAs compared to blank group, therapy groups' tumor growth was inhibited and the inhibition was dose- and time-dependent. The inhibition rates of 300 μCi and 400 μCi groups were significantly different (P < 0.05). The nuclei of tumor cells showed karyopycnosis, structure disorder and rupture after treatment by pathological exam. In some regions, the tumor cells were necrotic or disappeared.
CONCLUSIONSThe results indicate that the radioimmunological drug made from lung cancer monoclonal antibody LC-1 IgM and ⁹⁰Y can specifically localise in tumor tissue and ensure radioimmunological targeting therapy, so has underlying clinical value.
4.Analysis of neonatal fibrobronchoscopic indications, findings and interventions

Objective To investigate the correlation between the indications,findings,interventions of fibrobronchoscopy(FB) in neonates and their correlative diseases with neonatal FB results and clinical data.Methods Retrospective case series of 243 consecutive patients of 28 days old or younger were investigated underwent FB for the first time from January 2010 to December 2014,at a tertiary care hospital.The common indications for FB and detection rate of respiratory tract diseases were collected.If the findings of FB had significant associations with premature birth and other diseases were analyzed.Associations between interventions and basic illnesses were also analyzed.Results Of the 243 patients undergoing 275 procedures of FB,201 cases were boys(73.1%).The age of FB was (13.34 ± 9.76) days and the weight was (3.08 ± 0.68) kg.Forty-five cases were premature infants (16.4%).A total of 254 procedures were found to have congenital diseases (92.4%),and 177 cases of them had congenital heart diseases (CHD) (64.4%).Common indications for FB were dyspnea(140 cases,50.9%),tachypnea(82 cases,29.8%),and stridor(71 cases,25.8%).A total of 188 upper airway lesions were found and the most common findings were laryngomalacia(56 cases,20.4%) and vocal cord paralysis(bilateral/unilateral,50 cases,18.2%).A total of 315 lower airway lesions were found and the most common findings were airway mucosal inflammation (98 cases,35.6%),trachea and main bronchial stenosis (73 cases,26.5 %).A total of 21 cases (7.6%) underwent supraglottoplasty during or after FB,while 17 cases (6.2%) underwent tracheal dilation and 10 cases (3.6%) underwent tracheotomy.Compared with non-CHD neonates,neonates with CHD were statistically significantly less likely to have congenital lesions statistically,such as laryngomalacia(15.8% vs.28.6%,P =0.012),bilateral vocal cord lesions(6.2% vs.21.4%,P =0.000) and congenital laryngeal dysplasia(0 vs.7.1%,P =0.001).The tracheotomy(0 vs.10.2%,P =0.000) and supraglottoplasty(2.3% vs.17.3%,P =0.000) were more rare.Nevertheless,they were more likely to have secondary lesions such as the left main bronchial stenosis caused by extrinsic compression (23.7% vs.1.0%,P =0.000),abnormal bronchial anatomy(9.6% vs.2.0%,P =0.018),left vocal cord paralysis(9.0% vs.1.0%,P =0.008) and airway mucosal inflammation(41.8% vs.24.5%,P =0.004).The tracheostenosis and main broncial stenosis (37.3% vs.7.1%,P =0.000) with long-term intubation(78.5% vs.58.2%,P =0.000) were more common.There was no significant difference between term neonates and premature infants in the detection rate of respiratory tract diseases (P > 0.05),tracheotomy (0 vs.4.3 %,P =0.322),supraglottoplasty (13.3 % vs.6.5 %,P =0.205) or long-term i ntubation (80.0% vs.69.6%,P =0.157).Complications caused by procedure were rare and mild.Conclusions FB can detect whether the neonates with dyspnea,tachypnea and stridor have laryngomalacia,vocal cord paralysis,airway mucous edema,tracheal and main bronchial stenosis and other signs,and FB may play an important role in diagnosis,treatment and prognosis evaluation of neonatal respiratory diseases.
5.Diagnosis and treatment of priapism due to neoplasma of penis.

OBJECTIVETo improve the clinician's ability for emergency treatment of priapism.
METHODSBoth cases received 2 mg to 8 mg of metaraminol injection at the root of cavernous body, and perfusion of heparinized saline at glans and root of cavernous body of the penis by contrecoup, but they had not good response to the above therapy. At last surgery was performed.
RESULTSTotal penectomy was performed for both cases. One case was diagnosed of penile sarcoma, and another was metastatic transitional cell carcinoma.
CONCLUSIONPriapism due to neoplasma is infrequent, it should not be misdiagnosed in case of emergency.

