1.Biomechanical effects of placement of a disc prosthesis and adjacent vertebral bodies

[Objective]To present a validated two-functional spinal unit C4~7 finite element model with simulated placement of a disc prosthesis treatment to predict stresses and strains within adjacent vertebal bodies. [Methods]Two finite element models of C4~7 were generated using the CT scans taken from a male patient following placement of a disc prosthesis treatment,which were included with adjacent bodies,intervertebral discs. Simulations were conducted imposing a compression preload combined to a flexion/extension moment,a pure lateral bending moment and a pure torsion moment,and to quantify the stress levels in intervertebral discs,endplate,cancellous bone,cortical bone and adjacent bodies following placement of a disc prosthesis under clinically relevant loading conditions.[Results]The BryanTM disc prosthesis allows to correctly reproduce a physiological motion at the implanted level,and the changes in stresses and strains in levels adjacent to a placement of a disc prosthesis treatment level were minimal.[Conclusion]The artificial cervical disc prosthesis well restores the biomechanical behavior of the intact spine. Moreover,the selection of surgical indications and proper manipulations shoule bve paid more attehtion to.
2.Determination of Active Ingredient Oleanolic Acid in She Medicine Clematis Florida var. Plena by HPLC

Objective:To establish a method for the determination of oleanolic acid in She medicine Clematis florida var. plena. Methods:The HPLC method was carried on a Lanbo-Kromasil C18 chromatography column using acetonitrile-water ( 86∶ 14 ) as the mobile phase. The detection wavelength was 205 nm, the flow rate was 1. 0 ml·min-1 , and the column temperature was 25 ℃. Re-sults:The calibration curve of oleanolic acid was linear within the range of 0. 52-15. 60μg (r=0. 999 9). The average recovery of oleanolic acid was 92. 13%(RSD=3. 0%, n=9). Conclusion:The method is simple, rapid, accurate and reproducible, which can be used as an ideal method for the quality control of She medicine Clematis florida var. plena. .
3.Comparative study on the efficacy and safety of robot assisted laparoscopic and retropubic radical prostatectomy

Objective To compare the efficacy and safety of robot assisted laparoscopic radical prostatectomy (RALRP) and retropubic radical prostatectomy (RRP).Methods From May 2004 to September 2013,data from 124 prostate cancer patients were collected.Of them,41 cases underwent RALRP,and 83 cases underwent RRP.The following clinical data was reviewed and analyzed,including operation time,blood loss,transfusion rate,duration of catheterization,time of postoperative hospital stay,postoperative pathology,continence and biochemical recurrence.Results All the operations were successfully performed,without RALRP converted to open surgery.The mean operation time of the RALRP group and the RRP group was 224 min and 165 min,mean blood loss was 266 ml and 659 ml,the transfusion rates were 20% and 86%,the mean postoperative hospital stay was 16.4 d and 17.9 d,the positive margin rates were 24% and 10%,the differences were significant (P < 0.05).The mean catheterization time was 27.2 d and 23.5 d (P > 0.05).The 6-month,1-year and 2-year biochemical recurrence rates were 8%,21% and 24% in RALRP group,and 13%,16%,31% in RRP group.The 3-month,6-month,1-year and 2-year continence rates were 56%,70%,80% and 94% in RALRP group,and 55%,70%,78% and 79% in RRP group.For biochemical recurrence and continence rates,there was no significant difference between the 2 groups.Conclusions RALRP possesses advantages of less bleeding and faster recovery.In addition,the effect of postoperative oncology and continence is similar to open surgery.
4.Diagnostic value of combined detection of procalcitonin,C reactive protein and white blood cell count in children with respiratory tract infection

