1.Treatment experience of the acute traumatic intracranial hematoma combined with Herniation

Objective To investigate the key procedures of the acute traumatic intracranial hematoma com-bined with herniation and the prognosis factors. Methods 45 cases of acute traumatic intraeranial hematoma com-bined with herniation from February 1997 to June 2008 were admitted in our hospital. Timely establishment of effec-tive ventilation and circulation and pre-operative examination were done to all the eases. Craniotomy hematoma clean was performed in 8 cases, hematoma clean and decompressive craniectomy was canducted in 33 cases and 4 cases were not operatively treated. Results 26 eases (58%) were cured,and 19 cases (42%) died. Conclusions The key procedures of the acute tranmatie intraeranial hematoma combined with herniation is timely establishment of ef-fective ventilation and circulation, and that is effective method to prevent secondary brain injury ; removing hematoma as soon as possible,and lifting the oppression of the brain stem are the keys to rescue patients. Prognosis is closely related to the degree of primary brain injury, eonseious level before operation and the time of herniation appearance.
2.Case-control study on effects of vacuum drainage on perioperative blood loss after total hip arthroplasty for the treatment of femoral neck fractures.

OBJECTIVETo compare postoperative blood loss under different negative pressures of drainage after total hip arthroplasty for the treatment of femoral neck fractures.
METHODSFrom January 1st to December 30th 2013, 74 patients with femoral neck fractures treated with total hip arthroplasty were randomly divided into two groups: high negative pressure drainage group and low negative pressure drainage group. In high negative pressure drainage group, there were 34 cases including 10 males and 24 females, with a mean age of (75.94 ± 9.02) years old, and the patients were treated with 60 kPa negative pressure of drainage. In the low negative pressure drainage group, there were 40 cases including 13 males and 27 females, with an average age of (74.93 ± 8.90) years old, and the patients were treated with 30 kPa negative pressure of drainage. The amount of total drainage, total blood loss, and hemoglobin change were compared between these two groups.
RESULTSAll the patients got primary healing without infections. In high negative pressure drainage group,the change of hemoglobin was (41.74 ± 15.69) g/L, total blood loss was (1,217.73 ± 459.50) ml and the drainage volume was (312.94 ± 103.44) ml; while in low negative pressure drainage group,the results were (34.90 ± 12.90) g/L, (904.01 ± 381.58) ml and (129.25 ± 44.25) ml separately. All the results in high negative pressure drainage group were higher than those in the other group. Three days after operation, the change of hemoglobin was (46.00 ± 13.29) g/L and total blood loss was (1,304.72 ± 421.75) ml; while in low negative pressure drainage group, the changes of hemoglobin was (43.87 ± 11.39) g/L and total blood loss was (1,196.78 ± 344.20) ml; there were no statistically significant differences between two groups.
CONCLUSIONWhen placing drainage devices after total hip arthroplasty for the treatment of femoral neck fractures, the level of negative pressure should be chosen according to preoperative level of hemoglobin and HCT in patients. For old patients with femoral neck fracture, low negative pressure is more suitable.

3.Study on genetic microarray for detection of katG mutations associated with Mycobacterium tuberculosis resistance to INH

Objective To establish and evaluate a gene microarray for determination katG mutations of Mycobacterium tuberculosis isolates associated with resistance to isoniazid(INH).Methods A panel of probes were designed and gene chips were prepared by dotting.Mycobacterium tuberculosis isolates resistance to 5 drugs was determined by proportional dilution methods.Amplicons of Mycobacterium tuberculosis isolates were detected by our chip and sequenced.Results The drug resistance rate of the isolates to at least one of the anti-tuberculosis drugs was 70.8%(97/137).45 strains out 137 Mycobacterium tuberculosis isolates was resistant to INH(32.8%).katG was successfully amplified from 100% of the susceptible strains and 88.9%(40/45)resistant strains.4 of 45 INH resistant isolates' katG were deleted.27 of 40(67.5%) katG has been detected to have katG 315 codon mutations.The mutations were 315 AAC(Asn,13/40), ACC(Thr,6/40),ACA(Thr,4/40),ATC(Ile,2/40),AGC(Arg,2/40).The mutation rate of katG analyzed by gene chips we prepared were identical to katG sequencing.Conclusion The gene microarray techniques we developed for determination of Mycobacterium tuberculosis resistance to INH are specific, sensitive and may be used as an alternative in clinical laboratory.
4.Estimation of effects of liuwei Dihuang Wan on anti-inflammation in rat by HPLC-UV based metabonomic method.

