2.Analysis of plague monitoring results and epidemic trend from 2000 to 2012 in Guizhou Province

Objective To master the prevalence of plague and its trend in Guizhou Province,and to analyze the plague monitoring results from 2000 to 2012.Methods The report of infectious disease,the information of plague natural focus and the epizootic monitoring data of Xingyi City,Anlong County and Dingxiao Distract of Guizhou Province from 2000 to 2012 were collected and the status of the plague natural focus was analyzed.Results There were 137 cases of gland plague in Xingyi City and Arlong county from 2000-2003,1 death,and mt plague occurred in 66 villages.Fifty-four strains of Yersiniapestis were detected and 49 rats were plague antigen F1 positive(49/160).No human plague occurred between 2004-2012.A total of 4 plague antigen F1 positive rats were detected in Dingxiao District and Xingyi City in 2005 and 2006.There was no Yersinia pestis and F1 antibody in 2007-2012.The epidemic stage of plague was from 2000-2003; the active stage was from 2004-2006; and the quiescent stage was from 2007-2012.The dominant species of the plague natural focus was Rattus flavipestus (42.83%,7 966/18 597),but was replaced by Rattus norvegicus at the epidemic stage (47.22%,1 480/3 134) and the active stage(35.35%,2 071/5 196).The density of rodents was 5.34% at the epidemic stage,which was higher than that of the active stage (3.27%) and the quiescent stage (1.71%,x2 =2 286.15,P < 0.01).Xenopsylla cheopis(56.34%,10 034/17 811) was the dominant species,and the index was 1.537 9,which was greater than those of the active stage(0.959 6) and the quiescent stage(0.540 4,x2 =492.68,P < 0.01).Conclusions The plague of Guizhou Province is at the quiescent stage.Both the density of rodents and the Xenopsylla cheopis index are lower than the national standard of controlling.
3.Epidemiologic analysis on five cases involved in an outbreak of anthrax in a village of Wengan County,Guizhou, China

We identified the routes of infection and evaluate the effect of disinfection on the field of an outbreak of an-thrax in a village of Wengan County ,Guizhou ,China ,thus trying to provide with basis for the implementation of policies for pre-vention and control of anthrax .The authors gathered the cases information by searching and interviewing the targeted persons house by house ,and reviewed the medical records in hospitals .The samples including patient’s discharging fluid ,residues of died horse ,and soil from the places where the villagers dismembered horse were gathered and cultured for Bacillus anthracis . The technique of multiple-locus variable-number tandem repeat analysis (MLVA-8) was applied for revealing the genetic rela-tionships among those isolated Bacillus anthracis strains .Five cases of cutaneous anthrax occurred in the outbreak and the total attack rate was 7 .58% (5/66) among those contactors evolved in the activity of carrying ,dismembering ,washing ,chopping and eating the died horse .The attack rate was 100% (3/3) for those who carried ,dismembered ,washed ,chopped and ate that horse ,100% (1/1) for those who carried ,dismembered and ate ,and 7 .14% (1/14) for those who washed ,chopped and ate . The 25% (1/4) of the samples of discharging fluid from the cases with cutaneous anthrax were positive .After disinfection , 15 .38% (4/26) of the soil samples retained positive .The genetic similarity was 100% among the 5 isolate strains .The results suggested that the outbreak of anthrax in villagers occurred through the activities of carrying ,dismembering ,washing and chop-ping the died horse .Strengthening the risk communication and disinfection of the dismemberment places should be the crucial strategies to prevent and control anthrax epidemics in Guizhou in the future .
4.Analysis of plague monitoring results from 2013 to 2017 in Guizhou Province

