2.Therapeutic effect ofHuoxue-Buqi-Qutan decoction for vertigo due to vertebrobasilar insufficiency

Objective To investigate the therapeutic effect ofHuoxue-Buqi-Qutan decoction for vertigo due to vertebrobasilar insufficiency.Methods A total of 120 patients with vertigo due to vertebrobasilar insufficiency (VBI) were randomly divided into a treatment group and a control group, with 60 patients in each group. The treatment group was treated withHuoxue-Buqi-Qutandecoction and the control group was treated with nimodipine injection. The clinical efficacy, the change of strength before and after treatment were observed; the mean blood flow velocities of the vertebrobasilar arteries were detected with transcranial Doppler.Results After the treatment, the total effective rate was 91.7% (55/60) and 80% (48/60) in the treatment group and the control group, respectively, with significant difference (χ2=12.234,P=0.001). The mean blood flow velocities of the basilar artery, left and right basilar arteries were significantly increased in both groups (tvalues were 2.909, 4.558, 5.424, 9.831 in the treatment group, and 8.388, 11.191 in the control group, allP<0.01), the improvements of the mean blood flow velocities in the treatment group were significant better than those in the control group (tvalues were 7.385, 6.940, 3.890, allP<0.01). The vertigo grade was improved in both groups after the treatment than those before the treatment, while the proportion of grade 0 and grade 3 in the treatment group was significantly better than the control group (χ2 values were 8.298, 4.5472,P=0.004, 0.033).ConclusionHuoxue-Buqi-Qutan decoction can obviously improve the vertebrobasilar blood supply and the vertigo grade in patients with vertigo due to VBI, and then improve the clinical efficacy.

Proinflammatory cytokines induced by peripheral immune challenge can activate the inflammatory reflex, which results in negative-feedback control of inflammation .Some recent studies revealed that it is the splanchnic nerve , not the vagus nerve , to constitute the efferent arm of the inflammatory reflex .Further researches are needed to iden-tify the neural construction and regulatory mechanism of the inflammatory reflex , which might be harnessed for the treatment of inflammation and the development of anti-inflammatory drugs .

6. Effect of Coptidis Rhizoma processed with ginger juice on its property based on metabonomics

Objective: To explore the difference of hot and cold property of Coptidis Rhizoma (CR) before and after being processed with ginger juice. Methods: To feed rats for 29 d with the decoction of un-processed CR (UPCR) and ginger juice-processed CR (GPCR), the rats' blood was collected at different administration time, the data by HPLC-MS/MS analysis were processed with PCA to determine the metabolites difference between the treatment groups. Results: On the day 29, the metabolic status of endogenous substances in rats' blood showed the biggest difference between the UPCR and GPCR groups. Through checking the endogenous biomarkers of the rats in each group, the content of markers related to amino acid energy metabolism of rats in the GPCR group was higher than that in the UPCR group, indicating the energy metabolism of the GPCR was stronger than that of the UPCR, which showed the differences in cold and hot property between the two drugs. Conclusion: The results show that the differences in potency of the UPCR and GPCR can be reflected by rats' different biological effects. The results are consistent with the traditional processing theory in Chinese materia medica that the cold property of CR could be moderated to be colder by processing with assistant materials with hot property (ginger juice). The work indicates that the metabonomic method is a valuable tool in the research of processing effect on properties of Chinese medicinal substances. The results are helpful to clarify the mechanism of processing CR with ginger juice in the future.

8.Clinic study to evaluate left ventricular chamber stiffness by catherization and simultaneous echocardiography

Objective To establish the method of depict ing the left ventricular (LV) pressure-volume loop automatically by computer which incorporates LV catherization simultaneous with LV acoustic quantification echocardiography and to assess echocardiography non-invasive evaluation LV chamber stiffness. Methods Thirteen patients with hypertrophic cardiomyopathy (HCM) underwent LV catherization simultaneously with echocardiography. LV pressure and volume curves were sent to computer, and LV pressure-volume loop was depicted automatically and modulus of LV chamber stiffness (Kc) was obtained. Pulsed Doppler echocardiography of mitral intraventricular flows was obtained in patients with HCM. Results Patients with HCM had significantly higher Kc obtained from LV pressure-volume loop ( 0.43 ? 0.11 vs 0.27 ). R-E 3/R-E 0, (R-E 2)-(R-E 1), (R-A 3)-(R-A 2) were correlated to Kc(r= 0.61 , 0.57 , 0.58 ,respectively). Conclusions This method provides a simple and reliable technique for automatically tracing pressure-volume loop and should facilitate further investigation of the left ventricular diastolic function in clinical practice. R-E 3/ R-E 0, (R-E 2)-(R-E 1), (R-A 3)-(R-A 2) offer new non-invasive indices in evaluating LV chamber stiffness.

Objective To investigate the independent factors associated with the apex of left atrial appendage(LAA) spontaneous echo contrast (SEC) in patients with valvular atrial fibrillation (VAF). Methods Seventeen normal subjects and 21 patients with VAF were included. Plasma fibrinogen (Fg),vov Willebrand factor (vWF),D-dimer (DD),P-selectin,thrombin-antithrombin complexes (TAT) and plasminogen activator inhibitor-1(PAI-1) were measured. 10.Clinical Observation of Ambroxol Hydrochloride in the Adjuvant Treatment of Elderly Patients with Chronic Obstructive Pulmonary Disease

OBJECTIVE:To observe the clinical efficacy and safety of ambroxol hydrochloride in the adjuvant treatment of el-derly patients with chronic obstructive pulmonary disease. METHODS:94 elderly patients with chronic obstructive pulmonary dis-ease were randomly divided into control group(48 cases)and observation group(46 cases). Control group was given oxygen inha-lation,antispasmodic asthma,expectorant and antitussive,antibiotics against infections,bronchodilation drugs,glucocorticoids and other conventional treatment;observation group was additionally given 30 mg Ambroxol hydrochloride injection,adding into 200 ml 5% glucose injection by intravenous infusion,twice a day. The treatment course for both groups was 2 weeks. The clinical efficacy,improvement of cough and sputum volume before and after treatment in 2 groups were observed,and incidence of ad-verse reactions was recorded. RESULTS:The clinical efficacy in observation group was significantly higher than control group,re-ducing degree of cough and sputum volume were significantly superior to control group,the differences were statistically significant (P<0.05);and there were no significant differences in the incidence of total adverse reactions,nausea,stomach heartburn,rash and indigestion between 2 groups(P>0.05). CONCLUSIONS:Based on the conventional treatment,ambroxol hydrochloride can effectively relieve symptoms and improve efficacy in the adjuvant treatment of elderly patients with chronic obstructive pulmonary disease,with no obvious adverse reactions and reliable safety.
