1.Clinical observation of gimeracil and oteracil potassium combined with oxaliplatin in treatment of advanced gastric carcinoma patients

Objective To study the curative effect and clinical safety of gimeracil and oteracil potassium combined with oxaliplatin in treatment of advanced gastric carcinoma patients.Methods From January 2009 to December 2011,116 cases with advanced gastric carcinoma were randomly chosen and divided into group A,B (58 cases in each group).Group A was given gimeracil and oteracil potassium combined with oxaliplatin,while group B was given oxaliplatin,fluorouracil and calcium folinate.Before and after dosing,the blood routine,liver and kidney function,chest CT scanning and gastroscope inspection were observed,the recent curative effect and adverse reaction of chemotherapy were analyzed.The patients' survival time and disease progression were compared.Results The curative effect of 116 patients could all be evaluated,and the effective rate of group A and group B were 48.3%(28/58) and 29.3%(17/58),while clinical benefit rate were 74.1%(43/58) and 55.2%(32/58),there was significant difference(P < 0.05).The survival rate of two groups at different time points had no significant difference (P >0.05).The rate of nauseated and vomiting were 34.5%(20/58),37.9%(22/58) in group A,and 67.2%(39/58),62.1%(36/58) in group B,there was significant difference (P < 0.05).The influence of drugs on blood system,common peripheral neurotoxicity,extremities syndrome and inflammation of the oral mucosa between two groups had no significant difference (P >0.05).Conclusions Gimeracil and oteracil potassium combined with oxaliplatin in treatment of advanced gastric carcinoma patients has better clinical effect and less complication,which is worthy of further promotion.
2.Celastrol induces caspase-dependent apoptosis through ROS/JNK path-way in Saos-2 cells

AIM:ToexploretheeffectofcelastrolonapoptosisofSaos-2cellsanditsmechanism.METH-ODS:Saos-2 cells were treated with various concentrations of celastrol , and the cell viability was measured by MTT assay . Apoptosis and reactive oxygen species ( ROS) production were determined by flow cytometry .The protein levels of cleaved caspase-9, cleaved caspase-3 and phosphorylated JNK were evaluated by Western blot .RESULTS:The viability of Saos-2 cells was significantly inhibited by celastrol .Celastrol significantly induced apoptosis of Saos-2 cells.Celastrol signifi-cantly induced ROS production in the Saos-2 cells.Western blot analysis demonstrated that celastrol significantly increased the protein levels of cleaved caspase-9, cleaved caspase-3 and phosphorylated JNK in the Saos-2 cells.CONCLUSION:Celastrol induces caspase-dependent apoptosis through ROS/JNK pathway in Saos-2 cells.
3.Clinical Study on Treatment of Shoulder Periarthritis with Electro-acupuncture on Point Jianyu (LI 15)

Objective: To compare the curative efficacy and safety of electro-acupuncture on Jianyu (LI 15) with oral administration of Diclofenac Sodium Sustained-release Capsules in treating shoulder periarthritis. Method: Randomized controlled trials (RCT) were adopted in the study. The patients were randomized into two groups, 30 cases in one group.Electroacupuncture was done on Jianyu (LI 15) 20 min every time in the treatment group,while 75 mg Diclofenac Sodium Sustained-release Capsules were orally administered in the control group. For all the patients in two groups, one treatment course contains 7 days. Then the curative efficacy was evaluated by the efficacy evaluation criteria after two consecutive courses. Result: The total effective rate of treatment group and control group were 93.3% and 56.7% respectively. Conclusion: Electroacupuncture on Jianyu (LI 15) is an effective therapy for shoulder periarthritis and has more significant effect than oral Diclofenac Sodium Sustained-release Capsules.
4.Disposable Whole Bed Sheet and Its Reel Device: Design and Clinical Application

OBJECTIVE To overcome shortcomings in use of bed sheet,and to improve it for examining bed.METHODS Based on the working principle of the reel toilet paper and industrial drive belt,a reel device for(disposable) whole bed sheet was designed,and used on the bed examining more than 300 times.(RESULTS) The new designed reel aevice with disposable whole bed sheet was operated easy and safely,with decreased cost and(contamination) in use.It could alleviate infection opportunity in favor of control againt hospital infection and(environmental) protection.(CONCLUSIONS) The device can mount on any size of examining bed and widely accept for use.
5.Intratumoral microvessel density and expression of vascular endothelial growth factor in hepatocellular carcinoma after chemoembolization.

