1.Clinical analysis of bladder and urethra leiomyoma combined with uterine leiomyoma

Objective To discuss the cause, clinical features, diagnosis, treatment and prognosis of bladder and urethra leiomyoma combined with uterine leiomyoma. Methods The clinical data of 5 patients with bladder and urethra leiomyoma combined with uterine leiomyoma was analyzed retrospectively. All patients underwent surgical treatment, 3 cases were treated with enucleation of bladder leiomyoma, 1 case was treated with transurethral resection of bladder tumor (TURBT), 1 case was treated with enucleation of urethra leiomyoma,at the same time all patients were underwent hysteromyoma surgery, 1 case was treated with enucleation of partial uterine, 4 cases were treated with enucleation of hysteromyoma. Results Pathology examination showed that 4 cases were bladder leiomyoma, 1 case was urethra leiomyoma,5 cases combined with uterine leiomyoma. Patients were followed up 1-3 years,no tumor recurrence and metastasis. Conclusions Bladder and urethra leiomyoma is a kind of benign non-epithelial tumor without specific symptom,clinical rare,often merging with uterine leiomyoma, ultrasonography ,CT and cystoscopy scan could be main techniques for diagnosis mainly with surgical resection,prognosis is good. From clinical discovery bladder leiomyoma must notice patient whether at the same time merges with uterine leiomyoma.
2.Significance of changes of plasma levels of homocysteine and adrenomedullin in elderly patients with sudden sensorineural hearing loss

Objective To explore the relationship between levels of plasma homocysteine (Hcy) and adrenomedullin (ADM) and sudden sensorineural hearing loss (SSHL) in elderly patients. Methods The 80 patients with SSHL but without any treatment were selected as SSHL group, while 80 age-and sex-matched subjects without SSHL were collected as the control group. The levels of plasma Hcy and ADM were measured by fluorescence method and radioimmunoassay, respectively. And the levels of plasma Hey, ADM, total cholesterol (TC) and triglyceride (TG) were compared between the two groups. Results The levels of plasma Hcy, ADM, TC and TG were significantly higher in SSHL group than in control group [(13. 81±2.88) μmol/L vs. (11. 58±2. 80)/xmol/L, (26. 40±3. 20)ng/L vs. (22. 45±2. 80)ng/L, (5.22±0. 87)mmol/L vs. (4. 52±0. 81)mmol/L, (1.80 ±0. 12)mmol/L vs. (1.52±0. 13)mmol/L;t=5. 03, 8. 22, 7.48, 3. 85, all P<0. 01). When the two groups with hypertension were compared, the levels of plasma Hey, ADM, TC and TG were (15.30±2. 77)μmol/L, (29.40±2. 80) ng/L, (5. 48±0. 98) mmol/L and (1.92±0. 18) mmol/L in SSHL group, which were higher than those in control group[(12. 80±2. 62) μmol/L, (25.20± 2. 60) ng/L, (5.01±1.03) retool/L, (1.62±0. 15) mmol/L3 (t=4.03, 7.01, 3.25, 7. 80, all P< 0. 01). There were no differences in systolic and diastolic blood pressure between the two groups(t= 0. 87, 0. 64;P=0. 38, 0. 55).Conclusions The changes of plasma levels of Hey and ADM in the elderly suggest that they could involve in the pathogenic process of SSHL, which may provide referential value for understanding the state of the disease and judging the prognosis
3.Clinical significance of detecting levels of serum and ascitic PCT in patients with liver cirrhosis complicating spontaneous bacterial peritonitis

