1.Clinical significance of laparoscopic-assisted radical gastrectomy in advanced gastric cancer

Objective To study the effect of laparoscopicassisted radical gastrectomy in advanced gastric cancer.Methods According to the inclusion criteria,164 patients with advanced gastriccancer who treated in the Cancer Institute and Hospital,Chinese Academy of Medical Sciences betweenJanuary and June of 2009 were enrolled into the study.Based on the operation methods,all cases were divided into the experimental group(laparoscopy group,n =79)and the control group(laparotomy group,n =85).Except for the laparoscopicassisted radical gastrectomy,patients received the same treatment in thethe experimental group and control group.Intraoperative indexes,postoperative pathological results and recovery,complications and 5year survival rate were observed and compared between groups.Results Therecovery time of patient's general condition in the experimental group was shorter than that of the controlgroup(P <0.05).The difference of serious postoperative complications rate between the two groups wassignificant(P <0.05).The difference of 5year survival rate between the two groups was not significant(P >0.05).Conclusion Laparoscopicassisted radical gastrectomy is safe and feasible in the treatmentof advanced gastric cancer.This technique could get equal treatment effect as laparotomy and it has obviousadvantage in postoperative recovery.
2.Investigation and analysis of expenses of outpatients at counties and townships medical institutions in Xinjiang Uygur Autonomous Region

ObjectiveTo understand condition about cost and economic burden of outpatients in countries and townships medical institutions in Xinjiang.MethodsThirty-one medical institution were selected and the cost and income of one-day outpatients were investigated,then the cost of different diseases,age groups and payment methods and the later disease cost burden were analyzed.ResultsOut-patient expenses of top ten common diseases was pneumonia100.12 yuan,injury 85.18 yuan,hypertension and coronary heart disease(CHD) 69.13 yuan,examination and diseases related to pregnancy 49.60 yuan,disease of the genitourinary system 41.71 yuan,enterogastrtis 34.80 yuan,bronchitis 30.72yuan,osteoarthrosis 24.60 yuan,upper respiratory infection ( URI ) 23.63 yuan,scytitis 21.14yuan;The outpatient expenses of those taking part in Neotype Countryside Cooperative Medical Care Insurance,whose family-month-income was less than 250 yuan,was 18.07 yuan,which disease cost burden was 25.56%.ConclusionThe expenses of infectious diseases in country and township hospitals was in the top ten.The cost of chronic non-communicable diseases was rising significantly;For those participating Neotype Countryside Cooperative Medical Care Insurance,the outpatient expenses was low and the disease economic burden was higher.
3.Application of 16-detector row spiral CT for pulmonary angiography and reconstruction techniques in diagnosis of pulmonary embolism in elderly patients

Objective To study the application of pulmonary angiography and reconstruction techniques with 16-detector row spiral CT in the diagnosis of pulmonary embolism (PE) in the elderly.Methods Twenty-four elderly patients with suspected pulmonary thromboembolism received detection of CT and pulmonary artery angiography ( CTPA ) on 16-detector MSCT.Post-reconstruction techniques included maximum intensity projection (MIP), volume rendering (VR)and multiple planar reformation (MPR). Results A total of 161 pulmonary artery thrombi were detected in 24 elderly patients. The direct signs of pulmonary embolism included total occlusion (16.8%, 27/161), partial filling defect (67.7%, 109/161) and central filling defect or track sign (15.5%, 25/161). A total of 161 pieces of pulmonary thrombi were detected in transect image, 153 (95.0%) in MPR, 113 (70.2%) in MIP and 69 (42.9%) in VR. The transect image excelled evidently MIP and VR image in displaying pulmonary thrombi, especially the thrombi in pulmonary lobe and pulmonary artery branch (χ2 =235.36 and 243.41, P<0.05). Conclusions The 16-detector row spiral CT pulmonary angiography is non-invasive, fast and high sensitive, it should be the first choice for the diagnosis of PE in the elderly.
4.Influence of CPAP treatment on maximal oxygen uptake etc.in patients with CHF complicated OS-AHS

Objective:To explore influence of nasal continuous positive airway pressure (nCPAP)on maximal oxygen uptake (V · O2 max )etc.in patients with chronic congestive heart failure (CHF)complicated moderate-to-severe ob-structive sleep apnea-hypopnea syndrome (OSAHS)and explore its significance.Methods:A total of 83 CHF com-plicated moderate-to-severe OSAHS patients were selected and randomly divided into routine treatment group (n=40)and nCPAP group (n = 43).Both groups were treated for six months.Left ventricular ejection fraction (LVEF),apnea hyponea index (AHI)and V · O2 max were measured and compared between two groups before and af-ter treatment.Results:Compared with routine treatment group after six-month treatment,there was significant re-duction in AHI [(27.5±6.2)vs.(6.8±1.2)],and significant rise in LVEF [(0.45±0.07)vs.(0.48±0.05)]and · V O2 max [(16.5±3.5)ml·kg-1 ·min-1 vs.(19.2±3.4)ml·kg-1 ·min-1 ]in nCPAP group,P <0.05 all.Con-· clusion:The nCPAP can improve ventilation function,heart function and V O2 max in patients with CHF complicated moderate-to-severe OSAHS,who have received basic medication.
5.Comparative study on different anti-adhesion agents in preventing tubal obstruction after interventional recanalization

