1.Comparison of the curative effect of transurethral plasma kinetic enucleation of prostate and transurethral resection of prostate on benign prostatic hyperplasia

Objective To compare the clinical efficacy between transurethral plasma kinetic enucleation of prostate(TUPKEP)and transurethral resection of prostate(TURP)on benign prostatic hyperplasia(BPH).Methods The clinical data of patients with BPH were retrospectively analyzed.The clinical results of 72 cases of TURP and 60 cases of TUPKEP were selected.Results The operation time,the amount of bleeding of the TUPKEP group were (51.3 ±5.9)min,(131.7 ±9.2)mL,which were significantly better than those of the TURP group [(62.5 ± 7.4)min,(174.2 ±10.5)mL](t =9.48,24.48,all P <0.05).The amount of prostate resection in the TUPKEP group[(62.4 ±7.0)g]was significantly higher than that in the TURP group[(45.9 ±6.1 )g],the difference was statistically significant(t =14.47,P <0.05).The incidence rate of complication in the TUPKEP group was 1.7%(1 /60),which was significantly lower than that of the TURP group 12.5%(9 /72),the difference was statistically significant(χ2 =5.50,P <0.05).The postoperative hospitalization time,bladder irrigation time,catheter indwelling time of the TUPKEP group were (4.3 ±1.0)d,(1.2 ±0.5)d,(2.6 ±0.3)d,which were shorter than those of the TURP group [(6.2 ±0.9)d,(2.3 ±0.9)d,(4.8 ±0.2)d],the differences were statistically significant(t =11.48, 8.44,50.27,all P <0.05).The maximum urinary flow rate(Qmax),residual urine volume(RUV),prostate function (IPSS)and quality of life(QOL)were significantly improved in the two groups before and after operation,the differ-ences were statistically significant(TURP group:t =2.46,18.49,2.55,4.37;TUPKEP group:t =2.88,17.59,2.64, 3.94;all P <0.05).But there were no significant differences in Qmax,RUV,IPSS score and QOL score between the two groups after surgery(t =0.24,0.50,0.56,0.94,all P >0.05).Conclusion The clinical efficacy of TUPKEP in the treatment of BPH is positive,it can relieve the symptoms of patients with difficulty in urination,improve the quality of life of patients,and the incidence of postoperative complications is low,it is worthy of clinical application.
2.The diagnostic value in bronchoalveolar lavage galactomannan antigen detection of invasive pulmonary aspergillosis in patients without granulocyte deficiency

Objective To study infection of the lung tissue by bronchoalveolar lavage for BALF and explore whether GM can help improve the he diagnosis efficiency in patients without granulocyte deficiency with IPA, and find the threshold of GM values. Methods Between January 2011 and December 2011, 173 cases of patients were considered possible invasive pulmonary aspergillosis in parallel BALF GM detection in the center of the respiratory endoscopic examination.. According to the diagnostic criteria at home and abroad, all the cases were divided into four groups, including group of IPA, group of tuberculosis, group of bacterial pneumonia and group of non-infection. Using ELISA method for determination of serum and BALF GM. The diagnostic value was made by statistical methods in BALF GM detection without patients of granulocyte deficiency with IPA. Results There were 11 cases in the group of IPA, 23 cases in the group of tuberculosis, 90 cases in the group of bacterial pneumonia and 49 cases in the group of non-infection. In patients without granulocyte deficiency with IPA,BALF GM value was higher comparing with the patients without IPA. There were significant difference between the two groups; Comparing the GM value in the serum, the detection of BALF GM was higher in IPA. ROC curve analysis found that when the BALF GM test value was 0.95 or higher, there was highest possible with IPA. Conclusion BALF GM testing is beneficial to patients of invasive pulmonary aspergillosis without granulocyte deficiency in early identification with IPA. This study found that BALF GM best value is 0.95 or higher.
3.Study on correlation between anterior circulation artery stenosis and lacune or lacunar infarction in elderly patients

