1.Development of Multifunctional and Multipoint Fixed Support Drainage Device for Digestive Tract.

OBJECTIVE:
To discuss the development of a multifunctional and multipoint fixed support drainage device for the digestive tract, as well as the effect of its application on animal experimental models and patients.
METHODS:
The digestive tract multifunctional and multipoint fixed support drainage device is designed according to the requirements of the various gastrointestinal surgery and interventional procedures. It has metal flaps and airbags to achieve multi point fixation. The cuffs and shears are used to achieve endoscopic removal. And through different tube diameters and lengths, surgeons can achieve different surgical purposes.
RESULTS:
A multifunctional and multipoint fixed support drainage device for the digestive tract was successfully designed and developed. The application experiment of the winged pancreatico-intestinal supporting drainage tube on animal models and patients, showed lower drainage fluid amylase level, faster amylase recovery speed, and better perioperative safety.
CONCLUSIONS
The support drainage device has the characteristics of simple operation, firm fixation, and good controllability of removal. It is an ideal choice among support drainage tubes in gastrointestinal surgery and interventional operations.

2.Effects of carbon components of fine particulate matter (PM2.5) on atherogenic index of plasma.

OBJECTIVETo evaluate associations between carbon constituents of fine particulate matter (PM2.5) and atherogenic index of plasma (AIP).
METHODSWe collected subjects from two communities by a system sampling, and 112 people aged over 60 years old without cardiovascular disease were recruited. The levels of cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) of objects, and personal exposure to PM2.5 were measured on December, 2011. Total carbon (TC), organic carbon (OC) and elemental carbon (EC) of PM2.5 were detected and AIP was calculated according to its definition.
RESULTSThe value of AIP among the 112 subjects was 0.05 ± 0.26. Personal exposure concentration of PM2.5 and its carbon components (TC,OC and EC) were (164.75 ± 110.67), (53.86 ± 29.65), (44.93 ± 26.37) and (9.49 ± 5.75) µg/m(3), respectively. The Pearson analysis showed the linear relationship between TC,OC,EC and AIP, all significant positive correlations. The correlation coefficients were TC (r = 0.307, P < 0.05),OC (r = 0.287, P < 0.05) and EC (r = 0.252, P < 0.05), respectively. The multiple logistic regression analysis showed that when the AIP risk categories were selected as dependent variable and low risk group as reference group, the regression coefficient of TC,OC and EC was separately 1.03 (95%CI:1.01-1.05), 1.03 (95%CI:1.01-1.05), 1.12 (95%CI:1.02-1.22) in the high risk group; while there was no statistical significance of the regression coefficient and OR in the middle risk group.
CONCLUSIONThere was stable associations between the carbon constituents (TC,OC and EC) of fine Particulate Matter (PM2.5) and AIP. The findings suggested that carbon components of PM2.5 should be considered as risk factors of atherogenic.

3.Effect of fluvoxamine on lipid metabolism in hospitalized adolescent patients with depressive disorder

BackgroundFluvoxamine is increasingly used in the treatment for depressive disorder in adolescents. However, little research has been done on the effect of fluvoxamine on lipid metabolism, and the disordered lipid metabolism would cause severe harm to the health of patients and affect relevant prognosis. ObjectiveTo analyze the effect of fluvoxamine on lipid metabolism in adolescent patients with depressive disorder and to investigate the safety of fluvoxamine treatment. MethodsFrom June 2022 to June 2023, 60 adolescent patients with depressive disorder were involved, who met the diagnostic criteria of International Classification of Diseases, tenth edition (ICD-10) and received inpatient treatments in Shanxi Mental Health Center. These cases were randomly divided into study group (receiving fluvoxamine treatment) and control group (receiving sertraline treatment) with 30 cases in each group. The treatment period was 4 weeks. At baseline as well as 2 weeks and 4 weeks after treatment, both groups' indexes of fasting lipid metabolism were measured, including serum total cholesterol (TC), triglycerides (TG), high-density lipoprotein (HDL) and low-density lipoprotein (LDL), and Hamilton Depression Scale-17 item (HAMD-17) was adopted. The levels of lipid metabolism indexes and HAMD-17 score were compared between the two groups at different follow-up time points. ResultsFor HAMD-17 score, the time effect was statistically significant (F=849.687, P<0.01), while the inter-group effect and interaction effect was not statistically significant (F=0.033, 1.760, P>0.05). For TC levels, the inter-group effect was not statistically significant (F=1.461, P=0.232), but the time effect and interaction effect were statistically significant (F=13.129, 5.029, P<0.05 or 0.01). The time effect and the inter-group effect of TG level were not statistically significant (F=0.825, 0.185, P>0.05), but the interaction effect was statistically significant (F=7.577, P=0.004). For HDL levels, the time effect, inter-group effect and interaction effect were not statistically significant (F=1.079, 0.160, 1.877, P>0.05). For LDL levels, there was no statistical significance in the inter-group effect (F=0.019, P=0.891), while statistical significance was observed in both time effect and interaction effect (F=6.721, 9.075, P<0.01). ConclusionFluvoxamine and sertraline have curative effectiveness of same level on adolescent depression disorder, and short-term application of fluvoxamine has little effect on lipid metabolism indexes of patients. 4.A surgical classification system for the management of axial primary malignant and aggressive benign tumors and its application in multiple tertiary centers

