Objective To evaluate CT in the diagnosis of small bowel obstruction in children. Methods Thirty one cases with small bowel obstruction manifestations on CT and plain radiograph were analysed. The age ranged from 9 months to 14 years, male 17 cases, female 14 cases. There were 29 acute small bowel obstructions, and 2 chronic obstruction. Seven cases had history of previous abdominal operation. Results Corred diagnosis was made in 19 cases and 29 cases respectively by plain film and CT. The accurate etiologic diagnosis could be made by CT in 18 cases, including small bowell stone 4 cases, intussusception, 5 localized infection and adhesion 6 intraluminal bowel duplication, 1 congenital umbilical hernia, congenital malrotation of the intestine 1 case.Combined with history of abdominal operation the accurate CT etiologic diagnosis increased to 25 cases.There were 4 CT false negative cases and 2 plain film false negative cases. Conclusion The CT is obviously superior to plain radiograph in detecting strangulation and gas in bowel wall,better dilineating the transitional segment of bowel stenosis thereby demonstrating the case of obstruction which is very helpful for clinical management.

Objective To evaluate the efficacy and safety ofGuipi Decoction vs. antidepressants for depression. Methods Articles about the randomized controlled trials (RCTs) ofGuipi Decoction for depression were searched in the databases of CNKI, VIP, Wanfang Data, CMB, Embase, PubMed, and Cochrane Library. Two researchers independently screened the articles, extracted the data, and evaluated the methodological quality. Meta-analyses were performed by using RevMan 5.2 software.Results A total of 9 RCTs articles involving 893 cases were included. Meta-analysis results suggested that as compared with antidepressants only,Guipi Decoction together with antidepressants was associated with a significantly greater reduction in HAMD scores (MD=-4.63, 95%CI [-5.95, -3.32];P<0.001). There was no significant difference in HAMD scores betweenGuipi Decoction used alone and antidepressants (MD=-0.30, 95%CI [-4.22, 3.62];P=0.88). As compared with antidepressants,Guipi Decoction together with antidepressants increased in the rates of total effective events (RR=1.26, 95%CI [1.14, 1.38];P<0.001). No serious adverse reactions were reported. Meta-analysis results suggested that the incidence of adverse reactions of Guipi Decoction(RR=0.48, 95%CI [0.40, 0.58];P<0.001) andGuipi Decoction together with antidepressants (RR=0.51, 95%CI [0.30, 0.85];P=0.010) were lower than that of antidepressants alone.Conclusion The present evidence suggests thatGuipi Decoction is effective and relatively safe for patients with depression. However, because the quality of included research was low, more large-scale RCTs with multicenter and high quality are needed for further verification.

Objective To explore the definition,classification and imaging characteristics of abdominal enterogenous cysts in in-fant.Methods 18 patients with abdominal enterogenous cyst in infant,confirmed by surgical pathology,with complete imaging data (including ultrasonography,CT and enhanced CT),have been enrolled in the study.The imaging findings and pathological character-istics have been analyzed,and the classification of diagnosis has been conducted.Results According to the positions and imaging fea-tures of the pathological changes,abdominal enterogenous cysts could be further classified as:gastrointestinal wall type,mesenteric type and retroperitoneal type.In this study,gastrointestinal wall type in 1 1 patients (outside intestinal wall in 2,inside intestinal wall in 8,and outside gastric wall in 1);mesenteric type in 4 and retroperitoneal type in 3 were identified.Conclusion Classification of diagnosis can be conducted according to the enterogenous cyst’s special imaging findings,which provides a reference for the surgical operation.

Objective To explore the therapeutic effectiveness and pathways of human umbilical cord mesenchymal stem cells (hUCMSCs) transplantation for acute hepatic failure in rats.Method hUCMSCs were isolated from umbilical cord with attachment culture method,and the surface antigens were tested by flow cytometry.Forty-eight male Sprague Dawley rats were randomly divided into four groups.The animal model of acute liver failure was induced by injecting intraperitoneally with 50％ olive oil solution of carbon tetrachloride (2.5 ml/kg).The treatment groups were injected with hUCMSCs suspension separately through the tail vein or injected into the liver 24 h post-modeling.Blood serum and liver tissues were collected at several time points to analyze the improvement of liver function and histological repair.Real-time PCR was used to detect the expression of human CK8,CK18 and AFP mRNA in liver tissues.Immunohistochemistry was used to detect the expression of human CK18 in liver tissues.Result There were statistically significant differences among liver functions after transplantation (P＜0.05).hUCMSCs improved histological status through enhancing hepatocellular regeneration and reducing inflammatory cells.Real-time PCR results showed that the expression of CK8,CK18 and AFP mRNA was obviously increased in the tail vein transplantation group and hepatic lobe injection transplantation group as compared with the model group (P＜0.05).Immunochemistry results revealed that transplanted hUCMSCs in animal liver could differentiate into functional hepatocyte-like cells that expressed human CK18 as hepatocyte-specific marker in the tail vein transplantation group and hepatic lobe injection transplantation group.No significant differences in histological repair and grade of differentiation were examined between the tail vein transplantation group and hepatic lobe injection transplantation group (P＞0.05).Conclusion hUCMSCs can prompt the repair of acute liver failure and enhance pathological repair.Transplanted cells in animal liver can differentiate into functional hepatocyte-like cells that expressing hepatocyte-specific markers.Transplantation of hUCMSCs via the tail vein or direct injection into the liver had the similar therapeutic effects.

