1.Surgical treatment of the cervical spine fractures combined with ankylosing spondylitis

Objective To study effects and other related problems of surgery for patients with cervical spine fracture and ankylosing spondylitis.Methods Twelve patients with cervical spine fracture and ankylosing spondylitis were treated with surgery from April 1986 to April 2004.All eases were studied retrospectively.They were followed up for an average of 67.5 months and their complete clinical data were kept.The neurological function was evaluated by ASIA (American Spinal Injury Association) impairment scale,and the image analyzing software (Image-Pro Plus 5.1) was used to measure the angles of superior and inferior vertebral bodies of the fusion segment at flexion and extension positions.The difference between flexion and extension angles,?,served as the parameter of interspinal movement.According to the definition of spinal fusion by FDA (Food and Drug Administration),the?≥4?was considered as nonfusion.Other related problems were discussed by descriptive study.Results The average improvement in the nine patients with neurological injury was 1.3 ASIA grades.The injured segments in 10 cases were treated with fusion,of whom nine were fused by internal fixation.The fusion rate was 100%.Three cases were scheduled to have laminoplasty,but two had to receive laminectomy instead because of intraoperative complete fracture at the hinged side.Ten patients were complicated by insufficient function of major organs preop- eratively,and endotracheal intubation was difficult to perform in eight cases.Postoperative complications occurred in three cases,but fortunately healed completely.There were no deaths or fatal complications.Conclusions The neurological function can be improved by surgery for patients with cervical spine fracture and ankylosing spondylitis. Although the cervical spine is instable for most of the patients,fusion with internal fixation is indicated and can be successful.Those who had preoperative systemic diseases are likely to suffer from postoperative complications.The difficult endotracheal intubation is a common intraoperative problem.
2.A enzymatic method of the determination of calcium in serum

Objective A enzymatic method for measurement of calcium in serum by using urea amidolyase was introduction. Methods Double reagent two-point method.TEA buffer(pH 8.0,200 mmol/L)was used as the buffer solution.The first reagent consisted of sodium bicarbonate 26.0 mmol/L,NADPH 0.4 mmol/L,2-oxoglutaric acid 8.0 mmol/L,urea amidolyase 115 U/L and GLDH 43.0 kU/L.The second reagent contained sodium bicarbonate 26.0 mmol/L and ATP 6.2 mmol/L in the buffer solution.Results The Linearity range was 0.5~5.0 mmol/L.The average rate of recovery was 102.7%.The average within-run and between-run CV were 2.13% and 2.69%.Camparison with OCPC(X1),showed that Y1=1.02X1-0.021,r=0.981,and with Arsenazo Ⅲ(X2),Y2=0.99X2+0.03,r=0.978.No interference from as much as 300 μmol/L of bilirubin,4 g/L of hemoglobin,2.5 mmol/L of magnesium,2.0 mmol/L of ammonium ion and 8.0 mmol/L of triglyceride.Conclusion This method was simple,accurate and ripid,suitable for automatic analysis of calcium in serum.
3.PTEN mRNA expression in proliferative LECs of the rabbit

· AIM: To observe the effects of PTEN and the tumor suppressor gene in proliferative LECs of the rabbit.·METHODS:Forty-two white rabbits were randomly divided into test group (36 rabbits) and control group (6rabbits). The transparent lenses of treated rabbits were operated with extra capsular cortex extraction, and the controls were kept untouched. The rabbits were sacririced at 1d, 3d, 1wk, 2wK, 1mo and 2mo after surgery.Immunohistochemistry was applied to detect proliferative cellular nuclei antigen (PCNA) as well as hybridization in situ and reverse-transcriptase polymerase chain reactions (RT-PCR) were applied to detect phosphatase as well as tensin homology deleted on chromosome 10(PTEN) mRNA in lens equartor.·RESULTS:PCNA expression increased significantly to a high level at 1wk, began to reduce 2wk later, and recovered to the normal level 1 or 2mo after the surgery.PTEN mRNA expressed positively in normal rabbit LECs.The relative PTEN mRNA contents reduced greatly at 1dafter operation and remained low level at 3d. It began to increase slightly at 1wk, kept rising at 2wk and regained the normal expression after 1 or 2mo. There was inverse correlation between the PTEN mRNA and PCNA expression.·CONCLUSION: PTEN mRNA expresses positively in normal rabbit LECs plasma. PTEN participates in LECs proliferation and correlates with lens proliferative conditions.
4.A clinical analysis of 20 cases with bone marrow granulomas

Objective To study the clinical feature and original diseases of bone marrow granulomas. Methods A total of 5217 bone marrow biopsies were retrospecitively analyzed in Peking Union Medical College Hospital from January 2001 to December 2007. Results Bone marrow granulomas were present in 20 cases, representing an incidence of 0.38% in the series and an annual incidence of 2. 9 cases per year. Finally, 13 of these cases (65%) were diagnosed with tuberculosis. Hematological neoplasms and viral hepatitis were found in 4 and 2 cases respectively. Acute interstitial nephritis was diagnosed in one case. Conclusions The finding of a granuloma in a bone marrow biopsy is not common and unspecific.Tuberculosis, hematological neoplasms and viral hepatitis are the most common underlying diseases. Bone marrow biopsy is an important method for the diagnosis of disseminated tuberculosis.
5.Screening of binding protein of hepatitis C virus NS4A by phage display from human liver cDNA library

Objective To screen the HCV NS4A binding protein. Methods By using HCV NS4A as a solidified selective molecule, the T7 select human liver cDNA library was biopanned and the positive clones were selected. After screening, the positive plaques was amplified and then cloned into the pGEM-Teasy vector. Two positive plaques were chosen for DNA sequencing. Results The binding protein of HCV NS4A was identified as mitogen-activated protein kinase (MAPK)-activated protein kinase 5 (MAPKAPK5) by BLAST. Conclusion This approach provides a new way for the study of the pathogenic mechanism of HCV infection.

