1.Changes in arterial inflow after flap grafting under various tensions

AIM: To explore the curative effects of various approaches to improve the arterial inflow following flap grafting with various tensions. METHODS: 135 patients with skin defects in limbs, who underwent flap grafting in Department of Orthopedics, Baoan People's Hospital of Southern Medical University between August 2002 and August 2006, were selected, and informed the consent. The wounds of every 45 cases were sutured under the tension of 1.0 kg, 1.5 kg, and 2.0 kg, respectively; meanwhile, all patients were randomly divided into hyperbaric oxygen group, dexamethasone group, and local injection of heparin group with 15 cases in each group, and given the corresponding treatment besides ordinary treatment. Seven days later, the flap survival was observed. RESULTS: 135 patients were all involved in the result analysis without any loss. The flaps of 90 patients recovered completely under the tension of 1.0 kg and 1.5 kg. But under 2.0 kg tension, the flaps of 10 penitents in the hyperbaric oxygen group recovered, 4 survived mostly and 1 necrosed; in the dexamethasone group, 5 flaps recovered, 3 survived mostly, and 7 necrosed; in the local injection of heparin group, 3 flaps recovered, 3 survived mostly, other 9 necrosed completely. CONCLUSION: The suture tension of 1.0 kg to 1.5 kg is feasible in flap grafting, and compared with heparin and dexamethasone, hyperbaric oxygen could better promote the arterial inflow under high tension like 2.0 kg.
2.Adipose-derived stem cell transplantation for osteoporosis rats:evaluation by bone mineral density and histomorphometry measurement

BACKGROUND:Stem cel transplantation is increasingly hoped to promote osteoblast differentiation and inhibit osteoclast proliferation in the treatment of osteoporosis.
OBJECTIVE: To study the therapeutic effect of exogenous adipose-derived stem cel (ADSC) transplantation on osteoporosis in ovariectomized rats.
METHODS:Thirty Sprague-Dawley female rats were equivalently randomized into sham, model, ADSC transplantation groups. Rats in al groups except the sham group underwent bilateral ovariectomy to make osteoporosis models. Surrounding adipose tissues instead of the ovary were removed in the sham group. After modeling, rats were given 2×106ADSCs at passage 4via the tail vein in the transplantation group and the same volume of normal saline in the model group, once a week. After 6 weeks, levels of serum calcium, phosphorus, and alkaline phosphatase as wel as bone mineral density and histomorphometry indicators were detected in rats.
RESULTS AND CONCLUSION:Compared with the sham group, the trabecular bone volume fraction was significantly decreased in the model group (P < 0.01), but remarkably increased after ADSC transplantation (P < 0.05). After modeling, the bone trabecular absorption surface percentage and rate of bone trabecular formation were elevated significantly (P < 0.05 orP < 0.01), while these increases were improved by ADSC transplantation (P < 0.05). Additionaly, the levels of serum calcium and alkaline phosphatase and bone mineral density were significantly decreased after modeling, but were increased after ADSC transplantation. In contrast, the serum level of phosphorus was significantly increased in the model group (P < 0.05) but decreased markedly in the ADSC transplantation group (P < 0.05). To conclude, ADSC transplantation can reduce the loss of bone mass in osteoporosis rats by ovariectomy.
3.Change of tear film function in seasonal allergic conjunctivitis between acute exacerbation and non-onset phase

Objective To investigate the change of tear film function in patients with seasonal allergic conjunctivitis (SAC) between acute exacerbation and non-onset phase.Methods This was a prospective controlled study.Functional assessment of tear film was performed in 30 eyes of 15 patients with SAC (SAC group) between acute exacerbation and non-onset phase and 15 healthy controls (control group).The tear film function included tear film break-up-time (BUT),Schirmer I test (SIt) and tear film interferometer imager measurement.Results BUT was significantly decreased in SAC group on acuteexacerbation compared with that on non-onset phase and control group [(6.97 ± 1.56) s vs.(11.27 ± 1.39),(12.00 ± 1.11) s],and there was significant difference (U =20.50,P =0.000;U =1.00,P =0.000).Moreover,BUT in SAC group on non-onset phase was similar as control group(U =322.00,P > 0.05).There was no significant difference in SIt among SAC group on acute exacerbation and non-onset phase and control group(P > 0.05).In tear film interferometer imager measurement,80.0%(24/30) was Ⅰ-Ⅱ grade,20.0%(6/30) was Ⅲ grade in control group,20.0%(6/30) was Ⅰ-Ⅱ grade,80.0%(24/30) was Ⅲ-Ⅴ grade in SAC group on acute exacerbation,60.0%(18/30) was I-Ⅱ grade,40.0%(12/30) was Ⅲ-Ⅴ grade in SAC group on non-onset phase,and there was significant difference between SAC group on acute exacerbaiion and SAC group on non-onset phase,control group (x2 =19.27,P =0.000; x2 =8.40,P =0.004),and there was no significant difference between SAC group on non-onset phase and control group (x2 =1.98,P>0.05).Conclusion SAC can cause the instability of tear film during the acute exacerbation,whereas this instability can be recovered within the non-onset phase of S A C,which is close to the normal control

