1.Observation on the effect of combined medicine and clinical characteristics of children with dyspepsia diarrheat

Objective To investigate the clinical characteristics of children with dyspepsia diarrhea and the effect of Clostridium butyricum powder combined with changweikang granules on the treatment of the children with this diseases.MethodsFrom February 2013 to October 2015 in department of pediatrics, Tonglu County the First People's hospital, 76 cases with dyspepsia diarrhea as the research object, were randomly divided into the experimental group and the control group, 38 cases in each group.The two groups were given changweikang granules, at this basis, the observation group were given clostridium butyricum powder.3 days for a course of treatment, the two groups were treated for 2coureses continuously.ResultsBefore treatment, there were no significant differences in clinical symptom scores between the two groups.After treatment, the scores of loss of appetite, abdominal pain, diarrhea, abdominal distension and stool in the experimental group were lower than those in the control group, the difference was statistically significant (P<0.05);Before treatment, the serum motilin, gastrin and somatostatin levels in the two groups were not statistically significant.After treatment, the serum motilin and gastrin levels in the experimental group were lower than those in the control group, and the level of serum somatostatin was higher than that of the control group, the difference was statistically significant;After treatment, the total effective rate in the experimental group was 92.11% higher than that in the control group (73.68%), and the difference was statistically significant (P<0.05).ConclusionPediatric indigestion diarrhea often showed loss of appetite, abdominal pain, diarrhea, abdominal distension and stool.The effect is good which clostridium butyricum powder combined with changweikang granules on the treatment of infantile indigestion diarrhea, it is worthy of clinical application.
2.The cerebral protective effect of intravenous propofol anaesthesia on the patients with acute severe brain injury

Objective To study the cerebral protective effect of intravenous propofol anaesthesia on the patients with acute severe brain injury.Methods 70 patients with acute severe brain injury were randomly divided into control group and observation group,each group had 35 cases.The observation group was given intravenously propofol 2mg/kg for anesthesia induction,then maintaining anesthetization by 4mg · kg-1 · h-1.The control group was given midazolam,then discontinuous indrawed isoflurane.Diprivan 1.2mg/kg,fentanyl 4μ g/kg and vecuronium bromide 0.15 mg/kg were also used intravenously for the maintenance of anesthesia.The changes of intracranial pressure (IP)and cerebral oxygen partial pressure(COP) before anesthetization,surgery,3 hours after surgery and the waking time of surgery were recorded.Results After treatment,IP of two groups was decreased and COP was increased sharply; the IP of observation group was (18.98 ± 3.98) mm Hg,which was lower than (21.76 ± 4.01)mm Hg of the control group(t =7.20,P < 0.05),COP was (17.09 ± 3.03) mm Hg,which was higher than (14.67 ± 2.63)mm Hg of the control group (t =7.23,P < 0.05).The wake time of observation group was (15.98 ± 2.01) h,which was shorter than (20.01 ± 1.23) h of the control group (t =7.96,P < 0.05).Conclusion The cerebral protective effect of intravenous propofol anaesthesia is good in patients with acute severe brain injury;it is worth to popularize in clinic.
3.Improving Technique of Coronary Intervention to Build Pig Model of Coronary Microembolization

Objective To develop miniature pig model of coronary microembolization (CME) by easy and cost-effi-cient technique. Methods A total of 11 miniature pigs were divided into control group (n=5) and CME group (n=6). Femo-ral artery was punctured using 21 gauge needle that is normally used for transradial procedures. Microspheres were injected into the left anterior descending artery of the CME group by 5 F coronary radiography catheter and 1.8 F coronary micro-guide catheter. Serum concentrations of brain natriuretic peptide (BNP) and cardiac troponin I (cTnI) were evaluated just be-fore CME and 6 hours after CME. Apical myocardial pathological lesions were evaluated by optical microscope 6 hours after CME. Results All miniature pigs in control group survived, but one died in the CME group. 5 F coronary radiography cathe-ter and 1.8 F coronary micro-guide catheter reached designated location successfully. Before CME, serum BNP (ng/L:143.00 ± 13.51 vs 134.00 ± 15.57) and cTnI (μg/L:0.39 ± 0.09 vs 0.38 ± 0.10) showed no significant differences between these two groups (t values are 0.976 and 0.294 respectively,both P>0.05). By contrast, serum BNP (561.00 ± 80.65) and cTnI (2.75±0.58) were much higher in CME group than those (BNP 139.00±13.87;cTnI 0.54±0.14 ) in control group after CME (t values are 11.530 and 8.337 respectively,both P<0.001). In CME group, microspheres, micro-infarction and inflammatory cell infiltration were seen under an optical microscope which are absent in control group. Conclusion Using new surgical consumables can successfully develop miniature pig model with CME. And the technique is simple, cost-efficient, practical so it is worth promoting.
4.Preliminary of research of effect of artesunate on invasion of human colon cancer Lovo cells