Objective To study the efficacy of different anti-adhesion agents used in preventing tubal obstruction after recanalization.Methods Five hundred and eight patients with tubal obstruction were divided into 245 cases in control group,108 cases in chitosan group;113 cases in sodium hyaluronate group and 42 cases in lipiodol group.The patients in control group were injected with anti-inflammation agents after recanalizatian,while other groups were injected with chitosan,sodium hyaluronate or lipiodol at dose of 2-3 ml in every therapeutic group.The rate of location of tubal obstruction and tubal recanalization were recorded during operation.Then patients in every group were followed up on tubal patency after 3 months,and pregnancy rate after 12 months.Results Among 1016 fallopian tubes in 508 patients,there were 330 tubes occlusion at isthmus portion and 563 tubes occlusion at interstitial portion of fallopian tube.Thirtyseven fallopian tubes were ablated because of ectopic pregnancy,86 fallopian tubes were unobstructed.(1)The recanalization rate were 95.7% (179/187) in chitosan group,97.9% (191/195) in sodium hyaluronate group,98.7% (75/76) in lipiodol group and 97.7% (425/435) in control group,which did not show statistical difference (P>0.05).(2) The rates of tubal patency after 3 months of 91.7% (99/108) in chitosan group and 88.5% (100/113) in sodium hyaluronate group were significantly higher than 71.4% (30/42) in lipiodol group and 74.3% (182/245) in control group (P <0.05).(3)The rates of intrauterine pregnancy after 12 months were 48.1% (52/108) in chitosan group and 41.6% (47/113) in sodium hyaluronate group,which were significantly higher than 23.8% (10/42) in lipiodol group and 24.1% (59/245) in control group (P < 0.05).Conclusion Chitosan and sodium hyaluronate could be effective to prevent tubal obstruction after interventional recanalization and increase pregnancy rate.
6.The clinical comparative study on the therapeutic effects of NICU patients implemented by NICU professional doctors and non-NICU professional doctors

Objective To explore the implementation styles on the therapeutic effects on the neurosurgical intensive care unit (NICU) patients. Methods Patients were enrolled during February 3, 2015 to February 3, 2016. The key point time was August 3, 2015 when the treatment in our NICU was fully implemented by NICU professional doctors. Based on this time point, all the enrolled patients were divided into non-NICU professional doctor implementing (NNPDI) group and NICU professional doctor implementing (NPDI) group. Thus non-NICU professional doctors and professional doctors were the leaders of diagnosis and treatment in tow groups. The length of hospital stay, complications, prognosis and other therapeutic outcomes were compared between two groups. Results The length of hospital stay was longer in NPDI group than that in NNPDI group (P<0.05). The incidence of water-electrolyte imbalance was lower in NPDI group than that in NNPDI group (P<0.05). There were no significant differences in the incidence of the ventilator-associated pneumonia (VAP), the hepatic and renal insufficiency, the intracranial infections and stress ulcers between the two groups (P>0.05). The proportion of referral to other wards and fatality rate were both lower in NPDI group than those in NNPDI group (P<0.05). And the discharge rate from NICU was higher in NPDI group than that of NNPDI group (P<0.05). There was no significant difference in the rate of patients left hospital without treatment between the two groups (P>0.05). Conclusion The NICU professional doctor implementing may be contribute to, at least in part, the improving of prognosis of NICU patients without obvious advantages in most complications. The level of professional management remains to be improved.
7.Clinical observastion of Bushen-Qiangjin capsule and calcium D on aromatase inhibitors-induced osteopenia in breast cancer patients

Objective To observe the clinical effect of the Bushen-Qiangjin capsule and calcium D in the treatment of aromatase inhibitors-associated bone loss (AIBL) in breast cancer patients. Methods A total of 65 patients were randomized into a treatment group and a control group, 32 in the control group taking calcium D, and 33 in the treatment group taking calcium D and Bushen-Qiangjin capsule. After a 3-month treatment, the bone mineral density T (BMD), bone-specific alkaline phosphatase (BALP), bone gla protein (BGP) and tartrate resistant acid phosphatase (TrACP) of two groups were evaluated. Results The BMD increased significantly after treatment in both groups (P<0.05), and the therapeutic efficacy of the treatment group was better than of the control group (P<0.05). After treatment, the level of BALP (308.76 ± 10.99 U/L vs. 280.00 ± 7.44 U/L, t=8.170) and the BGP (42.21 ± 3.04 ng/ml vs. 34.38 ± 2.06 ng/ml, t=6.818) of the treatment group were significantly higher than those of the control group (P<0.01). The level of TrACP decreased significantly after treatment in both groups (P<0.05), and the TrACP (60.12 ± 4.58 U/L vs. 67.25±4.06 U/L, t=1.653) of treatment group was significantly lower than that of the control group (P<0.05). Conclusions The Bushen-Qiangjin capsule can produce a content efficacy in treating AIBL in breast cancer patients, improving the BMD and bone metabolism.
8.Effects of curcumin on proliferation, cell cycle and apoptosis of human thyroid cancer TPC-1 cells

