Multidisciplinary treatment of rectal cancer and precise surgical strategy and total mesorectal excision(TME) are the basis for effective treatment of rectal cancer.At present,the best scheme of neoadjuvant therapy for rectal cancer has not been established.The neoadjuvant chemotherapy,neoadjuvant short and long range radiotherapy,radiotherapy adverse reaction,the timing of surgery for rectal cancer are summarized as follows.

4.Effect of dialyzate flow on the dialysis adequacy

Objective To analyze the dialysis adequacy of the maintenance hemodialysis patients under different dialyzate flow.Methods Forty-eight patients under maintenance hemodialysis were divided into four groups according to dialyzate flow:500 ml/min group,600 ml/min group,700 ml/min group and 800 ml/min group,with 12 patients in each group.Each group was treated 6 weeks.The albumin (Alb),hemoglobin(Hb),hematocrit(Hct),blood urea nitrogen (BUN),serum creatinine(SCr) and parathyroid hormone(iPTH) levels before and after treatment were examined,Kt/V and urea reduction ratio(URR) were calculated separately.Results There was no significant difference in Kt/V between 500 ml/min group and 600 ml/min group.Kt/V was no increased when the dialyzate flow rate increased from 500 ml/min to 600 ml/min,that was to say they could not improve the dialysis adequacy.There was statistically significant difference in Kt/V among 500 ml/min group,600 ml/min group,700 ml/min group and 800 ml/min group,and 800 ml/min group on the dialysis adequacy was better.Different dialyzate flow on the impact of the dialysis adequacy was compared in self-control method.Kt/V increased along with the increase of dialyzate flow,and the dialysis adequacy and dialyzate flow showed positive correlation.Conclusion The high dialyzate flow of dialysis treatment can improve Kt/V and has significant effect in enhancing the dialysis adequacy.
5.Preliminary study on the role of endothelin in the pathogenesis of primary biliary cirrhosis

Objective To identify the relationship between the concentration of endothelin and the progression of primary biliary cirrhosis (PBC) by measuring the endothelins (ET) level of PBC patients as well as the PBC patients who were treated withUDCA. In addition, the role of ET on regulating TNF-α and IL-6 secretion of PBMC is explored. Methods The ET level in serum of 24 PBC patients was measured by ELISA. Then PBMCs were stimulated by ET and the concentration of TNF-α, IL-6 levels were measured by ELISA. cDNAs from all RNA extracted from the PBMCs were synthesized, and the expression level of TNF-α was detected by semi-quantitative RT-PCR. Results ET concentration in PBC patietns was significantly higher than healthy controls [(6.0±2.8) vs(2.3±3.4) pg/ml, P<0.05]. The level of ET was not related to other serum biochemical markers.. After UDCA treatment, the level of ET in PBC patients had decreased [(7.5±1.6) vs (6.2±2.7) pg/ml, P=0.084]. After ET-1 stimulation, the secretion of TNF-α by PBMCs in PBC patients increased significantly, and the secretion of 1L-6 decreased, but without statistically significant difference. RT-PCR showed that the expression level of TNF-α mRNA increased significantly after ET-1 stimulation. Conclusion The level of ET in PBC serum is much higher than that of the healthy controls. UDCA treatment can decrease the level of ET in PBC patients. ET can increase the expression of TNF-α mRNA. The increased secretion of TNF-α further activates inflammatory cytokine cascade that plays an important role in the development of PBC.
6.Laparotomy and Cool-tip radiofrequency ablation for large liver tumors: short-term results

