[ABSTRACT]OBJECTIVETo evaluate the efficacy of combined oral and intratympanic glucocorticoids treatment for severe to profound sudden sensorineural hearing loss(SSNHL).METHODSFifty-four patients with severe to profound SSNHL were retrospectively studied. They were treated with oral steroids, vasodilators, anticoagulants. The patients who accepted intratympanic methylprednisolone sodium succinate were included the therapy group, the others were included in the control group. Hearing improvements were analyzed respectively after two weeks.RESULTSHearing in both groups were improved significantly after treatment(P<0.01), but there was no difference in hearing improvements between two groups(P=0.194). The efficacy for severe SSNHL patients in therapy group was equal to those who were in control group (P=0.251), so was the efficacy for profound SSNHL patients in both groups(P=0.380).CONCLUSIONThe combined systemic and intratympanic glucocorticoids treatment was effective for patients with severe to profound SSNHL, but while with no better efficacy than oral glucocorticoid alone.

Objective Random allocation concealment is important in ensuring high-guality randomized controlled clinical trial.In this paper , we aimed to design a more convenient and effective two couplet random allocation method through improving the random allocation concealment technology which uses the envelope method . Methods According to the randomized sequence , a two-couplet random allocation should be prepared for every subject and bound into a book in order , which named random assignment book with cover and instructions .The first couplet is to collect the enroll information and the second couplet is to show the allocation informa -tion.The serial numbers of the two couplets which around sealant are the same , and leave blank at the same areas of the two couplets for signing the enter information .The content of the first couplet can be completely copied to the second couplet .In order to avoid expo-sing the allocation information in advance , the back of the second couplet should be black-printed. When the subjects are sure qualified, the researchers selected the corresponding two couplet in a sequential order .Exposing the allocation infromation in the second couplet, and the subjects will be allocated to the group designated on the second couplet .This random allocation book should be entrusted to professional printing mechanism ,printing according to the random assignment sequence table and in the strict quality control .For the multicenter clinical trials , it can be printed in parts . Results The method has been implemented in nearly ten randomized controlled trials and obtained the consistent high praise . Conclusion The random allocation book method is easy to produce , simple operation and convenient in monitoring , which can effectively achieve the random allocation concealment in a clinical trial as well as having a sig -nificantly practical value in ensuring the quality of randomization .

OBJECTIVE:To observe the efficacy and safety of domestic cefepime in the treatment of patients with acute exacerbation of chronic obstructive pulmonary diseases (AECOPD) complicated with respiratory failure. METHODS:A total of ninety-six patients with AECOPD complicated with respiratory failure were randomly assigned to receive cefepime 2.0 g bid iv gtt (Group A,n=48) or cefoperazone/sulbactam (Group B,n=48) at a dose of 2.0 g bid iv gtt for 7～14 day. RESULTS:The effective rate was 82% for Group A versus 62% for Group B(P0.05). CONCLUSION:The efficacy of cefepime is higher than that of cefoperazone/sulbactam in the treatment of patients with AECOPD complicated with respiratory failure,but both had good safety.

OBJECTIVE:To evaluate the cough-relieving and sputum-eliminating efficacy and safety of Guaifenesin Pseudoephedrine Hydrochloride-Codeine Phosphate Oral Solution.METHODS:By a randomized control study,a total of 60 patients characterized by nonproductive cough andlittle sputum were randomly assigned to receive Guaifenesin Pseudoephedrine Hydrochloride-Codeine Phosphate Oral Solution(Trial Group)or Ambroxol Hydrochloride Oral Solution(Control Group)l0 mL tid for 5～7 d.RESULTS:Both groups had a significant improvement in symptoms including cough and expectoration after treatment(P0.05).CONCLUSION:Guaifenesin Pseudoephedrine Hydrochloride-Codeine Phosphate Oral Solution is more effective in relieving cough than Ambroxol Oral Solution,and it has certain effect in eliminating phlegm.In addition,it is safe and has little adverse reaction.

AIM: To investigate the effect of PMA(phorbol 12-myristate 13-acetate) on the expression of CD18 and the mechanism. METHODS: The technique of quantitative RT-PCR analysis was used to measure the expression of CD18 mRNA in U937 cells treated by PMA. RESULTS: PMA could significantly induce CD18 mRNA expression in a dose and time-dependent manner. The induction effects of PMA on CD18 mRNA could be inhibited obviously by Myr (2 ?mol/L), a specific inhibitor of PKC, and APDC, an inhibitor of NF-?B, but not be inhibited by curcumin, a inhibitor of transcriptional factor AP-1. CONCLUSION: PMA enhanced the expression of CD18 via the pathway of PKC. Transcriptional factor NF-?B, but not AP-1, was essential for the gene transcription of CD18 in U937 cells treated by PMA.

Objective To investigate the change of HepG2 liver tumor perfusion after microbubble enhanced ultrasound cavitation treatment and observe the related pathological injury.Methods Twenty eight Balb/c(nu/nu) nude mice transplanted subcutaneous HepG2 tumor were divided into three groups randomly,including the microbubble enhanced ultrasound cavitation group,the ultrasound group and the sham group.Microbubble enhanced ultrasound cavitation treatment was performed by 0.1 ml microbubbles intravenous injection combined with pulse ultrasound emission in experimental group,while in control groups only ultrasound exposure or microbubble injection were applied.The perfusion of tumors was imaged using contrast-enhanced ultrasonography before and after treatments.Time-intensity curve and peak intensity were analyzed.The tumors were then harvested for histological examination.Results The perfusion of HepG2 tumors almost vanished immediately after treatment in experimental group,with the peak intensity reduced from (26.9 ± 10.9)％ to(8.2 ± 5.8)％ (P ＜0.05).There was no significant changes before and after treatments (P ＞ 0.05) in the two control groups.Histological findings were disruption of the endothelia,significant hemorrhage and increased intercellular fluid.Conclusions Microbubble-enhanced ultrasound cavitation can significantly reduce tumor blood perfusion and disrupt tumor vascularture.This new ultrasound therapy can potentially become a new physical anti-angiogenetic therapy for liver tumor.

Objective To systematically review the effectiveness and safety of percutaneous transhepatic biliary drainage (PTBD) versus endoscopic biliary drainage (EBD) for preoperative biliary drainage in patients with Klatskin Tumors.Methods The Pubmed,Embase,Web of Science,CNKI,VIP and WanFang Data from January 1998 to December 2017 were searched for published studies which compared endoscopic biliary drainage (EBD) with percutaneous transhepatic biliary drainage (PTBD) for preoperative biliary drainage.A Meta-analysis was then performed using the Revman 5.3 software.Results Seven cohort studies were included.There were 366 patients in the PTBD group,and 400 patients in the EBD groups.When compared with EBD,PTBD was associated with a lower risk of cholangitis (OR=0.31,95％ CI 0.20～0.48,P＜0.05),a lower risk of pancreatitis (OR=0.11,95％ CI 0.04 ～ 0.34,P＜0.05),and a lower risk of overall complications (OR=0.48,95％ CI 0.30 ～ 0.77,P=0.002).The rate of conversion from one procedure to the other was significantly lower in the PTBD group than the EBD group.The initial technical success rate and postoperative morbidity and mortality rates were similar in the 2 groups.Conclusion In patients with Klatskin tumors who require PBD,PTBD is a better initial biliary drainage method with lower incidences of procedure-related cholangitis,pancreatitis overall complications and conversion to other drainage procedures.