Objective To treat the patients with primary dysmenorrhea by puncturing Shiqizhui (EX-B8) and observe the change and regularity of the analgesic effect with different needle-retaining time, for providing evidence for the optimal needle-retaining time. Method Ninety eligible subjects with primary dysmenorrhea were randomized into a 20 min group of 30 cases, a 30 min group of 30 cases, and a blank control group of 30 cases. Visual Analogue Scale (VAS) was used to observe the change before and after intervention with the time, and to compare the intergroup differences. Result The acupuncture groups started to show analgesic effect 10 min after needling, and the effect increased with the needle-retaining time (P＜0.05);the therapeutic efficacy of the 30 min group was significantly superior to that of the 20 min group (P＜0.01). Conclusion Acupuncture at Shiqizhui can produce a real-time analgesic effect for patients with primary dysmenorrhea, and retaining the needle for 30 min is superior to 20 min.

The forkhead box O (FoxO) transcription factor family plays an important role in cell functions, including metabo-lism, apoptosis, cellular proliferation, stress reactions, DNA repair, and immune response. As a member of this family, forkhead box O3a (FoxO3a) regulates its target genes by modulating histone modifications, including phosphorylation, acetylation, and methylation. FoxO3a expression is abnormally downregulated in urologic neoplasm. Protein modifications and FoxO3a activity are mainly con-trolled by PI3K/Akt signal pathway and other signaling pathways. FoxO3a is also involved in the initiation, progression, and prognosis of urologic neoplasm. This review focuses on the function of FoxO3a in urologic neoplasm and elucidates the regulatory mechanisms involved. This article will provide novel strategies to clinical diagnosis and drug therapy of urologic neoplasms.

Objective To evaluate effects of acitretin on HaCaT cells cultured in hypoxic condition,and to preliminarily explore the possible therapeutic mechanisms of acitretin in psoriasis.Methods HaCaT cells were divided into several groups to be cultured in hypoxic condition with the presence of acitretin at concentrations of 10-5,10-6,10-7 and 10 8 mol/L respectively,with cells treated with dimethyl sulfoxide (DMSO) as DMSO control group and those receiving no treatment as blank control group.Cellular proliferative activity was evaluated by CCK-8 assay after 12-,24-and 36-hour hypoxic culture in vitro.The mRNA and protein expressions of hypoxia-inducible factor-1α (HIF-1α) and vascular endothelial growth factor (VEGF) were determined by reverse transcription (RT)-PCR and Western-blot analysis,respectively,after 24-hour hypoxic culture.Results After 24-hour hypoxic culture,the cellular proliferation rate was inhibited by 13.31％ ± 1.15％,21.86％ ± 5.31％,32.05％ ± 2.99％ and 37.28％ ± 3.21％ in the 10 8-,10-7-,10-6-and 10-5-mol/L acitretin groups respectively.With the increase of culture duration and acitretin concentrations,the degree of inhibition on cellular proliferation increased gradually.Compared with the blank control group,the 10-5-mol/L acitretin group showed significantly decreased protein expression of HIF-1α (0.319 ± 0.180 vs.1.196 ± 0.088,P ＜0.05),as well as decreased mRNA and protein expressions of VEGF (mRNA:0.442 ± 0.090 vs.1.108 ± 0.073;protein:0.216 ± 0.066 vs.1.174 ± 0.186;both P ＜ 0.05).However,no significant difference was found in the mRNA expression of HIF-lα between the 10-5-mol/L acitretin group and blank control group.Conclusion Acitretin can suppress the in vitro proliferation of HaCaT cells cultured in hypoxic condition,and down-regulate the expressions of HIF-1α and VEGF proteins as well as VEGF mRNA.