Objective To analyze the monitoring results on plague from 2013 to 2017 in Guizhou Province,to master the epidemic dynamics of the plague and to provide a basis for developing countermeasures.Methods Using a retrospective analysis method,plague monitoring data from 9 monitoring sites in Xingyi City,Anlong County,Ceheng County,Xingren County,Pan County,Liuzhi Special Zone,Si'nan County,Dejiang County and Yanhe County in Guizhou Province from 2013-2017 were collected and analyzed.Results In 2013-2017,there were no plague F1 antigen and F1 antibody.A total of 3 575 murine animals were captured indoors,with Rattus norvegicus as the dominant species,accounting for 49.99% (1 787/3 575).Rattus norvegicus and Mus musculus were common species,accounting for 35.64% (1 274/3 575) and 11.36% (406/3 575),respectively;the density of indoor rat was 3.48%(3 575/102 641),there was no significant difference in rat density between different years (x2 =3.655,P > 0.05).A total of 1 674 murine animals were captured outdoors,and the Rattus norvegicus was the dominant species,accounting for 34.29% (574/1 674).S.serrata and Rattus norvegicus were common species,accounting for 18.28% (306/1 674) and 13.20% (221/1 674),respectively;the density of outdoor rat was 1.67% (1 674/100 180),there was no significant difference in rat density between different years (x2 =6.241,P > 0.05).A total of 9 541 murine animals were examed and 2 270 rats with flea were detected,the flea infestation rate of rat was 23.79%,the rat body flea index was 1.019 4,Xenopsylla cheopi was the dominant flea species and the index was 0.557 3.Conclusions The plague natural focus of Guizhou is at a quiescent stage.The plague monitoring work should be strengthened,health education should be done well,and plague epidemics should be stricdy prevented.
5.Analysis of plague monitoring results from 2018 to 2020 in Guizhou Province

Objective:To analyze the monitoring results of plague foci in Guizhou Province, to master the epidemic dynamics of the plague and to formulate the prevention and control measures.Methods:The plague monitoring data from 2018 to 2020 in Guizhou Province were collected from China Disease Prevention and Control Information System Plague Prevention and Control Information Management System to analyze the plague epidemic situation. The murine animals capture methods used were cage clip method, 5 m cage clip method and day-by-day rodent capture method. Bacterial culture and hemagglutination were used in laboratory.Results:A total of 2 273 murine animals were captured indoors with cage clip method, and the Rattus norvegicus was the dominant species, accounting for 47.29% (1 075/2 273), Rattus flavipectus and Mus musculus were common species, accounting for 43.55% (990/2 273) and 5.10% (116/2 273). A total of 1 460 murine animals were captured outdoors with 5 m cage clip method, and the Rattus rattus sladoni was the dominant species, accounting for 35.48% (518/1 460), Rattus flavipectus and Eothenomys miletus were common species, accounting for 16.03% (234/1 460) and 8.70% (127/1 460). A total of 5 742 murine animals (including 3 733 animals captured with cage clip method and 5 m cage clip method, 2 009 animals captured with day-by-day rodent capture method) were examined and 1 176 with flea, the flea infestation rate of murine animals was 20.48%, Xenopsylla cheopi was the dominant flea species [56.63% (3 020/5 333)] and the index was 0.53. Laboratory test results were all negative. Conclusions:The plague foci of Guizhou Province is at a quiescent stage, but the reignition cannot be ruled out. The monitoring of plague should be strengthened, the quality of the monitoring should be improved, and health education and publicity should be carried out to prevent the occurrence of human plague outbreaks.
6.Evaluation on the effects of prevention and control programs regarding typhoid fever and paratyphoid fever in Guizhou province,from 2007 to 2012

