2.Application of spatio-temporal image in normal fetal heart ultrasonography
Chinese Journal of Medical Imaging Technology 2009;25(10):1841-1844
Objective To explore the basic method and clinical application of spatio-temporal image correlation (STIC) in the routine scanning of normal fetal heart. Methods Totally 120 volume datasets of normal fetal heart were acquired with 2D and STIC, respectively. The difference in time between the two methods were compared and the basic imaging method was explored and analyzed. Results Satisfactory gray-scale volume acquisitions were accomplished in all 120 cases. The quality of the images derived from volume datasets were comparable to that directly obtained from 2D echocardiography. No significant difference of visualization rate was found between the two methods (P>0.05). Some views that could not be showed with 2D were acquired with STIC. In fetal heart screening, STIC (n=104) and routine ultrasonography (n=85) took (4.47±2.32) min and (5.26±2.01) min per case, respectively. Conclusion STIC provides more views and information than 2D, simplifies the picture-gathering process, decreases the dependence of examiner, increases the efficiency, and decreases the exposure upon fetal heart obviously.

The patient is an 11-year old boy, who was born with universe alopecia as well as dry and coarse skin. When he was 3 months old, photophobia was noticed, and since then, upper respiratory tract infection had occurred twice a month complicated by frequent diarrhea. He had short stature with slight conjunc- rival congestion, corneal vascularization, opacity, coarseness and poor vision. No abnormality was found in the teeth, sweating ability, or hearing. He had universal alopecia; his skin was dry and rough with generalized rhombus- or polygon-shaped scaly patches. Particularly thick brown scales were observed on the upper limbs. Moreover, there were spiny follicular papules on the abdomen and axillae, hyperkeratosis of palm and sole, and dystrophic nails. Hyperextensibility of proximal interphalangeal joints of the third, fourth and fifth fingers was noticed. He also suffered from mental retardation, the verbal intelligence quotient being 52, performance intelligence quotient lower than 40, full intelligence quotient lower than 40, but no abnormality was found in the heart, lung, liver or spleen. Histopathology of skin on the abdomen suggested a change characteristic of ichthyosis. Chromosome analysis revealed a karyotype of 46, XY. This is the first diagnosed case of ichthyosis follicularis with atrichia and photophobia syndrome in China.

Chinese Journal of Ultrasonography 2003;0(12):-
Objective To analyze strain characteristics of hypertrophic cardiomyopathy(HCM) and explore the clinical value of strain rate imaging in differentiating HCM from hypertensive left ventricular hypertrophy(H-LVH). Methods Parameters of strain and strain rate were obtained and compared among 23 HCM patients, 30 patients with H-LVH and 20 healthy people(control group). Results In HCM group, systolic strain(?_ sys ),peak systolic strain rate(SR_ sys ), early diastolic strain rate(SR_ dia ) obtained from inter-ventricle septum(IVS) were smaller than that obtained from left ventricle posterior wall(LVPW), while SR_a increased in IVS. ?_ sys ,SR_ sys ,SR_ dia in both patients with HCM and H-LVH were smaller than that in control group, while atrial systolic strain rate(SR_a) increased in HCM group and H-LVH group. ?_ sys , SR_ sys , SR_ dia , SR_a obtained from IVS and ?_ sys , SR_ dia obtained from LVPW in HCM group showed statistic differences compared with H-LVH group. Conclusions Strain rate imaging could accurately detect the abnormal regional myocardial function of HCM and can be applied as a useful tool in discriminating HCM from H-LVH.

BACKGROUND: Nestin is a specific antigen of neural stem cells which widely expressed in lesion of nervous system and brain regeneration. Thus, nestin expression is commonly used to assess whether lesion or damage of the nervous system can promote neural regeneration.OBJECTIVE: To investigate the effects of erythropoietin(EPO)on nestin expression in neural stem cells after hypoxia-ischemia brain damage(HIBD)in neonatal rats from the angles of neural regeneration and activation of neural stem cells.METHODS: HIBD model was established by ligation of the right common carotid artery along with 2-hour 8% hypoxia exposure in neonatal rats. The control group was not subjected to hypoxia-ischemia,and the right common carotid artery was dissociated. The treatment group received an intraperitoneal injection of recombinant human erythropoietin(rh-Epo,5 000 IU/kg)once a day for three days after hypoxia/ischemia,while the two other groups intraperitoneally received normal saline at the same time.In each group, rats were randomly executed immediately,at 4,7,14 days after operation(n = 8).The nestin expression in hippocampal dentate gyrus region was examined by immunohistochemical staining and image quantitative analysis respectively.RESULTS AND CONCLUSION: The number of nestin-positive cells was significantly increased in HIBD group compared to control group at all time points(P < 0.05),and it was also significantly increased in treatment group than the other two groups at all time points(P < 0.05).The numbers of nestin-positive cells in hippocampal dentate gyrus region were significantly increased, and peaked on day 7 after operation in the three groups. The results showed that exogenous rh-Epo could enhance the expression of nestin in hippocampal dentate gyrus region of neonatal rats with HIBD,and promote the proliferation of neural stem cells,rh-Epo plays an important role in the regeneration and repair of neurocytes damaged by hypoxia-ischemia.

