1.Preventing peripheral central venous catheter-related bloodstream infections through process management

Objective To explore the role of process management in preventing peripheral central venous catheter (PICC)-related bloodstream infections (CRBSI). Methods A standard process management policy was established for the uniform management of PICC in our hospital. The incidences of CRBSI before and after the implementation of this policy were compared. Results The incidences of CRBSI was 6.0‰ (48/79 793 ) every 1000 catheter-days before process management and 2.9‰ (29/99643) after process management (P =0.000).Conclusion Standard process management can effectively prevent and control CRBSI caused by PICC.

Objective:To investigatethe transdermal delivery characteristics of methylphenidate hydrochloride (MPH) in vitro. Methods: Characteristics of MPH crossing nude rats skin were studied with Franz diffusion cells. A high performance liquid chromatographic (HPLC) method was established to determine the concentration of MPH crossed the skin. The permeability coefficient (P), steady state flux (J) and lag time(LT) for MPH through the skin of nude rats treated with various enhancers were compared with those of control. 3.Combined application of MS-275 and curcumin in inhibiting cell survival signaling pathways and inducing apoptosis of prostate cancer cells

Objective Curcumin and MS-275,an inhibitor of histone deacetylase (HDAC),are promising anti-tumor agents. The aim of present study was to investigate the mechanisms of apoptosis induced by combined use of MS-275 in low dosage and curcumin in prostate cancer cell line DU145. Methods MTT assay was used to evaluate the lethal effect on DU145 cells by solitary use of MS-275 and or combined with curcumin. The changes in cell life cycle was detected by flow cytometry. The expressions of the survival signaling pathways were determined by Western blotting. Results Solitary application of MS-275 or curcumin may inhibit the growth of DU145 cells in a time and dose-dependent manner. The combination of MS-275 (1?mol/L) and curcumin (20?mol/L) exhibited obvious cytotoxic effect on cell viability,which was only 45.9% 48h after the combined treatment. Cell cycle assay showed that the combination of MS-275 and curcumin resulted in obvious appearance of sub-G1 phase in DU145 cells,implying that the cell apoptosis had been induced. The results of Western blotting showed that after treatment of MS-275 or curcumin singly,the phosphorylation level of Akt and ERK kinase declined slightly,however,when MS-275 and curcumin were used together,there appeared a prominent inhibitory effect on Akt and ERK kinase,indicated by a sharp decline of their phosphorylation level,and at the same time,the level of cleaved PARP,a hallmark of apoptosis,was increased in DU145 cells. Conclusion Combined use of MS-275 and curcumin may exert a synergistic cytotoxic effect on the viability of DU145 cells,and exhibit an inhibitory activity on Akt and ERK kinases to induce apoptosis.
4.Experimental study of antisense epidermal growth factor receptor enhancing the radiosensitivity of human lung cancer cell line spc-a-1

Objective To explore whether antisense-EGFR could enhance the radiosonsitivity of human lung cancer spc-a-1 cell line.Methods The spc-a-1 cells were transfected with antisenso.EGFR-pcDNA3 by lipofectamine 2000(pcDNA3 antiEGFR group).Two other groups were used for comparison:control group(spc-a-1 cell without transfection)and pcDNA3 group(spc-a-1 cell transfeeted with pcDNA3 which did not contain antisense EGFR).Cell clones that stable expressing antisense-EGFR wa8 selected with G41 8 and the suppression of the expression of EGFR mRNA and protein were detected by RT-PCR and Western blot.The influence of antisense-EGFR on cell cycle was testified by flow cytometry assay.The cell apoptosis was analyzed by flow cytometry after 8 Gy irradiation.Further,cells of each group were irradiated with X-rays at the dose of 0,2,4,6 and 8 Gy.Dose-survival curve of each group was established by colony-forming assay.Results The expression of EGFR mRNA and protein were significantly inhibited after antisense-EGFR-pcDNA3 transfection.The cells arrested at the G2/M phase in the pcDNA3 antiEGFR group,control group and pcDNA3 group were (29.53±1.91)%,(13.7±1.30)%and(12.40±1.34)%,respectively.The apoptosis index of spc-a-1 cells in the antisonse-EGFR combined with irradiation group was obviously higher than that of the comparable groups [(39.24±1.57)%,(13.79±0.63)%and(15.02±0.85%)].The values of D0,Dq,SF2 of pcDNA3 antiEGFR group declined obviously compared with the control group(2.11,2.49,0.84 vs 1.19,0.15,0.32).Conclusions Antisense-EGFR could induce the G2/M cell cycle arrest,promote cell apoptosis and inhibit the ability of sublethal cell damage repair induced by irradiation,80 that it could significantly improve the radiosensitivity of spc-a-1 cell in vitro.
5.Mutuality analysis on quickly evaluate the traumatic degree of abdominal solid-organs with contrast-enhanced ultrasonography

