1.Comparison of the effect of different dose of omeprazole in the treatment of chronic gastric ulcer

Objective To compare the treatment effect of different dose of omeprazole in the treatment of chronic gastric ulcer.Methods 153 patients with gastric ulcer were selected as the research object,according to ran-dom number table.The patients were randomly divided into three groups,with each group of 51 cases.In the first three days,group A was given omeprazole 40mg added to 0.9% sodium chloride injection 20mL,intravenous drip,4 times/d. Group B was given omeprazole 40mg added to 0.9% sodium chloride injection 100mL,intravenous drip 2 times/d. Group C was given omeprazole 40mg added to 0.9% sodium chloride injection 100mL,intravenous drip 1 time /d. Then took omeprazole magnesium enteric -coated tablets oral,20mg/times,1 time /d.The treatment course was 4 weeks.The treatment effectiveness and adverse reactions were observed.Results The effective rates of 3 groups had significant difference(χ2 =11.573,P =0.003).The effective rate of group A was 98.3%,which was significantly higher than 84.3% of group B(χ2 =6.725,P =0.010)and 74.5% of group C(χ2 =11.922,P =0.001).The effec-tive rate of group B was higher than that of group C,but the difference was no statistically significant(χ2 =1.499,P =0.221).The patients of 3 groups had occasional headache,nausea,vomiting,fatigue,skin rashes and other adverse reactions,the symptom was lighter,there was no impact to the normal treatment.Conclusion In the first three days of the treatment,omeprazole added to 0.9% sodium chloride injection 20mL,intravenous drip,4 times/d,then taking omeprazole magnesium enteric -coated tablets oral to treat gastric ulcer has higher curative rate,less adverse reac-tion,the effect is better than that of middle and low doses of the drug,which has clinical value.
2.Changes and meaning of Serum IL -4,IL -10,IFN -gamma,TGF -beta 1 level in patients with delayed en-cephalopathy after carbon monoxide poisoning

Objective To study changes and meaning of serum IL -4,IL -10,IFN -gamma,TGF -beta 1 level in patients with delayed encephalopathy after carbon monoxide poisoning.Methods From January 2011 and June 2014 in our hospital,40 cases of DEACMP patients were selected as group A;the 40 patients with DEACMP was divided into 12 cases of mild cognitive dysfunction,mild cognitive dysfunction in 20 cases,8 cases of severe cognitive dysfunction.40 cases with acute carbon monoxide poisoning (ACMP)patients were selected as group B;40 cases of healthy subjects were selected as healthy controls.Serum interleukin 4 and interleukin 10(IL -4)(IL -10),interfer-on gamma (IFN -gamma),conversion,growth factor beta 1 (TGF -beta 1)content were tested.Results In Group A and group B serum IL -4 levels were lower than that of healthy controls (P <0.01),and serum levels of IL -10 in group B were higher than that in group A and healthy controls (P <0.01),serum level of IFN -gamma in group A and group B was higher than the healthy controls (P <0.01),and in group A IFN -gamma levels were higher than group B (P <0.01),while in group A serum TGF -beta 1 level was lower than B group and the control group (P <0.01),and in group B TGF -beta 1 level was higher than the control group (P <0.01);With cognitive impairment aggravating of DEACMP patients,IL -4,IL -10,TGF -beta 1 levels were on the decline,and IFN -gamma level was increasing (P <0.01).Conclusion DEACMP patients serum and cerebrospinal fluid of IL -4,IL -10,IFN -gam-ma,TGF -beta measures such as abnormal expression,showed that the onset of DEACMP may be related to neural immune injury.Serum IL -4,IL -10,IFN -gamma and TGF -beta of DEACMP patients exists abnormal expression Serum IL -4,IL -10,IFN -gamma and TGF -beta levels are also related to the degree of cognitive impairment of DEACMP patients.The onset and progress of DEACMP may be associated with neural immune injury.
3.CT-guided Percutaneous Lung Puncture for the Diagnosis of Solid Pulmonary Nodules: A Single-center Experience Summary.

BACKGROUND:
To investigate the diagnostic significance of percutaneous lung puncture for solid pulmonary nodules (diameter ≤15 mm).
METHODS:
This study retrospectively included 20 patients with solid pulmonary nodules who underwent percutaneous puncture from January 2014 to December 2018, including 11 males and 9 females. The diameter of the lesion is between 0.5 cm-1.5 cm, excluding severe organ dysfunction, and patients with coagulopathy.
RESULTS:
All 20 patients were successfully selected, and 19 patients were diagnosed with pathological diagnosis. Among them, 11 patients found malignant tumor cells, which were clearly malignant tumors of the lungs, 5 cases of chronic inflammation of the lungs, 2 cases of fibrous tissue hyperplasia, and 1 case of lung cartilage tissue, no tumor cells were found in 1 case. One patient with a small amount of pneumothorax after puncture and one patient with a small amount of pleural effusion on the puncturesite.
CONCLUSIONS
Percutaneous lung puncture has a high effectiveness and safety for the diagnosis of solid pulmonary nodules.
4.Effects of bisphenol A exposure at different circadian time on hepatic lipid metabolism in mice

Background Lipid metabolism in liver shows circadian-dependent profiles. The hepatotoxicity of environmental chemicals is dependent on circadian time. Objective To observe the effects of bisphenol A (BPA) exposure at different zeitgeber time (ZT) on hepatic and blood lipid metabolism and decipher the underlying mechanisms related to circadian rhythm in mice. Methods Thirty-five female C57BL/6J mice were sacrificed every 4 h in a light-dark cycle (12 h/12 h). The liver tissues were collected to describe the circadian profiles of hepatic Rev-erba, Bmal1, Clock, Srebp1c, and Chrebp mRNA expression levels within 24 h. Thirty female mice were divided into 6 groups by the timing (ZT3 represents the 3 h after light on, ZT15 represents the 3 h after light off) and dose (50 or 500 μg·kg−1·d−1) of BPA exposure to observe hepatotoxicity. Mice were gavaged with designed doses of BPA once per day for 4 weeks. Mice were maintained with ad libitum access to food and water and measured body weight weekly. After the experiment, mice were euthanatized and liver tissues were separated to determine the biochemical indicators of lipid metabolism and lipid metabolism- and circadian-related gene mRNA expressions. Results Hepatic Rev-erba, Bmal1, Clock, Srebp1c, and Chrebp mRNA expression levels were rhythmic during a 24 h period in mice. At ZT3 and ZT15, BPA did not alter body weight, plasma glucose, plasma total cholesterol, plasma low density lipoprotein cholesterol, and plasma triglycerides (P>0.05). The plasma high density lipoprotein cholesterol decreased in the 50 μg·kg−1·d−1 BPA group at ZT3 by 14.56% compared with the control group (P<0.05). The liver triglycerides increased in the 50 μg·kg−1·d−1 BPA group at ZT15 by 115.20% compared with the control group (P<0.05). BPA decreased Srebp1c mRNA expression level when dosing at ZT3 and increased Chrebp, Srebp1c, and Acc1 mRNA expression levels when dosing at ZT15 compared with the control group (P<0.05). BPA increased Bmal1 mRNA expression level and decreased Rev-erbα mRNA expression level at ZT3 exposure and decreased Bmal1 and increased Rev-erbα mRNA expression level at ZT15 exposure (P<0.05). Conclusion BPA exposure at light or dark period has different effects on hepatic lipid metabolism in mice. Hepatic lipid deposit appears when BPA is dosed at dark period. Rev-erbα-Bmal1 regulation circuits and the subsequent upregulation of Srebp1c and Chrebp and the target gene Acc1 may be involved.
