Objective To investigate the changes of actin (HHF35), serum albumin (A1), fibronectin (Fn) in cardiac cells and cardiac conducting sysytem (CCS) in order to probe into postmortem diagnosis of sudden manhood death syndrome (SMDS). Methods Immunohistochemical detections of actin, serum albumin and fibronectin (Fn) in the cardiac cells and CCS were performed by S-P method in cases with SMDS and cases died from coronary heart disease and non-cardiovascular diseases which were used as the control. Results Of the 22 cases with SMDS, 17 were found of actin-negative staining, 18 serum albumin-positive and 15 Fn-positive staining with evident pathological lesions in CCS in 8 cases. Conclusion It is suggested that SMDS is associated with early myocardial ischemia or infarction, which results in abnormality in actin, serum albumin and fibronectin (Fn) in the cardiac cells and CCS.

In order to explore the significance of cardiomyocyte apoptosis in coronary heart disease (CHD), the TUNEL and transmission electron microscopic (TEM) techniques were performed. The results showed that the permillage of cardiomyocyte apoptosis in infarct area in patients of sudden death from CHD was significantly higher than that in control group (557.94? 144.10‰ VS 34.30?20.68‰, p

Objective To investigate the incidence and influencing factors of aldosterone escape in patients with non-diabetic nephropathy by RASI-therapy. Methods A total of 104 patients with non-diabetic nephropathy were treated with ARB or combination therapy of ACEI and ARB in a mean follow-up period of 12 months. Aldosterone escape was determined according to the change of plasma aldosterone concentration before and after treatment during 6-month and 12-month ACEI/ARB treatment, while the influencing factors of aldosterone escape in patients with non-diabetic nephropathy was also analyzed after therapy with RASI . Results In 12 months, the incidence of aldosterone escape was significantly higher than that in 6 months (26.92% vs. 14.42%, P = 0.007). After 12-month treatment, the difference was statistically significant in incidence of aldosterone escape among different stages of CKD (P = 0.027). Compared with 6-month incidence of aldosterone escape in the losartan group, 12-month incidence increased evidently (P = 0.020). The Ald level was positively correlated with urinary protein excretion and the Scr level (r = 0.431, P = 0.003 and r = 0.336, P = 0.009, respectively), and negetively correlated with levels of the eGFR (r = -0.275, P = 0.006). Univariate Logistic regression demonstrated that risk factors of aldosterone escape included pre-treatment values of the urinary protein excretion (OR = 3.671, P = 0.028) and the eGFR (OR = 0.972, P = 0.019). Multivariate Logistic model revealed pre-treatment values of the eGFR was positively associated with aldosterone escape (OR = 0.970, P = 0.012). Conclusion The incidence of the aldosterone escape increases along with the time of treatment. Renal function has correlated with aldosterone escape and pre-treatment value of the eGFR is an independent risk factor of aldosterone escape.

Objective To investigate the effects of tegaserod on visceral sensitivity and explore the regulating mechanism. Methods Forty-two male Spragre-Dawley rats, which were induced colonic inflammation by intraluminal administration of trinitrobenzenesulfonic acid(TNBS),were randomly divided into eight groups. In the three colorectal distention(CRD) treated groups(n=6),abdominal contractions were recorded after 3,7 and 14 days of intra-gastric administration of tegaserod 2mg/kg d.In the three CRD control groups(n=4),abdominal contractions were recorded after 3,7 and 14 days of intra-gastric injection of saline 2.0mL/d.In immunohistochemistry(IH) treated group(n=6) and IH control group(n=6),samples of colon were removed and processed for SP and CGRP immunohistochemistry after 7 days of intra-gastric administration of tegaserod and saline, respectively. Results Abdominal contractions induced by colonic distention decreased significantly at 1.2mL and 1.6mL distention volume after 3 days of tegaserod administration(P

Objective To evaluate the clinical efficacy and safety of capecitabine continuous maintenance in treatment of re-currence of triple-negative breast cancer, which had responded to combined chemoradiotherapy. Methods The triple-negative breast cancer was defined as negative of ER,PR and HER2.A total of 46 patients of triple-negative breast cancer were divided into the treatment group (23 patients)and the control group (23 patients). The treatment group was given capecitabine continuous mainte-nance after 6 cycles of chemotherapy. The control group was given 6 cycles of combined chemotherapy only. The clinical efficacy and safety was evaluated between the two groups. Results The PR,PD,RR,and DCR in the treatment group were significantly higher than those in the control group(P <0.05).The acute toxic effects (except for hand-foot syndrome)were similar in two group(P >0.05).Conclusion Capecitabine continuous maintenance after combined chemoradiotherapy in treatment of recurrence of triple-neg-ative breast cancer is more effective, lower toxicity and tolerable.

Objective To investigate the Mahaim's fibers of the cardiac con duction system (CCS) of human hearts and study their morphological features. Methods The CCS from 165 cases of sudden death without extracardiac causes and tho se from 760 cases of non-cardiac death were sampled using a method established by the authors. Serial sections were prepared for histological observation. Results Mahaim's fibers were discovered in 7 cases, 5 cases were in the sudden dea th group, accounting for 3% in that group, 2 cases were in the non-cardiac deat h group, accounting for 0.26% in that group. All cases belonged to the bundle-ve n tricular branch type. The characteristics of Mahaim's fibers were found to be a s follows. Their transverse diameter was 10~35 microns. The bodies of the Mahaim 's fibers that originated from A-V bundle were slender with nucleus in the middle of the cell. While the shape of cells from the left bundle branch varied accord ing to the origin of the Mahaim's fiber with size enlarging from up downward. B oth types of cells showed abundant lightly stained cytoplasm, similar to A-V bu n dle and left bundle branch cells. The cells were not bifurcated and some of them were surrounded by a thin layer of fibrous tissue. Conclusion Mahaim's fibers are not a rarity and it is an abnormal development of an A-V by-path.