Objective To investigate the diagnostic value of procalcitonin (PCT) ,C‐reactive protein(CRP) and white blood cell (WBC) count in children with respiratory tract infection .Methods A total of 358 children inpatients with respiratory tract infec‐tion in the pediatric department of our hospital from 2014 January to June 2015 were selected and divided into the bacterial infection group and non‐bacterial infection group according to the throat swabs and sputum culture results .The venous blood was collected before and after treatment for detecting PCT ,CRP and WBC count ;meanwhile 30 healthy children were selected as the control group .Results The PCT ,CRP and WBC levels in the bacterial infection group were significantly increased compared with the non‐bacterial infection group and control group(P<0 .01) ,while which in the non‐bacterial infection group were relatively close to those in the control group(P>0 .05);but the PCT ,CRP and WBC count levels after 1‐week treatment in the bacterial infection group were significantly decreased compared with before treatment (P<0 .01) ,while which in the non‐bacterial infection group had little change between before and after treatment (P>0 .05) .Conclusion The combined detection of PCT ,CRP and WBC count has an important application value in the differential diagnosis and medication guidance of the children′s respiratory tract infection .
5.Establishment and Benefit Analysis of PACS in Small & Medium-Sized Hospitals

Objective To improve work efficiency and lower management cost. Methods Based on experiences from the 95th hospital in six years, PACS application benefits were analysed from such seven aspects as digital film management,economic benefit, efficiency and quality of diagnosis, evidence support for solving medical dispute,simplified treatment procedures, social effect, and remote disgnosis. Results Better economic benefits and social effect were obtained. Conclusion It is proved by practice that construction of PACS in small & midium-sized hospitals is necessary and feasible.
6.Research progress on the prevention and treatment of liver cancer intervened by Traditional Chinese Medicine on related signaling pathway

There are many abnormal signaling pathways in the occurrence, development and microenvironment of liver cancer. The abnormal activation or inhibition of cell signaling pathway can regulate cell proliferation, apoptosis and migration, and then affect inflammation, angiogenesis and tumor migration. Experimental studies have shown that Traditional Chinese medicine (TCM) can prevent and treat hepatocellular carcinoma by intervention with the transduction of related signaling pathways, which mainly includes hedgehog signaling pathway, IL-6/STAT3 signaling pathway, NF-κB signaling pathway, PI3K/Akt/mTOR signaling pathway, MAPK signaling pathway, toll-like receptor signaling pathway and Wnt signaling pathway. At present stage, TCM has a definite effect on the prevention and treatment of liver cancer, but the specific mechanism has not been fully elucidated, and the regulation of TCM on liver cancer related signaling pathways still needs further exploration.
7.Analysis of clinical features and liver histopathology of autoimmune liver diseases: an analysis of 109cases

Objective To analyze the clinical characteristics of autoimmune hepatitis (AIH), primary biliary cirrhosis (PBC), primary sclerosing cholangitis (PSC) and AIH/PSC or/PBC overlap syndrome in order to further understand the autoimmune liver diseases (AILD). Methods One hundred and nine patients with AILDs confirmed pathologically were collected between Jan. 2004 and June 2006. Of 109 patients, AIH was found in 27 eases, PBC in 67 cases, PSC in 4 eases, AIH-PSC overlap syndrome in 1 case and AIH-PBC overlap syndrome in 10 cases. The clinical and laboratory data of all patients were assessed retrospectively. Results The AILD was predominantly found in middle-aged women (73.3% ,80/109), and the main clinical manifestations were jaundice, malaise, anorexia and pruritus. The age distribution of patients with AIH showed a single peak at 50 years. Elevated serum gamma globulin and IgG were found in patients with AIH, of whom 62.9% (17/27) were positive for anti-nuclear antibody (ANA) and 3 were positive for liver-kidney microsomes type 1 antibody. The main histological changes in severe AIH cases included interface hepatitis (77.7 %) and bridging necrosis. Most of the PBC patients were presented with elevated serum alkaline phosphatase, glutamyl transpeptidase and IgM. Fifty patients (74.6%) were positive for anti-mitochondrial antibody (AMA) and AMA-M2. The pathological examination showed that 28. 3% of the cases were in Ⅰ or Ⅱ stage and 71.7% in Ⅲ or Ⅳ stage in patients with PBC who received liver biopsy. The pathologic change of reduction or even disappearing of bile ducts was found in 62. 6% patients with PBC. The clinical and pathological manifestations in patients with AIH-PBC overlap syndrome had both characteristics of PBC and AIH. Three out of 10 patients with AIH-PBC overlap syndrome were positive for ANA and AMA/AMA-M2. Conclusion Since AILD is not rare in Chinese, its diagnosis should be based on the clinical presentation, biochemical, immunological and histologic changes.
8.Three-dimensional finite element analysis on artificial cervical disc replacement for activities of lower cervical spine

