1.Process control of maintenance management for large medical equipment

Objective:To explore the process of maintenance management for large medical equipment, and the structure design and core connotation of the maintenance management software of them.Methods: Medical linear accelerator of Precise series of Elekta company was combined to achieve the objective. From the aim of designing normal operation rate to start, the failure mode and effects analysis (FMEA) was operated to analyze the maintenance strategies and maintenance manners which were established on the basis of PDCA(Plan, Do, Check, Action) cycle and the analysis mode of SWOT (Strength, Weakness, Opportunity, Threat), and it could achieve the aim of normal operation rate.Results: Through explored the connotation of maintenance management and the preliminary framework of the software of maintenance management, the detail and process of maintenance management that based on personalized accelerator of process control were established, and the element factors of software of maintenance management of accelerator also was established at the same time.Conclusion: The effective maintenance management of large medical equipment is based on informatization process management of process control, and the element factor of management software need personalized customization. On the other hand, failure mode and degradation mechanism of equipment were the basis for personalized customization, and the creation has practical value.
2.THE STUDY OF THE CONTENT OF THE ENDOGENOUS DIGOXIN-LIKE SUBSTANCE WITHIN THE PLASMA OF PATIENTS WITH CONGESTIVE HEART FAILURE

In this study, endogenous digoxin-like substance (EDLS) in the plasma was measured by radioimmunoassay in patients with congestive heart failure The EDLS levels were significantly increased in patients with heart failure and were positively correlated correlated with the heart function state, Na~+/K~+-ATPasc activity in red cells and the electrolyte change in blood cells. We believed that digoxin-like substance may participatc in the process of the compensatory mechanirsms of the heart failure.
3.A preliminary study on the correlation between gastric lymphoid follicles and Helicobacter pylori infection

Objective To investigate the correlation between H.pylori strain possessing cagA gene and the formation of gastric lymphoid follicles(GLF),and the effect of H.pylori infection on the occurrence of GLF.Methods Antral biopsy specimens from 655 patients (chronic gastritis:n=479,peptic ulcer:n=176)were used for detection of H.pylori infection and histological analysis. CagA gene was examined in 70 clinical isolates by means of PCR- amplification. Results The incidence of lymphoid follicles in gastric mucosa is significantly higher(60.14% ) in patients with H.pylori infection than those without infection(17.06% ).GLF is easier to be detected in patients with active gastritis than in those with inactive gastritis. There is no significant difference in the presence of GLF among H.pylori associated gastroduodenal diseases,such as chronic gastritis ,gastric ulcer. Moreover, there is also no significant relationship between H.pylori strains possessing cagA gene and GLF. Conclusion The presence of GLF might directly related with H.pylori infection, and can be observed as a constant morpholgical marker of H.pylori related gastroduodenal disease ;The formation of gastric lymphoid follicles is not related to the cytotoxin of cag A gene of H.pylori.
4.Osteopontin regulation by protein kinase B in HepG2 cells

Objective It has been unclear how osteopontin, one of metastasis-associated genes, could be regulated in HepG2 cells. The aim of this study was to investigate the effect of protein kinase B(Akt) on the expression of osteopontin in HepG2 cells, and to explore the relationship between Akt and osteopontin expression. Methods HepG2 cells were transfected with constitutively active Akt and dominant negative Akt by lipofectmine, and transfectants were confirmed using Western blot for Akt. Osteopontin expression was detected by Northern blot and Western blot. Results HepG2 cells were successfully transfected with Akt genes including constitutively active Akt and dominant negative Akt, and overexpression of exozegenes Akt could be detected in HepG2 cells by Western blot. Using Northern blot and Western blot, we found that Akt gene regulated osteopontin expression in RNA level and protein level. In serum-free condition, HepG2 cells constitutively expressed low levels of osteopontin. Transfection of constitutively active Akt increased osteopontin mRNA and protein expression. Transfection of dominant negative Akt decreased osteopontin expression.Conclusions The fact that osteopontin gene expression can be regulated by Akt indicates that osteopontin synthesis can be blocked by inactivation of Akt gene and that metastasis of liver cancer might be inhibited by this intervention.
5.On syntactic equivalence in medical English translation

Starting from the syntactic features of Medical English , namely extensive use of nominalization structure, passive voice and complex sentences, the present research attempts to carry out an in-depth study of Medical English translation on the sentence level guided by Nida 's Functional Equivalence Theory . It attempts to combine the translation theory with the translation practice of Medical English, with a view to improving students' translation proficiency and promoting Medical English teaching.
6.The diagnosis and treatment of 65 patients with abdominal aortic aneurysm

Objective To summarize our experience of the diagnosis and treatment in patients with abdominal aortic aneurysm(AAA).[WT5”HZ]Methods [WT5”BZ]From 1980 to 1999 sixty five AAA patients were hospitalized with 40 cases of general type, 18 cases of rupture type,4 cases of inflammatory type and 3 cases of infective type. In situ artificial graft transplantation was performed in 62 cases and nonanatomy pathway in 2 cases. The remaining one with inflammatory type was treated by interventionel therapy.[WT5”HZ]Results [WT5”BZ]57 cases were cured and were followed up from 1 to 12 years without complications.Death occurred in 7 cases.[WT5”HZ]Conclusions [WT5”BZ]The abdominal pulsate mass should draw much attention. Ultrasound is the selected examination and DSA, helix CT are accurate image appliance. It can elevate the curative ratio and decrease the mortality through rational operation based on the location, type and individual condition of AAA patients.
7.Role of spinal NADPH oxidase in development and maintenance of neuropathic pain in rats

