1.An analysis on clinical efficacy of Qinchai Kangdu mixture for treatment of infantile EB virus infectious mononucleosis with flaring heat in Qifen and Yingfen

Objective To observe the clinical efficacy of self made Qinchai Kangdu mixture for treatment of infantile EB virus (EBV) infectious mononucleosis and explore its potential mechanism. Methods A prospective study was conducted. One hundred and twenty children with pediatric EBV infectious mononucleosis and traditional Chinese medicine (TCM) syndrome of flaring heat in Qifen and Yingfen in Wuhan Children Hospital were randomly divided into observation group and control group (each, 60 cases). Conventional western medical treatment was given to the two groups for 7-10 days;the observation group was additionally given Qinchai Kangdu mixture which included the following ingredients:Scutellariae Radix 10 g, Bupleuri Radix 10 g, Artemisiae Annuae Herba 10 g, Forsythiae Fructus 20 g, Arctii Fructus 10 g, Paeoniae Radix Rubra 10 g, Cinnamomi Ramulus 6 g, Moutan Cortex 10 g, Trionycis Carapax 10 g, Isatidis Radix 20 g, Citri Reticulatae Pericarpium 10 g, Glycyrrhizae Radix 10 g, orally, once daily for 7-10 days. The times of returning to normal for clinical symptoms and signs, white blood cell count (WBC), peripheral blood abnormal lymphocyte ratio and the level of alanine aminotransferase (ALT) were observed in the two groups. And the clinical efficacy, changes in score of TCM syndrome and adverse reactions were also investigated in the two groups. Results The total effective rate in the observation group was significantly higher than that in the control group [93.33%(56/60) vs. 76.67%(46/60), P<0.05]. In observation group, the duration of fever (days:6.9±2.3 vs. 7.7±3.4), and the recovery times to normal for body temperature (days:2.9±1.4 vs. 4.8±1.7), angina (days:7.2±3.3 vs. 8.0±3.2), lymph node reduction in size (days: 6.6±2.1 vs. 10.2±2.0), enlarged liver bounce back in size (days: 7.8±1.7 vs. 9.6±2.0), WBC (days:7.22±1.78 vs. 10.67±1.97), peripheral blood abnormal lymphocyte ratio (days:7.24±1.86 vs. 11.15±1.65), and ALT (days:8.44±1.83 vs. 11.43±2.65) were all significantly shorter than those in the control group (all P < 0.05). No adverse reactions occurred in the therapeutic course in the two groups. Conclusions Qinchai Kangdu mixture can effectively improve the clinical symptoms of infantile EBV infectious mononucleosis with TCM syndrome of flaring heat in Qifen and Yingfen. The mixture may also promote the enlarged liver and spleen to contract in size at early stage and peripheral blood abnormal lymphocyte ratio and other laboratory indexes return to normal in time.

Objective To evaluate the efficacy of traditional Chinese medicine which eliminating necrotic tissues and promoting granulation on pressure ulcer of Ⅲ to Ⅳstage using Meta analysis.Methods RCTstudies of therapy of eliminating necrotic tissues and promoting granulation traditional Chinese medicine for stage Ⅲ~Ⅳ pressure ulcer were searched,in which the control groups used routine methods,while the experimental groups adopted necrotic tissues and promoting granulation traditional Chinese medicine.Two reviewers assessed the documents'quality separately and completed data extraction.The statistical analysis was performed using the Cochrance collaborations' Rev Man 5.1 software.Results The cure rates for pressure ulcers of the experimental groups were higher than the control groups.The time of recovery of the experimental groups were shorter than the controls.The dressing change frequency of the experimental groups was less than the controls.Conclusions Traditional Chinese medicines which eliminating necrotic tissues and promoting granulation can significantly improve the cure rate,shorten the therapeutic time and reduce the frequency of dressing change.
3.The design and application of the machine for making animal model for pressure ulcer

