2.Effect of postoperative intravenous analgegia using dezocine on uterine contraction-induced pain after cesarean section

Objective To study the effect of postoperative patient-controlled intravenous analgegia.using dezocine on uterine contraction-induced pain after cesarean section.Methods Sixty patients undergoing cesarean section were randomly divided into two groups with 30 cases each.Group D received patient-controlled intravenous analgesia(PCIA)with dezocine and tramadol.Group T received PCIA with tramadol.Ten minutes before completion of the operation,loading dose was given:in group D intravenous dezocine 0.1 mg/kg,in group T intravenous tramadol 2 mg/kg.Visual analogue scale(VAS)of wound pain and uterine contraction-induced pain,adverse effects were recorded at 4,8,16 and 24 h during analgesia.Results There was no siguiciant difference on VAS of wound pain between the two groups(P > 0.05).The VASs of uterine contraction-induced pain at the4,8,6 and 24th hour after surgery were(1.53 ±0.37),(2.28 ±0.15),(2.43 +0.56)and (2.01 ±0.64)respectively in Group D,which were significantly lower than those of(4.73 +0.43),(5.86 ±0.35),(5.83 ± 0.27)and(4.18 ± 0.53)in Group T(Ps < 0.05).Ramsay sedation scores were(3.97 ±1.41)and(3.12 ±0.06)at the 4 and 8 h,which were significantly lower than those of(1.12 ±0.49)and (1.09 ±0.73)in Group D(Ps <0.01)).However,there was no significant difference between the two groups in adverse effect(P > 0.05).Conclusion Intravenous patient-controlled analgesia with dezocine composite tramadol is not only effective for wound pain scores,but also can suppress uterine contraction-induced pain with less adverse effect in patients underwent cesarean section.

Calcium-sensing receptor(CaSR)is expressed in human parathyroid cells and renal tubular epithelial cells mainly.CaSR could control parathyroid hormone(PTH)secretion and the renal tubular's handing of the electrolytes and water.This receptor plays a central role in extracellular calcium homeostasis,and involves in the process of bone remodeling.The change of CaSR function,including anti-CaSR antibodies,CasR gene mutations or any part impairment in CaSR signaling pathway,not only results in the imbalaance of PTH and calcium homeostasis.but also relates to disorders of bone remodeling and renal tubular's mishandling in the electrolytes and water,which contribute to hyperparathyroidism.It makes possible clinical Use of cinacalcet in hyperparathyroidism in theory.
4.Heparin for treatment of sepsis: a systemic review

Objective To systemically review the efficacy and safety of heparin for treatment of sepsis.Methods Database search of IM/MEDLINE,Cochrane Library,SCIE,CBM,CNKI,VIP Data,WanFang Data (from January 2000 to June 2012) was conducted.The quality of included randomized controlled trials (RCTs) about heparin for treatment of sepsis was assessed,and relevant data were extracted according to the inclusion and exclusion criteria.Then meta analysis was performed using RevMan 5.1.Results 17 trials with 1 167 participants were included.The results of meta-analysis showed:compared with the control group,heparin significantly decreased 28-day mortality in patients with sepsis [odds ratio (OR) =0.59,95% confidence interval (95%CI) 0.45-0.77,P=0.0001] ; heparin did not deteriorate coagulation disorders,but corrected sepsis-induced platelet (PLT) count reduction [mean difference (MD) =13.94,95% CI 10.15 to 17.72,P<0.000 01],while it had no significant effect on the activated partial thromboplastin time (APTT) and prothrombin time (PT,APTT:MD=-3.18,95%CI-6.88 to 0.53,P=0.09; PT:MD=-0.68,95% CI-1.48 to 0.12,P=0.09).There was no significant difference between the two groups in the incidence of bleeding either.Acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score of heparin group was significantly lower than that of the control group (MD=-2.58,95%CI-3.29 to-1.87,P<0.000 01),and the incidence of multiple organ dysfunction syndrome (MODS) was significantly lower than that of the control group (OR=0.32,95%CI 0.17 to 0.61,P=0.000 6).In addition,heparin could shorten intensive care unit (ICU) stay (MD=-4.43,95%CI-6.79 to-2.07,P=0.000 2),whereas it showed no significant effect on the total length of hospital stay.Conclusions Heparin can ameliorate sepsis,and has high degree of safety and lower hospital expense.Due to limitation of the quality of included studies,larger sample and well-designed RCTs are needed to further support this conclusion.
5.Biomechanical properties of the lumbosacral spine and application of internal fixation materials

