NPC is a high incidence of malignant tumors of the head and neck, and is currently used mainly radiotherapy based, supplemented by a comprehensive treatment of chemotherapy, radiotherapy and chemotherapy, which have serious complications and serious impact on the treatment of patients and quality of life. Polyphenols are the main component of tea. Studies have shown that tea polyphenols have a significant anti-tumor effect of im proving the effect of radiotherapy and chemotherapy, reducing radiation damage, reducing conventional chemo therapy drugs IC50 and reducing the complications of chemotherapy. Tea polyphenols in the treatment of nasopharyngeal carcinoma has also made great progress. It has a strong inhibition of nasopharyngeal carcinoma cells, and can greatly reduce the occurrence of xerostomia after radiotherapy, which is of important clinical research value.

Objective To investigate the influence of ethanol on the toxicokinetic profiles of ketamine and its main metabolite norketamine in rabbits.Methods Ketamine hydroehloride Wills administered orally to the rabbits at a dose of 15mg/kg in the ketamine-treated group.Ketamine hydrochloride combined with ethanol at a dose of 15 mg/kg and 3.0g/kg respectively was administered orally to those of the ethanol-coadministration group.The serum and urine samples were collected before administration and at different time points after drug delivery.The concentrations of ketamine and norketamine were determined by GC and GC/MS.Compartment model and toxicokinetics parameters were assessed by WinNorLin program.Results The mean serum concentration-time profile of ketamine after oral administration was fitted to a two-compartment open model with first order kinetics and not affected by ethanol.The K_(10),AUC and β of ketamine in rabbits of ethanol-coadministration group increased as compared with those of ketamine-treated rabbits,while T_(1/2K_(10)),T_(1/2β),A and C_(max)decreased(P＜0.05).Meanwhile,the K01,A,B and C_(max) of norketamine,the metabolite of ketamine increased in ethanol-coadministration group and T_(1/2K01) and Tmax were lowered than those in ketaminetreated group(P＜0.05).Difference of the other toxicokinetics parameters including V/F,K_(10),K_(12),K_(21),AUC,T_(1/2K_(10)),T_(1/2α),T_(1/2β) and β were not statistically significant between two groups(P＞0.05).Conclusion Ethanol may accelerate elimination of ketamine and the metabolism of ketamine to norketamine and has little effect on the absorption of ketamine,suggesting that interaction between ethanol and ketamine should be considered in cases of co-abuse of the two drugs.

Objective To investigate the status and influencing factors of pleasant events among community-dwelling dementia patients. Methods Totally 266 community-dwelling dementia patients were recruited from Bei-jing,Guangzhou and Tianjin,and were investigated with the Chinese-version Pleasant Events Schedule-AD(PES-AD). Results The overall score of PES-AD was (14.0±6.7). The factor scores from high to low were sensory stimulation activities(0.9±0.5),family activities(0.8±0.5),emotional stimulation activities(0.8±0.4) and autonomy activities(0.6± 0.5). Multiple linear regression showed that the severity of disease,without chronic disease,educational level of de-mentia patients and their caregivers were significant influencing factors of pleasant events,which explained 41.1% of the total variance. Conclusion Sensory stimulation activities of community-dwelling dementia patients were relatively satisfactory,followed by family activities and emotional stimulation activities,but autonomy activities were relatively unsatisfactory. It is suggested to choose appropriate pleasant activities according to the severity of disease for de-mentia patients to improve their quality of life.

Objective To investigate the potential role of soluble triggering receptor expressed on ayeloid cells-1(sTREM-1) expression in serum,endotracheal aspiration (ETA),bronchoalveolar lavage fluid (BALF) and exhaled breath condensate (EBC) as early biomarkers for the diagnosis of ventilator-associated pneumonia (VAP) in patients with acute ischemic stroke.Methods One hundred and thirty-two patients with clinically suspected VAP were prospectively included in this multicenter study.The levels of sTREM-1 in serum,ETA,BALF and EBC were analyzed for diagnostic evaluation at the time of VAP clinically suspected.The bacterial count over 104/CFU as a gold standard for VAP,and the receiver operating characteristic curves were used to identify the ideal cutoff values.Results VAP was confirmed in 76 patients (57.58％).In VAP patients (VAP group) and non-VAP patients (non-VAP group),the level of sTREM-1 in BALF was 32.35 (30.08-41.72) and 18.92(11.89-31.72) ng/L,and the level of sTREM-1 in EBC was 1.57 (1.02-2.61) and 0.41(0.19-1.61)ng/L respectively.The level of sTREM-1 in BALF and in EBC in VAP group was significantly higher than that in non-VAP group (P ＜0.05).The optimum cutoff value for sTREM-1 in BALF according to the maximum Youden index was 23.61 ng/L.This cutoff value had 85.5％ sensitivity and 73.1％ specificity,with 0.813 area under the curve.sTREM-1 in BALF had excellent correlation with that in EBC (R2 =0.78,P ＜ 0.05).Conclusions The results of this prospective study suggest that sTREM-1 levels in BALF and EBC have better roles in facilitating the diagnosis of VAP and thus may be practically recommended to guide the administration of antibiotics when VAP is suspected.