Aim To investigate the effect of artesunate on the invasion of human colon cancer Lovo cells and the possible mechanisms. Methods After Lovo cells were treated with different doses of artesunate(20,80, 160 μmol·L - 1 ), the soft agar colony formation test was adopted to observe the anchorage-independent pro-liferation of Lovo cells. Transwell assay was used to determine the effect of artesunate on the invasion abili-ty of Lovo cells. And the protein expressions of HMGB1 and MMP-2 were investigated by western blot. Results Artesunate could significantly inhibit both proliferation and invasion ability of Lovo cells in a dose-dependent manner(P < 0. 01). The experimental group treated with artesunate significantly down-regula-ted the protein expressions of HMGB1 and MMP-2 compared with control group(P < 0. 05). Conclusion Artesunate could inhibit the invasion of human colon cancer Lovo cells by down-regulating HMGB1 and MMP-2 expressions.
5.Bioeffects of canine myocardium under microbubble destruction via diversity of ultrasonic intension

Objective To explore the bioeffects of canine myocardium under microbubble destruction via diversity of ultrasonic intension, in order to optimize ultrasonic intension for experiments. Methods Nine mongrel dogs were randomly divided into 3 groups. Ultrasound (1 MHz) in diversity of different intension (0.5 W/cm~2, 1.0 W/cm~2, 2.0 W/cm~2) was applied to expose canine myocardium after intravenous injection microbubbles of 2.0 ml. All the dogs were killed after being exposed for 5 min. The myocardium was harvested for HE staining and observed with transmission electron microscope for the tissue microstructures. Results The myocardium of hyperemia, disfiguration and necrosis wer observed in all groups. Myocardial edema but not hemorrhage appeared with 0.5 W/cm~2 , mild myocardial hemorrhage and slight inflammatory cell infiltration happened with 1.0 W/cm~2, whereas obvious hemorrhage and certain degree of inflammatory cell infiltration occurred with intension of 2.0 W/cm~2. With the augmentation of ultrasonic intension, myocardium trend to aggravate. Conclusion Ultrasound of diversity intension can induce different bioeffects of canine myocardium. Ultrasound mediated microbubble destruction with the intensity of 1.0-2.0 W/cm~2 can provoke a certain degree of inflammatory reaction with mild myocardial damage.
6.Study on the relationship between promoting apoptosis effect of artesunate and Wnt/β-catenin pathway in colon cancer cells

Aim To investigate the promoting apoptosis effect of artesunate( ART) on human colon cancer Lovo cells and its mechanisms. Methods MTT assay was performed to determine the anti-proliferative effect of artesunate. Flow cytometry assay and electron micros-copy( EM) were used to evaluate the apoptotic effect of artesunate. Luciferase reporter assay was introduced to measure the activation of Wnt/β-catenin pathway. Western blot was used to detect the pathway-related protein levels of β-catenin, GSK-3β,c-Myc and apop-tosis-related protein level of casepase-3 . Results Compared with the control group, the inhibitory rate of cell proliferation at 72 h and 320 μmol·L-1 ART was (78. 99 ± 1. 95 )% ( F =898. 301, P =0. 000 ); the cell apoptotic rate at 24 h and 160 μmol · L-1 ART was(19. 00 ± 0. 05)% and morphological signs of cell apoptosis were found by EM;the transcriptional activi-ty of TCF4/LEF at 24 h and 160 μmol·L-1 ART was (0. 36 ± 0. 30)%(F =470. 954,P <0. 01); the ex-pressions of caspase-3 and GSK-3β were significantly increased, whileβ-catenin and c-Myc were significant-ly decreased when treated with different concentrations of ART for 48 h ( P <0. 01 ) . Conclusion ART may significantly inhibit proliferation and promote apoptosis of Lovo cells probably by inactivating Wnt/β-catenin pathway.
7.Experimental Study on Effect of Shexiangbaoxin Pills on Morphosis and Function of Left Ventricle after Myocardial Infarction in Rat

Objective: To investigate the effect of Shexiangbaoxin Pills on morphosis and function of left ventricle after myocardial infarction in rat. Methods: The rat myocardial infarction models induced by ligaturing coronary artery were randomly divided into the myocardial infarction control group, Shexiangbaoxin Pill group and captopril group. The hemodynamic parameters and cardiac functions in 2 or 6 weeks after treatment were determined. Meantime, the morphological parameters of left ventrile (left cardiac chamber size, attenuation proportion of ventricular wall of infarction area, and expansion index of left ventrile), were studied. Results: Shexiangbaoxin Pills could reduce the distention of left ventrile and expansion of infarction area of rat in 6 weeks after infarction. It could obviously improve the contraction function of left ventrile after myocardial infarction. Conclusion: Shexiangbaoxin Pills can improve left ventricular reconstitution after myocardial infarction.
8.Expression of Myocardial Levels of Connexin 43, 40 in Experimental Dog Model of Sympathomimetic Atrial Fibrillation

