1.Clinical Application of Fiberoptic Bronchoscopy in Treatment of Ventilator Associated Pneumonia

Objective To study the value of fiberoptic bronchoscopy in treatment of ventilator associated pneumonia. Methods The 168 cases of ventilator associated pneumonia in ICU were randomly divided into treatment group and control group, each group of 84 cases, were given oxygen, anti-inflammatory, expectorant treatment. Treatment group was given fiber bronchoscope sputum suction and irrigation, control group was given sputum suction tube, then we observed the oxygenation changes after sputum suction of two groups of patients, compared clinical efficacy, sputum culture positive rate, antibiotic using time and staying time in the ICU between two groups of patients. Results Compared with control group, in treatment group SaO2, PaO2 and oxygen index were significantly improved (P< 0.05) ; in treatment group CRP, PCT and IL-6 were significantly improved (P <0.05) . In the treatment group, the total effectiveness is significantly higher than the control group;antibiotic using time and staying time of ICU were obviously shorter than the control group (P< 0.05) . Conclusion Fiberoptic bronchoscopy irrigation treatment can improve oxygenation, antibiotic using time and staying time in ICU of ventilator associated pneumonia patients, and is worthy of clinical application.
2.Screening of infection of malaria parasite with, atypical eosinophil distributions in the WBC scattergram and alarm in Sysmex XE-2100 hematology analyzer

Objective To explore a quick and feasible method of screening malaria parasite by using a Sysmex XE-2100 hematology analyzer though alarm information on high eosinophil count and atypical eosinophil distributions in the WBC scattergram. Methods Sysmex XE-2100 hematology analyzer was used for complete blood cell analysis. Microscopic review was used when high eosinophil count and atypicaleosinophil distributions in the WBC scattergram were found. If the review showed normal eosinophil cells, wecontinued to focus on red cell for searching malaria parasite. Results Among 1 501 specimens showing higheosinophil counts and atypical eosinophil distributions, nine cases with normal eosinophil cells were indisaccordance with the hematology analyzer, six of them showed high eosinophil count in the Sysmex XE-2100 hematology analyzer, whose distribution was located close to neutrophil clusters in scattergram. The otherthree had an abnormal WBC scattergram. There was no gap between eosinophil clusters and neutrophilclusters, which brought no classified results. But all the nine specimens had been found the trophozoite,schizont and merozoite in blood smears. Conclusions There were great possibility of the existence of themalaria parasite in specimens when hematology analysis showed high eosinophil count and atypical eosinophildistributions in the WBC scattergram in a Sysmex XE-2100 hematology analyzer, although these alarm wasnot comfirmed by microscopic review. This method provides not only a simple and reliable way for quickscreening malaria parasite but also has a great value in preventing the undetected ratio on malaria parasite.
3.Effect factors for language and memory dominance evaluation in patients with refractory epilepsy

Objective To investigate the effect factors for language and memory dominance testing in propofol Wada test in refractory epilepsy patients.Methods From January 2010 to August 2013,total of 23 patients underwent propofol Wada tests for language and memory dominance evaluation.According the performance in language test and memory test,the patients were divided into two groups,respectively:succeed with test or failed in test.The relevant factors including age,duration of epilepsy,handedness,FIQ (full scale of intelligence quotient),lateralization of lesions,language dominance,dose of propofol and the adverse effects of propofol were recorded as independent variables.Logistic regression was adopted to screen the effect factors for memory dominance evaluation.All statistical analyses were processed with SPSS software.Results All 23 patients succeeded with language test,but only 13 patients succeed with memory test.The difference of success ratios between language test and memory test was significant (P=0.001).For memory test,the difference of duration of epilepsy and FIQ in success group and failed group was significantly different (duration of epilepsy:(10.57±4.68) vs (18.30±9.30),P=0.032,FIQ:(81 ±25.33) vs (64.2±10.86),P=0.046).More grade Ⅱ adverse effects of propofol occured in the patients who failed in memory test (P=0.011).Logistic regression results indicated the grade Ⅱ adverse effects of propofol were the main risk factor for memory test failure (OR=28.0,P<0.01).Conclusions Propofol Wada test is an effective method for language dominance evaluation.However,the effect factors for memory dominance evaluation are much more than language evaluation,such as duration of epilepsy,FIQ and grade Ⅱ adverse effects of propofol,and grade Ⅱ adverse effects of propofol may be the most significant risk factor.
4.Role of heterogeneity of cancer-associated fibroblasts in targeted therapy of pancreatic cancer

