1.Diagnosis of vascular cognitive impairment

Vascular eognitive impairment (VCI) is a large class of syndromes from mild cognitive impairment to dementia, which are caused by the cerebrovascular risk factors (eg, hypertension, diabetes and hyperlipemia), obvious (eg, cerebral infarction and intracerebral hemorrhage) or unobvious cerebrovascular diseases (leukoaraiosis and chronic cerebral ischemia). Accurate early diagnosis provides the best treatment opportunity and slows down the progress of the disease. In recent years, the research on VCI has made a great progress. This article reviews VCI from the aspects of the etiology, clinical manifestation, neuropsychology, imaging, neuroelectrophysiology, biomarkers, and diagnostic criteria.
2.Application of magnetic resonance diffusion tensor imaging in the treatment of cognitive impairment of cerebrovascular disease with nimodipine

Objective:To explore the efficacy of nimodipine in treating cognitive impairment of cerebral small vessel disease (CSVD) and the value of magnetic resonance diffusion tensor imaging (DTI) in CSVD.Methods:A total of 80 patients with CVSD and cognitive dysfunction who admitted to Suzhou Municipal Hospital from February 2018 to February 2019 were selected, and the patients were divided into control group (40 patiengts) and observation group (40 patients) by random number table method. The control group received basic treatment and donepezil, and the observation group added nimodipine on the basis of control group. All patients were followed up for 12 months. Before treatment, 6 months after treatment and 12 months after treatment, the patients’Montreal cognitive assessment scale (MoCA) score, daily living ability scale (ADL) score, and Tinetti balance and gait analysis(TGA) were recorded. The levels of total cholesterol (TC), triacylglycerol (TG), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C), homocysteine (Hcy) and high-sensitivity C-reactive protein (hs-CRP) before and after treatment were recorded. The changes of DTI data before and after 12 months′ treatment in the two groups were recorded.Results:The MoCA score, ADL score, and TGA score of the two groups were higher than those before treatment and at 6 months and 12 months after treatment, and those 12 months after treatment were higher than those at 6 months after treatment. There were significant differences ( P<0.05). The MoCA and ADL scores of the observation group were higher than those of the control group at 6 and 12 months after treatment at 6 months: (24.61 ± 2.54) scores vs. (22.21 ± 2.83) scores, (71.53 ± 6.25) scores vs. ( 69.51 ± 6.81) scores; 12 months: (26.39 ± 2.16) scores vs. (23.76 ± 2.64) scores, (78.39 ± 5.76) scores vs. (72.39 ± 6.12) scores, P<0.05. At 6 months and 12 months after treatment, the visual space and execution ability, delayed memory, and orientation scores were higher than those before treatment, and in the observation group 12 months after treatment was higher than those 6 months after treatment: (5.97 ± 1.37) scores vs. (5.36 ± 1.29) scores, P<0.05. The observation group′s visual space and executive ability, abstract thinking, delayed memory, and orientation score were higher than those in the control group at the same period ( P<0.05). The levels of TC, TG, LDL-C, Hcy and hs-CRP of the two groups after treatment for 6 and 12 months were lower than those before treatment, and the levels of these index at 12 months after treatment were lower than those at 6 months after treatment ( P<0.05). The levels of Hcy and hs-CRP in the observation group were lower than those in the control group at 12 months after treatment, and Hcy was lower than that in the control group at 6 months after treatment ( P<0.05). After treatment, the fractional anisotropy (FA) values of the center, frontal lobe, and parietal lobe of both sides of the two groups were higher than those before treatment, and the difference in the observation group before and after treatment was higher than that of the control group ( P<0.05). After treatment, the mean diffusivity (MD) values of both frontal lobe and parietal lobe of the two groups were lower than those before treatment, and the difference between the observation group before and after treatment was higher than that of the control group ( P<0.05). Conclusions:Nimodipine has a good effect on the treatment of CVSD, which can effectively improve the patients′ cognitive impairment and other symptoms. DTI can sensitively sense the white matter integrity and cerebral blood flow perfusion of patients, which has a positive significance for the prevention and treatment of CVSD.
3.Tolerance to Low Temperature of Transgenic Aloe Plants with TaDREB Gene from Wheat

Plant expression cassette for TaDREB from wheat was constructed into plasmid pBIR1.Aloe stems were used as explants for the transformation mediated by Agrobaterium.Infected tissues were selected using G418 to generate transformants.In total,58 resistant plantlets to the antibiotics were obtained from the infected explants.The designed primers according to the selective gene npt II and the target gene TaDREB were used to analyze all of the G418 resistant plantlets.PCR results demonstrated that TaDREB were successful transferred into aloe genomic with the transformation efficiency of 0.5%.The transgenic aloe plants were treated under 4℃ for two weeks and then at-20℃ for 30min.The treatment showed that the leaves of negative plants appeared severe evidence of freeze injury with brown,withered and translucent,while the positive plants appeared good growing condition.The activities of enzymes such as peroxidase(POD)and superoxide dismutase(SOD)of transgenic plants which were stressed for 14 days under low temperature were analyzed.The results indicated that the trend of SOD and POD activities in transgenic plants was down-up-up-up,and that in non-transgenic plants was down-up-down-down.The average value of relative electrical conductivity in the positive plants was 0.456 which was lower than 0.685 in the negative plants.It is supposed that transformation of the kind of gene could improve the resistant ability of aloe to low temperature.
4.Food intolerance in 2962 health check-up receivers

