1.Effect of numerical simulation of vascular wall thickness on fluid-structure interaction analysis of complex intracranial aneurysms

BACKGROUND:Intracranial aneurysms have a high mortality, and finite element analysis to predict fracture risk has become a hot topic at present. Finite element analysis requires reliable fluid-structure interaction model, blood model of aneurysm is very easy to obtain, but the vascular wal model can not be obtained directly, only by artificial settings, which may have an impact on calculation results.
OBJECTIVE:To investigate the effects of vascular wal thickness on fluid-structure interaction analysis in finite element modeling of complex intracranial aneurysms, and provide a more reliable method of finite element modeling for the numerical simulation study of intracranial aneurysms.
METHODS:A three-dimensional numerical model of tandem left intracranial internal carotid artery aneurysms of a 67-year-old man was obtained by three-dimensional angiography. Four fluid-structure models were got postoperatively by thickening vascular wal , which were artificial y set for 0.3 mm, 0.4 mm, 0.5 mm, 0.6 mm. According to intraoperative measured data, dynamic characteristics of fluid-structure interaction of tandem internal carotid artery aneurysms were simulated by the finite element method, comparing four models to calculate the difference between the results.
RESULTS AND CONCLUSION:Among the four models, there were no difference in blood flow chart, blood pressure drop chart and wal shear stress chart (P>0.05). The deformation of the vascular wal was the most obvious in C 2 segment of the internal carotid artery, and the thicker vessel wal was accompanied by the more apparent deformation (P<0.01). Von Mises stress in the vessel wal of the four models reached a local maximum in the I and J points, the thinner vessel wal was accompanied by the larger local maximum (P<0.01). The settings of vascular wel may affect the fluid-structure interaction analysis of complex intracranial aneurysms and appropriate thickness settings wil obtain accurate calculation.

A cycle is built ranging from the application for to the implementation of a medical order for blood transfusion. Such measures as repeated blood types verification by two departments, dual-database management of blood type files, and bar coding are called into play to ensure information correctness. These measures can make sure that the correct blood type is transfused to the correct patient and rule out accidents in blood transfusion. Indications for blood products are set up under real-time surveillance, which makes available limited blood products supply to those in need and minimizes waste and abuse of such products. The full-range and real-time management of clinical blood use will confine blood transfusion indications and ensure patients' safety with 100% match ratio.
3.Diagnosis and treatment of neonatal hypoglycemia

Neonatal hypoglycemia is a common metabolic problem. The definition and clinical management of neonatal hypoglycemia remains controversial. The concerned reasons are mainly as follows:the course of a physiological decrease and recovery in the level of blood glucose is usually existed in newborn infants after birth;asymptomatic hypoglycemia(many newborn infants' blood glucose level is very low but without any clinical signs or symptoms);and the lack of the complete comprehension for the relationship between neonatal hypoglycemia and the long-term neurologic prognosis. The review describes the etiology,definition and clinical management of hypoglycemia in newborn infants.
4.Current application of sentinel lymph node biopsy in cutaneous malignant melanoma

Sentinel lymph node biopsy has a very important function in the accurate staging, diagnosis, and treatment of malig-nant melanoma. The operational technique of sentinel lymph node biopsy (SLNB) has been basically mature, but its indications remain unclear, especially for thin melanomas. Adequate evidence to show that SLNB combined with a complete lymph node dissection can prolong the survival of patients with metastasis is unavailable. Given the low incidence of malignant melanoma in the country, the use of SLNB has not been extensively investigated in China and the relevant methods are outdated. This review summarizes the current ap-plication of SLNB in cutaneous malignant melanoma by analyzing related studies conducted in recent years.

6.Present perceptions and trends of surgical margin in soft tissue sarcoma in limbs and trunk

Limb salvage surgery with comprehensive treatment is the standard care for soft tissue sarcoma in the limbs and trunk. Marginal status is a key factor in determining the quality of surgical treatment, as well as in guiding the selection of adjuvant therapies. The goal of surgery is to obtain an appropriately negative margin, although no consensus exists regarding the adequate thickness of normal tissue cuff. With the development of auxiliary treatments, surgery for soft tissue sarcoma is becoming less extensive. Marginal resection combined with adjuvant therapies may be necessary to preserve limb functions when the tumor is close to critical vessels, nerves, or bones. When preparing the preoperative plans, additional considerations should include histologic type, location, presence of barriers, and response to adjuvant therapies.
7.Comparison of Serum Levels of Thyroid Hormones and Thyroid-Stimulating Hormone in Excess-Heat and Deficiency-Heat Patients

