2.Clinical Study of the Collagenase Injection Combined Oxygen-Ozone for the Treatment of Lumbar Disc Herniation

Objective To assess the therapeutic effect of oxygen-ozone combined collagenase injection for the treatment of lumbar disc herniation.Methods Oxygen-ozone and collagenase were injected into the lumbar disc or outside the lumbar disc to treat the lumbar disc herniation.After the treatment,the patients were followed-up for 6~12 months,then the results were assessed according to the Macnab criteria.Results Of 183 patients,the effect was excellent in 144,good in 18,fair in 16 and poor in 5.The curative rate was 88.5%,the effective rate was 97.3%.No serious complications occurred in this group.Conclusion Oxygen-ozone combined collagenase injection is very effective,safe,and cheap method for treating lumbar disc herniation.
3.Effect of combination of tripterygium glycosides and valsartan on chronic glomerulonephritis complication with hypertension and effect on inflammatory cytokines

Objective To explore the clinical curative effect of combination of Tripterygium glycosides and Valsartan on chronic Glomerulonephritis complication with hypertension and the influence on inflammatory cytokines.Methods 90 cases of patients with chronic glomerulonephritis complication with hypertension in our hospital were selected from January 2015 to January 2016,To adopt randomized single blind controlled design methods,were divided into the treatment group Tripterygium glycosides combined with Valsartan,and the control group Tripterygium glycosides,each group were 45 cases,continuous medication for 24 weeks.Before and after treatment respectively observe the changes in blood pressure,24h urinary protein,endogenous creatinine clearance rate(Ccr),blood urea nitrogen(BUN),serum creatinine(Scr),glomerular filtration rate(GFR),inflammatory cytokines such as tumor necrosis factor-α,hypersensitive C-reactive protein,interleukin-6 as reference indicators.Results All the patients were observed for 24 weeks,no adverse effects were found.The effective rate of the treatment group was 95.55%,significantly higher than 75.55%of the control group(P<0.05).After treatment,systolic blood pressure(SBP),diastole blood pressure(DBP),24h urinary protein,SCr and BUN were all decrease,while Ccr and GFR were improved,and inflammatory cytokines such as tumor necrosis factor-α,hypersensitive C-reactive protein,interleukin-6 were reduced in the treatment group and the control group,the difference was statistically significant(P<0.05).Compared with the control group,blood pressure,24h urinary protein,Ccr, Scr,GFR,TNF-α、hs-CRP were improved in the treatment group(P<0.05).Conclusion Tripterygium glycosides combined with Valsartan treatment has good clinical curative effect,It can effectively relieve the symptom of chronic Glomerulonephritis complication with hypertension.
4.A clinical study of modified incision for video-assisted interruption of patent ductus arteriosus(PDA)

Objective To study the clinical advantages of modified incision for PDA interruption under video-assisted thoracoscopy. Methods Sixty patients suffering PDA were operated on by the modified incision technique through the left subaxillary 3rd interspace 2.0cm-2.5cm mini-incision as manipulation incision and the front axillary line of the same interspace 0.4cm-1.0cm thoracoscope incision the operative difficulty required surgical time surgical field exposure,complications,postoperative pain and,cosmetic and surgical results were compared with those of the conventional PDA operation. Results The operation on the 60 cases were all successful.Actual intra-thoracic operation duration was (20.39?9.63)min.Due to the minor surgical trauma,the patients were able to ambulate two days after the surgery.Only eleven patients needed analgesia after the operation.Heart murmur completely disappeared postoperatively.No complications occurred.Postoperative echocardiogram showed no residual or recurrent shunt ,even in three years follow-up. Conclusions The modified incision for PDA interruption with the video-assisted thoracoscopy causes minim surgical trauma and is a safe and simplified surgical technique.
5.The clinical study of twice chemonucleolysis with collagenase for the treatment of lumbar disc herniation

Objective To study the feasibility and indication of twice chemonucleolysis with collagenase for the treatment of lumbar disc herniation.Methods Eighty two patients of lumber disc herniation were treated with twice collagenase chemonucleolysis.All patients were followed up for 3 to 12 months and then the clinical results were assessed according to the Macnab criteria retrospectively.Results Eighty two cases were followed up from 3 to 12 months postoperatively.Fifty one cases were excellent,13 cases good,8 as fair and 10 were poor.The rate of excellent plus good reached 78%,the effective rate was 88%.Conclusion Twice chemonucleolysis with strict indications together with prevention of allergic reaction before,during and after the operation;is safe and effective for treating lumbar disc herniation.(J Intervent Radiol,2007,16:258-259)
6.Comprison among curative effects of different transplantion of BMSCs on rabbit model of cirrhosis

