2.Early diagnosis of ulnar impaction syndrome

Objective To explore early diagnostic methods for ulnar impaction syndrome on the basis of suggested criteria.Methods From December 1998 to December 2004,123 cases complained of ulnar pain.They were checked and diagnosed according to the criteria of Yu-dong Gu,and especially,the results of wrist MRL Forty-eight of them were diagnosed as ulnar impactinn syndrome.A retrospective study was done to analyze the char- acteristies of X-ray and MRI in examining ulnar impaction syndrome,clinical symptoms of the wrist,and the association between Chun & Palmer's scoring systems and imaging manifestations.Results Most of the cases of ulnar impaction syndrome had positive lunar variance (68.8%).Carpal avascular necrosis was found in about 27.1% of the cases through X-ray examination of the wrist,64.7% of whom were lunar osteonecrosis.Abnormal changes in signal intensity occurred in the MRI findings of the syndrome cases.The carpal necrosis was always located at the ulnar side of lunare or (and) at the waist and bottom of triquetrum.There was a close relationship between clinical symptoms and Chun & Palmer's grading systems and carpal imaging,MRI in particular Conclusion Early diagnosis of ulnar impaction syndrome can be made easily on the basis of deep understanding of the syndrome,clinical symptoms,and findings of imaging,especially MRI.

4.Expression of human telomerase reverse transcriptase and c-myc transcripts in endometrial carcinoma and its precursors.

OBJECTIVETo study the possible role of hTERT and c-myc in endometrial carcinogenesis.
METHODSThe expression of hTERT and c-myc mRNA was examined by in situ hybridization of endometrial samples from 14 cases with simple hyperplasia, 10 with complex hyperplasia, 8 with atypical hyperplasia and 42 with endometrioid carcinoma.
RESULTSExpression of hTERT was demonstrated in samples with simple hyperplasia, complex hyperplasia, atypical hyperplasia and carcinoma at frequencies of 2/14, 4/8, 8/10 and 39/42 (92.9%), respectively. The prevalence and intensity of the hTERT signal was greater in the carcinomas and lesions with atypical hyperplasia than those with simple or complex hyperplasia (P < 0.05). The expression of c-myc was demonstrated in samples with simple hyperplasia, complex hyperplasia, atypical hyperplasia and carcinoma at frequencies of 3/14, 1/8, 5/10 and 23/42 (54.8%), respectively. The frequency of c-myc expression was higher in carcinomas and hyperplastic lesions with atypia than those in lesions with simple or complex hyperplasia without atypia (P < 0.05). The expression of hTERT was shown to be correlated with the level of differentiation (P < 0.05), while the c-myc expression appeared to be associated with the depth of myometrial invasion (P < 0.05). The expression levels of hTERT and c-myc were not found to be correlated with each other in the tissues examined (P > 0.05).
CONCLUSIONSThe expression of hTERT and c-myc may be involved in the progression from the endometrial aypical hyperplasia to invasive carcinoma. The correlation between hTERT and c-myc in endometrial hyperplasia and carcinoma are not found.

5.Elective single blastocyst transfer is more suitable for normal responders than for high responders.

BACKGROUNDEmbryo quality and receptivity of the endometrium are two factors that determine the results of in vitro fertilization/intra-cytoplasmic sperm injection-embryo transfer (IVF/ICSI-ET). There is no consensus of the optimal transfer strategy for normal responders or high responders. The current study aimed to find the optimal transfer strategy for different subgroups of patients.
METHODSFrom April 2010 to December 2010, patients who meet the following criteria were included in this study; primary infertility, female age ≤ 35 years, FSH level on female cycle day 2 - 3 ≤ 12 mIU/ml, at least six good quality embryos available on day three. The clinical outcomes using different transfer strategies between normal responders and high responders were reviewed and compared.
RESULTSFor the normal responders, the clinical pregnancy rate of day three double-embryo transfer (DET) was comparable to that of day five elective single blastocyst transfer (eSBT), 64.04% vs. 60.33% (P > 0.05). For the high responders, the clinical pregnancy rate of day five eSBT was significantly lower than that of day three DET, 43.35% vs. 57.21% (P < 0.05). For the high responders, the rates of clinical pregnancy and implantation in frozen-thawed embryo transfer (FET) cycles were notably higher than in eSBT cycles (64.56% vs. 43.35% and 62.11% vs. 43.35% respectively) (P < 0.05).
CONCLUSIONSFor normal responders, eSBT might be an applicable strategy to reduce multiple pregnancy rates while maintaining acceptable overall pregnancy rates. And in order to reduce multiple pregnancies and increase the chance of pregnancy of high responders, FET may be a preferable strategy.

6.Surgical treatment and prognosis of gastrointestinal stromal tumor.

OBJECTIVETo investigate the outcome of surgical treatment for gastrointestinal stromal tumor(GIST) and the associated factors.
METHODSA total of 277 patients with GIST underwent primary surgical treatment from January 1990 to February 2010 at the Cancer Center of Sun Yat-sen University. The clinical data were retrospectively reviewed and the pathological examination was reviewed. Follow-up was performed.
RESULTSThere were 176 males and 101 females. The age ranged from 20 to 81 years old (median,57). Location of the tumor included colorectum (n=28),small bowel(n=76), stomach(n=173). All the patients had en bloc resection, including local excision in 98 patients, organ resection in 64, and extended resection in 115. The 5-year survival rates were 83.5%, 71.9%, and 61.9% in the three different procedures, respectively, and the difference was not statistically significant(P>0.05). Cox model showed that the tumor size, recurrence and metastasis were independent risk factors associated with the prognosis in GIST patients(P<0.05).
CONCLUSIONSSurgery remains the major approach for gastrointestinal GIST. Complete resection is the principal treatment. Extensive resection or extended lymph nodes dissection is not associated with improved survival.