Objective To investigate the set-up error of CR in total body irradiation localization and lung shielding.Methods TOR 18FG software was employed to measure the image quality of images at kV and MV levels.The clinical processes were established and the positioning error was analyzed.Results The low contrast resolution and spatial resolution of MV level images were much worse than those at kV level in the condition of total body irradiation,but the image at MV level could be used to identify the high contrast tissues and employed in total body irradiation.The longitudinal errors were (0.50 ± 1.65) cm for left lung and (1.16 ± 1.56)cm for right lung in A P direction,while (1.12 ± 2.22)cm and (0.41 ± 2.16)cm respectively in PA direction.The errors of lateral were (0.81 ± 1.19)cm for left lung and (0.43 ±1.20)cm for right lung in AP direction,while (0.31 ± 1.64)cm and (0.55 ± 1.49)cm respectively in PA direction.Conclusions Application of CR in total body irradiation could make positioning in treatment much easily and reduce the localization errors.

Objective To study the vitamin D nutritional status in the elderly people from organs and institutions in Jinan urban district. Methods 564 cases were available from institutions enjoying a public medical care in Qianfoshan Hospital by stratified random sampling.The subjects aged (69.5±6.5)years were divided into two groups:60-74 years and≥75 years.The [25(OH)D] levels was measured by enzyme linked immunosorbent assay (ELISA).It was defined that [25(OH) D] levels ≥ 75 nmol/L was sufficient,50.0 74.9 nmol/L was critical,25.0-49.9 nmol/L were insufficient and ＜25 nmol/L was deficient.All data processing and statistical analysis were finished by SPSS13.0. Results The numbers of sufficient,critical,insufficient and deficient vitamin D in 564 subjects were 114(20.2％),180(31.9％),234(41.5％) aud 36(6.4％),in subjects aged ≥75years were 13(9.8％),36(27.3％),70(53.0％) and 13(9.8％),and in subjects aged 60-74 years were 101(23.4％),144(33.3％),164 (38.0％),23 (5.3％),respectively. There was significant difference in the vitamin D nutritional status between the two age groups (x2=19.097,P＜0.01 ).The numbers of sufficient,critical,insufficient and deficient vitamin D in the 246 male were 57 (23.2％),91(37.0％),86 (35.0％％) and 12(4.9％),in the 318 female were 57(17.9％),89 (28.0 ％ ),148(46.5 ％ ) and 24(7.5 ％ ).The percentages of insufficient and deficient vitamin D in the female were higher than in the male (x2 =11.445,P＜0.01 ). Conclusions Insufficient and deficient vitamin D nutritional states are common in the elderly people from organs and institutions in Jinan urban district and are increasing with age,especially in the female.

Objective To investigate the bioavailability and pharmacokinetics of silybin A and silybin B in rats,respectively.Methods Following iv and ig administration of silybin to 20 Wistar rats,the plasma samples were collected at different time points up to 12 h.Sample pretreatment was involved in one-step protein precipitation with acetonitrile.Silybin A and silybin B were simultaneously determined by LC-MS/MS.Results After ig dosing silybin 28,56,and 112 mg/kg to rats,the t1/2β values were 5.48,5.08,and 5.73 h for silybin A,and 4.56,4.12,and 5.53 h for silybin B; The Cmax were 674.3,1349.4,and 2042.5 ng/mL for silybin A,and 671.0,1365.4,and 2066.2 ng/mL for silybin B; The Tmax were 0.20,0.23,and 0.20 h for silybin A,and 0.20,0.23,and 0.20 h for silybin B; The AUC were 454.4,845.9,and 1219.5 h·ng/mL for silybin A,and 432.0,817.1,and 1153.6 h·ng/mL for silybin B.The absolute bioavailabilities of silybin A and silybin B were 2.86％ and 1.93％,respectively.Conclusion Silybin A and silybin B have very low bioavailability after ig administration,and there is no significant difference in the pharmacokinetic parameters between silybin A and silybin B,which indicates that the two diastereoisomers have similar pharmacokinetic behavior in rats.