Objective To develop a kind of new machine for making animal model for pressure ulcer,and inspect its effect through experiments,in order to lay the foundation for the research of pressure ulcer experiments on animals.Methods This study developed the machine after reviewing the domestic and foreign literature,making full use of the existing experimental platform of our university.Then 55 Sprague Dawley(SD) rats were selected,after anesthesia and the skin preparation,the researchers imposed certain pressure with 70 mmHg/cm2 (1 mmHg=0.133 kPa) on the skin and muscle tissue on the inner left thigh of SD rats by using self-designed machine,pressing for 2 h,then reperfusion for 30 min,3 times a day,a total of 7 days.Results The authors developed a kind of new machine for making animal model for pressure ulcer,and successfully prepared Ⅲ phase pressure ulcers model in SD rats with success rate of 98.2%(54/55).Conclusion This machine can prepare Ⅲ phase pressure ulcers model on animals,it's easy to use and efficient,it can be used for researches in the field of prevention and cure of pressure ulcers.
4.Relationship of BRCA1 andβ-tubulinⅢmRNA expression with efficacy of taxanes for advanced esophageal carcinoma

Objective:To investigate the relationship of the mRNA expression of BRCA1 andβ-tubulinⅢwith docetaxel-resistance in esophageal cancer. Methods:The genes BRCA1 andβ-tubulinⅢwere determined at the mRNA level using RT-qPCR in 36 esophageal carcinoma specimens. Results:The mRNA expression of BRCA1 andβ-tubulinⅢwas determined in the 36 tumor samples using RT-qPCR. The median BRCA1 mRNA expression level in relation to that ofβ-actin was 6.27. The medianβ-tubulinⅢmRNA expression level in relation to that ofβ-actin was 4.44. The patients were divided into two groups using these cutoff values. The BRCA1 mRNA expression level was not correlated with the sensitivity of esophageal cancer patients to docetaxel (P=0.733). The response rate of the tumors with highβ-tubulinⅢexpression was (38.9%), which is significantly lower than in patients with lowβ-tubulinⅢexpression (83.3%) (P=0.015). Conclusion:Theβ-tubulinⅢexpression levels in the tumor tissues were probably an important biomarker for the efficacy of docetaxel chemotherapy in esophageal cancer patients. Our study may provide new insights into taxane chemotherapy for advanced esophageal cancer patients.
5.Correlation between Stress Reaction and other Stress Related Factors in Neurosis Patients

Objective: To investigate the correlation between stress reaction and other related psychological stress factors such as life events, social support, and coping style in neurosis patients comparing with the group of healthy persons. Methods: 88 neurosis and 100 healthy persons were examined with LES (Life Event Scale), PSSS (Perceived Social Support Scale), TCSQ (Trait Coping Style Questionnaire), SRQ(Stress Reaction Questionnaire). Results: The scores of stress reaction, family events and negative coping style of neurosis group were significantly higher than those of healthy group, and the scores of social support out of family and positive coping style were significantly lower than those of healthy group. The regression analysis and path analysis revealed that in neurosis group stress reaction was directly correlated with negative coping style and social events, but in healthy group it was correlated with negative coping style and family events. Conclusion: Stress reaction may be directly affected with negative coping style and social events in neurosis patients, whereas it may be done with negative coping style and family events in healthy persons.
6.Construction of nursing comprehensive experimental evaluation index system

Objective To construct a comprehensive nursing content and experiment evaluation index weights for the nursing profession to provide objective and comprehensive evaluation criteria for nursing comprehensive experimental evaluation. Methods Literature search method and the Delphi method(Delphi)were used to explore indicators of evaluation and appraisal the analytic hierarchy process was adopted to calculate index weights and test its logical consistency. Results 28 experts were selected from 10 hospitals of five provinces such as Shanghai, Guangzhou, Chongqing, etc. Three round screening investigations' effective return-ratio Wag 93.9%、96.8%and 100.0%respectively.The authoritative coefficient was above 0.70.3 first-level tar-gets were established,14 second-level targets and the corresponding target connotations, all levels of target weight satisfied the logical uniformity request Conclusions The Delphi method and the analytic hierarchy process jointly determine the nursing comprehensive experimental evaluation index system to ensure the scien-title and reliable characteristic of the evaluation index system. It is able to comprehensively measure the overall quality and ability of students for the nursing profession and provide the basis for a comprehensive experimental evaluation.
7.Establishment of weights of risk factor indicators for lower limbs deep vein thrombosis based on Delphi method and AHP