The incidence of pancreatic cancer is increasing year by year, but the clinical diagnosis and treatment progress is limited and the prognosis is poor. Tumor microenvironment (TME) is closely related to the invasion, metastasis and chemotherapy resistance of pancreatic cancer. Cancer-associated fibroblasts (CAFs) are fibroblasts in a state of continuous activation, which are the most prominent components in TME. CAFs can promote the malignant biological behavior of pancreatic cancer through a variety of molecule-mediated mechanisms. Moreover, several attempts targeting CAFs for the treatment of pancreatic cancer have been largely unsuccessful. It may be related to the heterogeneity of CAFs in pancreatic cancer. Therefore, in-depth study of its heterogeneity and accurate targeting of some specific phenotypes and functional CAFs subtypes in the matrix on this basis may be more promising in the clinical treatment of pancreatic cancer.
5.Relationship between blood pressure variability and left ventricular diastolic function in patients with ;essential hypertension

Objective:To observe the relationship between dynamic blood pressure variability (BPV) and left ventric‐ular diastolic function in patients with essential hypertension (EH) .Methods :A total of 140 newly diagnosed EH pa‐tients were selected .According to total coefficient of variation (CV) of 24h systolic blood pressure BPV ,they were divided into high CV group (n=70 ,CV>12.16% ) and low CV group (n=70 ,CV≤12.16% ) .Another 70 healthy subjects with corresponding gender and age were enrolled as healthy control group simultaneously .Echocardiography was used to measure mitral early diastolic peak flow velocity (E) ,late diastolic peak flow velocity (A) and decelera‐tion time of E peak (DT);meanwhile ,tissue Doppler mode was used to record early diastolic peak velocity (Em) and late diastolic peak velocity (Am) .All above indexes were compared among all groups .Results:Compared with healthy control group ,BPV significantly rose in EH patients (P<0.05 or < 0.01) .All BPV indexes in high CV group were significantly higher than those of low CV group except daytime mean diastolic blood pressure variability (dDBP‐BPV) and nighttime mean diastolic blood pressure variability (nDBP‐BPV) , P<0.05 all .Compared with healthy control group ,there were significant reductions in E and Em/Am ,and significant rise in DT and E/Em in EH patients ,P<0.05 or <0.01 ;compared with low CV group ,there was significant rise in E/Em [(10.32 ± 3.20) vs .(14.22 ± 2.20) ,P=0.033] in high CV group .Conclusion:Left ventricular diastolic dysfunction is more severe in hypertensive patients with higher dynamic blood pressure variability .

OBJECTIVE:
To screen and identify the frequency and characteristic of mutation in stereocilium-related gene Taperin of Chinese prelingual nonsyndromic hearing impairment with DNA microarray combined with PCR.
METHOD:
One hundred and thirty-four patients of prelingual nonsyndromic deafness and one hundred health individuals in China were investigated in this study. Genomic DNA was extracted from the patients and was subjected to DNA microarray to screen mutations in 4 most common genes. The samples that carried none of the common mutant alleles were subjected to PCR and sequenced to detect mutations in Taperin gene.
RESULT:
Ninteen out of one hundred and thirty-four patients of prelingual nonsyndromic deafness were detected carring common deafness gene with DNA microarray. Taperin gene were detected in one hundred and fifteen patients with PCR. A187S was detected in Taperin as hetrozygous state in 2 patients and their unaffected members of their family. It occurred at the evolutionary conservation of the amino acids of taperin according to alignment analysis. Two polymorphism, 157C>T and 318C>T, were found in the patients and the control group.
CONCLUSION
A novel Taperin mutation, A187S was detected in Chinese patients with prelingual nonsyndromic hearing loss, which may be relevant to hearing loss. Two polymorphism, 157C>T and 318C>T, were found in Chinese in our research. The carrier frequency for Taperin mutation is about 1.74% of prelingual nonsyndromic deafness in Chinese patients.

8.Progress on biogas technology and engineering.

Dwindling supplies of conventional energy sources and the demand to increase the share of renewable energy for sustainability have increased the significance of biogas, the product of synergistic fermentation of biodegrable organic wastes from municipal, agricultural and industrial activities by microbial populations under anaerobic conditions. With extensive research and engineering practice, many technologies and modes have been developed for biogas production and application. Currently, the most widely used mode is the complete-mixing mesophilic fermentation. Europe, especially Germany, is leading the world in the combined heat and power production (CHP) from biogas. In this paper, updated progress in biogas technologies is reviewed, with focuses on anaerobic microorganisms, bioreactor configurations and process development, biogas production and applications, in which perspectives of biogas as a clean and renewable energy are projected.