Objective To quickly evaluate the traumatic degree of abdominal solid-organs using contrast-enhanced ultrasonography (CEUS) and analysis on related factors with clinical treatment.Methods 52 patients with abdominal traumatic were observed by CEUS,and the traumatic degree was judged according to American Association for the Surgery of Trauma (AAST).The change of peritoneal fluid was observed with ultrasonography,and active bleeding and involve adjacent vessels their branches were observed with CEUS.In this way,a method of quickly evaluate the traumatic degree was established,and the correlation between indifferent grade trauma and appropriate interventions that include surgical and conservative treatment was studied.Results 52 patients with 71 lesions,compound injuries accounted for 82.7% (43/52).Among them,37 lesions were Ⅰ-Ⅲ grade trauma,34 lesions were severe trauma of Ⅳ-Ⅴ grade.The lesions complicated with active bleeding were 76.1% (54/71).The amount of peritoneal fluid was increased significantly within 30 min (P <0.05) in traumatic lesions with rapid bleeding.Among of 50lesions associated with active bleeding,the surgical treatment was 24.0% (12/50),the conservative treatment was 76.0% (38/50).Among of trauma lesions involving the two following vessels,Ⅰ-Ⅲ grade was 97.3% (36/37),Ⅳ-Ⅴ grade was 61.8% (21/34).Trauma involvement above level 2 focal blood vessels,surgical treatment accounted for 23.1% (12/52),conservative treatment accounted for 44.2%(23/52).Conclusions The severity of the trauma can not be a comprehensive response by AAST,becauce it is not only related to the scope of the traumatic lesions,vascular level,also involved with the trauma associated with active bleeding,bleeding speed and amount of peritoneal effusion and other factors.
6.Combined haemostatic percutaneous injection therapy for renal trauma under guidance of contrast-enhanced ultrasonography

Objective To study feasibility of combined haemostatic percutaneous injection therapy guided by contrast-enhanced ultrasonography (CEUS) in treatment of renal injuries. Methods Eighteen New Zealand rabbits were inflicted with kidney injury imitating grades Ⅲ-Ⅳ blunt injuries. The animals were randomly and equally divided into three groups, Group A ( treated with hemocoagulase),Group B ( treated with hemocoagulase and Alpha-cyanoacrylate) and Group C ( control group, given normal saline). The hemostatic time, hemostatic effect, and perirenal hematoma were observed. Results A perirenal hematoma was observed one hour after treatment. The perirenal fluid thickness was (0.200 ±0.012) cm in Group A, (0.050 ±0.002) cm in Group B and (0.400 ±0.009) cm in Group C, with statistical significance between two test groups and Group C (P < 0.05 ). At days 7 and 14 following treatment, lesion length and cross section was ( 1. 107 ±0. 143) cm and (0.433 ±0. 163) cm in Group A, (0.567 ±0.082) cm and (0. 160 ±0. 078) cm in Group B, and (0.980 ±0. 203) cm and (0.686 ± 0. 157) cm in Group C. There was statistical significance between the test groups (Groups A and B) and Group C (P<0. 01) at day 14. The lesion size in Group A was lager than that in Group B (P < 0.01 ). One month after treatment, a slight nephrohydrosis occurred in Group B. Conclusions Either injection of simple hemocoagulase or combined use of hemocoagulase and Alpha-cyanoacrylate guided by CEUS can attain positive hemostatic effect, but the latter one is more rapid and reliable.
7.Haemostatic percutaneous injection therapy for renal trauma guided by contrast-enhanced ultrasonography

