1.Alteration of Superxied Dismutase in Diabetic Nephropathy After Type 2 Diabetes

Objective To investigate superoxide dismutase(SOD) activity in peripheric blood of type 2 diabetic patients(NIDDM) complicated with diabetic nephropathy(DN) and SOD alteration after insulin treatment.Methods Three groups of individuals were selected.Group A included 46 cases of NIDDM patients with DN.Group B included 43 cases of NIDDM patients with stable blood sugar.Group C included 40 cases of normal individual.Xanthine enzyme method was used to test SOD.Results The total SOD(T-SOD),CuZn-SOD,Mn-SOD in group A,B and C were 94 7?10 7,58 3?6 2 and 36 4?4 3;101 6?9 3,61 5?5 6 and 39 6?3 7;104 4?8 9,63 7?5 3 and 40 7?4 1;respectively.The SODs level in group A were significant lower than those in group B and group C(P
2.A Retrospective Analysis on the Management of Triplet and Quaternion Pregnancy in Prenatal Period

Objective To summarize the expreiences of management of triplet and quaternion pregnancy mothers and their in our hospital. Methods The clinical data of seven cases of multiple pregnancy (6 case of triplet and 1 case of quaternion) in our department from 1992 to 2003 were analyzed retrospectively. Results All seven patients had middle or severe anemia and hypoproteinemia in second and third trimester. The preterm sign happened in 2/3 cases in 32-36 gestation weeks. The patients were admitted in hospital in 27-34 gestation weeks because of obstetric complication. 6 triplet cases delivered during 35th-37th gestation weeks, except the quaternion one delivered at 33 gestation weeks. There were 22 newborns in those cases, the all babies Apgar's scores were above 9, the birth weights of all babies were above the 3th percent of Chinese singleton gestation and newborn estimated curves, and the birth weights of half babies were above the 10th percent position. Conclusion The key points for managing the multiple pregnancies, especially larger than 3 babies were the following ⑴All pregnant women had a good prenatal care. ⑵Pregnant women rested well, anemia must be corrected, and the adequate nutrition was supplied, especially in third trimester. ⑶Using ultrasound to determine the cervix length and evaluate the risk of preterm delivery, when the preterm delivery sign happened, the inhibiting drugs must be used cautiously and rationally. ⑷ After the antenatal dexamethasone therapy was applied around 36 gestation weeks, selective cesarean section was preferred to deliver the babies. If good prenatal care can be applied, a good outcome for triplets and quaternion can be achieved, close to that of same gestation weeks singleton.

Due to its effectiveness and specificity,RNA interference (RNAi) technology is widely used in the treatment of tumor,viral infectious and metabolic diseases.Noninvasively tracing the delivery of therapeutic RNA molecules in vivo and evaluating the therapeutic effects are important to promote the clinical application of RNAi technology.Molecular imaging allows noninvasive visualization of biological processes at the cellular and molecular level in vivo.It can be used to monitor the biodistribution of therapeutic RNA molecules and to assess the effects of the RNAi.This article reviews the advance of molecular imaging in monitoring the delivery of RNAi molecules and evaluating the therapeutic effects of RNAi.
4.Atrial myxoma: a clinical and pathologic analysis

Fifteen cases with atrial myxoma in our hospital from May 2001 to Dec 2008 were retrospectively analyzed.Patients presented with a variety of symptoms mimicking mitral valve disease,including palpitation, shortness of breath and activity intolerance.Pulmonary and systemic embolizations occurred in three patients and sudden syncope in one.Radiograph and echocardiography can help to reveal atrial myxoma.Definite diagnosis was made based on histological examinations.In 6 months - 6 years'follow-up one case recurred.Surgical treatment should be prompted once diagnosis established.Tumor pedicle must be completely removed to prevent recurrence.
5.Precision medicine is ammunition for anesthesiologists to improve patient outcomes during the perioperative period:genetic variability of beta-blockers and opioids

The evolving practice of precision medicine allows physicians to make disease treat-ments and prevention decisions based on a patient's individual genetic and molecular profile.In recent years,gene sequencing and related techniques are becoming more affordable and more accessible to healthcare providers,and their use in various medical fields continues to expand.In particular,there are numerous opportunities for the use of precision medicine in the perioperative setting.For example, individual polymorphisms in alpha and beta adrenergic receptors can improve the efficacy of beta blockade,or predispose a patient to adverse drug reactions including hypotension and bradycardia. Likewise,particular polymorphisms in opioid receptors can increase or decrease the effectiveness of various opioid medications for achieving adequate postoperative analgesia.In addition,mutations in the cytochrome P4502D6 (CYP2D6)enzyme can drastically affect the clinical response to a particular subset of beta blockers and opioids by accelerating or decelerating their metabolism and clearance. Preoperative genetic testing would allow anesthesiologists to identify these and other relevant molecu-lar characteristics in their patients,and choose appropriate perioperative therapies accordingly in order to maximize clinical outcomes while minimizing the incidence of adverse events.It is the time for anes-thesiologists and perioperative care providers to practice precision medicine.
6.Evaluation of right ventricular global and regional function in patients with metabolic syndrome by two-dimensional speckle tracking imaging