OBJECTIVETo investigate intratumoral microvessel density (MVD) and expression of vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (BFGF) in hepatocellular carcinoma (HCC) after transcatheter arterial chemoembolization (TACE) and to evaluate their significance.
METHODSMVD and expression of VEGF and BFGF in cancerous tissues were examined in forty specimens resected from patients with HCC using immunohistochemical methods. Among these patients, 20 patients received 1 to 7 treatments of TACE prior to II-phase surgical resection (TACE group), the other 20 patients were treated by operation without receiving any other treatment preoperatively (surgical group). There was no significant difference in clinical features between the two groups. MVD was assessed by counting immunostained endothelial cells within a certain area, and staining intensity of VEGF was assessed quantitatively with computer-assisted image analyzer. The expression of BFGF was determined by cell-positive or cell-negative.
RESULTSThe average MVD was 130.51 75.5 in TACE group and 152.35 58.80 in surgical group. There was no significant difference between the two groups (t=-1.021, P=0.341). Staining intensity of VEGF was 645.60 543.27 in TACE group, higher than in surgical group (158.28 188.48, t=281, P<0.001). BFGF-positive rate was 35% in TACE group and 40% in surgical group. There was no significant difference (x(2)=0.107, P=0.744).
CONCLUSIONSThe results indicate that survived cancerous tissue has rich vascularity and the expression of VEGF of the cancerous cells can be enhanced by TACE which may play an important role in reestablishment of blood supply to tumor after TACE.

6.Reliability and validity of Chinese Pediatric Quality of Life Inventory in influenza cases

Objective:
To explore the reliability and validity of the Chinese Pediatric Quality of Life Inventory(PedsQL)in influenza children.
Methods:
From January 2017 to February 2018,we selected laboratory-diagnosed influenza cases and healthy children according to age and gender ratio by stratified random sampling and systematic sampling method. We employed Chinese version of PedsQL 4.0 to investigate their quality of life,used Cronbach's α to evaluate the reliability,and used Pearson correlation coefficient, t-test,confirmatory factor analysis(CFA)and ROC curve to evaluate the validity.
Results:
Totally 300 influenza cases and 300 healthy children were surveyed,with 294(98.00%)and 295(98.33%)valid questionnaires recovered. The general Cronbach's α was 0.89,and the Cronbach's α of each dimension ranged from 0.79 to 0.84. The Pearson correlation coefficients between the items and their belonged dimension ranged from 0.537 to 0.755,between the items and other dimension ranged from 0.203 to 0.384. The CFA resulted in RMSEA of 0.06,GFI of 0.88,AGFI of 0.90,CFI of 0.91 and NFI of 0.89. The scores in total and in all the dimensions in influenza cases were significantly different with those in healthy children(P<0.05). Taking the scores in influenza cases as a golden standard,the area under the ROC curve was 0. 985(P<0.05),the sensitivity was 0.92,and the specificity was 0.95. When the limit score was 82.18,the Youden index was the largest.
Conclusion
The Chinese version of PedsQL4.0 has good reliability and validity, which can be applied to quality of life assessment in children.
7.Effects of meditation therapy on fear of disease progress and mental health among acute myocardial infarction patients

Objective:To investigate the effects of meditation therapy on fear of disease progress and mental health among acute myocardial infarction (AMI) patients.Methods:Totally, 120 cases of acute myocardial infarction patients admitted to Affiliated Hospital of Jiangnan University were divided into experimental group and control group according to the enrolled time. 60 patients with AMI treated from June to November 2018 served as the control group and 60 patients with AMI treated from January to May 2019 served as the experimental group. The patients in the control group received routine nursing, the experimental group carried out 4-week meditation therapy based on the routine nursing. Before and after intervention, the effect was assessed by Fear of Progression Questionnaire-Short Form (FoP-Q-SF) and Symptom Checklist-90 (SCL-90), respectively.Results:After intervention, the physical health and social family function dimension scores in FoP-Q-SF as well as total FoP-Q-SF scores were (12.78±3.47), (10.45±2.44), (23.24±4.25) points, significantly lower than in the control group (14.33±2.72), (11.59±2.82), (25.91±3.89) points, the difference was statistically significant ( t values were 2.623, 2.277, 3.499, P<0.05); the scores of somatization, compulsion, anxiety, depression and total SCL-90 were (1.26±0.19), (1.42±0.23), (1.19±0.28), (1.20±0.16) and (121.81±9.59) points, significantly lower than (1.83±0.25), (1.68±0.37), (1.82±0.41), (1.71±0.33) and (145.85±9.12) points in the control group, the difference was statistically significant ( t values were from 4.580 to 13.659, P<0.001). Conclusion:Meditation therapy can effectively decrease disease progress and promote mental health in patients with acute myocardial infarction.
8.Disease burden of influenza in schools and child care settings in rural areas of Hangzhou, 2016-2018