Objective To study the changes of serum and ascitic procalcitonin (PCT ) level in the patients with liver cirrhosis complicating spontaneous bacterial peritonitis (SBP) and to investigate the value in its early diagnosis .Methods 92 patients with cirrhotic and ascites were detected the levels of serum and ascitic PCT by the immunofluorescence method and the detection results were compared with the ascites routine and the bacterial culture results .Results Serum levels of PCT in the SBP group and the non-SBP group were obviously higher than those in the control group ,the difference was statistically significant (P<0 .05);Serum and ascitic levels of PCT in the SBP were significantly higher than those in the non-SBP group ;The sensitivity of serum PCT for di-agnosing peritonitis was 89 .3% ,the sensitivity of ascitic PCT for diagnosing peritonitis was 71 .4% .Conclusion The levels of ser-um and ascitic PCT have the important value for the early diagnosis of liver cirrhosis ascites ,moreover serum PCT level has higher value for early diagnosing liver cirrhosis ascites .

5.A modified liver cirrhosis portal hypertension model in canines induced by subcutaneous injection of carbon tetrachloride

Objective To find an ideal liver cirrhosis portal hypertension model in big animals suitable for surgical study.Methods Twenty canines were treated by subcutaneous injection of 60% CCl4 colza oil emulsion into the back,at a dose of 1.0-1.3 mL/kg every 10 days for a total of 6-8 times.At the same time,it was combined with the control of food intake.All canines were fed with rice mixed with 10% hog fat.The amount of rice was 15 g/kg per day from the first day to the forth day,but the amount of food was not controlled from the fifth day to the tenth day.During the process,the canines′ general condition was observed and the changes of hepatic functions such as ALT,TP,ALB,G and A/G were measured.The changes of liver morphology,the diameter and blood flow of portal vein were monitored with color Doppler.Every two weeks,a piece of hepatic tissue to observe the pathological change of liver was excised operatively.Results During the process,the body weight of the canines decreased continually;ALT increased during early and middle stages and decreased during advanced stage;TP and ALB always decreased,and A/G continuously decreased,but G did not change significamtly.Eight weeks after injection,liver became progressively smaller.its density was not well-distributed,and liver particles gradually became coarse,and liver capsule became rough.The diameter of portal veins became larger and the velocity of portal vein became slower.Liver cirrhosis with psudolobules was found on light microscopy from the tenth week to the twelfth week.Three canines died,with mortality of 15.0%.Conclusions It took 10-12 weeks to establish this liver cirrhosis portal hypertension models in canines by subcutaneous injection of CCl4.This method is convenient,has high success rate,and is suitable for use in surgieal research.
6.Controlled study on different hepatic blood flow occlusion in hepatectomy

Objective To evaluate the indications of different hepatic blood flow occlusion in hepatectomy of cancer patients. Methods One hundred and fifty-six patients admitted between 1991-2001 underwent hepatectomy with hepatic blood flow occlusion in different ways, among them 48 cases underwent hepatic segmentectomy with segmental portal vein occlusion by a balloon catheter, 71 cases underwent hepatectomy with porta hepatis occlusion by Pringle method, 37 cases treated by hemihepatectomy or partial hepatectomy with hemihepatic occlusion. Results Intraoperative blood loss in patients using balloon catheter was smaller and postoperative liver function recovered faster compared with other ways of blood flow occlusion. Conclusion The preliminary result shows that hepatic segmentectomy with segmental portal vein occlusion by a balloon catheter is safe and useful technique for hepatectomy.
7.Effect of the chloride channel activity on vascular tone in the spontaneously hypertensive rats