6.Investigation of selenium levels in the environment and human body in Gaomi City and Zichuan District of Shandong Province in 2005

Objective To investigate selcnium(Se) levels of environment and human body in Gaomi City and Zichuan District of Shandong. Methods Lijiaying Township in Gaomi City of Weifang City, Zhaili Township and Longquan Township in Zichuan District of Zibo City were selected. Two farming soil samples at different spot, local wheat and corn, residents nail samples from 3 to 4 families were collected in each natural village in the investigated towns. The contents of Se were detected by 2,3-diamino naphthalene fluorescence method. Results Se level of the soil, wheat, corn, and nails in Lijiaying [(0.054 ± 0.019), (0.022 ± 0.009), (0.018 ± 0.007), (0.365 ± 0.108)mg/kg] was significantly lower than that in Zhaili [(0.425 ± 0.080), (0.130 ± 0.043), (0.098 ± 0.026), (0.751 ± 0.134)mg/kg] and Longquan[(0.487 ± 0.153), (0.112 ± 0.030), (0.097 ± 0.029), (0.735 ± 0.145)mg/kg;P < 0.01]. In Lijiaying, Se was deficient in soil, wheat, corn(< 0.200, < 0.025 mg/kg), above Se deficiency diagnosis and below Se-adequate level in the nail, while in Zhaili and Longquan, the Se level in the soil (0.425, 0.487 mg/kg), wheat(0.130, 0.112 mg/kg), corn (0.098, 0.097 mg/kg), nails (0.751, 0.735 mg/kg) was adequate (≥0.400 mg/kg). Conclusions The external environment is Se-deficient in Lijiaying, Se-adequate in Longquan and Zhaili. The selenium level in human body is consistent with the external environment.
7.Predictive value of umbilical cord blood bilirubin level for subsequent neonatal jaundice.

OBJECTIVETo investigate the predictive value of umbilical cord serum (UCS) bilirubin for subsequent jaundice in healthy term newborns.
METHODSFive hundred and twenty-three healthy term newborns (275 boys, 248 girls) were selected. The cord blood total serum bilirubin concentration and the serum albumin concentration were determined. All the infants were assessed for jaundice daily by measurement of transcutaneous bilirubin (TCB). When the infant's TCB was >or= 18 within the first 24 h after birth, >or= 21 at 48 h, >or= 25 at or after 72 h, the venous total serum bilirubin (TSB) was determined and treatment against jaundice was applied as needed. The infants were aligned into four groups according to their UCS bilirubin levels, starting from < 30 micromol/L(group 1); >or= 30 micromol/L(group 2); >or= 36 micromol/L(group 3); >or= 42 micromol/L(group 4). The frequency of hyperbilirubinemia and phototherapy (PT) were compared among the four groups. An analysis of UCS bilirubin as a predictor of later development of jaundice was performed. The characteristics of the infants who became jaundiced (jaundiced group) were compared with the normal infants (non-jaundiced group).
RESULTSA clear correlation between UCS bilirubin level and the development of hyperbilirubinemia was found in all populations of the four groups. Only eight of the 194 infants in group 1 showed a TCB index >or= 25. TSB values > 205 micromol/L but < 257 micromol/L were observed in 2 newborns. None of the infants in this group showed TSB > 257 micromol/L or needed PT. Thirty-two infants in group 2 showed TCB >or= 25, 12 infants had TSB > 205 micromol/L but < 257 micromol/L, 2 infants had TSB > 205 micromol/L and received PT. In group 3, one infant developed hyperbilirubinemia at 48 h after birth and received PT. Thirty-nine infants showed TCB >or= 25, 16 infants TSB > 205 micromol/L but < 257 micromol/L, 2 infants had TSB > 205 micromol/L and also received PT. In group 4, 4 infants showed a range of TSB from 200 to 215 micromol/L at 48 h and received PT. Twenty-two infants showed TCB >or= 25, 17 of them showed TSB > 205 micromol/L but < 257 micromol/L, and 5 of them had TSB > 205 micromol/L and received PT. The frequency of TSB > 205 micromol/L increased from 1.03% in group 1, 5.77% in group 2, 19.75% in group 3 and to 42.5% in group 4. None of the 194 newborns in group 1 needed phototherapy, whereas 0.96%, 3.70% and 22.5% of the newborns in groups 2 - 4, needed PT. The frequency of patients with hyperbilirubinemia or phototherapy increased with increasing UCS bilirubin levels. For the prediction of TCB >or= 25 using a UCS bilirubin cut-off level, such as >or= 35 micromol/L, we found a positive predictive value of 45.68% and sensitivity of 68.27%. It is significant to predict neonatal jaundice by UCS bilirubin levels (P < 0.001). In the jaundiced group (TCB >or= 25) UCS bilirubin levels were significantly higher than those in the non-jaundiced group (t = 10.96, P < 0.001). No significant differences were found in the cord blood serum albumin concentration (t = 2.38, P > 0.05), the gestational age (t = -0.90, P > 0.05), and birthweight (t = 0.10, P > 0.05) between the jaundiced and non-jaundiced groups.
CONCLUSIONSUCS bilirubin level is useful in predicting the subsequent jaundice in healthy term infants. The use of UCS bilirubin values may help detect infants at low or high risk for hyperbilirubinemia and minimize an unnecessary prolongation of hospitalization.