Objective: To explore the expression of myocardial levels of connexin 43(Cx43), Cx40 in experimental dog model of sympathomimetic atrial fibrillation (AF). Methods: 15 mongrels dogs were randomly divided into 3 groups: Control group, Rapid atrium pacing (RAP) group and RAP+isoprenaline (ISO) perfusion group. n=5 in each group. The hearts were taken to establish in vitro langendorff cardiac perfusion model. Atrial effective refractory period (AERP) and AF inducing rate were tested;intracellular expression and distribution of nerve growth factor (NGF) and tyrosine hydroxylase (TH) were examined by immunohistochemistry, total protein contents of Cx43 and Cx40 were measured by Western blot analysis, mitochondria morphology was observed by transmission electron microscope and mitochondria reactive oxygen species (ROS) generation was detected by fluorescent colorimetric method. Results: AERP was similar between Control group and RAP group (166±5.1) ms vs (160±3.2) ms which cannot induce AF; while it was shortened in RAP+ISO group (148±3.7) ms, P<0.05 which may successfully induce AF.Compared with Control group, mitochondria was slightly swollen in RAP group and the matrix was intact, while mitochondria was obviously swollen in RAP+ISO group and part of matrix was transparent; total protein contents of Cx43 and Cx40 were lower in both RAP group and RAP+ISO group, P<0.05; in addition, they were even lower in RAP+ISO group than RAP group, P<0.05. Compared with Control group and RAP group, RAP+ISO group had increased expression and distribution of NGF, TH and mitochondria ROS generation, P<0.05; NGF, TH and ROS in RAP group were higher than Control group, P<0.05. Conclusion: Sympathetic AF has been related to the contents and changes of myocardial levels of CX43 and Cx40; sympathetic nerve might trigger AF by oxidative stress induced down-regulation of myocardial CX43 and Cx40 in experimental dog model.
9.A Case of Disseminated Cryptococcosis with Cutaneous Manifestations and Osteomyelitis: Clinical and Laboratory Investigations

Objective To report a case of disseminated cryptococcosis with cutaneous manifestations and osteomyelitis. Methods and Results A 33 year old female was admitted due to multiple nodules and ulcers on the upper arms, shoulders, buttocks and thighs for one year. The patient was pregnant when admitted, and gave birth to a premature baby during her illness. The nodules increased half a month after delivery, which was suspected to be hematogenously disseminated pulmonary tuberculosis and was given anti tuberculous therapy for three months but failed. Physical examination showed there were 39 nodules or ulcers on the face, gum, trunk, buttocks and extre mities. The bone structure of the left tibia and fibula destroyed and a sinus developed on the left fibula. Microbiologic examination showed that lots of spores were seen in the smear of pus and necrotic tissues, which produced yeast like colonies in culture with positive urease and caffeic acid test. Cryptococcus neoformans, serotype A was identified by API yeast reaction band and serology. Inoculation with mice and rats showed that their brains, lungs and livers were involved easily. Further identification as C.neoformans var.neoformans was obtained based on sequence analysis of ribosomal internal transcribed spacer region 2. The anti tuberculous therapy was stopped and anti fungal therapy was initiated at once. Intravenous and topical amphotericin B in combination with fluconazole were chosen in the initial therapy and itraconazole for maintenance. The nodules disappeared after 30 days and the last ulcer in the left tibia healed completely after 200 days. The anti fungal therapy was discontinued after 277 days and the patient was completely cured.
10.Comparison of clinical characteristics between rifampin-dependent and rifampin- resistant patients with pulmonary tuberculosis.

OBJECTIVETo compare the clinical characteristics of rifampin-dependent (R-dependent Mycobacterium tuberculosis) and rifampin-resistant (R-resistant Mycobacterium tuberculosis) patients with pulmonary tuberculosis.
METHODSThe clinical data including the demographic data, age groups, course of disease, history of chemotherapy with anti-TB drugs, and results of drug susceptibility test were collected from 61 cases of R-dependent pulmonary tuberculosis and 148 cases of R-resistant pulmonary tuberculosis treated between October, 2008 and January, 2012.
RESULTSMost of the R-dependent and R-resistant patients were between 30 and 44 years of age. The R-dependent patients included 12 receiving the first treatment patients and 49 with previous treatments, and the R-resistant patients included 11 without and 137 with previous treatments. The multi-drug resistant rate was 80.3% in R-dependent group, as compared to 92.6% in R-resistant group.
CONCLUSIONMost of the patients infected with R-dependent Mycobacterium tuberculosis are young or middle-aged, often having serious disease conditions. Detecting rifampin dependence is important for patients with initial treatment failure. Multi-drug resistance therapy guideline should be applied to patients infected with R-dependent Mycobacterium tuberculosis to improve the cure rate.