Objective To analyze the correlation between anterior circulation artery stenosis and lacune or lacunar infarct(LI) in elderly patients.Methods A retrospective analysis of data was performed in 111 patients with intracranial artery atherosclerosis,ischemic cerebral infarction or transient ischemic attack from January to December 2016 in our hospital.All the patients underwent non-contrast brain MRI or CT scan,as well as one-stop dynamic whole brain 4D CT angiography and CT perfusion scan(CTA-CTP/ perfusion).Imaging data were retrospectively analyzed.Intracranial 4D CTA was produced by using MIP and CPR post processing.The correlation of intracranial internal carotid artery(ICA)and middle cerebral artery (MCA)stenosis with lacunar infarct or lacune was analyzed by chi-square test using IBM SPSS Statistics 22.0 software.Results The average age of 111 patients was(68.4± 6.8)years.A total of 19 phases with 6080 images were obtained by one-stop scanning.The optimal phase of artery visualization was selected from 19 phases for evaluating artery stenosis.Intracranial ICA and/or MCA stenosis were revealed in 94 patients,including 73(65.8%)patients with LI or lacune and 21 patients(18.9%)without LI or lacune.17 patients without intracranial ICA or anterior circulation of MCA stenosis included 7 patients(6.3 %)with LI or lacune and 10 patients(9.0%)without LI or lacune.Anterior circulation vessels stenosis was positively correlated with lacunar infarction or lacune(x2 =7.794,P=0.005).94 patients with anterior circulation vessels stenosis were further divided into 2 subgroups:unilateral and bilateral stenosis.39 cases showed unilateral vessels stenosis,including 25 cases (26.6 %) with LI or lacunein,and 14 cases (14.9 %) without LI or lacunein.And 55 cases showed bilateral vessel stenosis,including 48 cases (51.1%)with LI or lacune,and 7 case (7.4 %) without LI or lacune.The risk for LI or lacunein was statistically higher in bilateral vessel stenosis than in unilateral vessel stenosis(x2 =7.061,P=0.008).Patients with anterior circulation vessels stenosis combined with LI or lacune were 73 cases,including 31 cases of grade Ⅰ,19 cases of grade Ⅱ,14 cases of grade Ⅲ,and 9 cases of grade Ⅳaccording to Trial criteria(NASCET)classification,with no significant difference between the different grades of anterior circulation vessels stenosis.Conclusions A correlation between ICA or MCA stenosis and lacunar infarction or lacune may exist.Risk for lacunar infarction or lacune is higher in patients with bilateral artery stenosis than in patients with unilateral artery stenosis.
4.Evaluation of the vaccinations with three strains of gene-deleted mutants from pseudorabies virus, PRV TK-, PRV gE-/gI and PRV TK-/gE-/gI- after exposure to the wild Fa strain

To evaluate the value of the vaccinations with 3 strains of gene-deleted mutants from pseudorabies virus(PRV), PRV TK-, PRV gE-/gI- and PRV TK-/gE-/gI- after to exposure to the wild Fa strain, these mutant strains from the PVR reference isolate Fa were used to vaccinate 4 weeks old PRV-free pigs with a dosage of 105 PFU each ,and followed by nasally challenged by the parental Fa strain with a dosage of 107 PFU at 14 days post vaccination. The pathological changes, virus discharge and distribution were evaluated after vaccination and challenge. It was found that the histopathological observations in the 10 collected samples including cerebrum, cerebellum, heart, liver, lungs, spleen, kidneys, tonsils, lymph nodes and trigeminal ganglion from these 3 mutant strains showed that the rates of occurrence of pathological changes in various organs were 4/10, 3/10 and 4/10 respectively, whereas that of the positive controls were 9/10. The damage in lungs was more serious in pigs vaccinated with PRV TK-mutant and positive control in comparison with other groups of pigs inoculated, and the damages in cerebrum, cerebellum and trigeminal ganglion in positive controls were more serious than those of pigs vaccinated with the 3 gene-deleted mutants. However, the tonsils, the main organ for latent infection were damaged mildly in the pigs inoculated with these 3 gene-deleted mutants in comparison with that of the positive controls. As demonstrated by Southern blot analysis, all the vaccinated pigs could discharge viruses by secretion through nasal cavity, but the soldier pigs were not infected successively by the gene-deleted mutants and the gene-deleted mutants were also unable to establish infection in cerebrum and cerebellum. Nevertheless, they could not effectively block discharge of PRV Fa after exposure to Fa virus, but could block effectively the virulent Fa virus invading into cerebrum and cerebellum. From these observation, it is evident that the deleted mutants of the TK, gE/gI , TK/gE/gI genes can block the invasion of virulent Fa virus into cerebrum and cerebellum and lessen the damages on multiple organs or tissues ,indicating that the deleted mutant of TK/gE/gI gene may be the most promising candidate of vaccine strain for development of the commercial vaccine.
5.Detection of QRS complexes using wavelet transformation and golden section search algorithm.

