1.Effects of Angong Niuhuang Bolus on Vital Organ Injury and Mortality in Rats with Sepsis

Objective To observe the effects of Angong Niuhuang Bolus(ANB) on vital organ injury and mortality in rats with sepsis.Methods SD rats were randomized into normal control group,model group,and low-,middle-and high-dose ANB groups(in the dose of 1.0,2.0 and 3.0 g?kg-1?d-1 respectively).Except that the normal control group,the rats in other groups received cecal ligation and puncture(CLP) method to induce sepsis through abdominal infection.ANB groups were given gastric gavage of ANB 0.5 hours before CLP and 6 hours after CLP.After CLP for 24 hours,the mortality was observed,serum levels of alanine aminotransferase(ALT),total bilirubin(TB) and creatinine were detected,and pulmonary myeloperoxidase(MPO) activity in all of the rats was examined.Results Serum ALT and TB levels,MAO activity and 24-hour mortality were increased in the model group(P

[Objective] To observe the effect of Hewei Jiangni Decoction (HJD) for the treatment of stress ulcer and to explore its possible mechanism. [Methods] Sixty SD rats were randomized into 6 groups: normal, model, ranitidine (0.05 g?kg-1?d-1), and HJD groups (1.25, 2,50 and5.00g?kg-1?d-1 respectively). Except the normal group, the rats in other groups were treated with water immersion method to induce stress ulcer. Then the drugs according to the experimental design were administered to ranitidine and HJD groups for 5 days successively , and the model and normal groups were given saline. Gastric mucosal blood flow was detected by laser Doppler flowmeter, and inducible nitric oxide synthetase (iNOS) mRNA expression in gastric mucosa was examined by reverse transcriptase polymerase chain reaction (RT-PCR) . Obj ective In the body of stereotactic radiotherapy ( SBRT ) , using Cone Beam is the matc-hing precision of soft tissue and bone anatomy ,translation between evaluation organization and internal organiza-tion in the rotation of the lung cancer patients with tumor location error ,location correction .Methods Applica-tion varian companies in the United States has the airborne imaging system clinac -ix linear accelerator apparatus treatment of 8 cases of lung cancer patients .By CBCT and SBRT comparison before and after the treatment for each patient and evaluate internal instant translation and rotation of the body of the patients with SBRT postures , tumor location and tumor center of baseline shift score changes .Results Based on the difference between of the anatomy of the bone and soft tissue match position is 3.0 mm(0~8.3 mm).The patient′s tumor location in 3-d median change is 1.4 mm(0~12.2 mm)and 12.2 mm(0~13.2 mm).The median baseline offset between 3 d score is 2.2 mm(0~4.7 mm).The rest of the system error and random error about 1 °.Conclusion In Lung cancer SBRT transformation to improve treatment of lung tumor process , change the position of tumor center is mainly affected by the body′s internal organization instant translation and rotation .
4.Function of Contour in the sphincter-preserving surgery in low rectal cancer

Objective To study the function of Contour stapler in the sphincter-preserving surgery in 121 cases with low rectal cancer. Methods One hundred and twenty-one cases enrolled were divided into 2 groups randomly and accepted Contour or other kinds of staplers after operation respectively. Further comparison were performed on the rate of sphincter-preserving,anastomosis stricture and leakage,infection of incision and stool frequency between two groups. Results We found no significant differences in the comparison of rate of anastomosis stricture or leakage,stool frequency,but the Contour group had significant higher rate of sphincter-preserving (98. 3%)than other stapler group(68. 9%),and lower rate of infection of incision(3.94%)than control(11.9%)(P <0. 05 respectively). Conclusions It is secure,reliable and practical to use Contour stapler in operation on low rectal cancer,which can increase the opportunity of sphincter-preserving,decrease the the rate of infection of incision.
5.Effect of neoadjuvant chemotherapy on EC50 of propofol administered by TCI causing loss of consciousness in breast cancer patients

Objective To investigate the effect of neoadjuvant chemotherapy on EC50 of propofol given by TCI which produces loss of consciousness (LOC) in breast cancer patients.Methods Ninety ASA Ⅰ or Ⅱ breast cancer patients aged 30-60 yr scheduled for elective surgery were allocated into 3 groups according to the chemotherapy the patients received (n=30 each):group Ⅰ control (group C) received no chemotherapy;group Ⅱ taxol and group Ⅲ adriamycine+cyclophosphamide+5-Fu.The patients were unpremedicated.The EC50 of propofol given by TCI was determined by up-and-down technique.The initial effect-site concentration (Ce) of propofol was 2.07μg/ml and the ratio between the 2 successive Ce was 1.09.Loss of response to verbal command and eyelash stimulation was used as sighs of LOC.The EC50 of propofol was calculated.Results The EC50 of propofol causing LOC was significantly lower in group Ⅱ and Ⅲ than in control group.There was no significant difference in the EC50 of propofol which produces LOC between group Ⅱ and Ⅲ.Conclusion The EC50 of propofol causing LOC in breast cancerpatients is decreased by adjuvant chemotherapy.
6.One-stage endoscopic sphincterotomy plus laparoscopic cholecystectomy for the treatment of cholecysto-choledocolithiasis

Objective To evaluate the safety and efficacy of one-stage endoscopic sphincterotomy plus laparoscopic cholecystectomy for the treatment of patients who have concurrent gallstones and CBD stones.Methods We conducted a prospective study from Mar 2008 to Nov 2011 in our department.A total of 63 consecutive patients of concurrent gallstones and CBD stones were included and divided into two groups:31 patients in test group first underwent ERCP and EST to remove CBD stones,and then during the same general anesthesia underwent laparoscopic cholecystectomy (LC) ; while 32 patients in control group first underwent ERCP and ES,and then LC 3-5 days later.Results The stone clearance rate in test group and control group were 96.9% and 96.7% (P > 0.05),respectively.The postoperative lung infection rate in the test group and the control group were 7.1% and 16.1% (P <0.05),respectively.The length of hospital stay (LOS) of the two groups was (7.5 ± 1.7) days and (12.6 ±2.5) days,respectively (P < 0.05).The total expense of hospitalization was (¥) (2 356 ± 126) and (¥) (37 056 ± 152),respectively (P < 0.05).Conclusions One-stage laparoendoscopic management of patients with concurrent gallstones and CBD stones is safe,effective and more economic approach than two-stage treatment for patients suffering from concurrent gall stone and choledocholithiasis.

Objective To investigate the effect of Angong Niuhuang Bolus(ANB) on mRNA expression of high mobility group box-1(HMGB1) protein and myeloperoxidase activity in the lung of rats with sepsis induced by cecal ligation and puncture(CLP) method.Methods Seventy-five male SD rats were randomly divided into normal control group,model group,AG490(a Janusk kinase inhibitor) treatment group(subcutaneous injection of AG490 8mg/kg 0.5 h before CLP),rapamycin(RPM,a signal transduction and transcription activator inhibitor) treatment group(subcutaneous injection of RPM 0.4 mg/kg 0.5 h before CLP),and low-,middle-and high-dose ANB groups(gastric gavage of ANB in the dose of 1.0,2.0 and 3.0 g?kg-1?d-1 respectively).The animals were sacrificed 24h after CLP,and then lung tissue samples were collected to determine the expression of HMGB1 mRNA by the method of reverse transcription polymerase chain reaction(RT-PCR).Meanwhile,the activity of pulmonary myeloperoxidase(MPO) was also measured.Results Compared with the normal control group,the mRNA expression of HMGB1 was significantly enhanced(P