Objective To establish the weights of risk factors indicators for lower limbs deep vein thrombosis through the scientific research methods.Methods 21 experts were consulted for two rounds of questionnaires survey with Delphi method,according to the experts'opinion,analytic hierarchy process (AHP) was used to calculate the weights of indicators in grade one and the experts'evaluation was used to calculate the weights of indicators in grade two.Results The weight coefficients of grade one including vascular endothelial injury,blood high condensation state and slow blood stream were 0.3325,0.5278 and 0.1396 respectively.The items with maximal weights of the 21 indicators in grade two was vascular trauma,in particular vein injury(0.0770).The Kendall's W of the indicators in grade two was 0.422.Conclusions Through qualitative and quantitative research methods,the risk factors indicators which provide the reference for the prevention of lower limbs deep vein thrombosis are scientific and reliable.
8.Qualitative research about impact of constructing national key clinical specialty on high quality nursing service

Objective The aim of this study was to investigate the impact of constructing national key clinical specialty on high quality nursing service.Methods Using the Colaizzi 7-step analysis method to analyze the information,which through using the phenomenology research methods by making interviews with 14 clinicians,nurses and nursing administrators.Results The clinical medical staffs experienced the construction of key clinical specialist,to some extent,that would promote the development of nursing discipline,enhancing the environment and equipment in hospital,having improvement on the ability of nursing service and quality simultaneously,and then the cooperation between medical staffs and patients were more closely,while nurses bearing increased work pressure,with obviously job burnout.Conclusions The national key clinical specialist construction would effectively launching nursing quality care in depth and improving patient satisfaction with medical care,while the working pressure of nurses and the imperfection of post performance management restricted the development of nursing discipline.
9.Construction of theory model of nursing humanistic care based on Confucianism

Objective To explore the inner relationship between the humanistic care of nursing and the four elements of benevolence, rites, confidence, harmony, and construct a theoretical model of nursing humanistic care based on Confucian thought. Methods Using semi-structured focus interview with 13 experts, the data was sorted, analyzed, encoded and refined, seeking to enrich the modern humanistic nursing care ideas and factors from the Confucian ideology by using grounded theory research method, make the theory research and clinical practice of nursing humanities phenomenon was closely related to the construction of nursing the theory of humanistic care model based on Confucianism. Results Obtained the cognition, feelings and ideas of the experts in nursing, humanistic care and Confucianism, and extracted four classification subjects:benevolence ritesconfidenceharmony as the spiritual core, carrier, code of conduct, value orientation of nursing humanistic care, and constructing a theoretical model of nursing humanistic care based on Confucianism. Conclusions The construction of nursing humanistic care theory model based on Confucianism enriches the connotation of nursing humanistic care, and provides references for the further study of nursing humanistic care theory, and instructs clinical nursing practice by nursing humanistic care theory based on Confucianism.
10.Risk factors of venous thomboembolism in ICU:a Meta-analysis

Objective To systematically analyze the risk factors of venous thromboembolism (VTE) occurrenced in intensive care unit(ICU).Methods The literatures on risk factors for ICU-acquired VTE were retrieved from PubMed,Cochrane Library,Embase,ScienceDirect,CNKI,WanFang,VIP and CBM Databases.Then the data was extraction and quality assessment was performed.The RevMan 5.3 software was used for conducting the Meta-analysis.Results Fifteen articles were included,involving 939 VTE cases.The meta analysis results of different factors were the renal insufficiency(OR =14.60),deep v ein thrombosin(DVT)/pulmonary embolism (PE) history(OR=8.23),rencent surgery history(OR =6.75),age> 60 years old(OR =4.84),shock(OR =4.36),malignant tumors (OR=2.76),central venous catheter(OR=2.54),heart function grade>3(OR=2.27),mechanical ventilation(OR=1.57),D-dimer(WMD=423.99),APACHE Ⅱ score(WMD=4.25).Conclusion The risk factors of developing VTE in ICU are renal insufficiency,DVT/PE history,recent surgery history,age≥60 years old,shock,malignant tumors,central venous catheter,heart function grade>3,mechanical ventilation,D-dimer and high APACHE Ⅱ score.