The extraction and identification of ECG (electrocardiogram) signal characteristic parameters are the basis of ECG analysis and diagnosis. The fast and precise detection of QRS complexes is very important in ECG signal analysis; for it is a pre-requisite for the correlative parameters calculation as well as for correct diagnosis. In our work, firstly, the modulus maximum of wavelet transform is applied to the QRS complexes detection from ECG signal. Once there are mis-detections or missed detections happening, we utilize the Golden Section Search algorithm to adjust the threshold of maxima determination. The correct detection rate of the QRS complexes is up to 99.6% based on MIT-BIH ECG data.

6.Obtaining of union expression virus-like particle of PPV VP2 and PCV2 ORF2 and its immunogenicity

The amplified VP2 gene (PPV strain SC-1) and PCV20RF2 gene were inserted into the eukaryotic expression vector pPI-2.EGFP.Then the recombinant plasmid named pPI-2.EGFP.VP2.ORF2 was obtained.Mediated by liposome,the recombinant plasmid was transfected into Veto cells and expressed.Using immunofluorescence assay,the fluorescence of expression products were first detected at 20 h after transfection and peaked 36 h.Under electronmicroscope,virus-like particles (VLPs) can be observed in the transfected cells.To confirm the obtained VLPs to be recombinant particles,piglets were immunized using purified VLPs.The dynamic variation of blood T lymphocytes and serum antibody level of PPV and PCV2 were measured.The results showed that the ratio of CD3+,CD4+ T lymphocyte in peripheral blood lymphocyte of immunized piglets raised in a certain degree,the number of CD8+ T lymphocytes fell at 7-14 d after immunization,and then raised.Relatively high level of PP-V,PCV2 specific antibody could be detected.This indicated that the expression of recombinant plasmid pPI-2.EGFP.VP2.ORF2 was successful,the virus-like particles were formed and showed favourable immunogenicity.
7.Effect of locking compression plate combined with scaled cancellous bone graft on postoperative nonunion of humeral shaft fracture

OBJECTIVE: To retrospectively evaluate the effect of locking compression plate (LCP) combined with scaled cancellous bone graft on postoperative nonunion of humeral shaft fracture. METHODS: A total of 19 cases with postoperative nonunion of humeral shaft fracture were collected from Department of Orthopaedics, Affiliated Hospital, Xiangnan University between August 2005 and January 2009. There were 12 males and 7 females, aging 28-59 years with the mean age of 36 years. All patients were treated with LCP fixation combined with scaled cancellous bone graft. The key points of the operation were as follows: beck-median or lateral approach, protection of the radial nerve and ulnar nerve, removal of the cicatrix, reopening of humeral canal, and limited periosteum striping. For minimally invasive plate osteosynthesis technique, length of LCP was appropriate, 3-4 locking screws at least were used on either stump, and double cortices were penetrated with locking screws and treated with scaled cancellous bone graft. RESULTS: All the fractures healed with a mean period of 7.2 months (from 6 to 8 months). Two cases showed temporary radial nerve palsy after the operation, which gradually recovered 3 and 6 weeks, respectively after appropriate treatment. Shoulder and elbow functions were somewhat impaired in 7 cases, but the dysfunction could not influence daily living. Infection, screw loosening, blade plate breakage, and other complications were not found. CONCLUSION:The integrity of locking screws and plate prevents screw breakage and plate loosening. The combination of autoallergic cancellous bone graft and scaled decorticating technique can improve healing of humeral shaft fracture.
8.A retrospective clinical study of the therapeutic effect of subscapularis transthoracic and posterolateral approach for upper thoracic tuberculosis