Objective To evaluate the role of spinal nicotinamide-adenine dinucletide phosphate (NADPH) oxidase in the development and maintenance of neuropathic pain (NP) in rats.Methods Twenty-four pathogen-free healthy male Sprague-Dawley rats,aged 200-250 g,were divided into 3 groups (n =8 each) using a random number table:sham operation group (S group),NP group and NADPH oxidase inhibitor group (NI group).The left sciatic nerve was exposed but not ligated in group S.The left sciatic nerve was exposed and 4 loose ligatures were placed on the sciatic nerve at 1 mm intervals with 4-0 silk thread to establish the NP model in group NP.In group NI,the NP model was established at 3 days after intrathecal catheterization,and NADPH oxidase inhibitor 5 μg/10 μ1 was injected intrathecally once a day for 7 consecutive days starting from 30 min before operation.The mechanical paw withdraw threshold (MWT) was measured at 1 day before operation and 1,3,5 and 7 days after operation.The rats were sacrificed at 8 days after operation,and the spinal cord was removed for determination of NeuN positive cells (by immuno-histochemistry) and expression of NADPH oxidase (by Western blot).Results Compared with group S,the MWT at each time point after operation and ratio of NeuN positive cells at 8 days after operation were significantly decreased in NP and NI groups,the expression of NADPH oxidase was upregulated in group NP,and the expression of NADPH oxidase was down-regulated in group NI (P<0.05).Compared with group NP,the MWT at each time point after operation and ratio of NeuN positive cells at 8 days after operation were significantly increased,and the expression of NADPH oxidase was down-regulated in group NI (P<0.05).Conclusion Up-regulated expression of spinal NADPH oxidase is involved in the development and maintenance of NP in rats.
8.Diagnosis and treatment of Leriche syndrome: a report of 33 cases

Objective To summarize the diagnosis and treatment of 33 cases of Leriche syndrome. Methods A retrospective review of the clinical data of 33 cases of Leriche syndrome was done. Results Claudication and impotence occurred in 79.9% and 70.4% of the cases. Color Doppler ultrasonography, especially combining with CTA or MRA, was helpful for the diagnosis. Aortic angiography or DSA was necessary for the determination of the clinical patterns and selecting the therapeutic methods. Surgical patterns selestion should be considering the patients' general status and conditions of the affected vessels. Surgical treatment was performed on 25 cases, including12 aortoiliac artery bypasses , 6 aortobifemoral artery bypasses , 4 axillo bifemoral artery bypasses, 2 embolectomies by Fogarty tube only and 1 aortal interposition with artificial vessel plus renal artery plasty. Aorta iliac artery bypasses get the best results with 1 year patency rate(100%) in all cases, and 5 year patency rate of 75.0%, which was significantly superior to those axillo bifemoral artery bypass grafts with 5 year patency rate of 37.5%. All the other 8 patients without operation died within 5 months. Conclusions Early diagnosis and comprehensive therapy should be adopted to improve the long term patency rates of grafts transplantation in Leriche syndrome.
9.Diagnosis and treatment of intussusception in adults:a report of 40 cases

Objective To assess the clinical characteristics,diagnosis and treatment of intussusception in adults. Methods The clinical data of 40 intussusception patients treated in our hospital from 1971 to 2001 were reviewed. Results The clinical manifestations mainly included abdominal pain, abdominal distention,vomit, abdominal mass, bloody stool or constipation. Patients with the triad signs of abdominal pain, abdominal mass and bloody stool only accounted for 17.4% (7/40). Those who had an acute attack of intestinal obstruction were usually diagnosed as acute mechanical intestinal obstruction, and in which intestinal strangulation took place more easily. Those who had a chronic process often had incomplete intestinal obstruction and were easily misdiagnosed.Of the 40 patients,39 underwent operation,37 cured, 2 died; and another one was cured by barium enema restoration. Conclusions B ultrasonography was helpful in early diagnosis. Bowel tumor, bowel polypus, inflammatory bowel diseases and anatomic abnormality are the main predisposing causes. So resection of the involved intestine would be the proper measure for eradication of the causes and prevention of recurrence.
10.Nano-hydroxyapatite/chitosan/alginate for repairing mandibular defects

BACKGROUND:Studies have confirmed that the nano-hydroxyapatite/chitosan/alginate composite materials have a certain flexibility and strength and possess a bioactivity similar to human bone.
OBJECTIVE:To explore the effect of the nano-hydroxyapatite/chitosan/alginate composite materials on the repair of rabbit mandible defects.
METHODS:Bilateral mandibular defect models of 10 mm × 5 mm × 5 mm were made in 18 healthy New Zealand white rabbits. Then, the rabbits were divided into two groups:experimental group was implanted with nano-hydroxyapatite/chitosan/alginate composite material, and control group was implanted with hydroxyapatite/chitosan composite. At 4, 8 and 12 weeks after implantation, cone-beam CT was applied to observe implant degradation, cal us growth and bone connection in the defect area;new bone formation was observed by hematoxylin-eosin staining.
RESULTS AND CONCLUSION:The gray values of the bone density in the experimental group and control group gradual y increased with time, and there were remarkably significant differences between the two groups at different time points (P<0.01). At the same time point, the experimental group was superior to the control group in gross observation, cone-beam CT observation, gray value of CT and histological observation (P<0.05). At 4-8 weeks after implantation, the implant materials in the two groups were gradual y degraded with a blurred junction between the defect and bone tissue, and a smal amount of new bone formed tightly integrated to the recipient bone tissue, in which the experimental group was more significant. And during 8 to 12 weeks, the degradation of implanted materials in the two groups was basical y complete, and the implant began to merge with the recipient bone tissue, with further generation of new bone tissue and gradual repair of bone defect area, in which, the experimental group was more obvious. Results show that the nano-hydroxyapatite/chitosan/alginate can repair bone defects effectively, and promote the new bone formation.
