2.Clinical study of standing bed for the orthostatic hypotension after cervical spinal cord injury

ObjectiveTo observe the therapeutic effect of standing bed on orthostatic hypotension with cervical spinal cord injury. Methods36 patients with cervical spinal cord injury were divided randomly into the standing bed group and routine treatment group. The blood pressure of all patients were measured every day during rehabilitation treatment. The function of spinal cord was assessed before and after treatment.ResultsThe function of sensory and motor in the two groups were all improved, but there was no statistical difference between the two groups in the grades of spinal cord function. There were 27 patients who had orthostatic hypotension in the study. All the patients with complete spinal cord injury suffered the orthostatic hypotension. There was no improvement in blood pressure of complete spinal cord injury after treatment. In incomplete spinal cord injury patients, there was a significant improvement and the difference of blood presure between lying and standing was decreased in standing bed group after treatment (P<0.05). Compared with routine treatment group, the blood pressure in standing bed group was improved (P<0.05). Conclusions Cervical spinal cord injury could have orthostatic hypotension. Complete spinal cord injury would suffer orthostatic hypotension in different degree. The standing bed treatment could improve the orthostatic hypotension in incomplete spinal cord injury.
3.The study of acupuncture for patients with vertebral-artery-type cervical spondylosis using TCD and BAEP

Objective To observe the effect of acupuncture on patients with vertebral-artery-type cervical spondylosis by means of transcranial Doppler sonography ~TCD) and brainstem auditory evoked potentials ~BAEP), and to explore the mechanism of acupuncture in treatment of cervical spondylosis by means of blood velocity and neuroelectrophysiology measurements. Methods Forty patients with vertebral-artery-type cervical spondylosis were divided into acupuncture treatment group ~20 cases) and routine treatment group ~20 cases). The TCD and BAEP were examined before the rehabilitation treatment and 2 week later. Comparing with the control group, the effect of acupuncture on the blood velocity and cerebral electrophysiology was analyzed. Results ~1)The total efficiency rates in the two treatment groups were 90% and 85%, respectively. The clinical heal rate was 45% in acupuncture group and 35% in routine group. There was no statistic difference with regard to the therapeutic effect and the length of treatment between the two groups. ~2)The blood velocity of vertebrobasilar artery ~VBA) in the patient group was decreased. The abnormal rate of TCD was 77.5% ~31/40 cases), characterized with the decrease of VBA. The abnormal rate of BAEP was 75% ~30/40 cases), characterized with brainstem abnormality type. ~3)The blood velocity in both groups was increased after treatment. Compared with the routine therapy group, the velocity of blood flow in vertebral artery in acupuncture treatment group was increased markedly ~P
4.Management of 240 cases of non-functioning renal graft

Objective To explore the methods for treating no-functioning renal graft.Methods The management of 240 cases of non-functioning renal graft between 1978 and December 2003 was retrospectively analyzed.Results Twenty-nine recipients with renal failure one month after renal transplantation were subjected to removal of the renal graft. One year after renal transplantation, the recipients with renal failure received resection of the renal graft, 4 were treated with renal artery embolism, and 114 did not receive special treatment.Conclusion The non-functioning renal graft required removal at early stage of renal transplantation. At later stage, no symptomatic cases of post-transplanted renal failure required no management after stopping immunosuppressant. The appropriate management is reserved for the symptomatic cases.
5.Effect of floating acupuncture on the processus transverses of vertebrae lumbales Ⅲ syndrome

ObjectiveTo explore the therapeutic effect of floating acupuncture on the the processus transverses of vertebrae lumbales Ⅲ syndrome.Methods40 patients with the processus transverses of vertebrae lumbales Ⅲ syndrome were divided into the floating acupuncture group (20 cases) and the traditional group (20 cases). The pain was evaluated with visual analogue scale (VAS) before and after treatment.ResultsCompared with traditional treatment, the floating acupuncture can relieve pain rapidly after 15 min and 1 d of treatment. ConclusionThe floating acupuncture shows preferable clinical curative effect to patients with the processus transverses of vertebrae lumbales Ⅲ syndrome.
6.Analysis of coronary artery anatomy pattern in 56 arterial switch operations and early outcome