Objective To evaluate the right ventricular global and regional strain and strain rate in patients with metabolic syndrome(MS) by two‐dimensional speckle tracking imaging(2D‐STI) ,and explore the correlation between different MS criteria and global strain and strain rate of right ventricle . Methods Forty‐four MS subjects ,35 pre‐MS subjects and 40 healthy subjects were enrolled . The basal ,middle and apical systolic longitudinal peak strain( Ss) and strain rate( SRs) ,early diastolic and late diastolic peak strain rate(SRe ,SRa) of right ventricular free wall were measured using 2D‐STI and global strain and strain rate ( GSs ,GSRs ) ,early diastolic and late diastolic peak strain rate ( GSRe ,GSRa ) of right ventricle were calculated . The correlation between different MS criteria and GS ,GSR of right ventricle were analyzed . Results Compared with control group ,Ss ,SRs ,SRe and SRa in basal and middle segments in MS group and Ss ,SRs in basal segment in Pre‐MS group decreased( all P < 0 .05) . Ss and SRs in basal segment in MS group were significantly lower than those in Pre‐MS group(all P <0 .05) . Compared with control group , GSs ,GSRs ,GSRe and GSRa in MS group and GSs ,GSRs in Pre‐MS group decreased ( all P <0 .05) . GS and GRS in MS group and Pre‐MS group had no statistical significance( all P > 0 .05) . There were good correlation of waist circumference(WC) ,systolic blood pressure(SBP) ,fasting blood‐glucose with GSs( r =-0 .73 ,-0 .69 ,-0 .46 , P < 0 .01) and GSRs( r = -0 .63 ,-0 .78 ,-0 .48 , P < 0 .01) . The multiple regression analysis of MS criteria showed that waist circumference and systolic blood pressure were independently associated with GSs and GSRs of right ventricle(β= -0 .44 ,-0 .77 , P <0 .01) . Conclusions 2D‐STI could accurately assess right ventricular global and regional function in MS patients .Among all MS risk factors ,waist circumference and systolic blood pressure are the most responsible for right ventricular function .
7.Proifling on Invention Patents ofXi-Xinin China

Xi-Xin, a commonly used traditional Chinese herbal medicine, was regarded as one of the top grade materia medica since ancient time. The main medical parts were the root and stem. TheChinese Pharmacopeiaregulated the types ofAsarum heterotropoidesvar. mandshuricum,A. sieboldiivar. seoulense, andA. sieboldii. Analysis onXi-Xin related invention and patent information was an important aspect to understand the patent application condition and development tendency. It will also promote the research, development and industrialization of this medicine and promote its scientific result transformation. The Kongfz Patent Search Engine was used in the patent searching. Python and R languages were used in the collection, screening, classification, statistics, digging and analysis. In-depth analysis was made on the domesticXi-Xin related medical invention, patent application condition and development tendency. It provided references for the development ofXi-Xin. The results showed that an ever-increasing number of patents onXi-Xin were available. TheXi-Xin related inventions and patent applications were mainly from China, Republic of Korea, Japan, Russia, the U.S.A., etc. The main clinical applications were common cold, cough, headache, rheumatism and arthralgia, antibiosis, antiphlogosis, and cardiovascular diseases.Xi-Xin was mostly combined with herbal medicine with the effects of invigorating blood circulation and removing blood stasis, relieving exterior syndrome, and tonifying deficiency.
8.Development Analysis ofGinkgo Bilobain the Perspective of Invention Patent

Ginkgo bilobaL. is one of the most common used medicinal plants worldwide. The mainly medicinal parts are dry leaf and dry seed. Root and stem also have medicinal value. It has been only found in the wild in Tianmu Mountain, Zhejiang Province, China, while cultivated in a much wider area. Since ginkgo is an important plant resources, a comprehensive overview is necessary, which can help understand the current status and the development trends. In this study, the patent information fromEspacenet and World Traditional Medicine Databaseis used to outline the technology development of ginkgo in the perspective of invention patents. The results showed that ginkgo development currently in the early or mid-term maturity period. Ginkgo got the best development in China. Most international exchange of intellectual property happened in United States. More attention has been paid to the development ofG. biloba leaf. Ginkgo’s medicinal application and compatibility were also analyzed in this study.
9.Establishment and Implementation of Management Method of Second Kind Psychotropic Drugs in Outpatient Pharmacy

OBJECTIVE:To establish scientific and rational management of second kind psychotropic drugs.METHODS:The account book of second kind psychotropic drugs was set up according to specification of narcotic drugs and first kind psychotropic drugs.Daily radix management was carried out by exclusive person,account book and counter,as compared with traditional management.RESULTS & CONCLUSIONS:In this management method,checking time was decreased to 20 min from 50 min.The rate of consistency between account and drug was up to 98% from 95%.The method is superior to traditional one.
10.One-stage endoscopic sphincterotomy plus laparoscopic cholecystectomy for the treatment of cholecysto-choledocolithiasis

Objective To evaluate the safety and efficacy of one-stage endoscopic sphincterotomy plus laparoscopic cholecystectomy for the treatment of patients who have concurrent gallstones and CBD stones.Methods We conducted a prospective study from Mar 2008 to Nov 2011 in our department.A total of 63 consecutive patients of concurrent gallstones and CBD stones were included and divided into two groups:31 patients in test group first underwent ERCP and EST to remove CBD stones,and then during the same general anesthesia underwent laparoscopic cholecystectomy (LC) ; while 32 patients in control group first underwent ERCP and ES,and then LC 3-5 days later.Results The stone clearance rate in test group and control group were 96.9% and 96.7% (P > 0.05),respectively.The postoperative lung infection rate in the test group and the control group were 7.1% and 16.1% (P <0.05),respectively.The length of hospital stay (LOS) of the two groups was (7.5 ± 1.7) days and (12.6 ±2.5) days,respectively (P < 0.05).The total expense of hospitalization was (¥) (2 356 ± 126) and (¥) (37 056 ± 152),respectively (P < 0.05).Conclusions One-stage laparoendoscopic management of patients with concurrent gallstones and CBD stones is safe,effective and more economic approach than two-stage treatment for patients suffering from concurrent gall stone and choledocholithiasis.
