1.The effect of flexible visiting policies in ICUs: A Systematic Review and Meta-Analysis

Objective:To investigate the effect of flexible visiting policies in ICUs by comparing with restrictive visiting policies.Methods:English database (Pubmed, embase, Cochrane library, scopus, OvidSP, Wiley Online library, Google scholar and web of science) and Chinese database (CNKI, VIP, CBM and Wanfang) are retrieved, the search time is up to February 2020. The included literatures were evaluated quantitatively and qualitatively.Results:The flexible visiting policies can reduce the risk of delirium in ICU patients [RR=0.59, 95%CI=(0.36,0.96)], shorten the length of stay [MD=-0.21, 95%CI=(-0.35,-0.06)], improve patients' anxiety symptoms [MD=-2.2, 95%CI=(-3.8,-0.61)], reduce the incidence of anxiety and depression and improve the nursing satisfaction of patients and their families, without increasing the risk of ICU acquired infection [RR= 0.77, 95% CI= (0.51, 1.16)] and patient mortality (RR= 0.82, 95% CI= (0.53, 1.26)]. Sensitivity analysis suggested that the results were stable.Conclusions:Compared with the current restrictive visitation policies, the flexible visitation system can reduce the risk of delirium, improve anxiety symptoms and depression, and obtain more satisfaction from family members and patients, and It can be tried further.
2.Effect of Biling Zhidai Tablets on Serum Levels of Monocyte Chemotactic Protein-1, Intercellular Adhesion Molecule-1 and Tumor Necrosis Factor-alpha in Rats with Sequela of Pelvic Inflammatory Diseases

Objective To investigate the effect of Biling Zhidai Tablets(BZT) on monocyte chemotactic protein-1 (MCP-1), intercellular adhesion molecule-1 (ICAM-1)and tumor necrosis factor-alpha(TNF-α)levels as well as hemorheology of rats with sequela of pelvic inflammatory diseases(SPID). Methods One hundred Wistar rats were randomly divided into normal control group , sham-operation group , model group , BZT group(0.54 g·kg-1·d-1)and Fuke Baidai Tablets group(0.225 g·kg-1·d-1), 20 rats in each group . The rats of the model group, BZT group and Fuke Baidai Tablets group were given mechanical damage plus injection of mixed bacteria to induce SPID. After modeling for 2 weeks, the latter three groups were given intragastric administration of the medicine for 21 days. The uterine swelling rate, the serum levels of MCP-1, ICAM-1, TNF-α as well as hemorheological indexes of rats in various groups were detected. Results The uterine swelling rate in BZT group and Fuke Baidai Tablets group was significantly decreased as compared with that of the model group, and the uterine swelling inhibition rate of BZT group was higher than that of Fuke Baidai Tablets group (P<0.01). The levels of MCP-1, ICAM-1 and TNF-α of BZT group and Fuke Baidai Tablets group were significantly decreased as compared with those of the model group, and the effect of BZT group was better than t hat of Fuke Baidai Tablets group, the differences being statistically significant(P<0.05or P<0.01). The indexes of hemorheology in BZT group were significant improved as compared with Fuke Baidai Tablets group, and the improvement in BZT group was superior to that in Fuke Baidai Tablets group (P<0.05 or P<0.01). Conclusion B ZT e xert certain therapeutic effect on treating SPID rats, and the mechanism is related with the relief of swelling of uterus, decrease of serum MCP-1, ICAM-1 and TNF-αlevels and improvement of the indexes of hemorheology.
3.Preparation, Characterization and Application for Genomic DNA Extraction of a Magnetic Microsphere with Surface Charge Switching