OBJECTIVETo understand the possibility of estimating the anti-inflammation effect of Liuwei Dihuang Wan (LWW) in rat by having developed HPLC-UV metabonomic technology.
METHODThe hydrophilic or lipophilic constituent group of LWW was extracted by distilled water or acetic ether respectively. The anti-inflammation effects of different LWW dosages and extractions were measured by traditional method respectively. Then, metabonomic analysis was performed.
RESULTThe high dosage of LWW extraction (16.5 g x kg(-1))could inhibit the swell of rat palm significantly, but there are not statistically significant effect of low dosage group. Nevertheless, the metabonomic study showed that LWW extraction could restore obviously the rat HPLC-UV urinary profiling disturbed by inflammation in low dosage, especially the hydrophilic constituent group.
CONCLUSIONOur study indicated that the developed metabonomic technology based on HPLC-UV might be used as a potentially powerful tool for estimating the anti-inflammation effect of LWW with sensitivity and integrity.

5.Expression of human telomerase reverse transcriptase and c-myc transcripts in endometrial carcinoma and its precursors.

OBJECTIVETo study the possible role of hTERT and c-myc in endometrial carcinogenesis.
METHODSThe expression of hTERT and c-myc mRNA was examined by in situ hybridization of endometrial samples from 14 cases with simple hyperplasia, 10 with complex hyperplasia, 8 with atypical hyperplasia and 42 with endometrioid carcinoma.
RESULTSExpression of hTERT was demonstrated in samples with simple hyperplasia, complex hyperplasia, atypical hyperplasia and carcinoma at frequencies of 2/14, 4/8, 8/10 and 39/42 (92.9%), respectively. The prevalence and intensity of the hTERT signal was greater in the carcinomas and lesions with atypical hyperplasia than those with simple or complex hyperplasia (P < 0.05). The expression of c-myc was demonstrated in samples with simple hyperplasia, complex hyperplasia, atypical hyperplasia and carcinoma at frequencies of 3/14, 1/8, 5/10 and 23/42 (54.8%), respectively. The frequency of c-myc expression was higher in carcinomas and hyperplastic lesions with atypia than those in lesions with simple or complex hyperplasia without atypia (P < 0.05). The expression of hTERT was shown to be correlated with the level of differentiation (P < 0.05), while the c-myc expression appeared to be associated with the depth of myometrial invasion (P < 0.05). The expression levels of hTERT and c-myc were not found to be correlated with each other in the tissues examined (P > 0.05).
CONCLUSIONSThe expression of hTERT and c-myc may be involved in the progression from the endometrial aypical hyperplasia to invasive carcinoma. The correlation between hTERT and c-myc in endometrial hyperplasia and carcinoma are not found.

6.Morphology and microleakage study of repairing subpulpal wall perforation with resinous inlay.

OBJECTIVEThe purpose of this study is to study the sealing ability and the furcal appearance of repairing subpulpal wall perforation with resinous inlay.
METHODSFifty newly extracted human molars were randomly divided into three experiment groups (group A, group B, group C, 15 teeth each) and one control group (5 teeth). In experiment groups, perforations were made perpendicularly to the center of the pulp chamber floor. Perforations of group A and B were repaired with resinous inlay and sealed by AH Plus sealer and luting glass-ionomer, respectively. Perforations of group C were directly repaired using light-cure composite resin. Perforations were not made in five teeth of control group. The furcal appearances were evaluated under stereomicroscope after repairing. Microleakage was measured by glucose oxidase detection.
RESULTSThe fineness rate of furcal appearances with resinous inlay repairing were 83.3%, while the fineness rate of furcal appearances with light-cure composite resin directly repairing were 46.7%. There were statistics difference between resinous inlay repairing and light-cure composite resin directly repairing (P<0.05). There were statistics difference among the daily microleakage of three experiment groups, group A CONCLUSIONUsing resinous inlay to repair the subpulpal wall perforation can improve the sealing effect and avoid material overextension. AH Plus can be used as perforation sealant because of its better sealing ability.

7.Stress changes of lateral collateral ligament at different knee flexion with or without displaced movements: a 3-dimensional finite element analysis.

OBJECTIVETo create a 3-dimensional finite element model of knee ligaments and to analyse the stress changes of lateral collateral ligament (LCL) with or without displaced movements at different knee flexion conditions.
METHODSA four-major-ligament contained knee specimen from an adult died of skull injury was prepared for CT scanning with the detectable ligament insertion footprints, locations and orientations precisely marked in advance. The CT scanning images were converted to a 3-dimensional model of the knee with the 3-dimensional reconstruction technique and transformed into finite element model by the software of ANSYS. The model was validated using experimental and numerical results obtained by other scientists. The natural stress changes of LCL at five different knee flexion angles (0 degree, 30 degree, 60 degree, 90 degree, 120 degree) and under various motions of anterior-posterior tibial translation, tibial varus rotation and internal-external tibial rotation were measured.
RESULTSThe maximum stress reached to 87%-113% versus natural stress in varus motion at early 30 degree of knee flexions. The stress values were smaller than the peak value of natural stress at 0 degree (knee full extension) when knee bending was over 60 degree of flexion in anterior-posterior tibial translation and internal-external rotation.
CONCLUSIONLCL is vulnerable to varus motion in almost all knee bending positions and susceptible to anterior-posterior tibial translation or internal-external rotation at early 30 degree of knee flexions.