Objective:To propose and verify a surgical classification system for the axial primary malignant and aggressive benign tumor.Methods:The CZH surgical classification system was originally developed for the axial primary malignant and aggressive benign tumor. The CZH surgical classification system includes seven types, according to the anatomic features and the extension of tumor violation. A total of 136 patients (79 males and 57 females) with axial primary malignant and aggressive benign tumor from multiple tertiary centers who received surgery from July 2006 to July 2019 were included. The average age was 44.40±17.55 years (8-83 years) old. There were 99 malignant tumors and 37 aggressive benign tumors included. The number of patients with each classification was presented as followed, Type I 13, Type II 15, Type IIIa 3, Type IIIb 20, Type IVa 43, Type IVb 12, Type Va 21, Type Vb 3, Type VI 2, Type VIIa 3 and Type VIIb 1. Surgical procedures were selected according to different types in classification. The inter- and intra-observer consistencies were evaluated by the Kendall's W test. The VAS, Frankel score, overall survival and recurrence free survival were recorded during the follow-up. Results:The inter- and intra-observer consistent coefficient was 0.973 and 0.996, respectively ( P<0.05). The single posterior approach was adopted for the Type II tumors. Other patients underwent surgery by the combined antero-posterior approach. The majority in anterior approach (113 cases) was the modified submandibular approach. The reconstruction modes included anterior "T" shape titanium mesh (112 cases) or the 3D printed prothesis (7 cases) combined with the posterior occipto-cervical fusion (92 cases) or the pedicle screw system (44 cases). The average surgical duration and the volume of intraoperative bleeding was 348.40±136.14 min (60-760 min) and 1 225.69±859.40 ml (80-4 000 ml), respectively. The operation duration and volume of intraoperative bleeding among each type were with statistical difference. The patients with Type IV, V tumors had longer operation duration than those with Type II tumors. Those with Type V and VII tumors had longer operation duration than those with Type I tumors. The patients with Type V tumors had more intraoperative bleeding than those with Type I-IV tumors. The average preoperative VAS score was 4.15±2.25 and then was reduced significantly to 0.62±0.71 and 0.38±0.59 at one and three months after operation, respectively. The Frankel score was also significantly ameliorated at one and three months postoperatively. There were 22 postoperative complications (16.2%). The complications included cerebral spinal fluid leak (12.5%), dysphagia and/or dysphonia (7.4%), dyspnea (5.1%), wound infection (3.7%), wound hemorrhage (2.2%) and pharyngeal dehiscence (1.5%). The incidence of postoperative complication was 25.9% in Type IV-VII tumors, while 11.8% in Type I-III tumors. Conclusion:CZH surgical classification system was verified with high observer consistency. This classification system could assist surgeons to select proper surgical approaches, resection modes and reconstruction modes, and thus ensure the safety of surgery and reduce the recurrence. The tumors in Type IV, V and VII may be with more challenging for surgeons. The incidence of postoperative complication in Type IV-VII tumors may be higher than that in Type I-III tumors.
5.Hei Xiaoyaosan Regulates Fas/FasL/Caspase-8/Caspase-3 Signaling Pathway to Inhibit Neuronal Apoptosis in AD Rats