SuperparamagneticSiO2@Fe3O4microspherewithcore-shellstructurewaspreparedbythe method of hydro-thermal and St?ber method, and modified by (3-aminopropyl) triethoxysilane ( APTES) . The obtained amino-Fe3 O4@SiO2 microsphere with controlled surface charge properties was characterized by SEM and TEM, and the surface electric property of amino-microsphere was investigated by Zeta potential measurement. The microsphere was used to extract DNA from human blood, and the solid phase extraction method by microspheres was developed. The mechanism of action between Aminated-Fe3 O4@SiO2 microsphere and DNA was explored, and the gel electrophoresis and PCR test were done on the extraction product. The result showed that genomic DNA with high purity was successfully extracted from whole blood by using amino-Fe3 O4@SiO2 microsphere, with the extraction rate about 70%, the saturated adsorption capacity for DNA was about 40 ng/μg, and the elution could be directly used for further bio-analysis.
4.R607C mutation of NOTCH3 gene and clinical features in 4 CADASIL families in Henan, China

Objective To investigate the NOTCH3 gene mutation and clinical features in cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) families.Methods The clinical features of 4 CADASIL probands in Henan,China were analyzed retrospectively,and the incidences of other members in their families were investigated.The NOTCH3 gene mutations in the 3rd,4th,llth,and 18th exons were detected and the results were analyzed in the patients and some family members.Results Gene sequencing showed that 6 patients in 4 families and 1 mutant carrier had NOTCH3 gene R607C mutation in exon llth,they all met the clinical features of CADASIL.Three patients accompanied with vascular risk factors.The clinical stroke patients had unilateral limb weakness.All 5 patients with complete head MRIdata had thalamic infarction.Conclusions In the 4 CADASIL families of R607C mutation,the clinical features of 6 patients with CADASIL were similar,but there were individual differences in different family members.Imaging examination has important role in the diagnosis of CADASIL.The vascular risk factors,such as hyperte.
5.Application of Electrical Driven and Cationic Exchange Membrane to Capture and Enrich Deoxyribonucleic Acid

Based on the selective permeability of cationic exchange membrane and the characteristic of electri cal driven, a new and non-damage apparatus that can be used to separate compounds with different electric property was built to capture and enrich DNA.λ-DNA, as a model compound, was used to study the charac teristic of capturing and enriching DNA in this device.Factors that might affect the efficiency such as voltage and flow speed were optimized and gel electrophoresis was done to study if DNA was denaturation, PCR was done to prove the concentration can be used to further bioanalysis.Results were got; the apparatus could used to capture and enrich DNA scathelessly under the condition of low-voltage, the concentrate multiple was 62, and the recovery efficiency was accessible to 47%, concentration can be used for the next bioanalysis.Auto matic, easy to handle and little organic reagent using made it be advantageous, it could be miniatured to com bine with other biochips.This method had a large application space in field of DNA extraction, purification and concentration.
6.Evaluation of the effect of neck exercises on senior high school students

Objective:
To explore the effect of neck exercises on improving the shoulder and neck symptoms of senior high school students in Zhuhai.
Methods:
From February 2019 to July 2019, 805 senior high school students from 20 classes in 10 middle schools in Zhuhai were selected by multi-stage random sampling method, and randomly divided into two groups based on the class. The final observation group consisted of 405 cases and the control group consisted of 400 cases. The observation group was intervened by neck exercise based with isometric contraction exercise for 12 weeks, while the control group was intervened by routine intervention. The changes of VAS, NDI and PSQI were compared before and after the intervention between the two groups, and the subjects who met the diagnosis criteria of cervical spondylosis were analyzed in subgroups.
Results:
A total of 112 students met the diagnostic criteria of cervical spondylosis, the prevalence rate was 13.9%. among them, 58 cases were diagnosed in the control group and 54 cases in the observation group. After the intervention, the VAS pain level of the observation group was lower than that of the control group (Z=-3.37, P<0.01), and the NDI score was significantly lower than that of the control group (t=-20.05, P<0.01). After the intervention, the PSQI subjective sleep quality, sleep time, sleep efficiency, sleep disorder, daytime dysfunction dimension scores and total scores of the observation group were significantly lower than those of the control group (t=-10.36, -7.66, -12.39, -9.78, -9.06, -11.31, -16.56, P<0.05).
Conclusion
The incidence of cervical spondylosis is high in senior high school students in Zhuhai. The neck exercise focusing on isometric strengthening can effectively improve the symptoms and sleep quality of cervical vertebrae.
7.Discussion on the determination of bicarbonate in hemodialysis.

