1.Diagnostic and therapeutic methods for liver injury

Objective To investigate the diagnostic and therapeutic methods for liver injury.Methods The clinical data of 80 patients with liver injury were retrospectively analyzed.Results The liver injury was srede Ⅰ in 12 patients.grade Ⅱ in 25 patients,grade Ⅲ in 20 patients,srede Ⅳ in 14 patients,and grade V in 9 cases.16 patients were not treated with operation,24 patients were treated with hepatic artery ligation,and 3 patients were treated with plugging.75 patients were cured,and 5 patients died.Conclusion Liver injury should be clearly diagnosed as early as possible,correct therapeutic methods should be selected according to the state of the injury,and operative manner should be reasonably selected according to intraoperative conditions.The treatment of complications and postoperative protection to the function of important organs should be paid attention to.

3.Relationship between different types of acute ischemic stroke in elderly patients with serum OX-LDL,blood sugar, CRP amd IL-6

Objective To investigate the relationship between different types of acute ischemic stroke in elderly patients and serum oxidized low-density lipoprotein (OX-LDL),blood sugar,C-reactive protein(CRP) and interleukin 6 (IL-6) levels.Methods 100 patients with acute cerebral infarction (group A) and 30 heathy patients (group B)were investigated.The levels of serum OX-LDL,blood sugar,CRP and IL-6 were detected by automatic biochemical analysis.Results The levels of OX-LDL(627.2 ± 174.7 ) μg/L,blood sugar ( 8.8 ± 1.7 ) mmol/L,CRP ( 107.5 ±19.4)mg/L and IL-6(79.5 ± 12.1 )ng/L in patients of group A were significantly higher than group B[ OX-LDL(506.1 ± 152.4) μg/L,blood sugar(6.9 ± 1.5) mmol/L,CRP(87.2 ± 13.6) mg/L and IL-6(58.5 ± 10.3) ng/L](t=2.74,3.18,2.61,2.91,allP<0.05).Conclusion Elderly patients with acute cerebral infarction were closely related to the elevated levels of plasma OX-LDL,blood sugar,CRP and IL-6.Increased plasma OX-LDL was a risk factor for the main artery atherosclerosis type,and hyperglycemia was primary factor for the small artery atherosclerosis type of acute cerebral infarction.
4.Expression of Glucocorticoid Receptor in Peripheral Blood Mononuclear Cells from Patients with Pemphigus

Objective To clarify whether the mRNA expression level of glucocorticoid receptor (GR) in the peripheral blood mononuclear cells (PBMCs) can predict the clinical response to glucocorticoid (GC) in patients with pemphigus. Methods Reverse-transcription polymerase chain reaction (RT-PCR) was applied to determine the mRNA expression of GR? and GR? in PBMCs from 30 patients with pemphigus and 30 healthy volunteers. Results GR? mRNA was detected in all patients and all healthy volunteers, and the expression of GR? mRNA in pemphigus patients was significantly lower than that in healthy volunteers (P = 0.044), but no significant difference was found between patients insensitive and sensitive to glucocorticoid. In contrast, GR? mRNA was detected in 10 of 12 patients insensitive to glucocorticoid, 3 of 18 patients sensitive to glucocorticoid, and 9 of 30 healthy Volunteers. 5.Analysis of clinical manifestations and emergency operations of Crohn disease:a report of 26 cases

Objective To discuss the clinical features,diagnosis and operative treatment in Crohn disease(CD)characterized by acute abdomen.Method The clinical data of 26 CD cases testified by pathology after emergency operations from April 1997 to February 2009 were retrospectively analyzed.Results In 26 cases of acute abdomen,17 cases characterized by mechanical obstruction,6 cases characterized by peritonitis,3 cases characterized by hemorrhage of digestive tract.Eight cases(30.8%)were diagnosed before operation,and 13 cases(50.0%)were diagnosed during operation,totally 80.8%(21/26).All patients received surgical operations as following:resection and anastomosis in 21 eases,resection and fistulation in 4 cases,ileotransverso-stomy in 1 case.Two cases died after operation,and others discharged including 1 case received second operation within 5 days.Conclusion It is difficult to be diagnosed before operation when CD characterized by acute abdomen,and appropriate emergency operations are needed according to the particular cases.
6.The effect of the ganoderma lucidum spore on the immunoregulation of the immunosuppression mice

Objective:To explore the effects of Ganoderma Lucidum spore on the immunity of the immunosuppression mice with Glucocorticiods(GC).Methods:Different dosages of Ganoderma Lucidum spore were intragastriced to the immunosuppression mice with Glucocorticiods, and the weight of the immune organs and the phagocytic function of the macrophages and neutrophile,the expression in of CD3+,CD4+,CD8+,CD49b+ and CD19+ in the blood cell were measured;And the total complement activity, the content of the cytokine and the valence of hemolysin in the mice serum were detected.Results:The immune function of the GC immunosuppression mice were remarkable effected with Ganoderma Lucidum spore, it could significantly increase the index of the thymus and spleen, improve the valence of hemolysin and the phagocytic function of the macrophages and neutrophile of the mice.The total complement activity, the content of the cytokine in serum,and the quantity of T,B and NK cell in the mice blood were influenced by Ganoderma Lucidum spore.Conclusion:The results show that Ganoderma Lucidum spore remarkable effect on the immunity of the immunosuppression mice with GC, and it can promote accommodation the immune function of mouse.
7.Rapamycin increased development of CD4+ CD25high Foxp3+T cells of peripheral blood in liver transplant recipients