7.Full-length cDNA cloning of flavonol synthase genes of Carthamus tinctorius and construction plant expression vector.

Flavonol synthase (FLS) is one of the key enzymes in flavonoids metabolic pathways. In this study, middle sequence was obtained from Carthamus tinctorius transcriptome sequencing results. Full-length cDNAs of FLS was cloned from petals of C. tinctorius to FLS by using RT-PCR and RACE technology. Its full-length cDNA was 1,201 bp, with an open reading frame of 1,101 bp and 336 encoded amino acids. The phylogenetic analysis showed that, FLS gene encoded amino acids in C. tinctorius were highly homologous with amino acids in congeneric Compositae species, especially Rudbeckia laciniata. The pBASTA-FLS plant expression vector was successfully built by the molecular biology method, which lays a foundation for further studying biology functions of the gene and biosynthesis mechanism of flavonoids.

8.Influence regarding micro-ecological environment of Snail habitats in Lake area on the distribution of Snails

Objective To explore the relationships between micro-ecological environmental factors and the density of Snails so as to provide information for the elimination of Snails and control of Schistosomiasis disease,under ecological methods.Methods A bottomland close to Junshan Park in Yueyang city,Hunan province was selected as the field for survey during 10,2007-10,2008,and a systematic sampling method was applied to determine the specific sites of Snail investigation.All the Snails in each frames were collected and the soil surface temperature and vegetation coverage in several frames were measured.30 g soil sample in each selected frames were also collected simultaneously.The number of live Snails in each frame was counted by dissection,and soil measured pH value and soil moisture were tested in the laboratory.The distribution of Snails and microecological environmental factors,fitted general additive model for the relationship of these factors and the Snail density were described.Results 104 frames were surveyed,with pH value as between 4.70-7.92,vegetation coverage as in 1% to 96%,soil surface temperature as in 14.5-32.7℃,the soil moisture as in 0.07-2.00.Under General additive model,data showed that there was no significant difference for vegetation coverage.However,other factors were all significantly different(P<0.001).It was found that a nonlinear relationship was existing between these factors and the Snail density.Conclusion Smoothing function relationship was noticed between the Snail density and micro-ecological environmental factors.It's suggested to fit general additive model to study the relationship between the distribution of Snails and its influencing factors,so as to adopt appropriate measures to change the related ecology to control the diffusion and reproduction of Snails.
9.Relationship between maximum body mass index and type 2 diabetes mellitus among adults in Heilongjiang province

Objective To determine the prevalence and distribution of type 2 diabetes mellitus (T2DM) and the relationship between maximum body mass index (MAXBMI) and T2DM. Methods From June to August, 2005, a stratified cluster sampling of 1071 permanent residents in communities, over 20 years old, from 4 districts and 1 county of Mudanjiang was chosen. The prevalence of T2DM, and the association between T2DM and different levels of the MAXBMI, current BMI were studied. Results The prevalence in the communities was 7.09% and in those with past maximum BMI≥28 kg/m~2, it was 12.10%. With the increase of past MAXBMI levels, the risk of T2DM patients also increased significantly(trend X~2=17.387 23, P<0.0001). Data from multifactor analysis showed that MAXBMI in the past was positively related to T2DM (OR=3.06, P=0.0013). In T2DM patients, the group with MAXBMI≥27.4 kg/m~2 had higher 2-hour postprandial blood glucose than those with lower MAXBMI (P=0.0408). When compared with low maximum BMI group in normal blood glucose population, the group with higher MAXBMI (≥ 25.4 kg/m~2) had higher blood glucose and greater change of BMI. Conclusion In both groups that patients with T2DM and with normal glucose, in order to control blood glucose better, researchers should not only concern about the influence of the MAXBMI in the past, but also pay attention to constantly keep BMI at the normal range.
10.Analysis of risk factors affecting operative outcome of small bowel obstruction.

OBJECTIVETo evaluate the risk factors affecting the early postoperative outcomes in patients with small bowel obstruction.
METHODSClinical data of 193 patients with small bowel obstruction undergone operation were analyzed retrospectively. A range of factors were investigated to estimate postoperative outcome, including gender, age, comorbidities, etiology of obstruction, presence of strangulated bowel (viable or nonviable), leukocyte count, temperature, and heart rate. Logistic regression analysis was used to study the prognostic value of each significant variable in terms of postoperation.
RESULTSThe major causes of small bowel obstruction were adhesion and hernia, contributing 38.9% and 37.8% of all cases, respectively. Strangulation occurred in 42.0% and caused nonviable bowel in 23.3% of obstructing episodes. Elderly (>or=70 years), diabetes, malignant tumors WBC >15x10(9)/L were independent significant factors associated with bowel strangulation. The overall complication rate was 16.1%, the 30-day mortality was 4.1%, and the median postoperative hospital stay was 13 days. Age >or=70 years and bowel resection were significantly associated with postoperative complications in the univariate analysis. Only elderly and malignant obstruction were significantly associated with operative mortality in multivariate logistic regression.
CONCLUSIONSSurgery for small bowel obstruction is still associated with significant mortality and morbidity. Elderly is significantly associated with an increased incidence of strangulation, operative mortality, and complications.