[Objective] To investigate the relationship of thyroid hormones with excess-heat syndrome and deficiency-heat syndrome. [Methods] Serum levels of thyroid hormones and thyroid-stimulating hormone (TSH) in 30 healthy volunteers (Group A) , 27 cases of excess-heat syndrome (Group B) and 35 cases of deficiency-heat syndrome (Group C) were detected by radioimmunoassay. [Results] Serum levels of triiodothyronine (T3) and thyroxine (T4) were higher and reverse T3 (rT3) lower in Group C than those in Group A (P 0.05). [Conclusion] Serum thyroid hormones level in deficiency-heat syndrome is different from that of excess-heat syndrome may be related to the hypothalamus-pituitary-thyroid function.
8.Interventional drainage through sinus tracts for the treatment of postoperative abdominal abscess

Objective To explore a new way to drain a postoperative abdominal abscess. Method Drainage catheter and sinography were employed to locate the site of the abscess, and a 10.2F~16F catheter with single or double lumen was inserted into the abscess by interventional technique. Result A 100% success was obtained for the 15 patients with 18 abscesses by inserting the drainage catheters, and all the abscesses were cured 7~65 days after drainage. Conclusions Compared with the surgical or percutaneous drainage, the present method is less traumatic, safer, more economical and effective, and it could be applies to all kinds of postoperative intraperitoneal abscesses which could not been drained adequately or with formation of sinus because of displacement of drainage tube.
9.Study on the relationship between the efficacy of IFN and ribavirin on chronic hepatitis C and the anti-IFN antibodies.

Objective In order to study the relationship between the efficacy of IFN and ribavirin in treating the patients with chronic hepatitis C and the anti-IFN antibodies.Methods 20 patients received 3MU rIFN ?-2b three times a week and ribavirim 1?050mg once a day for 24 weeks;32 patients were treated with 3MU rIFN ?-2b and without ribavin.These patients were all followed up 24weeks after stopping treatment,meanwhile the anti-IFN antibodies were detected.Results In first group,50% of the patients had complete response,which was higher obviously than 18.7% in the second group;25%relapse developed in the first group,which was lower than 56.3% in the second group.25% of the patients had no response in each groups.The relapse was related to the anti-IFN antibodies,especially neutralizing antibodies.Conclusion The efficacy of combined treatment with IFN?-2b and ribavirin is superior to the treatment with only rIFN?-2b.The relapse is related to anti-IFN antibodies,especially neutralizing antibodies.
10.Expression and role of IL-33 and its receptor ST2 in eosinophilic and non-eosinophilic chronic rhinosinusitis with nasal polyps.

OBJECTIVE:
To investigate the expression and role of Interleukin-33 (IL-33) and ST2 in the nasal polyps of human Eosinophilic and non-Eosinophilic chronic rhinosinusitis with nasal polyps (ECRS and non-ECRS).
METHOD:
IL-33 and ST2 protein expression in nasal polyps of ECRS and non-ECRS as well as in seemingly normal mucosa of the inferior turbinate tissue was investigated by immunohistochemical staining and messenger RNA (mRNA) expression of IL-33 and ST2 was assessed by realtime polymerase chain reaction (PCR) in 27 subjects with ECRS, 33 subjects with non-ECRS, and 11 control subjects.
RESULT:
(1) The ST2 was found both in nasal polyps of ECRS and non-ECRS,especially in ECRS, yet hardly found in the normal mucosa of the inferior turbinate tissue; (2) The expression of ST2 mRNA in nasal polyps of ECRS was higher than that in non-ECRS and normal inferior turbinate tissue, and the difference was both prominent in statistics (P<0.01); (3) The expression patterns of IL-33 at both mRNA and protein levels were not significantly different among the three groups (P>0.05).
CONCLUSION
The IL-33 and its receptor ST2 were both expressed in human nasal polyps including ECRS and non-ECRS, meanwhile the expression patterns of ST2 at both mRNA and protein levels were significantly higher in nasal polyps of ECRS. The current study suggests that IL-33 and its receptor ST2 may play important roles in the pathogenesis of chronic rhinosinusitis with nasal polyps, especially in ECRS through the increased expression of ST2 in Eosinophils as a hypothesis.