Objective To compare the therapeutic effect of debridement,bony fusion and internal fixation through subscapularis transthoracic and posterolateral approach in treating upper thoracic tuberculosis.Methods Between January 2002 to February 2014,a total of 43 patients with upper thoracic tuberculosis were analyzed retrospectively.There were 21 patients in subscapularis transthoracic approach group and 22 patients in posterolateral approach group.Several parameters of the two groups were compared,including gender,age,segmental tuberculosis,intraoperative blood loss,operative time,complications of operation,hospital stay,the cure rate,the Cobb angle,Frankel scale and so on.Results There was no significant difference in gender,age,segmental tuberculosis,ESR,Frankel scale,Cobb angle between the two groups before surgery.The operative time was 196± 10 min in subscapularis transthoracic approach group,whereas it was 142±13 min in posterolateral approach group.The operative time of subscapularis transthoracic approach group was significantly longer than posterolateral approach group.There was no significant difference in intraoperative blood loss,complications of operation,hospital stay and the cure rate.The followed up of subscapularis transthoracic approach group and posterolateral approach group was 28.0± 14.7 months and 27.3± 15.5 months respectively.The preoperative Cobb angle was 36.5°±4.8° and the postoperative Cobb angle was 24.8°±5.6° in subscapularis transthoracic approach group.The correction rate was 30.9%.The preoperative Cobb angle was 34.5°±5.2° and the postoperative Cobb angle was 10.2°±2.6° in posterolateral approach group.The correction rate was 68.5%.Thus,the correction rate of posterolateral approach group was significant better than subscapularis transthoracic approach group.The neurological score of Frankel scale was significantly restored in two groups after operation,and there was no significant difference between two groups.Conclusion For upper thoracic tuberculosis,the therapeutic effect of debridement,bony fusion and internal fixation through subscapularis transthoracic and posterolateral approach could achieve the same good clinical results.However,the posterolateral approach was better than subscapularis transthoracic approach in surgical trauma,surgical duration and kyphosis correction.Therefore,it is recommended to use the posterolateral approach in treating upper thoracic tuberculosis.

Objective: To investigate the expression of peroxisome proliferator actived receptor ? (PPAR ?)and the inducement of apoptosis by PPAR ? ligand in renal cell carcinoma(RCC) derived cell lines.Methods:RT-PCR and Western blot analysis were performed to determined the expression of PPAR ? mRNA and protein in two RCC derived cell lines(786 O and A498) and two normal kidney(NK) derived cell lines(HK 2 and HMCC). Two RCC cell lines were treated with 50 ?mol/L troglitazoned for and evaluated for the effects of antidiabetic thiazolidinediones (TZDs) on the cells apoptosis by fluorescence microscopy and DNA ladder assay.The mutative expressions of Bcl 2 and Bax before and after TZDs treatment were also performed by western blot analysis. Results: The expression of PPAR ? was observed to be stronger in 786 O and A498 cells than in HK 2 and HMCC cells by RT-PCR and Western blot analysis. Treated with 50 ?mol/L troglitazone (for 48 h) it induced typical apoatosis in 786 O and A498 cells. After treatment, a decrease in Bcl 2 expression in RCC cells was observed by Western blot analysis,and the expression of Bax,however,was up regulated.Conclusion: The results reveal that troglitazone has the tumor suppressive effect on RCC cells. High affinity PPAR ? ligands (TZDs) may be the candidates for a novel approach to the treatment of this refractory neoplasm.
10.Frequent somatic mutations of the von Hippel-Lindau (VHL) tumor suppressor gene and its meaning in sporadic human renal clear cell carcinoma

Objective : To investigate the mutation of VHL gene, an important tumor suppressor gene in primary sporadic human renal cell carcinoma (RCC),and analyse its relationships with pathological stage and grade of renal cell carcinoma. Methods: We analyzed 57 cases of primary sporadic Chinese renal clear carcinoma using the polymerase chain reaction (PCR) and denaturing high performance liquid chromatography(DHPLC).All positive cases in DHPLC analysis were further characterized by direct sequencing. Results: Somatic mutations were detected in 30 (53%) of 57 clear cell renal carcinomas including 13 deletions, 2 insertions, and 15 missense mutations. These mutations mainly occurred in the last one third region of exon 1, 2,and 3. Conclusion: VHL tumor suppressor gene is one of the major tumor suppressor genes in human renal cell carcinoma,and there are frequent mutations of VHL in primary sporadic Chinese renal clear cell carcinomas. The mutations of VHL gene were irrespective of the age and pathological grade and stage of patients.