During the routine tests, it's found that the testing method related to Hemodialysis mentioned in the industry standard YY0598-2006 cannot reflect the real bicarbonate concentration in Hemodialysis. To discuss the reaction principle and its interference factors deeply by studying the assay determination of bicarbonate in Hemodialysis, and it could be cited as reference for amending the quality standard or troubleshooting. The improved way of determining bicarbonate concentration in Hemodialysis is also put up.

Objective To analyze the clinical, imaging characteristics and NOTCH3 mutations of cerebral autosomal dominant arteriopathy with the subcortical infarcts and leukoencephalopathy (CADASIL) in Henan, China.Methods CADASIL patients diagnosed by gene or biopsy in People′s Hospital of Zhengzhou University between 2012-2016 were recruited.Clinical and imaging features of these patients were analyzed retrospectively.The distribution of NOTCH3 gene mutations hotspots was described in Henan region at the same time.Results There were 37 patients from 19 families who were diagnosed as CADASIL by genetic testing or biopsy, 27 of whom had symptoms of CADASIL.Two families were confirmed by pathological examination and 17 by genetic testing.Of these 17 families, 13 mutations were found.Mutations in exon 11 were found in eight families, in exon 4 were detected in four families, and in exon 13 were found in two families.Mutation in exons 3, 8 and 20 was detected in one family respectively.Most patients presented with stroke and several presented with cognitive decline.Twelve patients had been attacked by risk factors.Magnetic resonance imaging (MRI) was performed on 22 patients.White-matter lesions were distributed in brain stem, basal ganglia, subcortical, temporal pole, external capsule.There were 19 patients with white-matter lesions in temporal pole and seven in capsula externa, showed as a high signal in T2WI.Conclusions CADASIL patients can be associated with risk factors.T2WI hyperintensities in the anterior temporal lobe were more common than that in the capsular external.Exon 11 and exon 4 were the hotspots for the NOTCH3 mutation in Henan patients.
9.Progress of SLC13A5 as a potential pharmacological target of metabolic diseases

The solute carrier (SLC) consists of more than 400 transport proteins mediating the influx and efflux of ions, nucleotides, sugars and other exogenous and endogenous substances across biological membranes. Over 80 SLC carrier proteins have been reported to be closely associated with human diseases, in which more than 30 SLC proteins have been regarded as the potential drug targets. SLC13A5 mediates transmembrane transport of substances such as citrate, which is connected with de novo lipid synthesis (DNL). Studies have found that SLC13A5 is related to metabolic diseases such as obesity, insulin resistance (IR), non-alcoholic fatty liver disease (NAFLD). At present, there is no specific drug for clinical treatment of metabolic diseases caused by lipid metabolism disorders, especially NAFLD. Therefore, this paper summarizes the effect of SLC13A5 on metabolic regulation and its potential as a pharmacological target for metabolic diseases treatment, aiming to provide a reference for the research and development of drugs related to metabolic diseases.
10.Design and development of early warning systems for unplanned extubation in decision-making-based critical patients

Objective:To develop early warning systems for unplanned extubation in critical patients to give the early warning and interference in order to reduce the risks of unexpected drawn tubes and guarantee the security of the patients.Methods:Took the model of risk warning system for unplanned extubation in critical patients as the core, obeyed the guide of the nursing intervention program, based on the information system of patients in ICU and relied on computer science technology to design the architecture for the interface layout, the partitions, the modular structure, the content and the function of the system. This warning system was designed from multiple angels and directions. This research retrospectively analyzed the 18 cases for unplanned extubation from January to December in 2016 using this system to verify its warning effectiveness.Results:The prototype of this warning system included login module, evaluation module, decision module, warning module, directive module and the module for canal fixed scheme. The system automatically determined the level of risk for critical patients to implement the layered pre-warning and screen the high-risk patients. Finally, individual nursing interfering method could be supplied. The detection rate of this warning system was 88.89% after retrospective analysis.Conclusion:The warning system for unplanned extubation patients is an automated, intellectualized and informationalized platform. It can effectively warn the high risk of the extubation patients and evade the risk of canal nursing.