Objective To study the effect of intra arterial thrombolysis in combination with basic fibroblast growth factor (bFGF), and the protecting mechanism of the bFGF. Methods 50 male Wistar rats were subjected to middle cerebral artery occlusion (MACO) for 4 hours by a single fibrin rich clot. The rats were randomly assigned to the following two expremental groups: 1) treatment with urokinase alone; 2) treatment with urokinase and bFGF. This review focuses on the fetal origins of adult disease hypothesis put forward by David Barker and his colleagues,recent advances in epidemiological studies and experimental research in this field.Barker Hypothesis states that environmental factors,particularly intrauterine nutrition,as indicated by birth weight,operate in early life to program the risks for adverse health outcomes in adult life.A large growing body of reports described the association between the early development and adult diseases,such as diabetes,hypertension,coronary heart disease,abnormal lipids metabolism,obesity and cancer,etc.Experimental studies show that the changes of some key genes' expression,caused by epigenetic modifications,lead to a permanent alteration of cellular proliferation and differentiation and finally the genesis in key tissues and organs.These results bring about the impairment in structures and functions and the increased susceptibility to chronic diseases in adult life.The hypothesis provides a new perspective for the prevention and therapy of chronic diseases.

9.Application of diffusion weighted imaging in severity assessment and prognosis prediction of diffuse axonal injury

Objective To discuss the value of diffusion weighted imaging (DWI) in severity assessment and prognosis prediction of diffuse axonal injury (DAI). Methods A retrospective study was performed on the clinical imaging data and the follow-up results at six months after injury in 29 patients with DAI. The detection rate of DAI lesion by DWI and conventional MRI was compared by means of one-way ANOVA. The correlation between the number of lesion in different areas with GCS and GOS was analyzed with Spearman rank correlation test. Results (1)The average detected DAI lesions were 19.24±5.72 on DWI, 14.41 ±4.50 on FLAIR, 10.58±3.79 on T2WI and 4.83 ±2. 11 on TIWI, with the highest detection number of DAI lesion on DWI (P < 0, 05). (2) There was a significant correlation of the number of central lesions (in corpus callosum, basal ganglia and brain stem) with GCS and COS (P < 0. 05), but there was no correlation of total lesion number or periphery lesion with GCS and COS (P > 0.05). Conclusions DWI is a potentially useful imaging modality in detecting DAI lesion, when the number of central lesion on DWI can be served as an objective marker in severity assessment and prognosis prediction of DAI.
10.Application of microcurrent detector to intraoperative diagnosis of 101 cases of tumor

Objective To investigate the relationship of tumor malignancy with Na+ microcurrent measured by microcurrent detector in order to develop a novel approach for rapid intraoperative diagnosis of tumor.Methods 101 cases of various tumors were studied.Na+ microcurrent of tumor tissue or surrounding normal tissue was measured by Na+ microcurrent detector.All tumor tissues were sampled for histopathological examination as well.Results In 97 cases(96%),the Na+ microcurrent level in tumor tissue was significantly different from that in surrounding normal tissue.It was greater than 30?A in tumor tissue with the maximum of 60-70?A,and averagely 20?A in surrounding normal tissue.The consistency of Na+ microcurrent detection and pathological examination was 96%.Conclusion the measurement of Na+ microcurrent by microcurrent detector is rapid,convenient and accurate,which may become a novel approach for rapid intraoperative diagnosis of tumor malignancy.