Objective To assess the effect of vitamin D insufficiency on the quality of life in the elderly.Methods A total of 205 cases in health examination center of our hospital were randomly divided into elderly group (aged ≥ 60 years) and middle-aged g roup (aged 45-60 years).The SF-36 Form was adopted to investigate status of body and mental health.Serum 25 (OH)D3 levels were measured by enzyme-linked immunosorbent assay (ELISA).Serum 25 (OH) Ds ≤ 49.9 nmol/L was defined as vitamin D insufficiency. ResultsThere were 55 cases (53.9％) with vitamin D insufficiency and 16 cases (15.7％) vitamin D sufficiency in the elderly.The scores of vitamin D insufficiency group in the elderly were lower than of vitamin D threshold and sufficiency groups in fourdimensions,including physical function,physical role,body pain and general health.The scores ofvitamin D insufficiency group in the elderly were also lower than of vitamin D threshold and sufficiency groups in three dimensions,including social functioning,vitality and mental health (F=10.28,9.20,6.60,all P＜0.05).Serum 25(OH)D3 played more important role in scores of the quality of life in the elderly than other factors by stepwise multiple linear regression. ConclusionsThe vitamin D insufficient status widely exists in the elderly,and it might influence their quality of life.

Objective:To explore the promoting effects of IL-7 and IL-2 on CD4+CD25-T cells proliferation in vitro and construct a stable culture system in vitro for CD 4+CD25+regulatory T cells from human umbilical cord blood.To compare the inhibiting effects between induced proliferated CD 4+CD25+Tregs and naturally isolated CD 4+CD25+Tregs on PBMCs functional activity.Methods:CD4+CD25-T cells and CD4+CD25+T cells were isolated from human umbilical cord blood mononuclear cells by magnetic activated cell sorting ( MACS) system and then expanded in vitro.Four different concentration levels of IL-7 combined with proper concentration of IL-2 were added as inducer and the efficiency and optimal concentration of IL-7 on inducing,CD4+CD25-T cells were analyzed via 4 different methods.Flow cytometry method was used to detect the changes of CD 4+CD25-T cells.The inhibitory effect of expanded CD 4+CD25+T cells on peripheral blood mononuclear cells (PBMCs) was tested by MTS.The expressions of Foxp3,IL-10 and TGF-βgenes in CD4+CD25+T cells were test by RT-PCR.Results:The CD4+CD25+T cells from each groups were expanded significantly after three weeks of culture.The results indicated that use of IL-7 combined with IL-2 resulted in the highest cell expansion comparing to the other groups.It was shown by the inhibitory test that the expanded CD 4+CD25+regulatory T cells could inhibit the proliferation of PBMCs ,but IL-7 induced CD4+CD25+regulatory T cells exerted weaker suppressor activity than natural regulatory T cells .Only IL-7 (4 ng/ml) and IL-2 (2 000 U/ml) induced CD4+CD25+regulatory T cells showed the strongest killing activity.Conclusion:We successfully expand CD4+CD25+regulatory T cells in vitro.The protocol is established in which the use of mAbCD 3/CD28 combined with IL-7 and IL-2 resulted in the highest cell expansion ,and intensely expressed cell phenotype of CD 4 and CD25.

Objective To study the postmortem distribution of Bromadiolone and its metabolite-Benzylideneacetone in dogs and provide an experimental evidence for the sampling of Bromadiolone poisoning cases. Methods The dogs were given 2LD50 and 4LD50 Bromadiolone by intragastric administration. Anatomy was conducted immediately after death and samples of body fluids and viscera (heart blood; peripheral blood, bile, urine, heart, liver, spleen, lung, kidney, brain, urinary bladder, left leg muscle, stomach, stomach contents, pancreas) were collected and detected after the dogs poisoning death. The Bromadiolon and its metabolite-Benzylideneacetone contents in samples were analyzed by GC/MS. Results Hemorrhagic symptoms came out at 3d after Bromadiolone delivery and deaths occurred at (178.40±20.94)h after intoxication. The postmortem distribution of Bromadiolon and its metabolite-Benzylideneacetone in dogs was as following: 2LD50 Bromadiolon: bile>urine, liver, heart, kidney>heart blood, peripheral blood, spleen, lung and so on. Benzylideneacetone: the content in bile, urine, heart blood, peripheral blood, lung, stomach contents are higher. 4LD50 Bromadiolon: bile, urine>liver, peripheral blood>heart blood, stomach contents and others. Benzylideneacetone:contents in bile, urine and lung are higher. Conclusion The postmortem distribution of Bromadiolon and its metabolite-Benzylideneacetone in dogs is uneven, contents in bile, urine, liver, heart blood and peripheral blood are higher, whichare suggested for forensic toxicological analysis in Bromadiolon poisonig case.