8.Short- and long-term effects of thrombolysis in patients with acute cerebral infarction and its influencing factors

[Objective]To analyze the short-term (24 h) and long-term (3 months) effects of thrombolysis in patients with acute cerebral infarction and its influencing factors.[Methods]Fifty-three patients with acute ischemic stroke,admitted to our hospital fiom January 2006 to December 201 i,were treated with urokinase (UK);the effects were observed by NIHSS and Modified Rankin Scale (mRS).Influencing factors were analyzed by Stepwise logistic regression.[Results] Twenty-eight patients had good outcome (52.8%) 24 h after thrombolytic therapy;multivariate logistic regression indicated that gender,interval between onset and receiving thrombolytic therapy,and fibrinogen level before thrombolytic therapy were the independent predictors of good outcome (P=0.007,0.017 and 0.001,respectively).Forty patients (75.4%) had good outcome 3 months after thrombolytic therapy;multivariate logistic regression indicated that the scores of NIHSS before thrombolysis and 24-h thrombolytic effect were the independent predictors of good outcome (both P=0.002).[Conclusion] Risk factors associated with short- and long-term thrombolytic effects might be different,which needs us make a difference in clinical work.
9.Clinical analysis of 29 cases with neuroendocrine neoplasm in the digestive system.

OBJECTIVETo investigate the diagnosis and treatment of neuroendocrine neoplasm (NEN) in the digestive system.
METHODSClinical data of 29 patients with NEN from January 2000 to December 2012 in The First Affiliated Hospital of Dalian Medical University were analyzed retrospectively and the prognosis was evaluated according to the new WHO classification.
RESULTSThere were 19 males and 10 females and the average age was 46.5 years. All the patients had no clinical manifestations of carcinoid syndrome, and they all received surgical treatment. Two cases were gastric neuroendocrine carcinoma(NEC), who received radical total gastrectomy and distal gastric resection respectively. Three cases had neoplasm in the duodenum, including 2 NEC and 1 neuroendocrine tumor(NET), and they all underwent Whipple's procedure. Two cases were small intestine NEC, who received partial small intestine resection. Three cases had neoplasm in the appendix, including 1 NEC treated with right hemicolectomy and 2 NET with appendectomy. One case was ascending colon NEC, who received right hemicolectomy. Eighteen cases had neoplasm in the rectum, including 4 NEC treated with low anterior resection and abdominoperineal resection respectively, and 14 cases of NET underwent low anterior resection, local resection, and endoscopic resection respectively. The 1- and 3- year survival rates of 13 NEC cases were 38.4% and 7.7% respectively. The 5-year survival rate of 16 NET cases was 81.3%.
CONCLUSIONSNEN of digestive system is located mainly in the rectum and the clinical symptom is unspecific. Radical resection of NEN is the preferred treatment. The prognosis of NEC is poor, and that of NET is better.

10.Effects of hemoperfusion on oxidative stress, matrix metalloproteinases and their inhibitors in acute paraquat poisoning rabbits.

OBJECTIVETo observe the effects of hemoperfusion on oxidative stress status and the levels of matrix metallo proteinase (MMP-2, MMP-9), tissue inhibitor of metalloproteinase (TIMP-1) in lungs, livers and kidneys in paraquat poisoning rabbits, and to explore the mechanism of therapeutic effects induced by HP on acute paraquat poisoning.
METHODSSeventy eight rabbits were randomly divided into normal control group (N group, n=6), exposure groups (PQ group, n=24), hemoperfusion treatment group (HP treatment group, n= 24) and blank control group (HP group, n=24). The PQ, HPQ and HP groups were divided into 4 observation time groups (1, 3, 7 and 21 d). N group was exposed to 5 ml normal saline and PQ group was exposed to 50 mg/kg PQ by oral gavage. In 1 h after PQ exposure, HPQ group was exposed to the activated carbon hemoperfusion for 2 h. The content or activity of MDA, SOD and GSH-Px in lungs, livers and kidneys were detected, the expression levels of MMP-2, MMP-9 and TIMP-1 were measured with immunohistochemical SP method for all groups.
RESULTSThe contents of MDA in lungs, livers and kidneys of PQ and HPQ groups decreased and the activities of SOD and GSH-Px in lungs, livers and kidneys of PQ and HPQ groups increased with observation time. The expression levels of MMP-2, MMP-9 and TIMP-1 in PQ and HPQ groups enhanced on the first day, PQ group was most obvious. Along with the observation time extended, all kinds of positive expression were still high. Compared with normal control group, the activities of serum SOD and GSH-Px in PQ and HPQ groups declined significantly, but the contents of serum MDA increased; the expression levels of MMP-2, MMP-9 and TIMP-1 in lung, liver and kidney tissues increased obviously, the ration between MMP-9 and TIMP-1 significantly increased (P < 0.05). Compared with PQ group, the activities of SOD and GSH-Px in HPQ group significantly increased, the content of MDA declined, the expression levels of MMP-2, MMP-9 and TIMP-1 in lung, liver and kidney tissues declined obviously, the ration between MMP-9 and TIMP-1 significantly declined, but higher than N group, the differences were statistically significant (P < 0.05).
CONCLUSIONThe oxidative stress and MMPs may be involved in the pathogenesis of tissue injuries induced by paraquat. The treatment with HP could obviously reduce oxidative stress and the expression levels of MMP-2, MMP-9 and TIMP-1, enhance the ration between MMP-9 and TIMP-1. So HP treatment could play a role in rescuing the PQ poisoning and protecting the organs function.