9.Study on the effects of gonadotropin-releasing hormone analogues in the inhibition of ovarian cancer transplanted tumors and in the protection of ovarian function after chemotherapy on nude mice

Objective To investigate the influence of gonadotropin-releasing hormone (GnRH) analogues on ovarian cancer and ovarian function in vivo.Methods ES-2 cells were cultured and xenotransplanted into 36 nude mice,which were divided into 6 groups:normal saline (NS) group:NS 0.1 nd/day subcutaneous injection,and then NS 0.2 ml/week peritoneal injection; cisplatin (DDP) group:NS 0.1 ml/day subcutaneous injection,and then DDP 5 mg/kg ( diluted to 0.2 ml ) per week peritoneal injection; goserelin group:100 μg goserelin ( diluted to 0.1 ml) per day subcutaneous injection,and then NS 0.2 ml/week peritoneal injection; goserelin + DDP group:100 μg goserelin ( diluted to 0.1 ml) per day subcutaneous injection,and DDP 5 mg/kg (diluted to 0.2 ml) per week peritoneal injection; cetrorelix group:100 μg cetrorelix (diluted to 0.1 ml) per day subcutaneous injection and NS 0.2 ml/week peritoneal injection; cetrorelix + DDP group:100 μg cetrorelix (diluted to 0.1 ml) per day subcutaneous injection and DDP 5 mg/kg ( diluted to 0.2 ml) per week peritoneal injection.All the peritoneal injection started from subcutaneous injection one week later.To compare the weight of nude mice,the volumes of transplanted tumors,the expression of Ki-67 antigen in transplanted tumors,the estrus,the ratio of atretic follicles,the ratio of primary and preantral follicles,the levels of serum anti-Mullerian hormone ( AMH ),folliclestimulating hormone ( FSH),estradio ( E2 ) and progesterone (P) in each group.Results There were no significant difference in the weight of nude mice among 6 groups ( P > 0.05 ),which on day 29 in NS group was ( 19.8 ±2.2) g,DDP group (20.5 ± 1.4) g,gosereline group ( 19.6 ±0.9) g,goserelin + DDP group ( 19.7 ± 1.6) g,cetrorelix group (20.7 ±2.2) g,and cetrorelix + DDP group ( 19.0 ± 1.7) g.The tumor volumes of different groups on the 12th day:NS group (241 ± 179) mm3,DDP group (78 ±20) mm3,gosereline group (78 t±55) mm3,goserelin + DDP group (64 ±48) mm3,cetrorelix group (78 ±64) mm3,or cetrorelix + DDP group (70 ± 19) mm3,in which there were significant difference between NS group and the other groups ( P < 0.05 ) ; and the same result was obtained on day 15,19,22,26 and 29 ( P < 0.05 ).The expression of Ki-67 in NS group was ( 33 ± 10 ) %,in which it was higher than those in DDP group 3.5%,goserelin group 8.8%,goserelin + DDP group 1.5%,cetrorelix group (23 ± 11 ) %,or cetrorelix + DDP group ( 8 ± 6 ) % ( P < 0.05 ).The ratio of primary and preantral follicles in goserehn group was (71.5 ± 8.1 ) %,in goserelin + DDP group was (62.4 ± 4.1 ) %,in cetrorelix group was (71.2 ± 7.4) %,and in cetrorelix + DDP group was (63.8 ±3.1 )%,in which they were much higher than that in DDP group ( 47.0 ± 4.8 ) % ( P < 0.05 ).The level of AMH in goserelin group was ( 98 ± 27 ) ng/ml,which was much higher than that in NS group (66.2 ± 17.4) ng/ml (P <0.05),while there were no difference in the levelsof FSH,E2 or P among different groups ( P > 0.05).Conclusion GnRH analogues could inhibit the growth of transplanted tumors in nude mice,meanwhile increase the secretion of AMH,decrease the frequencies and prolong the lasting time of estrus,decrease the ratio of atretic follicles,raise the ratio of primary and preantral follicles,which may be protect the ovarian function of nude mice.

Experimental animal plasma lipid and metabolite p roduct obviously decreased against the control group. The result is shown as fo llows:in Rbl group total cholesterol (TC) 51.20%, P<0.01,triglyceride (TG) 35.45% P<0.05; Malouylaldehyd(MDA) 47.0 %P<0.01;but in Rg1 group TC 38.5;TG 26.28% P <0.05; MDA 25.9% P<0.05.