ObjectiveTo explore the effect and mechanism of Hei Xiaoyaosan in regulating the tumor necrosis factor receptor superfamily member 6 (Fas)/Fas ligand (FasL)/cysteine protease-8 (Caspase-8)/cysteine protease-3 (Caspase-3) signaling pathway to intervene in neuronal apoptosis and prevent Alzheimer's disease (AD). MethodNinety SPF-grade SD male rats of 4 months old were selected and randomly grouped as follows: 10 rats in the blank group, 10 rats in the sham group (bilateral hippocampus injected with 1 μL normal saline), and 70 rats in the modeling group [bilater hippocampus injected with 1 μL amyloid-beta protein 1-42 (Aβ1-42) solution for the modeling of AD]. Fifty successfully modeled rats were selected and randomly assigned into model, donepezil hydrochloride (0.45 mg·kg-1), and high-, medium-, and low-dose (15.30, 7.65, 3.82 g·kg-1) Hei Xiaoyaosan groups. Rats were administrated with corresponding agents by gavage once a day for 42 days. Terminal-deoxynucleoitidyl transferase-mediated nick end labeling (TUNEL) was employed to observe the apoptosis of neurons in the cortex and hippocampus, and immunohistochemistry (IHC) was used to detect the expression of B-cell lymphoma-2 (Bcl-2) and Bcl-2-associated X protein (Bax) in the hippocampus. Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) was employed to determine the mRNA levels of Fas, FasL, and Fas-associated protein with death domain (Fadd). Western blot was used to determine the protein levels of Fas, FasL, Fadd, Caspase-3, cleved Caspase-3, Caspase-8, and cleved Caspase-8. ResultCompared with the blank group and sham group, the model group showed increased apoptosis rate in the cortex and hippocampus (P<0.01), elevated Bax level (P<0.01), lowered Bcl-2 level (P<0.01), up-regulated mRNA levels of Fas, FasL, and Fadd in the hippocampus (P<0.01), and up-regulated protein levels of Fas, FasL, Fadd, cleaved Caspase-3, and cleaved Caspase-8 (P<0.01). Compared with the model group, donepezil hydrochloride and Hei Xiaoyaosan at high and medium doses decreased the apoptosis rate in the cortex and hippocampus (P<0.05, P<0.01), lowered the Bax level (P<0.01), elevated the Bcl-2 level (P<0.01), and down-regulated the mRNA levels of Fas, FasL, and Fadd and the protein levels of Fas, FasL, Fadd, cleaved Caspase-3, and cleaved Caspase-8 (P<0.05, P<0.01) in the hippocampus. Low-dose Hei Xiaoyaosan decreased the apoptosis rate in the cortex and hippocampus (P<0.05, P<0.01), lowered the Bcl-2 level (P<0.01), and down-regulated the mRNA levels of FasL and Fadd (P<0.05) and the protein levels of Fas, FasL, Fadd, cleaved Caspase-3, and cleaved Caspase-8 (P<0.05) in the hippocampus. ConclusionHei Xiaoyaosan can protect neurons in the cortex and hippocampus of AD rats by inhibiting the apoptosis mediated by the Fas/FasL/Caspase-8/Caspase-3 signaling pathway.
6.Hei Xiaoyaosan Affects Synaptic Plasticity of Hippocampal Neurons in APP/PS1 Double Transgenic Mice of AD via cAMP/PKA/NMDAR Signaling Pathway

