1.Effect of occupational injury on quality of life: 234 cases in the mining area of Tongling, Anhui

BACKGROUND: Quality of life, which is induced as a completely evaluating system to reflect new healthy viewpoints and medical patterns, plays a key role in supplying traditional therapy and evaluating rehabilitation efficacy in occupational injury.OBJECTIVE : To analyze the effects of occupational injury on quality of life in mining area of Tongling, Anhui.DESIGN: Case contrast questionnaire, Logistic single factor analysis and multiple factors analysis.SETTING: Workers General Hospital of Tongling Nonferrous Metal Group; Mining Safety Office of Tongling Nonferrous Metal Company.PARTICIPANTS: A total of 234 subjects with occupational injury were selected from four subgroups of Tongling Nonferrous Metal Company from January 2000 to December 2005 and regarded as the occupational injury group.Injured grade was evaluated by the department of safety in production. In addition, 206 employees of the same sex were regarded as the control group. All subjects provided the confirmed consent.METHODS: Questionnaires were finished in December 2006. Besides those whose educational level was lower than primary school or illness state was severe, their questionnaires were filled by somebody else through asking the feelings,and other subjects filled in the questionnaires by themselves. ① Social demography characteristics were surveyed in the two populations. ② The World Health Organization Quality of Life Assessment-100 (WHOQOL-100), which was characterized by excellent reliability, validity and sensitivity, was used to survey influencing factors (sex, age, occupation,educational level, marriage, smoking, drinking, pay, type of work in production, grade of occupational injury, injured site and type, other diseases, average income of per family member, housing area of per family member, accidental types,character styles, sleeping status, etc.) and various dimensions reflecting quality of life [physiological function (body ache,vigor, sleeping), mental state (well-being feels, confidence, and other three aspects), independence (daily living ability,work ability, and other two aspects), social relationship (interpersonal relation, social communication, and other one spect), material life (reliability, housing environment, and other eight aspects) and faith (individual faith)]. There were 24 aspects and every aspect included four items; in addition, there were 4 items in general healthy status. Therefore, there were 100 items in total. LinearitY method was used to score each item (from 1 tc 5 points). Self-scored quality of life had only one item, which was scored based on hundred-mark system. ③ Quality of life of subjects in the occupational injury group was analyzed with one-way analysis of variance. Variously dimensional indexes of quality of life in 6 fields were regarded as dependent variance, and effect factors were regarded as independent variance. The data were analyzed with multiple stepwise regression analysis.MAIN OUTCOME MEASURES: ① Characteristics of social demography; ② scores of variously dimensional indexes of quality of life; ③ correlation between quality of life and effect factors in occupational injury group.RESULTS: All 234 patients in the occupational injury group and 206 subjects in the control group were involved in the final analysis. ① There were no significant differences in sex, age, occupation, educational degree, working duty,marriage, drinking, smoking, pay, having other diseases and occupational types between the two groups (P > 0.05). ②Scores of physiological field, independence field and total quality of life were lower in the occupational injury group than those in the control group (P < 0.05). ③ Effect factors on quality of life in the occupational injury group contained intoxication and asphyxia; ankle, foot and cranial injuries; avulsed wound and task degrees. Multiple factor analysis demonstrated that quality of life of patients in the occupational injury group was affected by waist injury, pay level and relationship with colleagues and family member.CONCLUSION: Quality of life is inferior in the occupational injury group to that in the control group. The effect factors are mainly focus on injured sites, accident and injured types, economic status and relationship with society and family member.

5.Expressions and its relationship of vascular interleukin-6 and signal transducer and activator of transcription 3 in patients with giant cell arteritis

6.Clinical observation of gimeracil and oteracil potassium combined with oxaliplatin in treatment of advanced gastric carcinoma patients

Objective To study the curative effect and clinical safety of gimeracil and oteracil potassium combined with oxaliplatin in treatment of advanced gastric carcinoma patients.Methods From January 2009 to December 2011,116 cases with advanced gastric carcinoma were randomly chosen and divided into group A,B (58 cases in each group).Group A was given gimeracil and oteracil potassium combined with oxaliplatin,while group B was given oxaliplatin,fluorouracil and calcium folinate.Before and after dosing,the blood routine,liver and kidney function,chest CT scanning and gastroscope inspection were observed,the recent curative effect and adverse reaction of chemotherapy were analyzed.The patients' survival time and disease progression were compared.Results The curative effect of 116 patients could all be evaluated,and the effective rate of group A and group B were 48.3%(28/58) and 29.3%(17/58),while clinical benefit rate were 74.1%(43/58) and 55.2%(32/58),there was significant difference(P < 0.05).The survival rate of two groups at different time points had no significant difference (P >0.05).The rate of nauseated and vomiting were 34.5%(20/58),37.9%(22/58) in group A,and 67.2%(39/58),62.1%(36/58) in group B,there was significant difference (P < 0.05).The influence of drugs on blood system,common peripheral neurotoxicity,extremities syndrome and inflammation of the oral mucosa between two groups had no significant difference (P >0.05).Conclusions Gimeracil and oteracil potassium combined with oxaliplatin in treatment of advanced gastric carcinoma patients has better clinical effect and less complication,which is worthy of further promotion.

9.The ultrastructural features and it's significance of the skip area with giant cell arteritis

Objective To investigate the ultrastructural features and it's significance of the skip area with giant cell arteritis(GCA).Methods 20 GCA patients and 7 non-GCA patients underwent temporal artery biopsies. The sections were stained with HE and methylene blue(MB) for light microscopy(LM), doubly stained with uranyl acetate and lead curate for electron microscopy (EM). 20 GCA patients were divided into skip area GCA group(14 cases) and active inflammatory cell infiltration GCA group(6 cases) according to the definition of skip area or skip lesion. The ultrastructural changes of each group were observed by EM and assessed by using 4 grades from 0 to 3 scores.Results The scores of ultrastructural changes were as follows: the skip area GCA group, 0 in 1 patient, 2 to 6 in 9 patients, and 8 to 9 in 4 patients; the active inflammatory cell infiltration GCA group, 7 to 15 in 6 patients; the non-GCA group, 0 to 1 in 6 patients, and 5 in 1 patient. The scores of the three groups were tested by H check, the difference was evidently significant( P0.05) in comparison with active inflammatory cell infiltration GCA group.Conclusions At electron microscopic level, the skip area of temporal artery biopsy with GCA showed obvious pathological changes, especially in the intima of artery. The results suggest that the investigation of ultrastructure features of the skip areas of temporal artery with GCA may be benefit for the diagnosis and treatment of this disease.
10.Study of the effect of Astragalus injection on the cell apoptosis of intracerebral hemorrhage in rats

Objective To investigate the rule of cell apoptosis after intracerebral hemorrhage (ICH) in rats, and to analyze the effect of Astragalus injection on the brain tissue water content and cell apoptosis perihematoma.Methods Experimental ICH models of rats were produced in by injection of Collagenase into right caudate nucleus using stereotaxis technique. After operation, 160 SD rats were randomly divided into 4 test groups according to different intervention time (operation meanwhile group, 6 h group,12 h group and 24 h group after operation, 32 rats in each group) and hemorrhagic control group (32 rats). The rats in test groups were treated with Astragalus injection intraperitoneally once a day. All ICH animals were killed at 4 d or 7 d after ICH. 12 rats from the above each group were used to measure brain tissue water content (dry-wet method). The effect of different initiation time point of treatment on brain tissue water content and cell apoptosis perihematoma was observed.