Objective:To investigate the effects of curcumin on proliferation, cell cycle, and apoptosis of human thyroid cancer TPC-1 cells.Methods:Logarithmic growth phase TPC-1 cells were treated with 0.0, 7.5, 15.0 and 22.5 μmol/L curcumin for 48 h. Cell proliferation levels were determined using the methylthiazolyl tetrazolium (MTT) assay. Cell cycle and apoptosis levels were measured using flow cytometry. Western blot method was used to determine the expression levels of B cell lymphoma-2 (Bcl-2) and Bcl-2-associated X protein (Bax).Results:The inhibition rates of cell proliferation in 7.5, 15.0 and 22.5 μmol/L curcumin groups were higher than that of 0.0 μmol/L curcumin group ( P < 0.05); the proportion of G0/G1 phase was higher than that of 0.0 μmol/L curcumin group, but the proportions of G2/M and S phase were lower than those of 0.0 μmol/L curcumin group ( P < 0.05); the cell apoptosis rate was higher than that of 0.0 μmol/L curcumin group [(14.13 ± 0.57)%, (25.27 ± 0.62)%, (36.01 ± 0.84)% vs (8.48 ± 0.43)%, P < 0.05]; the protein expression of Bcl-2 was lower than that of 0.0 μmol/L curcumin group, and protein expression of Bax was higher than that of 0.0 μmol/L curcumin group ( P < 0.05). Conclusion:Curcumin can inhibit the proliferation of TPC-1 cells, block the cell cycle in G0/G1 phase, and promote cell apoptosis by downregulating Bcl-2 expression and upregulating Bax expression.
9.Lung nodule detection algorithm using improved YOLOv7 network model

To address the issues in the current lung nodule detection for tuberculosis where the existing object detection algorithms have limited precision for small nodules and often predict bounding box locations inaccurately,a lung nodule detection method based on YOLOv7 is presented for obtaining small lung nodules more effectively and realizing the continuous convergence of target detection box.Based on the framework of YOLOv7 network model,the improvements are made in the following 3 aspects.(1)The cross-channel information and target airspace information are obtained with the effective SimAM channel attention mechanism embed in the Head network,so as to highlight the target features and enable the model to identify the regions of interest more accurately.(2)SIOU boundary loss function is used to increase the angle cost on the original loss function,and redefine the distance cost and shape cost to improve the convergence rate and reduce the loss value.(3)SIOU-NMS is used to replace the non-maximum suppression algorithm for reducing the error suppression due to target occlusion.The results of experiments on a custom lung nodule dataset show that compared with the original YOLOv7,the proposed method improves accuracy and recall rate by 2.9%and 3.1%,and the mean average precision at a confidence threshold of 0.5 is increased by 3.7%.The model can effectively assist in the diagnosis of lung nodules.
10.Biventricular segmentation using U-Net incorporating improved Transformer and convolutional channel attention module

A U-Net incorporating improved Transformer and convolutional channel attention module is designed for biventricular segmentation in MRI image.By replacing the high-level convolution of U-Net with the improved Transformer,the global feature information can be effectively extracted to cope with the challenge of poor segmentation performance due to the complex morphological variation of the right ventricle.The improved Transformer incorporates a fixed window attention for position localization in the self-attention module,and aggregates the output feature map for reducing the feature map size;and the network learning capability is improved by increasing network depth through the adjustment of multilayer perceptron.To solve the problem of unsatisfactory segmentation performance caused by blurred tissue edges,a feature aggregation module is used for the fusion of multi-level underlying features,and a convolutional channel attention module is adopted to rescale the underlying features to achieve adaptive learning of feature weights.In addition,a plug-and-play feature enhancement module is integrated to improve the segmentation performance which is affected by feature loss due to channel decay in the codec structure,which guarantees the spatial information while increasing the proportion of useful channel information.The test on the ACDC dataset shows that the proposed method has higher biventricular segmentation accuracy,especially for the right ventricle segmentation.Compared with other methods,the proposed method improves the DSC coefficient by at least 2.83%,proving its effectiveness in biventricular segmentation.