BACKGROUND:In recent years, the spinal internal fixation technology has made rapid development based on biomechanics and material sciences.
OBJECTIVE: To review the biomechanical characteristics of the lumbosacral spine and the application of various internal fixation materials in the reconstruction of spinal stability after lumbosacral spinal tuberculosis.
METHODS:A computer-based search of Medline and Chinese Journal Ful-Text Database was performed for relevant articles using the keyword of “lumbosacral spinal tuberculosis, biomaterials materials, fixation” in English and Chinese, respectively.
RESULTS AND CONCLUSION: Rigid internal fixation is a conventional treatment for lumbosacral tuberculosis, which improves the spinal alignment and stability during the spinal reconstruction. Metalic materials such as stainless steel, titanium and titanium aloys have been widely used in rigid internal fixation, but metal sedimentation, non-transparency, stress shielding and osteoporosis after internal fixation impact the fusion effects and imaging observation. Absorbable materials as newly-developing materials have good biocompatibility and biodegradability in orthopedic internal fixation. To select the appropriate material for internal fixation, the biomechanical properties of internal fixation materials wil be investigated according to the degree of vertebral damage and lumbosacral stability.

Electronic portal imaging device(EPID) is now been used widely.EPID was initially used for the purpose of checking set-up error.There are two ways to verify set-up errors-on-line and off-line.With advanced knowledge about the dosimetry characteristics of EPID,the use of EPID for dosimetry verification was adapted from the research study to the clinic.EPID plays an important role in quality assurance of radiotherapy accessories including multileaf collimator(MLC)that has been most studied in the past couple of years.This article briefly reviews the clinical use of EPID.

8.Clinical analysis of coronary artery bypass grafting:a report of 286 consecutive cases

Objective To evaluate the technique of coronary bypass grafting and its early result in 286 consecutive patients.Methods Two hundred and eighty-six patients from June 2002 to December 2005 underwent coronary artery bypass grafting.The average unmber of grafts was 3.3.Two hundred and eighty-three patients had left internal mammary artery bypassed to the left anterior descending branch.Other grafts were from saphenous veins.Results All patiens were clinical recovery.Respirator assisted within 0~28 hours(average 10).Conclusion Coronary artery bypass can be safely done with low mortality excellent surgical results even in patients with multi-vessel coronary artery disease.
9.OBSERVATION OF THE KINETIC CHANGE ON IgG,T-CELL SUBSETS,IL-2 LEVEL OF MICE INFECTED TRICHINELLA SPIRALIS

AimTo explore the kinetic change of the IgG,T-cell subsets and IL-2 level from the mice infected with T. Spiralis. Methods The level of specific IgG A band IL-2 was determined by ELISA,the percentage of CD4+and CD8+ T-cells were examined by flow cytometry on 7, 14,21,28,35 days after mice infected with T. spiralis respectively. ResultAfter infected with T. spiralis,the level of IgG in mice rised gradually,and reached its peak on the 35th day. The change of T-cell subsets showed that CD4+T cells decreased,which CD8+Tcells increased. The ration of CD4+/CD8+cells decreased,and which was the obvious on the 14thday. It did not recover to normal level even on the 35th day. The IL-2 of level reached the peak on the 7th day after infected,then IL-2 level decreased quickly and lower than that of normal mice on 35th day after infected. Conclusion When the acute phase of T. spiralis infection,the immune function of host was inhibited. The protective immunity of against T. spiralis infection was cellular immunity mainly ,in cooperation with humoral immunity.
10.Immune Responses to Challenge Infection in Mice Immunized with Trichinella spiralis Adult Worm Soluble Antigen

Objective To study the immune response induced by the mice immunized with Trichinella spiralis(T.spiralis) adult worm soluble antigen(AWSAg). Methods T.spiralis AWSAg was prepared to immunize Kunming strain mice. Dynamic changes of IgG, IL 2 and T lymphocyte subsets from immunized mice were determined after challenge infection on d 7, d 14 , d 21 , d 28 and d 35 . Results On d 7 after challenge infection, IgG and CD4 + T cells of immunized group were markedly elevated and persisted higher over the observation period. In contrast, CD4 + T cells and CD4/CD8 ratio were significantly decreased in unimmunized group resulted from immune suppression after infection. IL 2 levels reached the peak on d 7 and persisted in high level from d 7-d 21 in both immunized and unimmunized group after infection, then decreased gradually. Till 35 days after infection, IL 2 level was still higher than the normal mice. Conclusion Mice immunized with AWSAg of T. spiralis produced a potential cellular and humoral immune response.
