2.Expression of Glucocorticoid Receptor in Peripheral Blood Mononuclear Cells from Patients with Pemphigus

Objective To clarify whether the mRNA expression level of glucocorticoid receptor (GR) in the peripheral blood mononuclear cells (PBMCs) can predict the clinical response to glucocorticoid (GC) in patients with pemphigus. Methods Reverse-transcription polymerase chain reaction (RT-PCR) was applied to determine the mRNA expression of GR? and GR? in PBMCs from 30 patients with pemphigus and 30 healthy volunteers. Results GR? mRNA was detected in all patients and all healthy volunteers, and the expression of GR? mRNA in pemphigus patients was significantly lower than that in healthy volunteers (P = 0.044), but no significant difference was found between patients insensitive and sensitive to glucocorticoid. In contrast, GR? mRNA was detected in 10 of 12 patients insensitive to glucocorticoid, 3 of 18 patients sensitive to glucocorticoid, and 9 of 30 healthy Volunteers. Objecitve To observe the effects of ursolicacid on contractile activity of uterine smooth muscle strips of virginal rats.Methods 20 Wistar virginal rats,weight 200-300g were selected,to record the effects of ursolicacid(1.0?10~(-8),2.0?10~(-8),3.0?10~(-8),4.0?10~(-8)g/L) on contractile activity of uterine smooth muscle strips with Biolap 410 Biological System.Diphenhydramine(2?10~(-6)mol/L),Phentolamine(2?10~(-6)mol/L),Verapamil(2?10~(-7)mol/L),Indomethacin(2?10~(-5)mol/L) were used as antagonists to study the mechanisms of ursolicacid on uterine smooth muscle strips.Results Ursolicacid could significantly potentiate uterine contractile activity of virginal rats.It could increase the frequencies,prolong the duration and enlarge the amplitude of contractile activity.The effect of which could be inhibited by the Indomethacin;but could not restrained by other antagonists.Conclusion The results of Ursolicacid may be mainly primarily with prostaglandin synthese or release,but not mediated by H_(1) receptor,? receptor and L-voltage-dependant calcium channels.
4.Surgical treatment of gastrointestinal stromal tumors: a report of 486 cases

Objective To investigate the surgical treatment of gastrointestinal stromal tumor (GIST) based on the clinical characteristics of GIST in different locations.Methods The clinical data of 486 GIST patients who received surgical treatment at the Peking Union Hospital from January 2003 to December 2012 were retrospectively analyzed.There were 461 patients with primary GIST and 25 with secondary GIST.The clinical characteristics and surgical treatment methods were analyzed.All data were analyzed using the chi-square test.Results Of the 461 patients with esophageal stromal tumors,6 received partial esophagectomy.Of the 234 patients with gastric stromal tumors,183 received partial gastrectomy,23 received proximal gastrectomy + cardiectomy,23 received distal gastrectomy,2 received total gastrectomy and 3 received exploratory laparotomy.Of the 51 patients with duodenal stromal tumors,34 received partial duodenectomy,9 received pancreatico-duodenectomy,5 received pylorus-preserving pancreaticoduodenectomy,3 received palliative surgery.Of the 116 patients with small intestinal stromal tumors,110 received partial small intestinal resection and 6 received palliative surgery.Of the 29 patients with rectal stromal tumors,13 received posterial transsphincteric surgery of the rectum,12 received transanal local resection of rectal tumors and 4 received abdominoperineal resection; laparotomy was performed on 25 patients with GIST in other positions.Of the 25 patients with secondary GIST,10 patients with liver metastasis of GIST received hepatic segmentectomy,1 received hepatobiopsy; 6 received abdominopelvic tumor resection; 5 received portial resection of the small intestine or colon; 1 received sigmoid colostomy; 1 received splenectomy and 1 received intracranial tumor resection.Of the 461 patients with primary GIST,patients who received combined devisceration accounted for 12.58% (58/461),and the ratio of combined cholecystectomy was the highest,which was 34.5% (20/58).Combined devisceration was considered for patients with duodenal stromal tumors,gastric stromal tumors and small intestinal tumors.Laparoscopic surgery accounted for 20.39% (94/461) of all the surgery,and the ratio of laparoscopic surgery which carried out in recent 5 years was 28.52% (77/270),which was significantly higher than 8.90% (17/191) of the earlier 5 years (x2=36.67,P < 0.05).Conclusion Different surgical treatment methods including minimally invasive surgery could be adopted according to different clinical characteristics of GIST in different locations,and radical resection of GIST is the main objective.
5.Effects of valsartan on the expression of acyl-coenzyme A:cholesterol acyltransferase-1 and peroxisome proliferator activated receptor-gamma in experimental atherosclerosis in rabbits