Objective To screen the HBV core promoter binding protein, and to investigate their potential role in the replication of HBV DNA. Methods By using HBV core promoter being used as a selective molecule, the T7select human liver cDNA library was biopanned and the positive clones were selected. Results After phage display screening, the positive plaques was amplified and then cloned into the pGEM-Teasy vector. Six positive plaques were chosen for DNA sequencing. The binding protein of HBV core promoter was identified as caboxypeptidase N(CPN) by BLAST. Conclusion The results suggest that phage display screening of binding protein of HBV core protein provides a new approach to study the replication mechanism of HBV DNA.
7.Expression of soluble human anti-idiotypic single chain Fv antibody to hepatitis C core protein in E. coli

Objective To express soluble human anti-idiotypic single chain Fv to hepatitis C core protein in E.coli. Methods Using phage display technique, the semisynthetic phage library was panned by HCV core monoclonal antibody which was coated in a microtiter plate. After three rounds of biopanning, 53 clones were identified specific to HCV core antibody. The specificity of anti-idiotypic scFv was determined by ELISA. After digested with Sfi/Not, the selected HCV core anti-idiotypic scFv positive clone was subcloned into the vector pCANTAB5E for the expression of E-tagged soluble anti-idiotypic scFv. The E.coli XL1-Blue was transformed and induced by IPTG. The specificity of anti-Id scFv was evaluated with ELISA. Results HCV core anti-Id scFv DNA digestion and sequence data showed that the scFv gene was composed of 774bp. ELISA results demonstrated that the soluble human HCV core anti-idiotypic scFv to HCV core monoclonal antibody had a specific combination character. The molecular weight of expressed HCV core anti-idiotypic scFv was 28kD as shown by SDS-PAGE. Conclusion HCV core anti-Id scFv has been successfully expressed in E.coli.
8.Experimental study of the bone in the injury of alternate current

Objective After the rabbits were electrocuted by 220V AC,we found that the pathological changes in the surface of the bones electrocuted could be one objective and quantitative index for the diagnosis of electrocution in the forensic pathology.Methods The experiment animals were divided into four groups,including: the antemortem electrical injury,postmortem electrical injury,putrefied and the normal.After specimens from each group being taken X ray examination,all specimens were then observed the morphological appearance of the surface of the bones with scanning electron microscopy(SEM),meanwhile,detected the kinds and contents of element in the electrical injury area with energy spectrum.Results①small holes on the surface of electrified area and the detection of contents of the elements in the high-density surrounding the holes could be a index for the diagnosis of AC injury and able to differentiate of the antemortem electrical injury,the postmortem electrical injury and the putrefied bones.②the morphologic changes of small holes and the alteration of the contents of elements such as calcium,phosphorus and carbon could be helpful to estimate the time of electrical injury.Conclusion The morphological appearances and the changes of elements in the surface of the bones electrocuted could be a useful index to analyse the differences among the antemortem electrical injury,postmortem electrical injury,putrefied and the normal bones.
9.Relationship between expression of COX-2 and clinicopathological features in esophageal carcinoma

AIM: To examine COX-2 expression in esophageal carcinoma,and to study relationships between COX-2 expression and clinicopathological features and prognosis of esophageal carcinoma patients.METHODS: 89 paraffin-embedded tissue samples from patients with esophageal carcinoma were collected,its clinicopathological features such as tumour differentiation,depth of invasion,length and site of the tumor,regional lymph node metastases,distant metastasis were recorded.Survival time of 81 cases were also recorded.By SP immunohistochemistry method,the expression of COX-2 in tumor samples was examined.RESULTS: COX-2 expression in esophageal carcinoma was markedly higher than that in nomal esophagus,the expression was higher in less differentiated and deeper invaded cases(P0.05).Cases of esophageal carcinoma with lower COX-2 expression had longer survival time than those with higher COX-2 expression(P
10.Modified pinning combined with external tension band for the treatment of Gartland type III humeral supracondylar fractures in children.

OBJECTIVETo explore therapeutic effects of Modified pinning combined with external tension band for the treatment of Gartland type III humeral supracondylar fractures in children.
METHODSFrom February 2009 to November 2010, 79 children with Gartland type III humeral supracondylar fractures were treated by modified pinning (pinning cross internal and external condyle assisted with lateral condyle) combined with external tension band (crossing needle end and hooking around) through lateral approach of elbow. There were 47 males and 32 females, with an average age of 8.7 years (ranging from 2.5 to 14 years). The time from injury to operation was 2 h to 8 d. The elbow joint function and Carrying angle were observed before and after treatment. The Flynn criteria were used to evaluate therapeutic effects.
RESULTSSixty-one patients were followed up (18 patients' data were lost) for 6-30 months (mean 13.5 months). According to Flynn criteria, 53 patients got excellent result, 7 good and 1 fair.
CONCLUSIONTreatment of child Gartland type III humeral supracondylar fractures with modified pinning combined with external tension band through lateral approach of elbow is believed to be an ideal method, which has advantages of reliable fixation, rapid recovery and less complications.