Objective To study the results of laparotomy and Cool-tip radiofrequency ablation to treat large liver tumors.Methods Laparotomy and Cool-tip radiofrequency ablation were carried out on 64 patients with large hepatic cancer.To destroy the tumor completely,for tumors of 3.0~4.0 cm in diameter,7 ablations were required; for 4.0~5.0 cm in diameter 15 ablations; for 5.0~6.0 cm in diameter 19 ablations; for 6.0~7.0 cm in diameter 40 ablations.Result The complete necrosis rate of laparotomy and radiofrequency ablation was 93.75% (60/64).The short-term results were good.Conclusions Laparotomy and Cool-tip multipoint overlapping radiofrequency ablation for large liver tumors (tumor diameter>3 cm) could result in a high complete necrosis rate and a low complication rate.It is a good radical treatment for unresectable and large liver cancer.
7.Clinical and Imaging Analysis of Benign Fibrous Histiocytoma of Bone

Objective To improve the clinical and imaging diagnosis of benign fibrous histiocytoma of bone.Methods Imaging findingsin 14 patients with pathologically confirmed benign fibrous histiocytoma of bone were analyzed.X-ray plain film was performed in allpatients,CT scan and MR scan were done in 9 cases and in 5 cases respectively.Results The clinical symptom was mainly pain in the local lesion,soft tissue mass could be felt around the lesion in 2 cases.The single-lesion was 12 cases and multi-lesion was 2 cases.There were 22 lesions in 14 cases.Most lesions localized in long bones,totally were 17 lesions.9 lesions localized in cancellous bone,8 lesions localizedin diaphysis compact substance.X-ray plain film showed centric or eccentric osteolytic destruction,unilocular or multiocular with clearborder or sclerotic border,expansion,pathologic fracture and speckled calcification in some destruction lesions.Soft tissue mass could be seen around the lesion in few cases.CT was better than X-ray plain film in displaying calcification,cyst change,pathologic fracture and soft tissue masses in the destruction portion.MR presented as homogeneous signal on T_1WI and hyper-signal on T_2WI,sometimes was inhomogeneous,speckle-like low-signal and non-signal could be seen in the lesion on T_1WI and T_2WI.Few lesion shows intermixed signal on T_1WI and T_2WI.Conclusion Based on the typical imaging and clinical features,the correct diagnosis of benign fibrous histiocytoma can be made in most cases.

Adrenomedulin is an angiogenesis peptide that was found recently.It plays a critical role as an autocrine/paracrine tumor cell survival factor.Recently,adrenomedulin has been proved that it is overex- pressed in tumor,such as breast cancer.It may have a significant role in tumor progression and differentiation by stimulating mitogenic activities,inhibiting immune response,inhibiting apoptosis and enhancing angiogen- esis.We may realize targeted therapy tumor by intervening adrenomedulin,its receptor,and the signal con- duction pathway.
9.Isolation and Characterization of a Moderately Halophilic Bacterium High-producing Esterase

A moderately halophilic bacterium strain DF-B6 capable of degrading Tween 20 was isolated from solar saltern brine by plate method.Based on the phenotype,physiological and biochemical character-istics,and the phylogenetic analysis of 16S rDNA sequence,strain DF-B6 was identified as Idiomarina.The substrate specificity test showed that the lipolytic enzyme from strain DF-B6 was an esterase,and not a li-pase.The maximum esterase activity was observed in 8% NaCl concentration at 50?C,pH 8.0.The effect of various divalent cations was studied on the activity of esterase from Idiomarina sp.DF-B6 at the concentra-tion of 10 mmol/L,Mg2+,Ca2+ and Mn2+enhanced the esterase activity,while Zn2+,Fe3+ and Cu2+ inhibited its activity.
10.Cloning and Expression Analysis of two Endosperm Promoters in Rice

The cloning of promoter is important for studying the genetic engineering and the regulation of gene expression in plants. Two promoters Os772 and Os359, which are predicted to be highly expressed in the endosperm of rice from the EST database were cloned. After construction of the Os772∶∶GUS and Os359∶∶GUS expression vectors, they were transformed into rice. X-Gluc staining of transgenic plants showed that Os772 and Os359 can promote GUS gene expression in matured endosperm but not in root, stem, leaf and flower. This result indicates Os772 and Os359 are two rice endosperm-specific promoters.