This study was aimed to establish the three-dimensional model of C_(3-7) segment of lower cervical spine after artificial disc implantation, to analyze the movement of lower cervical spine after artificial disc replacement. According to CT films of 1 patient at 6 months after artificial disc implantation, three-dimensional finite element model that included Bryan~(TM) artificial cervical disc prosthesis of the lower cervical spine was established using finite element method, then introduced into Ansys 9.0, the vertebral cortical bone, cancellous bone and intervertebral disc were meshed and analyzed by using under several states such as flexion/extension, lateral bending and rotation, thus understanding their motion characteristics. By comparison with previous research findings, test results nearly accorded with or exhibited identical trend with previous study. The results suggest that, cervical disc replacement can basically guarantee the stability of lower cervical spine movement.
9.Immune hyper-activation of multiple lymphocyte subsets in acute coronary syn-drome

Objective:The process of myocardial infarction is generally characterized by the activation of host immune cells and the occurrence of inflammation. However, it is unknown which immune cells are preferentially activated and participated into the progression of myocardial infarction. Methods:A total of 55 patients with myocardial ischemia including 13 of stable angina ( SA) ,25 of unstable angina (UA) and 17 of acute myocardial infarction (AMI) as well as 12 of healthy controls (HCs) were enrolled in the study. The frequency and the immune activation marker CD38 expression by peripheral CD3 T cells,CD4 T cells,CD8 T cells,CD4+NKT cells, CD4- NKT cells, CD3-CD56+ NK cells and B cells were comprehensively analyzed. Results:There was no significant difference in the frequencies of these immune cell subsets in peripheral blood among these four groups. Importantly,it was found that CD38 expression was significantly increased on CD8 T cells,NKT cells and NK cells in patients with acute coronary syndromes ( ACS) including UA and AMI patients as compared with those in SA and HC subjects. These data indicated that multiple immune cells were activated in ACS patients,which were possibly participated into the pathogenesis of ACS. Conclusion:The activation of multiple immune cells was closely associated with the progression and outcome in ACS patients. This study provides immune hyper-activation mechanism underlying the development of ACS and may favor for finding a novel immune marker to predict the progression of ACS.
10.Cranio-maxillofacial resection for the treatment of oral, maxillofa-cial, head and neck tumors involving the skull base:a 10-year ret-rospective study at a single center

Objective:To review our patients who underwent cranio-maxillofacial resection in the recent 10 years and explore the indication of the operation. Methods:From 2003 to 2013, 116 patients underwent cranio-maxillofacial resection in our department for the treatment of tumors involving the skull base. Tumors that involved the skull base were divided into 3 types according to skull base invasions shown in the coronal planes of CT and MRI scans. Type 1 tumor was adjacent to the skull base with free bone (n=45), type 2 tumor involved the skull base with intact dura (n=30), and type 3 tumor involved dura with free brain (n=41). All patients underwent cranio-maxillofacial resection by oral and maxillofacial surgeons and neurosurgeons. The defects after cranio-maxillofacial resection were reconstructed immediately with adjacent local or regional flaps (n=62) and free vascularized flap (n=54) according to different de-fects, respectively. Results:Cranio-maxillofacial resection was successfully performed in all patients. No intraoperative complication was found. The overall success rate of soft tissue flaps and free flaps was 98.3%and 96.4%, respectively. Three patients with intracrani-al infection (n=2) and bleeding in the internal carotid artery were dead postoperatively even though they underwent salvage surgery. The overall rate of complications was 14.7%, and the dead rate was 2.6%. Recurrence or distant metastasis was found in 36 patients dur-ing the follow-up period. Conclusion: For the indication of cranio-maxillofacial resection, the balance between tumor resection and postoperative function, survival rate, and quality of life should always be considered. This technique includes the balance between func-tion and form, survival and quality of life, donor and recipient sites, and primary and secondary functions.