Objective To evaluate food intolerance sensitivity among 2962 health check-up receivers from the First Hospital of China Medical University.Methods Serum samples of 2962 adults were collected from January 2008 to June 2010.Serum allergen-specific IgG antibodies of 14 kinds of food were detected by ELISA.The data were analyzed by using One-way analysis of variance.Results In this study,the rate of food intolerance was 0.4%(mushrooms) to 38.0%(eggs).Higher rates of food intolerance were found in egg,crab,shrimp,milk,and cod fish(Eggs 32.5%,crabs 26.0%,shrimps 19.8%,milk 13.8%,and cod fish 12.0% respectively).However,wheat,pork,and mushroom showed relatively lower rates of food intolerance(Wheat 0.8%,pork 0.4%,and mushrooms1.4% respectively).Positive reaction to food intolerance was even stronger in women(9.4% versus 8.2%).Conclusion It is discovered by study that in general population,the diversity of intolerance among different food is associated with 4 factors: gender; age; feature of food; individual diversity.It is recommended that the status of intolerance to food in the general population be investigated by health management.
6.Selection of words in co-word analysis

After the factors that influence the selection of words were analyzed according to the problems in selec-tion of words for co-word analysis, a hypothesis for how to select the word sets with its verification analysis was pro-posed, the method for selecting words was thus decided according to the word frequency-covered literature area and the number of high frequency words. However, its practical use in large data sets needs to be further improved.
7.Use of Antimicrobials in Operating Departments of Hospital

OBJECTIVE To investigate and analyze the application of prophylactic antibiotics in perioperation period in our hospital.METHODS According to the designed table with unified contents and standards,a retrospective investigation was made in 325 patients who had been discharged from our hospital.RESULTS Of the 325 patients in our study,antimicrobials use in 281 patients was rational,44 patients had irrational antimicrobials use.CONCLUSIONS The results show that it is very important to strengthen and improve the management of the antimicrobials use in a normal and standard way in our hospital.
8.Chinfloxacin hydrochloride inhibits HERG potassium channel at open state.

This study is designed to investigate the effects of chinfloxacin hydrochloride (CFX) on the kinetics of HERG K+ channel. Whole cell patch clamp technique was used to record HERG K+ currents from HEK293 cells transiently transfected with cgi-HERG-GFP plasmids and channel kinetics were assessed in the absence and presence of CFX and moxifloxacin hydrochloride (MOX). Results demonstrated that the open state of HERG K+ channel was inhibited by CFX in a concentration- and time-dependent manner, with an IC50 of 162.1 +/- 14.2 micromol x L(-1), two folds higher than its positive control MOX. But there were no significant effects on channel kinetics. In addition, the inhibitory effect of CFX on HERG was enhanced when cells were subjected to altered extracellular K+ concentration.

9.Effects of shuxuening injection on the levels of serum matrix metalloproteinase-9 and tissue inhibitor of metalloproteinase-1 in acute exacerbated chronic obstructive pulmonary disease patients.

OBJECTIVETo observe the effects of Shuxuening Injection (SI) on the levels of serum matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase (TIMP-1) in acute exacerbated chronic obstructive pulmonary disease (COPD) patients.
METHODSSeventy-nine patients with acute exacerbated COPD were randomly assigned to the treatment group (39 cases) and the control group (40 cases). Routine therapies for COPD were given to patients in the control group, while 15 mL SI was given to those in the treatment group by intravenous dripping, twice daily for total 14 days. The forced expiratory volume in one second (FEV1) and forced vital capacity (FVC) were detected by Spirometer. The FEV1/FVC (%) and the FEV1% were calculated. The levels of serum MMP-9 and TIMP-1 were detected using ELISA before and after treatment, and compared with 20 healthy subjects as the control.
RESULTSThe FEV1, FVC, FEV1/FVC (%), and FEV1% were significantly improved after treatment in the treatment group when compared with before treatment and with the control group (P < 0.05, P < 0.01). When compared with before treatment and with the control group, the levels of serum MMP-9, TIMP-1, and the ratio of MMP-9/TIMP-1 decreased more significantly in the treatment group after treatment (P < 0.01). Correlation analyses showed that obvious negative correlation existed between the levels of serum MMP-9 and TIMP-1 and FEV1/FVC (%) (r = -0.677, -0.629, P < 0.01). Obvious negative correlation existed between the levels of serum MMP-9 and TIMP-1 and FEV1% (r = -0.562, -0.661, P < 0.01). Furthermore, obvious negative correlation also existed between the ratio of MMP-9/ TIMP-1 and FEV1%, as well as FEV1/FVC (%) (r = -0.732, -0.891, P < 0.01).
CONCLUSIONSSI could improve the pulmonary ventilation function of acute exacerbated COPD patients. One of its mechanisms might be correlated with lowering the serum levels of MMP-9 and TIMP-1, and correcting the imbalance of MMP-9/TIMP-1.

10.Research in effect of physic temperature dropping by ice blanket on incidence of pressure ulcer of stroke patients in ICU

Objective To explore effect of physic temperature dropping by ice blanket on incidence of pressure ulcer of stroke patients in ICU, and normalize the basic nursing procedure during temperature reduc-tion.Methods 230 fever patients were randomly divided into the group A(control temperature with ice blan-ket,108 cases)and the group B (control temperature with routine measure,122 cases).Thc incidence of pressure ulcer and surface temperature of pressed position in the two groups were compared.Results The incidence of pressure ulcer and surface temperature of pressed position in group A was lower than those of group B.Conclusions The incidence of pressure ulcer can be reduced by using ice blanket for physic temperature dropping.The pressure of pressed position were descended by alternative prostration and lateral position.