Objective The objective of this study was to analyze the disease burden of influenza in schools and child care settings in rural areas of Hangzhou. Methods Hospital visit influenza cases aged 3-17 years in hospitals that reported based on influenza surveillance system from 2016 to 2018 in Chun′an county, Hangzhou city were selected as study subjects and a total of 294 confirmed cases of influenza were selected using system sampling method. Questionnaires were designed to investigate the basic information and data on inpatients and outpatients among, health care and life quality, etc.. Direct and indirect economic burden and disability adjusted life year (DALY) were analyzed and compared among different age groups. Results The mean age of investigated subjects was (8.88±3.92) years. A total of 143 (48.64%) investigated cases were male. In total of 283 (96.26%) cases were outpatients. The total economic burden was 124 743.95 CNY. The mean economic burden was 424.30 CNY per person. The mean direct and indirect economic burden was 361.33 and 62.97 CNY per person respectively. The difference of the mean direct, indirect and total economic burden per person between different age group was statistically significant (P<0.001). The 3-5 years age group showed the highest economic burden with the median value of direct, indirect and total economic burden per person being 276.24, 50.98 and 321.26 CNY, respectively, while the 12-17 years age group showed the lowest values with 175.30, 26.54, 201.79 CNY, respectively. The DALY of 294 influenza cases was 1.18, and the median of burden strength was 3.21 DALY/thousand. The difference of the burden of strength between different age group influenza case was statistically significant (P<0.001), of which the 12-17 years age group showed the highest value with 4.25 DALYs/thousand while the 3-5 years age group showed the lowest value with 2.60 DALY/thousand. Conclusion The disease burden of influenza was heavy in schools and kindergartens in rural areas of Hangzhou city, with the cases aged from 3 to 5 years showing higher economic burden and cases aged from 12 to 17 years showing greater burden strength.

10.Effect of tension on recovery of the donor site of a modified forearm flap

Objective :
To investigate the effect of tension on donor site recovery of modified forearm flaps (closed forearm donor site skin flaps).
Methods :
From October 2018 to April 2021, 12 patients with oral cancer underwent forearm flap repair at Xuzhou Central Hospital. Handgrip strength and wrist motion were recorded before surgery. During the surgical incision at the donor site of the forearm, the triangular full-thickness skin of the donor site was used to close part of the surgical incision, and the other part of the surgical incision was directly closed and sutured. Tension was measured with a tension meter during the surgical incision at the donor site. Patients were followed up after surgery to observe whether there were postoperative complications such as necrosis and poor healing of donor site wounds, as well as forearm grip strength, wrist range of motion and appearance satisfaction.
Results:
Among the 12 patients, there was only one elderly patient with postoperative necrosis near the wrist in the donor site skin, and the average tension of the patient was 0.65 kg; the lesion healed after dressing changes. All of the other patients recovered well without postoperative complications. The mean tension of 12 patients was (0.51 ± 0.05) kg. The preoperative and postoperative grip strength of 12 patients was (23.7 ± 10.3) kg and (22.3 ± 10.7) kg, respectively, and the difference was not statistically significant (t=5.872, P<0.001). The mean range of motion of the wrist was (47.6 ± 8.3)°, (45.8 ± 5.8)°; dorsiflexion (54.6 ± 3.2)°, (53.9 ± 2.3)°; radial deviation (37.0 ± 2.3)°, (36.1 ± 2.2)°; ruler deviation (27.1 ± 1.9)°, (26.4 ± 1.3)°, respectively. The t values were 1.64, 1.636, 2.116 and 1.412, and the P values were 0.129, 0.130, 0.058 and 0.186, respectively. All 12 patients were satisfied with the appearance of the donor site of the forearm.
Conclusion
When the average tension during suturing is less than 0.5 kg, there is a lower risk of complications at the donor site of the forearm one month after surgery, a lower risk of impaired donor site function, and a greater possibility of patient satisfaction with the appearance of the donor site.