ObjectiveTo explore the effect and mechanism of Hei Xiaoyaosan in modulating the synaptic plasticity in APP/PS1 mice by regulating the cyclic adenosine monophosphate (cAMP)/protein kinase A (PKA)/N-methyl-D-aspartate receptor (NMDAR) signaling pathway. MethodTwelve 4-month-old male C57BL/6J mice were selected as the blank control group, and 60 4-month-old male APP/PS1 double transgenic mice were randomized into model, KW-6002 (adenosine receptor antagonist, 3 mg·kg-1), and high-, medium-, and low-dose (22.10, 11.05, 5.53 g·kg-1, respectively) Hei Xiaoyaosan groups, with 12 mice in each group. Mice were administrated with corresponding drugs for 90 days. Transmission electron microscopy was employed to observe the synaptic ultrastructure of hippocampal neurons, and Golgi staining was used to observe the dendritic spine density of neurons in hippocampal CA1 region. Western blot was employed to measure the protein levels of cAMP, PKA, N-methyl-D-aspartate receptors 1, 2A, and 2B (NR1, NR2A, and NR2B, respectively), postsynaptic density protein 95 (PSD95), and synapsin 1 (SYN1). Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) was performed to determine the mRNA levels of cAMP, PKA, and NR1. Enzyme-linked immunosorbent assay was employed to determine the content of interleukin-12 (IL-12) and interleukin-4 (IL-4) in the hippocampus. ResultCompared with the blank group, the model group showed blurred boundaries between presynaptic membrane and postsynaptic membrane in hippocampal CA1 region, reduced and scattered synaptic vesicles, and decreased density of postsynaptic membrane, and irregular, disarranged, and loosened dendritic spines of neurons in hippocampal CA1 region (P<0.01). In addition, the model group presented down-regulated protein levels of cAMP, PKA, NR1, NR2A, NR2B, PSD95, and SYN1 and mRNA levels of cAMP, PKA, and NR1, elevated IL-12 level, and lowered IL-4 level in the hippocampus (P<0.01). Compared with the model group, the drug intervention groups showed clear and intact boundaries between presynaptic membrane and postsynaptic membrane in hippocampal CA1 region, increased synaptic vesicles with dense arrangement, increased density of postsynaptic membrane, and improved morphology, arrangement, and density of neuronal dendritic spines (P<0.05, P<0.01). In addition, the drug interventions up-regulated the protein levels of cAMP, PKA, NR1, NR2A, NR2B, PSD95, and SYN1 (P<0.05,P<0.01) and mRNA levels of cAMP, PKA, and NR1 (P<0.01), lowered the IL-12 level (P<0.01), and elevated the IL-4 level (P<0.01) in the hippocampus. ConclusionHei Xiaoyaosan can improve the structure and morphology of hippocampal neurons in APP/PS1 mice by activating the cAMP/PKA/NMDAR signaling pathway and repairing synaptic plasticity.
7.Hei Xiaoyaosan Regulates RAS/RAF/MEK/ERK Signaling Pathway to Ameliorate Oxidative Stress in Rat Model of AD

ObjectiveTo investigate the role and mechanism of Hei Xiaoyaosan in intervening in oxidative stress in the rat model of Alzheimer's disease (AD) via modulating the rat sarcoma (RAS)/rapidly accelerating fibrosarcoma (RAF)/mitogen-activated protein kinase kinase (MEK)/extracellular signal-regulated kinase (ERK) signaling pathway. MethodOne hundred 4-month-old SPF-grade Wistar male rats were randomly grouped as follows: 10 in the blank group, 10 in the sham group (bilateral hippocampus injected with 1 μL normal saline), and 80 in the modeling group [bilateral hippocampus injected with 1 μL amyloid beta protein 1-42 (Aβ1-42) solution for the modeling of AD]. Fifty rats qualified for modeling were selected and randomized into the model, donepezil hydrochloride (0.5 mg·kg-1), and high-, medium-, and low-dose (15.30, 7.65, 3.82 g·kg-1, respectively) Hei Xiaoyaosan groups. The rats were administrated with corresponding drugs by gavage once a day for 42 consecutive days. At the end of gavage, Morris water maze test was performed to examine the learning and memory abilities of the rats, and Nissl staining was used to observe the pathological changes of neurons in CA3 region of the hippocampus. The immunofluorescence assay was used to observe Aβ deposition and tau phosphorylation. Western blot was employed to determine the protein levels of RAS, RAF, phosphorylated (p)-RAF, MEK, p-MEK, ERK, and p-ERK in the hippocampal tissue. Biochemical methods were used to determine the levels of reactive oxygen species (ROS), malondialdehyde (MDA), and superoxide dismutase (SOD) in the hippocampal tissue. ResultCompared with the sham group, the model group showed prolonged escape latency (P<0.01), shortened swimming distance in the target quadrant (P<0.01), reduced and uneven stained Nissl bodies, enhanced fluorescence intensity of Aβ and p-tau (P<0.01), up-regulated protein levels of RAS, p-RAF, p-MEK, and p-ERK in the hippocampal tissue (P<0.01), increased ROS and MDA content (P<0.01), and decreased SOD activity (P<0.01) on day 5. Compared with the model group, donepezil hydrochloride and high-, medium-, and low-dose Hei Xiaoyaosan shortened the escape latency (P<0.01), increased the swimming distance in the target quadrant (P<0.01), improved the arrangement, morphology, and structures of neurons and the number and distribution of Nissl bodies, decreased the fluorescence intensity of Aβ and p-tau (P<0.01), up-regulated the protein levels of RAS, p-RAF, p-MEK, and p-ERK (P<0.05, P<0.01), decreased the ROS and MDA content (P<0.01), and increased the SOD activity (P<0.01) on day 5. ConclusionHei Xiaoyaosan may ameliorate oxidative stress, reduce Aβ and p-tau levels, and inhibit hippocampal neuronal damage by regulating the RAS/RAF/MEK/ERK signaling pathway, thus improving learning and memory abilities.
8.Hei Xiaoyaosan in Treating Alzheimer's Disease: A Review