Objective This study was designed to investigate the relationship of the coronary anatomy pattern in 56 arterial switch operation (ASO) with the early outcome.Methods From May 2010 to September 2013,a total of 56 patients with transposition of the great arteries complicated with other defomities(39 males and 17 females,with age of 5 days to 126 months),who were undertook ASO in Capital Medical University affiliated Beijing Anzhen Hospital.There were 19 patients with complete transposition of the great arteries and intact ventricular septum,35 patients with complete transposition of the great arteries and ventricular septal defect.and 1 patient with double outlet right ventricle with subpulmonary ventricular septal defect (Taussing-Bing).33 were complicated with atrial septal defect and 26 with patent ductus arteriosus,20 had severe pulmonary hypertension,2 had pumonic valve stenosis and 1 had coarctatioin of aorta.All patients undertook ASO,coronay distribution were detected during the procedure and coronary artery were grafed.Results Coronary artery:Yacoub type A were 46 cases:4 patients died during the 30 days after the operation,Yacoub type B were 5 cases:2 patients died.Yacoub type C were 2 cases:1 died of low cardiac output on the operating day.Yacoub type D were 1 case and Yacoub type E were 2 cases:none of them died.Conclusion Patients with common coronary variants have undergone ASO without added mortality rate.Those with intramural or single coronary arteries have significant added mortality rate.
7.Factors for anemia after kidney transplantationA data analysis of 826 patients from one institute during 10 years

BACKGROUND: Anemia after kidney transplantation has a clinical incidence rate of 30%-40%, is the important risk factor for cardiovascular diseases and kidney failure after kidney transplantation and is also the independent prediction index of patient's death. OBJECTIVE: To analyze the factors related to anemia after kidney transplantation. RESULTS AND CONCLUSION: In the anemia group (n = 225, 27.2%, aged 26-65 years), the incidence rate of anemia in female and male patients was 23% and 37%, respectively (P < 0.05), 46 patients had hypertension and used angiotensin-converting enzyme inhibitor or angiotensin Ⅱ receptor antagonist and 16 patients had chronic erosive gastritis or upper gastrointestinal tract ulcer, with the human survival rate of 85.3% and kidney failure rate of 25.3%. In the non-anemia group (n = 601, 72.8%, 405 males, 196 females, aged 18-71 years), 35 patients had hypertension and used angiotensin-converting enzyme inhibitor or angiotensin Ⅱ receptor antagonist and 14 patients had chronic erosive gastritis or upper gastrointestinal tract ulcer, with the human survival rate of 92.1% and kidney failure rate of 12.6%. There was significant difference in above-mentioned indices between anemia and non-anemia groups (P < 0.05). These results suggest that gender, age, kidney function, digestive tract disease history, and drug application are closely related to anemia after kidney transplantation.
8.Corneal histopathological changes and interleukin-6 level in aqueous humor of a rabbit model of penetrating corneal trauma combined with seawater immersion*

BACKGROUND: Following trauma caused by seawater, cells often exhibit special pathological changes because of the special physico-chemical properties of seawater.OBJECTIVE: To observe corneal histopathological changes and interleukin-6 level in aqueous humor of a rabbit model of penetrating corneal trauma combined with seawater immersion.METHODS: The rabbit eye models of penetrating corneal trauma caused by firecrackers were established in 16 adult healthy grey rabbits. A 3-mm whole-layer incision was made in the cornea. The right eyes served as experimental sides and the left eyes served as controls. Seawater was injected into the aqueous humor of the right eyes via the corneal incision. The eye surface was flushed with seawater for 30 minutes. Physiological saline was used for the left eyes. RESULTS AND CONCLUSION: Optical microscopy results showed that at 1, 2, 3 days after model establishment, corneal cells on the experimental side exhibited severe necrosis and abscission, obvious swelling of substantia propria layer complicated by cellular infiltration. At 1 and 2 days after model establishment, the pathological changes on the control side were the same as the experimental side, but they were mild, but at 3 and 5 days, they were obviously alleviated. At 1, 2, 3 days after model establishment, interleukin-6 level in aqueous humor was significantly higher on the experimental side than on the control side (P < 0.05). These findings suggest that the degree of injury on the experimental side was more severe than that on the control side, indicating that seawater may be an important causative factor of corneal injury.
9.Types and spontaneous Ca2+ waves of ICC in the bladder of guinea pig

