Chinese Journal of Medical Instrumentation 2014;38(2):145-148
Based on the analysis of the present status of medical devices management, this paper optimized management process, developed a medical devices management system with Web technologies. With information technology to dynamic master the use of state of the entire life-cycle of medical devices. Through the closed-loop management with pre-event budget, mid-event control and after-event analysis, improved the delicacy management level of medical devices, optimized asset allocation, promoted positive operation of devices.

3.Immune response and chronic obstructive pulmonary disease.
Chinese Journal of Practical Internal Medicine 2001;0(04):-
Chronic obstructive pulmonary disease(COPD) is a chronic inflammatory disease characterized by infiltration of T-lymphocytes,which plays a key role in the immune response of human.Exploration on the characters of immune response in the pathologic process related to COPD may be useful to explore the mechenism of immunity response in COPD patients, thus, contrubute to improve the knowledge of COPD and investigating new avenues of treatment.

Objective To explore the feasibility of endoscopic thyroidectomy without the use of ultrasonic scalpel. Methods Monopolar high frequency electrosurgical unit was used to complete endoscopic thyroidectomy in 6 cases of thyroid benign tumors. Results All the operations were completed successfully. The operation time was 80~200 min(mean,110 min) and the intraoperative blood loss was 25~50 ml(mean,36 ml).The patients got out-of-bed activities and took liquid diets at a mean of 24 hours postoperatively. No complications was found. The drainage tube was removed on 2 days after operation and the patients were discharged from hospital at 3~5 days. Conclusions Use of high frequency electrosurgical unit for endoscopic thyroidectomy is safe and feasible.

With the development of neuroimaging techniques and the extensive application in clinical practice, the case reports about migrainous cerebral infarction have increased progres- sively, and have received extensive attention. However, its exact pathogenesis still remains unclear. In recent years, with the application of functional neuroimaging techniques, people have a new insight into the pathophysiological mechanisms of migrainous cerebral infarction,This article reviews the advanced in research on the pathogenesis of migrainous cerebral infarction.

Objective To explore the clinical effect of two-points approach vascular stenting technique (via both the femoral artery and posterior tibial artery of the affected side) in repairing blunt popliteal artery injuries. Methods The clinical data of eleven patients with blunt popliteal arterial injuries, who were treated with two-points approach vascular stenting technique during the period from March 2011 to March 2015 as single-approach stenting repair failed, were retrospectively analyzed.First, antegrade puncture of the femoral artery on the affected side with subsequent angiography was performed to evaluate the arterial injury, when the attempt to push the guide-wire to pass the injured arterial segment by twisting manipulation failed the puncture of posterior tibial artery was immediately carried out, then,a 4-French single-bend catheter together with a goose neck snare was inserted.Guide-wire capture technique was used to gasp the guide-wire in the injured artery segment, and then the guide-wire was pulled to pass through the injured artery segment. In this way,the guide-wire working circle was reliably established, and along the guide-wire the vascular stent was inserted and delivered to the right site to repair the injured artery.A 6-French guiding catheter was inserted to perform thrombus aspiration for eliminating the distal outflow obstruction caused by thrombus.Thrombolytic therapy with small dose of urokinase through a indwelling catheter was employed for the treatment of peripheral arterial thrombosis. Results The technical success rate was 100%.Angiography revealed that partial transection of artery was detected in 7 patients, and complete transection of artery in 4 patients.A total of 16 stents were implanted. No perioperative mortality or procedure-related complication occurred. All patients were followed up for 13-24 months, with a mean of (18.7±3.8) months.In-stent thrombosis occurred in 2 patients, which was treated with thrombolytic therapy and the blood flow recovered;severe in-stent stenosis (>70%) was seen in 3 patients,in whom the blood flow returned to normal after balloon dilation therapy;mild in-stent stenosis (<30%) was found in 6 patients, and no special treatment was given.During the follow-up period,no stent fracture, deformation,or displacement was seen, and no patient needed to have an amputation surgery. Conclusion In treating blunt popliteal artery injuries,two-points approach vascular stenting technique can effectively reduce the operation time, rapidly recover the blood flow of the injured popliteal artery, and shorten the time of limb ischemia. This technique has very satisfactory clinical shortterm effect.

BACKGROUND: Various artifacts are found to be appeared in the nuclear magnetic resonance; the frequently appearing ones are the motion artifacts, plait artifacts, metallic artifacts, partial volume effect artifacts, chemical shift artifacts, phase artifacts, zipper artifacts and so on. However, there is still has no report on the controlling and revising method of the zipper artifacts which is produced in the spin-echo sequence imaging process. OBJECTIVE: Based on the discussion of the mechanism of the production of zipper artifact in the spin-echo sequence imaging process, a new impulse sequence is worked out to avoid the zipper artifact. METHODS: Standard SE sequence was complied to obtain the standard image, and compiling the artifact sequence to obtain the linear disturbance image and zipper artifact image, compiling the modified sequence to obtain the modified zipper artifact image. RESULTS AND CONCLUSION: Usually, the linear disturbance image could obtained in the instantaneous gradient of 5 degree with the echo sequence of 120 degree, and the zipper artifact will be obtained in the instantaneous gradient of 90 degree with the echo sequence of 120 degree. Modified sequence instantaneous gradient of 90 degree with the echo sequence of 120 degree can completely eliminate zipper artifact. The interesting result obtained in this paper is that the zipper artifact can be totally avoided with this new impulse sequence.

It is often neglected that hypercapnia may occur during upper epidural anesthesia. To evaluate the necessity and accuracy of P_(ET)CO_2 monitoring through a nasotracheal oropharyngeal cathter during upper epidural anesthesia, 16 patients undergoing radical excision of mammary carcinoma, were investigated for the changes of PaCO_2 and PET CO_2, and the correlation between them. The results showed that the increases of PaCO_2 during the anesthesia were significant (P
