1.Determination of Residual Chloramphenicol in Wuji Baifeng Pills by HPLC-MS/MS with Normal Solid-phase Extraction

Objective:To establish a method for the determination of chloramphenicol residue in Wuji Baifeng pills by liquid chro -matography tandem mass spectrometry with normal solid-phase extraction .Methods:A normal solid-phase extraction column was used for the sample pretreatment and enrichment , and high performance liquid chromatography tandem mass spectrometry was used to deter-mine chloramphenicol residue in Wuji Baifeng pills .The analysis was performed on an Agilent-ODS C18 (250 mm ×4.6 mm,5 μm) column .The mobile phase was composed of methanol and water with gradient elution at 40℃ (0-7 min, 30% methanol;7-15 min, 30%-80%methanol;15-25 min, 80%-30%methanol).The flow rate was 1.0 ml· min-1 , and the quantitation ion was m/z 152.1 (the negative ionization) under the mode of multiple reaction monitoring .Results:The limit of detection was 0.016 ng.The calibra-tion curves were linear within the range of 0.187-3.749 ng.The method recovery was 94.3% with RSD of 3.19%(n=9).Conclu-sion:The method is simple with accurate results .It is suitable for the determination of chloramphenicol residue .
2.Risk factors analysis of deep vein thrombosis in patients with hip fracture

Objective To analyze the characteristics and risk factors of preoperative deep vein thrombosis in patients with hip fracture,and provide a theoretical basis for clinical treatment and intervention.Methods 470 patients with hip fracture were chosen as the objects of study.The clinical data of the patients were retrospectively analyzed,including age,gender,injury,combined with other systemic diseases,fracture type (whether old fractures),two D-dimer level.All the patients with deep venous thrombosis of lower extremity were divided into DVT group and nonDVT group.There were 83 cases in DVT group,387 cases in non-DVT group.The factors like aboves as independent variables,presence of deep vein thrombosis as dependent variable,,preoperative deep venous thrombosis of the characteristics and risk factors were analyzed.Results 83 patients had DVT,the incidence rate was 17.7%.There were significant difference in age (40/83 vs 268/387),combined with other system diseases (57/83 vs 283/387),prevention measures (58/83 vs 196/387),D-thrombosis two dimer level (16/83 vs 122/387) of the two groups (x2 =13.712,14.836,9.876,5.313,all P < 0.05).3 age ≥ 40 years,with other system diseases and injury,did not take to prevent the formation of thrombus measures were independent risk factors influencing the occurrence of DVT.Conclusion Age ≥40 years,combined with other system diseases and injury,don't take to prevent the formation of thrombus measures are independent risk factors influencing the occurrence of DVT.
3.Efficacy observation of percutaneous vertebroplasty combined with zoledronic acid in treatment of spinal metastatic carcinoma

Objective To observe the efficacy of percutaneous vertebroplasty (PVP) combined with zoledronic acid in treatment of spinal metastatic carcinoma.Methods Fifty cases of spinal metastatic carcinoma were divided into control group and treatment group by random digits table method with 25 cases each.The control group was given zoledronic acid 4 mg,intervals of 28 d intravenous drip 1 time.The treatment group was given PVP besides zoledronic acid.The visual analog scale score,activity ability score and analgesic usage score after treatment for 3 months were observed and compared.The quality of life was evaluated and the occurrence of relapse of fracture was also recorded.Results After treatment for 3 months,the visual analog scale score,activity ability score and analgesic usage score were (2.00 ± 0.17),(2.12 ± 1.38),(1.47 ± 0.10) scores in treatment group,and (7.84 ± 0.18),(3.21 ± 0.13),(3.30 ± 0.09) scores in control group,and there were significant differences between two groups (P < 0.01 or < 0.05).The occurrence of relapse of fracture was 2 cases (8%,2/25) in treatment group and 9 cases (36%,9/25) in control group,and there was significant difference between two groups (P < 0.05).Conclusions PVP combined with zoledronic acid in treatment of spinal metastatic carcinoma has many advantages such as small incision,short surgery time and rapid relief from pain.It can obviously improve the quality of life,and it is worthy of spreading in the clinic.
4.Effects of PDCA Cycle Quality Management Model on Multi-drug Resistant Organism Infection Control in Reha-bilitation Hospital