8.Study on mechanism of down-regulation of SLP-2 gene expression on proliferation and apoptosis of glioma cells

Objective:To investigate the effect of downregulation of SLP-2 gene expression on proliferation and apoptosis of brain glioma cells.Methods:The targeting siRNA sequence of SLP-2 transfected U251 human glioma cells,blank group (without any cell treatment)and negative control group (cell transfected nonsense siRNA sequence)were set,Western blot was used to detect the expression of SLP-2,Bcl-2,Bax,Notch1,Hes1 protein after transfected 48 h;cell proliferation was detected by CCKg;cell apoptosis was detected by flow cytometry.Results:The protein expression of SLP-2 in negative control group had no significant difference with the control group (P>0.05),and the expression of SLP-2 protein in SLP-2 knock down group was significantly lower than control group (P<0.05);cell survival rate,apoptosis rate,mRNA expression of IL-6,TNF-α and Bcl-2,Bax,Notch1,Hes1 protein expression in the negative control group and the blank group had no significant difference (P>0.05),cell survival rate,mRNA expression of IL-6 and TNF-α and the expression of Bcl-2,Notch1,Hes1 protein in SLP-2 knock down group was significantly lower than control group,the apoptosis rate and the expression of Bax protein was significantly higher than control group (P>0.05).Conclusion:Downregulation of SLP-2 gene expression can significantly inhibit the proliferation and induce apoptosis of glioma cells,down regulate the expression of inflammatory cytokines IL-6 and TNF-α,and the mechanism is related to the inhibition of Notch1 signaling pathway.
9.The post-injury regenerating ability of temporomandibular joint of rats exposed to the repeated + Gz forces.

OBJECTIVETo observe the post-injury regenerating ability of temporomandibular joint of rats exposed to the repeated + Gz forces.
METHODSOne hundred and eight male rats were randomly assigned to 9 groups, with 12 rats in each group. Group A was normally fed serving as the blank control group. Group B was only fixed with rat-kept devices for 5 minutes as the fixed control group. Group C was borne +1 Gz for 5 minutes. In the same position as group C, group D, E, F, G, H and I were repeatedly exposed to +10 Gz for 30 seconds each time with 5 times a day and +1 Gz for 1 minutes as a interval. The 6 groups were treated in the condition above for 4 days a week and 4 weeks in total. The rats in group A, B, C and D were killed on the next day after the centrifuge. The rats in group E, F, G, H and I were killed at 1, 2, 4, 8 and 12 weeks after the centrifuge, respectively. After the rats were killed, temporomandibular joint were taken for observation under optical microscope, scanning electron-microscope (SEM) and transmission electron-microscope (TEM).
RESULTSThere was no significant difference observed in group A, B and C. But in group D, E, F, G, H and I pathologic injuries and self-regeneration were seen in TMJ in different degrees.
CONCLUSIONSThe injury of TMJ could regenerate gradually with time.

10.Clinical trial of icotinib combined with thoracoscope minimally invasive in the treatment of non-small cell lung cancer

Objective To investigate the expression and prognosis of serum medium factor (MK),vascular endothelial growth factor-C (VEGF-C) for icotinib combined thoracoscope minimally invasive treatment of non-small cell lung cancer(NSCLC).Methods A total of 94 patients with NSCLC were randomly divided into control group (n =47) and treatment group (n =47).Control group was treated by thoracoscope minimally invasive treatment.Treatment group was orally given icotinib 125 mg,3 times a day,30 d a cycle,and treated for 2 cycles.The recent clinical effect and adverse drug reactions were observed in two groups.To evaluate the quality of life with the scale named quality of life instruments for cancer patients:lung cancer (QLICP-LU).Enzyme linked immunosorbent assay (ELISA) method was used to determinate the serum MK and VEGF-C.Results The disease control rates in treatment group and control group were 76.60% (36/47 cases),48.94% (23/47 cases),with significant difference(P < 0.05).After treatment,the physical function,psychological function,social function,common symptoms and side effects,specific module and total quality of life scores were 32.32 ± 6.24,37.87 ± 6.14,29.44 ±5.29,34.02 ±6.32,35.20 ±6.29,168.85 ±30.28 in treatment group,had significant difference with those in control group,which were 24.11 ± 4.17,32.05 ± 4.68,21.26 ± 3.97,28.47 ± 4.81,31.83 ± 5.61,137.72 ±23.24 (P<0.05).The levels of MK and VEGF-C were (0.42 ±0.13),(0.11 ±0.06) ng · mL-1 in treatment group,had significant difference with those in control group,which were (0.64 ±0.16),(0.16 ±0.08) ng · mL-1 (P < 0.05).The main adverse drug reactions in two groups were diarrhea,nausea and rashes,the incidence rates of adverse reactions in treatment group and control group were 12.77% (6/47 cases),10.64% (5/47 cases),with no significant difference (P > 0.05).Conclusion The therapy of icotinib combined thoracoscope for minimally invasive treatment of NSCLC can improve the clinical effect,improve the quality of life,and reduce MK and VEGF-C levels.