Objective To investigate the possible influence of immunosuppressive therapy,including sirolimus (SRL) and calcineurin inhibitors (CNI, tacrolimus), on level of Treg in the liver allo-graft recipients, Methods Forty-seven liver transplant recipients with stable liver function were assessed for at least 2 years, and divided into two groups: one composed of 15 patients receiving SRL,and another, of those receiving CNI (32 patients with tacrolimus). Thirty-eight age-matched healthy subjects (HS) were used as normal controls. We examined the expression of CD4, CD25, and Foxp3 in peripheral blood monouclear cells. Treg of every group was analyzed using Cell Quest software.Results SRL significantly increased the prevalence of CD4+ CD25high T cells, compared with HS and CNI group. The prevalence of CD4+ CD25high T cells of CNIs group was significantly lower than that of HS. The percentage of CD4+ CD25high T cells in the total CD4+ T cells was 1.88 % (1.56 %-2. 60 %), 1. 15 % (0. 57 %-l. 48 %) and 0. 84 % (0. 46 %-1. 45 %) in SRL, NS and CNI group,respectively (P<0. 01 or P<0. 05). Foxp3 was expressed in more than 95 % of CD4 + CD25high T cells and less than 20 % of CD4+ CD25low T cells, whereas not expressed in CD4+ CD25- T cells.Conclusion Different immunosuppressive therapy (SRL or CNI) might have different roles in tolerance induction in liver transplant recipients. Namely, SRL promoted the induction of allo-antigen tolerance, but CNI hampered the progression.
8.Pedicled preputial double-faced island flap urethroplasty for hypospadias in children

Objective To discuss the indication and efficacy for treating hypospadias with ure-throplasty of Pedicled preputial double-faced island flap(PPDIF). Methods Records of 99 children (mean age, 5.1 years; age range, 4 months-12 years)with hypospadias who underwent PPDIF oper-ation were reviewed. Of them, 56 cases were penile type of hypospadias, 24 cases of penile root type, 13 cases of scrotal type and 6 cases of perineal type according to Barcat classification. Based on Duck-ett technique, the partial preputial cortical plate remained and was displaced to gastr-penis with ure-thra. , and finally the new urethra would be covered by the cortical plate. Results During follow-up of average 2 years, 8 patients(8. 1%) developed urethral fistula, 1 (1.0 %) developed mild meatal ste-nosis, and 1(1.0 %) developed flap necrosis. The success rate for total PPDIF procedure was 89.90 % (89/99). And the success rate of simple PPDIF for cases with penile and penile root hypospadias, and PPDIF+Duplay for cases with serotal and perineal hypospadias were respectively 91.25 % (73/80) and 84.21%(16/19). respectively. Conclusions PPDIF is good for treating most hypospadias. Furth-more it make penis better shape. In the severe cases, PPDIF+Duplay prededure is recommended and the effect was affirmative.
9.Establishment and application of networked drug-cognitive behavioral sequential management model in patients with chronic insomnia

Objective To construct the sequential management model of networked drug-cognitive behavior in patients with chronic insomnia,and to explore the effects of this model on treatment and management of patients with chronic insomnia. Methods A total of 160 patients with chronic insomnia treated from January 2014 to June 2015 were randomly divided into the experimental group and the control group. The experimental group was treated with networked drug-cognitive behavior sequential therapy and management. Through the establishment of patient management files in the network management system,the disease was assessed,treatment programs were developed, remote implementation of 8 weeks of drug-cognitive behavior sequential treatment was conducted,12 months of net-work remote dynamic management and efficacy evaluation was performed. The control group received 8 weeks of traditional medical care with face-to-face drug-cognitive behavior sequential treatment and 12 months outpatient fol-low-up management. Results Comparison of management core indicators:there were significant differences between two groups in number of visiting hospital,exit status,treatment completion and documentation,sleep diary comple-tion,sleep scale completion and patient satisfaction. Comparison of sleep quality:after 2 months of treatment,there was no difference in quality of sleep between two groups; after 12 months of treatment,there were significant dif-ferences in sleep latency,awakening time after sleep,total sleep time and sleep efficiency between two groups. Scale score:after 12 months of treatment,there were significant differences in Pittsburgh sleep quality index,sleep personal beliefs and attitude scale score between two groups. Conclusion Network-based management improves the compliance of patients with chronic insomnia,reduces the loss of follow-up rate,improves sleep cognition,increases sleep quality,saves patients' time and cost,increases patient satisfaction,which is worth promoting in clinical application.
10.The metabolic fingerprint of the compatibility of Radix Aconite and Radix Paeoniae Alba and its effect on CYP450 enzymes.

Using a UPLC-MS/MS (MRM) and cocktail probe substrates method, the metabolic fingerprint of the compatibility of Radix Aconite (RA) and Radix Paeoniae Alba (RPA) and its effect on CYP450 enzymes were investigated. These main CYP isoforms include CYP 1A2, CYP 2C, CYP 2E1, CYP 2D and CYP 3A. Compared with the inhibition effect of RA decoctions on CYP450 isoforms, their co-decoctions of RA and RPA with different proportions can decrease RA' inhibition on CYP3A, CYP2D, CYP2C and CYP1A2, but can not reduce RA' effect on CYP2E1. The metabolic fingerprints of RA decoction and co-decoctions with different proportions of RPA in CYP450 of rat liver were analyzed by UPLC-MS. Compared with the metabolic fingerprints of RA decoction, the intensity of diester-diterpenoid aconitum alkaloids decreased significantly, while the intensity of monoester-diterpenoid alkaloids significantly increased in the metabolic fingerprints of co-decoctions of RA and RPA. The results suggest that RA coadministration with RPA increased the degradation of toxic alkaloid and show the effect of toxicity reducing and efficacy enhancing.