Alzheimer's disease (AD) is a neurodegenerative disease that predominantly affects the elderly. It belongs to the category of dementia in traditional Chinese medicine (TCM), with the onset and progression closely associated with the functions of the kidney, liver, and spleen. The classic TCM formula Hei Xiaoyaosan, which regulates the three Yin of liver, spleen, and kidney, shows broad prospects in treating neurodegenerative diseases. This article reviews the experimental studies reported in the past decade to summarize the mechanisms of Hei Xiaoyaosan and its active components in intervening in AD. Hei Xiaoyaosan can treat AD via multiple targets, levels, and aspects comprehensively. The clinical studies have demonstrated that Hei Xiaoyaosan alone or in combination with other therapies has a definite therapeutic effect on AD. Specifically, it can ameliorate the cognitive impairment, mitigate oxidative stress, and inhibit the overexpression of soluble apoptotic factors in AD patients. This review aims to provide a theoretical basis for the treatment of AD with Hei Xiaoyaosan and explore new research directions. Moreover, it gives new insights into the clinical application of Hei Xiaoyaosan and the development of products with both medicinal and edible values.
9.Dosimetric study of intensity-modulated radiotherapy with different gantry angle for lumbar spinal metastases

Objective To compare the dose difference of intensity-modulated radiotherapy (IMRT) with different gantry angle between target volumes and surrounding normal tissues in lumbar spinal metastases therapy. Methods Ten patients with lumbar spinal metastases were enrolled in the study. Three plans with the same prescription dose of 3000 cGy/10f were designed by seven-equal beams (plan-A), five-back beams (plan-B) and seven-back beams (plan-C). All the plans were designed with the same objective function and dose limiting condition. The difference of dosimetric parameters of planning target volume (PTV), organ at risk (OAR), normal tissues, and treatment parameters in all the plans were compared with SPSS 21.0 statistical software. Results All the plans satisfied the clinical requirement. There was no significant difference in the Dmean, D2%, D98%, CI and HI of PTV between plan-A and plan-C (P >0.05), and these parameters of plan-A and plan-C were better than plan-B (P < 0.05). Compared with the other two plans, plan-B reduced the dosimetric parameters of bilateral kidney (P < 0.05), whereas plan-B increased the Dmax of spinal cord (P < 0.05). The V5 and V10 of normal tissue of plan-B were lower than the other two plans while the V15, V20 and V25 showed inverse relationship (P < 0.05). plan-B had certain advantages in shortening the monitor units and treatment time (P < 0.05).. Conclusions The seven-equal beams (plan-A) and seven-back beams (plan-C) IMRT plans can provide better target dose distribution, and reduce the Dmax of spinal cord. Five-back beams (plan-B) IMRT plan had certain advantages in protecting bilateral kidney and shortening treatment time.