Objective To analyze the effect of PDCA cycle quality management model on multi-drug resistant organism (MDROs) infec-tion control in a tertiary rehabilitation hospital. Methods From March, 2013 to December, 2015, targeted surveillance of MDROs infection control was performed with PDCA cycle quality control. The MDROs detection rate, the awareness rate of MDROs prevention and control, the rate of doctor issuing isolation orders and execution rate of medical sanitation disinfection and isolation were analyzed. Results The de-tection rates of methicillin-resistant staphylococcus aureus and Carbapenem-resistant Pseudomonas aeruginosa decreased (χ2>3.922, P<0.05), while no significant difference was found in the detection rate of multidrug-resistance Acinetobacter baumanii (χ2=8.775, P=0.071). The awareness rate of MDROs prevention and control, the rate of doctor issuing isolation orders and execution rate of medical sanitation dis-infection and isolation significantly improved (χ2>399.17, P<0.001). Conclusion PDCA cycle quality management model played an impor-tant role in the prevention and control of MDROs in rehabilitation hospital.
5.Effects of human glucagon-like peptide-1 analog gene transfection on blood glucose,insulin levels and pancreatic island in diabetic rats

Objective To explore the effects of recombinant expression plasmid of human glucagon-like peptide-1 (hGLP-1) analog gene (2?Val2-hGLP-1) on blood glucose, serum insulin level and pancreatic island in diabetic rats. Methods The diabetic rat model was reproduced by intraperitoneal injection of streptozotocin. The rats were then divided into 3 groups randomly (8 each): recombinant plasmid pIRES2-EGFP/Val2-hGLP-1 transfection group, empty plasmid pIRES2-EGFP transfection group, and diabetic rat model control group. Moreover, 8 untreated SD rats were set as normal control. Each rat in empty plasmid transfection group and recombinant plasmid transfection group was injected via tail vein with 110?g plasmid, whibe those in diabetic model control group and normal control group were treated with equal volume of normal saline solution. Blood glucose and serum insulin levels of rats were determined 30 days after experiment, and glucose tolerance test and insulin tolerance test were performed to estimate insulin sensitivity. The pathological changes in pancreatic island and insulin secretion were evaluated with HE and immunohistochemistry staining, respectively. Results Compare with normal group, diabetic model group and empty plasmid transfection group, the blood glucose level significantly lowered (P0.05). Meanwhile, the insulin secretion was increased and the pathological changes in pancreatic islands were alleviated in recombinant plasmid transfection group compared with that in diabetic model control group. Conclusions hGLP-1 analog gene transfection may be able to promote the proliferation of pancreatic islands and enhance sensitivity to insulin, thus significantly lower blood glucose level and ameliorate the lesion of pancreatic islets in diabetic rats.
6.Clinical observation of Tiaomai-Yinon the treatment of tachyarrhythmia with the TCM patterns of deficiency of Qi and Yin, and heat of blood stasis

Objective To study the effect ofTiaomai-Yin with the therapy method of clearing heat and cooling blood to treat the tachyarrhythmia with the TCM patterns of deficiency of Qi and Yin, and heat of blood stasis.MethodsA total of 60 patients with tachyarrhythmia were randomly recruited into a control group and a treatment group, 30 patients in each. The treatment group was treated withTiaomai-Yin, while the control group was treated with Metoprolol. After both groups were treated for one month, the changes of TCM symptoms, ECG and 24 h DCG were observed.Results The total effective rate of the treatment groups was significantly better than the control group(86.67%vs. 56.67%;χ2=7.957,P<0.05).Tiaomai-Yin significantly improve the symptoms of shortness of breath (1.40 ± 2.11vs. 2.10 ± 1.87;t=-3.341,P<0.01), tiredness (1.30 ± 2.33vs. 2.10 ± 1.84;t=-3.262,P<0.01), dry mouth (0.09 ± 1.34vs. 2.50 ± 2.41;t=-3.194,P<0.01), and tongue (1.90 ± 1.65vs. 3.90 ± 1.81;t=-3.217,P<0.01) and pulse (2.60 ± 1.71vs. 5.10 ± 2.55;t=-3.283,P<0.01).ConclusionTiaomai-Yin had effects in treating tachyarrhythmia with the TCM patterns of deficiency of Qi and Yin, and heat of blood stasis with improvement of symptoms and fewer side effects.
7.Association between serum 25-hydroxyvitamin D3 concentration, parathyroid hormone, and arterial stiffness in patients with type 2 diabetes

Objective To evaluate the association between serum 25-hydroxyvitamin D3 [25 (OH) D3],parathyroid hormone,and arterial stiffness in patients with type 2 diabetes.Methods Serum 25 (OH) D3 and parathyroid hormone(PTH) were determined in a cross-sectional sample of 258 patients aged 30 years and over.Arterial stiffness was assessed by pulse wave velocity(PWV) obtained with a VP-1000 pulse wave unit.Fasting plasma HbA1c,lipid profile,calcium,and high sensitive-C reactive protein were determined.Results (1)The prevalence of vitamin D3 deficiency was high(79.84%) in patients with type 2 diabetes.(2) Those with lowered serum vitamin D3 levels had raised PWV [(1610.76 ± 142.70 vs 1527.95 ± 58.02) cm/s,P<0.05].(3) Multiple stepwise regression analysis showed that 25 (OH) D3 was an impact factor of PWV risk score,which was independent of age,duration of diabetes,and systolic blood pressure(β =-0.256,P<0.01).(4) Serum PTH was positively correlated with PWV (r =0.210,P < 0.05) and systolic blood pressure (r =0.229,P < 0.05),but negatively correlated with 25 (OH) D3 (r =-0.153,P < 0.05).Conclusions 25 (OH) D3 deficiency is common in patients with type 2diabetes,and a low serum 25 (OH) D3 level is significantly associated with increased arterial stiffness in these patients.The association of serum PTH with arterial stiffness may result via changes in vitamin D and blood pressure.