Objective To discuss the types of ICC and characteristics of spontaneous Ca2+ waves of different types of ICC in the bladder of guinea pig. Methods Frozen-sections were made from the bladder of guinea pig and ICC were cultured in vitro. Cells were stained by indirect immunofluorescent method and detected by Laser scanning confocal microscope. The ICC cultured in vitro were divided randomly into 4 group: dimmer ICC,monomer ICC,dimmer ICC treated with 2-APB group and monomer ICC treated with 2-APB group according to the cell morphology and disrupted with 2-APB.The calcium concentration of ICC cultured in vitro were marked with Fluo-4 AM and disrupted by 2-aminoethoxydipheylbrate (2-APB, 100 μmol/L) in dimmer ICC treated with 2-APB group and monomer ICC treated with 2-APB group. The calcium oscillation function of ICC was observed under Laser scanning confocal microscope. Results For the monomer ICC and dimmer ICC in frozen sections and cultured in vitro,there were increased frequency (P<0.01) and amplitude (P<0.05) of spontaneous Ca2+ waves in dimmer ICC compared with the monomer monomer ICC. But after the cells disrupted by 2-APB after 15 min,There were decreased frequency (P<0.01) and amplitude (P<0.01) of spontaneous Ca2+ waves in the dimmer ICC treated with 2-ABP group compared with the dimmer ICC. The changes(P>0.05) of spontaneous Ca2+ waves was not statistical significance in monomer ICC treated with 2-ABP group compared with monomer ICC group. Conclusions The bladder of guinea pig may exist 2 different types of ICC, dimmer ICC and monomer ICC. The excitability of spontaneous Ca2+ waves of dimmer ICC could be higher than in monomer ICC. The special structure of dimmer ICC may contribute to the formation of high spontaneous Ca2+ waves.
10.Expression and significance of c-kit messenger ribonucleic acid and c-kit protein in diabetic cystopathy in guinea pig

AIM: To determine the expression of c-kit mRNA and protein in the bladders in guinea pigs with diabetic cystopathy (DCP) and to explore the correlation and mechanisms between c-kit expression and DCP. METHODS: Sixty guinea pigs were divided randomly into control group (n=20) and experimental group (n=40). The guinea pigs in experimental group were injected with streptozotocin (STZ) to induce diabetes mellitus. After fed for 10 weeks, the animals in both groups were tested with urodynamics, and the guinea pigs in experimental group were divided into the subgroups of DCP and the diabetic no-cystopathy (NDCP) group according to the results of urodynamics. mRNA expression of c-kit was detected by reverse transcription polymerase chain reaction (RT-PCR) and protein expression of c-kit was tested and analyzed by laser scanning confocal microscope. RESULTS: Decreased expression of c-kit mRNA was observed in DCP group compared to control and the NDCP group. The ratio of c-kit mRNA and GAPDH was 5.66±0.54 in controls (P<0.05), 5.54±1.28 in NDCP group (P<0.05) and 4.65 ±0.47 in DCP group. c-kit protein expression significantly declined in DCP group. The mean value of fluorescence intensity was 856.52± 53.03 in control group, 844.67± 59.24 in NDCP group and 548.69± 48.51 in DCP (P<0.01).CONCLUSION: The declined expression of c-kit) gene at transcription and translation levels destroys the SCF/c-kit signal pathway, leading to the dysfunction of Cajal-like) cells in DCP guinea pig, so the abnormal expression of c-kit gene is involved in the pathogenesis of DCP.