Objective To investigate the features of event-related potentials (ERP) P300, auditory evoked potential(AEP) and brainstem auditory evoked potential(BAEP) in senile depression patients and the dynamic chan-ges before and after treatment of sertruline. Methods P300, AEP and BAEP were recorded in 31 senile depression patients using Chinese WOND 2000C Instrument, and a comparison between the patients and 33 normal controls in terms of these evoked potentials were made. Some of the depression patients treated with oral intake of sertraline were followed electophysiologically after 12 months of treatment. Results Compared with normal controls, senile depres-sion patients showed delay of latency of P300-P3 and BAEP-Ⅲ, decreased amplitude of P300-P3, AEP-P2 and BA-EP-Ⅲ(P <0.05 or 0.01). The 12-month follow-up measures of ERP showed significant improvement in terms of la-tency and amplitude of P3/P300,AEP-P2 and BAEP-Ⅲ after treatment of sertraline. Conclusion Senile depres-sion patients showed abnormality on several ERP indexes (such as P3/P300, AEP-P2 and BAEP-Ⅲ), and these in-dexes might provide important evidences to monitor the effect of medicine.

Objective To investigate clinical significance of transforming growth factor-beta 1 ( TGF-β1 ) in patients with chronic idiopathic thrombocytopenic purpura(CITP). Methods The serum level of TGF-β1 in 38 pa-tients with initial CITP were detected using enzyme-linked immunosorbent assay(ELISA). Results The serum level of TGF-β1 in initial patients with CITP was significantly higher than that of the controls [( 132.57±5.17) μg/L vs ( 76.81±4.42) μ/L] ( P <0.01 ). The serum level of TGF-β1 in those having good response after therapy was sig-nificantly lower than before treatment[(81.26±3.78)μg/L] (P <0.01 ). There was no difference in TGF-β1 be-tween nonremission [(123.49 ± 4.31 ) μg/L] and initial patients (P > 0.05 ). There was negative correlation between TGF-β1 and platelet count(r = -0. 342 ,P < 0.05 ) ,there was positive correlation between TGF-β1 and megakaryo-cyte count (r = 0.409, P < 0.01 ). Conclusions TGF-β1 partakes in the pathogenesis of CITP, the determination of which in patients with CITP is useful to judge the state of illness, which can be regarded as an assistant index of cur-ative effect.

Objective To investigate the expressions of Dapper1 in gastric carcinoma and elucidate its relationship with survivin and its role in tumor cell apoptosis. Methods Dapper1 mRNA was detected with RT-PCR using specimens from 30 cases of gastric carcinoma and the corresponding normal gastric mucosa. The pcDNA3.1-Dpr1 plasmid was transfected into SGC-7901 cells with LipofectamineTM 2000.The effect of upregulation of Dpr1 on SGC7901 cell apoptosis was determined by flow cytometry. The downregulation of survivin、Dvl-2 and β-catenin protein expression were detected by Western blot analysis. Results Downregulation of Dpr1 gene expression was observed in 17(57%)of 30 human gastric cancer and the downregulation was significantly correlated with the depth of invasion and the degree of differentiation (P<0.05).Also,upregulation of Dpr1 mRNA and downregulation of survivin mRNA were detected after transfecting pcDNA3.1-Dpr1 plasmid in SGC7901 cells, which led to downregulation of survivin、Dvl-2、β-catenin protein and increase of the SGC7901 cell apoptosis rate from 2.89%to 13.96%.Conclusion Downregulation of Dp1l gene expression is common in human gastric carcinoma, and upregulation of Dpr1 results in significant inhibition of survivin expression which can induce apoptosis of SGC7901 cells.

To report a case of 16-month-old boy with anhidrotic ectodermal dysplasia with immunodeficiency who experienced disseminated herpes simplex infection. From 2 months of age, the patient experienced multiple pyrexial episodes of undetermined origin, which responded well to anti-inflammatory agents after undressed. Abnormal sweat with dry skin was noted; therefore, the skin biopsy of right axilla was performed at 7 months of age, and suggested a diagnosis of anhidrotic ectodermal dysplasia. Since 6 months of age, he developed recurrent upper respiratory infections and 2 episodes of pneumonia. Twenty days before, several glossal erosions occurred in the patient, supervened by painful and erosive eruptions and numerous blisters around the mouth and both hands with hyperpyrexia. Four days before, the patient was transferred to the department owing to skin lesion exacerbation. Cutaneous examination showed multiple crested or ulcerated plaques distributed eriorificially (mouth and nasal cavity) on the face. Several irregular, demarcated ulcers were scattered on the buttocks, scrotum and lower limbs, surrounded by grouped and umbilicated vesicles arising on erythema. Both hands were swelling, crusting and painful. Dentition was abnormal, and the patient had only 2 upper conical incisors. Routine investigation revealed that white cell count and C-reactive protein extremely elevated. Immunologic profile showed an abnormal distribution of lymphocyte subsets with decreased CD3+ T cells, CD8+ T cells and NK cells. Serum IgM level was slightly low. IgM antibodies to herpes simplex virus type 1 (HSV-1) were detected by serological testing. Based on the above-mentioned features, a diagnosis of anhidrotic ectodermal dysplasia with immunodeficiency and disseminated herpes simplex infection was confirmed. The patient was resolved favourahly after intravenous ganciclovir and antibiotics for 3 weeks without relapse of skin lesions.