Objective To investigate the effect of bone marrow mesenchymal stem cells (bone marrow stem cells,BMSCs) through different approaches in the treatment of transplanted cirrhotic model in rabbit effect difference.Methods Establishment of rabbit model of carbon tetrachloride cirrhosis with liquid olive oil mixture of subcutaneous inj ection and ethanol fed,the liver cirrho-sis models were randomly divided into cirrhosis model group (n=10),hepatic artery transplantation group (n=10),ear vein trans-plantation group (n=10).Healthy New Zealand rabbits bone marrow,using the method of whole bone marrow adherent cultured bone marrow BMSCs,and then were transplanted and serum albumin,alanine aminotransferase in 3 ways;by HE staining,the de-gree of hepatic cirrhosis and detection of serum albumin by Western blot expression in liver.Results Through the vein group and ar-tery transplantation group BMSCs to cirrhotic model in rabbit,4 weeks after the discovery of liver transplantation group rabbits were significantly improved,serum albumin is liver cirrhosis without transplantation group increased significantly(P<0.05),transami-nase is liver cirrhosis without transplantation group decreased significantly (P<0.05).The therapeutic effect of 2 ways of BMSCs transplantation on rabbit liver cirrhosis had a significant difference (P<0.05).Conclusion Transplantation of BMSCs through the ear vein and the hepatic artery of rabbit after 4 weeks of treatment of liver cirrhosis,liver function can be improved.But the hepatic artery transplantation therapy is obviously better than that of intravenous transplantation of treatment effect.
7.Effects of infusion of esmolol on the expression of hypoxia inducible factor-1α in ischemia-reperfusion injury to spinal cord in rats

ObjectiveTo investigate the effect of infusion of esmolol on expression of hypoxia inducible factor-1α following spinal ischemia-reperfusion (I/R) in rats.MethodsThirty-six healthy male Wistar rats weighing 300-350 g were randomly assigned into 3 groups (n =12 each):group Ⅰ sham operation (group S); group Ⅱ spinal I/R and group Ⅲ esmolol pretreatment (group E).Spinal ischemia was produced by cross-clamping of abdominal aorta distal to renal artery for 20 min in I/R and E groups,Infusion of esmolol 200 g· kg- 1 ·min- 1 was initated 30 min before spinal ischemia and continued for the subsequent 1 h reperfusion in group Ⅲ (E).In groups S and I/R the animals received equal volumes of NS instead of lidocaine.Neurological behavior was assessed according to Tarlov scoring system at 24 and 48 h of reperfusion.The lumbar segment ( L4、5 ) spinal cord was resected at 24 and 48 h ofreperfusion for microscopic examination.The expression of HIF-Ia in spinal cord was detected by immunohistochemistry analysis.ResultsCompared with group S,the expression of HIF-Iα in spinal cord was down-regulated,and Tarlov score was significantly decreased in groups S and l/R.The spinal cord injury was attenuated in group E compared with group I/R.CondusionEsmolol infusion can protect the spinal cord against I/R injury,and inhibition of the expression of HIF-1α is involved in the mechanism.
8.Therapeutic Effect of Collagenase and Oxygen-Ozone in Patients with Lumbar Disc Herniation

Objective To evaluate the clinical effect of chemonucleolysis with collagenase and oxygen-ozone in lumbar disc herniation.Methods 90 cases with lumbar disc herniation confirmed by clinic,CT or MRI were randomly divided into the control group (collagease was injected outside the lumbar disc) and treatment group (based on the treatment of control group,oxygen-ozone was injected into lumbar disc).Results 90 cases were followed-up after the operation for 3 months. The excellent rate was 37.8% in the control group, 68.9% in the treatment group. The excellent or good rate was 93.3% in the control group, 95.6% in the treatment group. 9.Treatment of carotid atherosclerosis stenosis with eversion carotid endarterectomy

Objective To evaluate the clinical value of eversion carotid endarterectomy in patients with carotid atherosclerosis stenosis. Method Summarized the clinical data of 20 patients who accepted eversiou carotid endarterectomy from March 2009 to August 2009, compared the outcomes with preoperative and postoperative image learnt materials and clinical symptoms. Results All cases finished successfully. All clinical symptoms were improved after operation. The stenosis were all improved completely on the image after operation. Conclusions Eversion carotid endarterectomy is safe, effective, and shortening operative time and hospitalization time. It has low restenosis rate, thicken endometrium and plaque can be stripped completely.
10.Intratumoral injection of emulsion with hot lipiodol and pirarubicin for treatment of liver metastases lack of blood supply

Objective To explore the efficacy of percutaneous intratumoral injection of mixed emulsion with hot lipiodol and pira-rubicin for treatment of liver metastases lack of blood supply.Methods 12 patients with liver metastases,confirmed by the clinical and pathological examination,were treated by intratumoral injection of mixed emulsion with heating lipiodol and pirarubicin using 21G biliary needle under CT guidance.The adverse events were recorded and the treatment efficacy was also assessed according to the imaging findings.Results 8 patients achieved a partial remission of lesions,and 3 ones had stable condition.No serious adverse reactions occurred.Conclusion It is effective,simple,convenient and safe for treatment of liver metastases lack of blood supply with percutaneous intratumoral injection of emulsion with hot lipiodol and pirarubicin.