Objective This study was to evaluate the effects of prevention and control regarding programs on typhoid fever and paratyphoid fever,in Guizhou province,from 2007 to 2012, to provide evidence for the improvement of related programs. Methods Data on typhoid fever and paratyphoid including information on epidemics,individual,cases,measures for prevention and control programs taken and relative government documents were collected and analyzed in Guizhou province,from 2007 to 2012. Information related to the average annual incidence,nature of outbreaks, time span before confirmed diagnosis was made,unit which carried the case report,proportion of laboratory confirmed diagnosed cases and case-management were compared between 2007-2009 and 2010-2012 descriptively while chi-square test with Excelland EpiInfo software were used for data analysis. Results In the period of 2007-2009,a total of 5 978 typhoid fever and paratyphoid fever cases were reported in Guizhou province with the average yearly incidence as 5.29/100 000. In the period of 2010-2012,2 765 cases were reported with the average yearly incidence as 2.57/100 000. When compared to the former,data from the latter period showed that the average yearly incidence had declined 51.31% in all the prefectures. There were still some outbreaks appeared but the total number of cases involved reduced 87.50%. The time span before the confirmation of diagnosis became shorter but the difference was not statistically significant (χ 2=0.08,P=0.99). Number of cases reported by hospitals at county or above had 11.51% of increase while those cases reported at the township hospitals or below decreased for 61.47%. The proportion of laboratory diagnosed cases increased 23.63%. Rates of timeliness on cards being filled in,inputted and audited showed increase of 8.44%,6.76%and 2.40%respectively. Conclusion Successful measures for prevention and control on typhoid fever and paratyphoid fever had been remarkably taken in Guizhou province,but the potential risk of outbreaks still existed in some areas,suggesting that health education and surveillance programs including laboratory diagnosis,should be strengthened.

Objective To understand the genetic and epidemiologic characteristic of Brucella (B.) melitensis strains isolated in Guizhou province in 2010-2012. Methods B. genus specific BCSP31-PCR and species-specific AMOS-PCR were used to identify the bacteria strain,while the identified strains were analyzed under MLVA-16 and cluster analysis of B. melitensis strains. The strains were isolated from Guizhou and other provinces. Results Six B. melitensis strains were identified as B. melitensis using the BCSP31-PCR and AMOS-PCR. Data from the MLVA-16 analysis revealed the differences of repeated numbers at parts of the VNTR locus in the six strains isolated in Guizhou province. The six strains from Guizhou province and 105 B. melitensis strains from other province could be divided into 72 MLVA types(MT). Strain ZY and ZA from Guizhou province were typed as MT63,and LL3,LL4 and LL11 were typed as MT67,while strain SQ was typed as MT72. Data from the clustering analysis showed that ZY,ZA,LL3,LL4 and LL9 were most closely clustered with B. melitensis isolates from Yunnan,Fujian and Guangdong provinces,but strain SQ was genetically remote from other strains. Conclusion PCR methods,combined with MLVA-16, identified the six B. melitensis strains isolated in Guizhou province in 2010-2012 as B. melitensis biovar 3,with the genetic diversity of the strains showed. Six strains were closely related to the B. melitensis strains from Yunnan,Fujian and Guangdong provinces. The results of this study provided scientific basis for the control and prevention of Brucellosis in Guizhou province.

9.Investigation on the causes of increased hepatitis E cases reported in Guizhou province.

OBJECTIVETo analyze causes of growing hepatitis E (HE) cases reported in Guizhou province, and probe into existing problems faced by medical institutions in diagnosis of clinical and laboratory-confirmed cases, for the purpose of improving the quality of HE surveillance system.
METHODSSix hospitals reporting greater HE cases from 2007 to 2011 were pinpointed, whose reported cases rose suddenly in 2011. Such cases were investigated by means of impatient medical record review, results of laboratory test and clinician interview.
RESULTS136 of the 354 reported HE cases investigated were found compliant with the diagnostic criteria of HE with an accordance rate of 38.42%. Difference of the HE diagnostic accordance rate among individual years, hospitals and reporting departments was statistically significant. Such rate of hospital reports was found to be the lowest in 2011, ranging from 0 to 18.18% respectively; HE cases reported by non-infectious departments accounted for 61.30% of total cases reported, with its accordance rate considerably below the infectious departments (8.29%). HE positive cases and HE positive rate in 2011 were significantly higher than that of preceding years.
CONCLUSIONSuch increase of reported HE cases in 2011 in the province was mostly attributable to more HE laboratory tests made by the hospitals, yet the accordance rates were lower than satisfactory. In this regard, the medical institutions in question were advised to enhance their competency training for HE diagnosis and case report quality.