Objective To study haemostatic percutaneous injection therapy for the management of vascular damage in patients with renal injuries guided by contrast-enhanced ultrasonography(CEUS).Methods Which of 56 patients with renal trauma were diagnosis by CEUS,37 cases with grades Ⅱ-Ⅳ renal injuries were brought into our study.According to wound degree and accompanying active bleeding,they were divided into experiment group (percutaneous injection hemostatic treatment)and control group(conservative treatment).Results Thirty-seven renal trauma manifest low perfusion in lesions by CEUS,and the contrast agent could be seen overflow to renal pelvi and the location of capsule in 13 patients.The patients were divided into experiment group(17 cases)and control group(20 cases).The color of hematuria of 9 patients in experiment group became gradually light at 30 mins after treatment.and the color of 7 cases become normal,and hematuria of the only one was iterative appear.The color of hematuria of 9 patients in control group became gradually light in 24-72 hours,others' hematuria became gradually light in 5-14 days.The time of color of hematuria become light of the former was shorter than those of the latter(P<0.05).Reexamination by ultrasound and renal function and urine routine at 1,3 and 6 months after treatment,the results of all patients indicated normal.Conclusions Haemostatic percutaneous injection therapy for renal trauma guided by contrast-enhanced ultrasonography has very obvious hemostatic efficacy.Its advantages included may be used for effective,minimally invasive control of renal injuries(grades Ⅱ-Ⅳ),and can be a feasible management of active bleeding at bedside.

9.The diagnosis applying effects of ocular vestibular evoked myogenic potentials in BBPV disease.

OBJECTIVE:
To investigate the diagnosis applying effects of ocular vestibular evoked myogenic potentials(oVEMP) in peripheral BPPV disease.
METHOD:
During September 2012 to January 2015, we selected 80 healthy people in our hospital medical center as the control group, choose the same period of primary benign paroxysmal positional vertigo as the observation group of 80 patients. Two groups were carried out fully functional auditory evoked potential analysis, determination of oVEMP and cervical vestibular evoked myogenic potentials (cVEMP) anomaly amplitude threshold, P1 latencies, N1 incubation period.
RESULT:
The cVEMP abnormal rate in the observation group was 28.8%, the oVEMP abnormal rate was 38.8%, while cVEMP and oVEMP abnormal rates in the control group was 1.3% and 2.5% respectively that compared to significant differences between the two groups (P < 0.05). The oVEMP test amplitude in the observation group was (5.98 ± 2.15) µv, the N1 incubation period was (10.03 ± 0.76)ms, while the control group were (4.09 ± 2.11)µv and (11.67 ± 0.78) ms that compared difference were statistically significant (P < 0.05). The cVEMP test amplitude in the observation group was (154.8 ± 43.9)2 µv, while the control group was (180.49 ± 45.34)µv, compared the difference was statistically significant (P < 0.05).
CONCLUSION
Paroxysmal positional vertigo patients ocular vestibular evoked myogenic potentials abnormal rate is relatively high, the utricle dysfunction for more severe than the balloon can be the subject of an objective function of the ear stone judgment, judgment in favor of the disease.

10.Identification of role of PI3K in mediating necroptosis of L929 cells induced by tumor necrosis factor alpha

Objective To identify the role of phosphatidylinositol-3-kinase(PI3K) in mediating necroptosis induced by tumor necrosis factor alpha (TNFα) and the involved mechanism.Methods Knockdown of p110α,receptor-interacting protein 1(RIP1) or both p110αand RIP1 was mediated by the specific short hairpin RNA (shRNA) lentivirus and verified by RT-PCR or Western blotting .In addition , Western blotting was used to detect phosphorylation of mixed lineage kinase domain-like protein(MLKL) and protein kinase B(AKT) or tetramerization of MLKL.Cell death was measured by micros-copy and flow cytometry.Results AKT phosphorylation and TNFα-induced necroptosis of L929 cells were suppressed by the inhibitors of PI3K or AKT, as well as p110αknockdown.Moreover, RIP1 knockdown did not inhibit L929 cell death induced by TNFαplus Z-VAD, but the RIP1-independent necroptosis was inhibited by p 110αknockdown.In addition, p110αknockdown suppressed MLKL phosphorylation and tetramerization induced by TNFαwith Z-VAD in L929 cells. Conclusion PI3K mediates necroptosis of L929 cells induced by TNFαby activating AKT and MLKL, respectively.