Objective To investigate the effects of valsartan on the expression of acyl-coenzyme A: cholesterol acyltransferase-1(ACAT-1) and peroxisome proliferator activated receptor-gamma(PPAR-?) in atherosclerotic plaques on rabbit aortic wall.Methods Twenty-four male Japanese white rabbits were randomly assigned into three groups(8 each): control group,valsartan group and high cholesterol feeding group.All rabbits were fed according to the experimental protocol for 12 weeks.Blood samples were taken from vein for measurement of serum lipids.The ratio of intima/media thickness of the aorta was measured.ACAT-1 mRNA/protein and PPAR-? mRNA/protein were determined by reverse transcription polymerase chain reaction(RT-PCR) and Western blotting,respectively.Results After 12 weeks,the levels of serum total cholesterol(TC),triglyeride(TG) and low density lipoprotein-cholesterol(LDL-C) in valsartan group and cholesterol group were significantly higher than those in control group(P0.05).The intima thickness and the ratio of intima/media in carotid arteries in cholesterol group were significantly higher than those in control group and valsartan group(P
6.Application of Vitreoretinal Surgery in Treating Complicated Retinal Detachment

Objective To explore the effect of the vitreoretinal surgery on the complicated retinal detachment.Methods We collected the clinical data of 50 cases with complicated retinal detachment treated by vitreoretinal surgery combined with the methods of vitrectomy,membrane excision,perfluorocarbon liquids,retinal coagulation of laser or condensate,exchange between silicon oil and gas perfluorocarbon liquids,silicon oil fill.The period of follow up was 3 to12months.Results The retina was attached completely in 46 cases(92%).4 cases did not reset auownting for 8%.The sight of all eyes was improved in varying degrees.Conclusion Vitreoretinal surgery is an effectial way for treat complicated retinal detachment.

0.05).Conclusions Plasma fibrinogen level is affected by -148C/T polymorphism of ?-fibrinogen gene. High plasma fibrinogen level is a risk factor for ACI in Chinese young adults. With other risk factors and environmental factors, T allele increases plasma fibrinogen level and may be a heritable risk factor for ACI in Chinese young adults.

Objective To explore the pathogenesis of invasive pituitary adenomas,and to improve the effect of clinical treatment.Methods 15 pairs of invasive pituitary adenoma tissues and noninvasive pituitary adenoma tissues were selected from 114 specimens collected and the Id1 gene expression and Id1 protein expression in invasive pituitary adenoma tissues and noninvasive pituitary adenoma tissues were tested by quantitative RT-PCR,Western blot and immunohistochemical methods;Then small interfering RNA (siRNA) was used to interfer Id1 gene expression in NQ-04 cells.Results The Id1 gene expression in invasive pituitary adenoma tissues was significantly higher than that in noninvasive pituitary adenoma tissues(n =15,t =2.725,P =0.013) ;The migration of pituitary adenoma cells interfered was significantly reduced.Conclusion The invasive pituitary adenoma tissues has high level of Id1 gene expression,which may be correlated with its invasion.And Id1 is expected to become a biological marker of diagnosis and determination in the prognosis of pituitary adenoma.

Objective To analyze the influence of usual processing steps on insoluble particles in Chinese medicine injection,with Shuanghuanglian injection as an example.Methods Light block method was adopted to analyze the influence of the different conditions of sterilization,ultra filtration and PH value on insoluble particles in Chinese medicine injection.Results Sterilization,ultra filtration and PH value caused the change of insoluble particles in Chinese medicine injection.Conclusion The comprehensive use of sterilization,ultra filtration,PH value can reduce the quantity of insoluble particles and clinical risk.
10.The effect of tramadol on perioperative insulin resistance in patients undergoing pulmonary lobectomy

ObjectiveTo investigate the effect of tramadol on perioperative insulin resistance in patients un dergoing pulmonary lobectomy.MethodsThirty patients undergoing pulmonary lobectomy,were randomly divided in to two groups :group T(tramadol group)and group C (control group), five minutes before skin incison, group T re ceived tramadol 1.5 mg/kg intravenously (IV), followed immediately by a continuous infusin of 0.5mg/kg per hour until the end of surgery, and the control group received normal saline instead of tramadol.Blood samples were takenfrom vein for determination of plasma blood sugar,insulin, IL-6 and TNF-α concentrations before induction of anesthe sia(T0) ,at the end of the operation(T1) ,four hours after operation(T2)and seven oclock on the next morning(T3),and the value of ISI was calculated.ResultsThe concentrations of blood sugar, insulin,TNF-α and IL-6 at T1 andT2 ,T3 increased significantly compared to baseline(T0) in both groups(P < 0.05).The concentrations of blood sugar,insulin,IL-6 and TNF-o at T1 ,T2 and T3 were significantly lower in group T than that in group C (P <0.05).Thevalue of ISI was significantly higher in group T than that in group C (P < 0.05).ConclusionTramadol could allevi ate perioperative insulin resistance in patients undergoing pulmonary lobectomy by inhibiting plasma blood sugar,insu lin,IL-6 and TNF-α.