AIM: To study the chloride channel activity [ICl_(Ca)] in vascular smooth cells of the spontaneously hypertensive rats (SHR). METHODS: The vascular beds of mesenteric arteries were isolated from the pentobarbital anesthetized rats and perfused with 37 ℃ PSS at a constant flow rate. The vasoconstriction response to norepinphrine (NE) was determined by changes in perfused pressure. The strips of the rat arteries were mounted in an organ chamber filled with 37 ℃ PSS and the vascular tension was measured. RESULTS: (1) The contractile responses of mesenteric arteries to NE in SHR were greater than that in Wistar rats. (2) The inhibitory magnitude of the contractile response by niflumic acid in SHR was significantly less than that in Wistar rats and showed dose-dependent manner. (3) Decreasing the extracellular Cl~- concentration increased the contractile response to NE significantly and the amplitude of enhanced contractile response in SHR was greater than that in Wistar rats. CONCLUSION: It can be concluded that NE-induced contraction is enhanced in SHR, which is partly due to an increase in Cl~- efflux through the Ca~(2+)-activated Cl~- channels. The chloride channel activity may increase in association with the elevation of vascular tone and blood pressure.
8.Effect of Herbal Tanreqing-injection Combined with Vancomycin in Treatment of MRSA Pneumonia and Antibiotic Resistance of the pathogens

OBJECTIVE To investigate(meticillin-resistant Staphylococcus aureus MRSA) the antibiotics resistance and curative effect of herbal Tanreqing-injection combined with vancomycin in the treatment of MRSA pneumonia.METHODS All data of microbiology about sputum culture between 2001-2007 were statistically analyzed with a retrospective method in a hospital,in order to understand the MRSA prevalence trends and their resistance.From 100 cases of MRSA pneumonia,50 cases were treated by vancomycin alone,another 50 cases were treated by Tanreqing injection plus vancomycin.RESULTS There were 3082 MRSA strains and 162(meticillin-sensitive S.aureus MSSA) strains in 3244 S.aureus strains;nosocomial infection cases accounted for 78.07% in MRSA-positive sputum samples;MRSA wasn't resistant to vancomycin,but resistant to other antibiotics.Resistance rates had a significant difference between MRSA and MSSA(P

Objective To evaluate clinical safety and efficacy of autologous tissue antigens loaded dendritic cells (DC)in the treatment of elderly patients with gastric cancer.Methods Fresh tumor cells were isolated from surgical specimens in twenty patients with gastric cancer. Single cell suspensions were obtained and induced to apoptosis by heat shock. The apoptotic tumor cells were frozen before use.Peripheral blood mononuclear cells were isolated and cultured with recombinant human granulocyte macrophage-colony stimulating factor (GM-CSF) and interleukin-4 (IL-4).DCs were loaded with apoptotic tumor cells. The immunological and clinical responses were examined after the final vaccination. ResultsVaccination of dendritic cells was well tolerated and no toxicity was observed. The cytokine levels in serum such as IL-2(46 pg/mg vs 89 pg/mg), IL-12(44 pg/mg vs. 86 pg/mg) and IFN-γ(38 pg/mg vs.82 pg/mg) were increased after vaccinations as compared with pretherapy. DTH test were positive in five patients (5/10). The antigen special cells for CD8+/ IFN-γ+ were increased in five patients(5/10). The size of retroperitoneal lymph node of one patient was reduced. The tumor marker CEA level were decreased in five patients, stable in eight patient and increased in two patients.Conclusions Autologous DC vaccines loaded with autologous tumor antigens could improve function of non specific immunity and elicit specific T cells immunologic response and is one of safe and effective treatments for gastric cancer.
10.The effects of TPN on postoperative patients with colon or rectum cancer(clinical analysis of 42 cases)

Objectives:To study effects of TPN on postoperative patients with colon or rectum cancer,including gut function recovery,wound healing and immune function. Medthods:42 elder patients with colon or rectum cancer were divided into two groups at randomization,TPN group and control group.From POD+1 to POD+8,TPN group was given TNA daily and control group was given solutions of glucose and electrolyte.Gut function recovery and wound healing were observed and albumin,pre-albumin,IgG,IgA,IgM,CD3+,CD4+ and CD8+ were determined on POD+1 and POD+8. Results:The recovery of gut function and wound healing in TPN group were better than those in control group,and levels of pre-albumin,IgG,IgA were higher than those in control group. Conclusions:TPN has positive effects in gut function recovery,wound healing and immune function in postoperative patients with colon or rectum cancer.