Cancer-related fatigue is one of the most common and refractory clinical sympotoms in patients with cancer. Investigators have tried to study the molecular pathogenesis of cancer-related fatigue, and the cytokine has become a research focus. At present, a number of cytokines, including transformation growth factor, tumor necrosis factor, interleukin, proteolytic inducing factors, inflammatory cytokine are proved to be correlated to cancer-related fatigue.
9.Effect of allo-human bone marrow mesenchymal stem cells on T and B cells from patients with rheumatoid arthritis in vitro

Objective To study the effect of allo-human bone marrow mesenchymal stem cells (BMSCs) on T and B cells from patients with Rheumatoid arthritis (RA) in vitro. Methods BMSCs were isolated from bone marrow samples of healthy volunteers and purified by density gradient centrifugation and cultured in vitro. Peripheral lymphocytes were isolated from patients with RA.Then, BMSCs and lymphpcutes were co-cultured. The modulatory effect of BMSCs on proliferation, activation and maturation of T and B lymphocytes of RA patients stimulated by PHA and SAC respectively was observed. The cell generation cycle and the degree of apoptosis were assessed by flow cytometry with PI/ Annexin V. After co-cultured with or without BMSCs for 72 hours, T cells were harvested, then they were labeled with anti-CD3, anti-CD4, anti-CD8, anti-CD25 antibodies and analyzed by flow cytometry. The density of IgG in the co-culture system was detected by ELISA. Results T and B cells proliferation was significantly suppressed when co-cuhured with bMSCs but did not induce T cell apoptosis. There was a significant decrease in the ratio of CD4+ CD3+ T cells in the co-cuhure group (34±6), as compared with the control group (44±7) (P<0.05). There was a decrease in CD25+ T cells and increase of CD4+ CD25+ cells in BMSCs co-cultured group (P<0.05). IgG was in creased in the cocuhure system. Conclusion Human BMSCs inhibit T and B cell activation and proliferation in patients with RA in vitro. And these immunomodulatory effects are not MHC restricted. The results of this study have provided evidence for the fact that BMSCs has the potential to be an effective treatment for RA.
10.Bioactivity of indolylpiperidine-piperazine derivatives on α1 -adrenoceptor-mediated inotropic response

OBJECTIVE To investigate the blocking activities of a series of potential α1-adrenoceptor (α1-AR) antagonists (Compounds B1 -B9) on α1-AR.METHODS ① A series of potential α1-adrenoceptor (α1-AR) antagonists,indolylpiperidine derivative (IPD) and Compounds B1 -B9,with indolylpiperidine moiety and different substitutes were synthesized through the coupling of indolylpiperidine and piperazine derivatives.② Inotropic responses experiment was used to examine blocking effects of IPD and Compounds B1 - B9 in isolated rat atria by phenylephrine (PE) stimulation.③ Blocking effect of IPD and Compounds B1 - B9 on phosphorylation level of extracellular signal-regulated kinase (ERK) in PE treated HEK293 cells was tested by Western blotting.RESULTS ① Potential α1-adrenoceptor (α1-AR) antagonists with indolylpiperidine moiety and different substitutes were synthesized successfully.② PE caused a dose-dependent inotropic response which was inhibited by pre-incubation of phentolamine (Phen),a non-selective α1-AR antagonist,IPD and Compounds B1,B3,B4,B7,B8 and B9,respectively; IPD and Compounds B4 and B8 caused an obvious rightward shift of inotropic response-curve,the pA2 values for IPD and Compounds B4 and B8 were 6.72 ± 0.21,6.86 ± 0.29 and 6.67 ± 0.19,respectively.③ Phosphorylation level of ERK1/2 was inhibited by pre-incubation with Compounds B1,B2,B3,B5,B6,B7,B8 and B9 or IPD in PE treated α1A-AR stably expressed HEK293 cells; PE-stimulated phosphorylation level of ERK1/2 was inhibited by pre-incubation with Compounds B2,B4,B7 or B8 in α1B-AR stably expressed HEK293 cells.CONCLUSION Compound B4 has a selective blocking activity on α1B-AR,and Compounds B1,B3,B5,B6 and B9 or IPD have a selective blocking activity on the phosphorylation level of ERK1/2.
