1.Clinical effect of intra-articular injection of sodium hyaluronate combined with rehabilitation training in treatment of knee osteoarthritis in the elderly

Objective To observe the clinical effect of intra-articular injection of sodium hyaluronate combined with rehabilitation training in treatment of knee osteoarthritis, and their effects on β endorphin and substance P levels in plasma. Methods All 106 cases with knee osteoarthritis were divided into treatment group and control group. Treatment group were treated by intra-articular injection of sodium hyaluronate combined with rehabilitation training, and control group underwent rehabilitation training only. Knee joint function was evaluated using Lysholm scores before and six weeks after treatment, and at 3-month and 6-month follow-up. Meanwhile, the concentrations of β endorphin and substance P in plasma were tested in the two groups. Results Compared with pre- treatment, the Lysholm scores were obviously increased after 6-week treatment, at 3-month and 6-month follow-up in both groups, with more obvious increase in treatment versus control group (P<0.01). The Lysholm scores were higher in treatment group than in control group (P<0.01). The concentration of β endorphin in plasma was significantly increased, and the substance P concentration in plasma was decreased after treatment in both groups. There were also significant differences in concentrations of β endorphin and substance P in plasma between treatment group and control group (P<0.01). Conclusions Intra-articular injection of sodium hyaluronate combined with rehabilitation training is effective in improving knee joint function in elderly patients, in relieving pain and in enhancing joint movement.
2.Clinical study on cilostazol combined with pancreatic kininogenase in treatment of LEADDP

Objective To compare clinical effects between cilostazol(CLT)alone and cilostazol combined with pancreatic kininogenase(PK)on lower extremity arterial disease in diabetic patient(LEADDP).Methods Patients with LEADDP were randomly divided into 2 groups:single medication group or the control group(53 cases)and combination group or the treatment group(53 cases).Sugar-reducing medicines were given before and after treatment to keep a steady blood glucose level.The control group took CLT orally 100 mg each time,twice a day.Besides receiving the same dose of CLT,the treatment group took 120 tablet of PK on an empty stomach,three times a day,with the duration of 3 months.Clinical symptoms,ankle/brachial index(ABI),blood rheology,nailfold microcirculation,intimal medial thickness(IMT)and plaque thickness were compared between the 2 groups after treatment.Results Symptoms in both groups improved after treatment.ABI was elevated significantly compared to that before treatment(P<0.05)and the change was more obvious in combination group.The difference of ABI after treatment between the 2 groups had statistical significance(P<0.05).IMT of the right foot dorsal artery,and left and right posterior tibial artery increased significantly compared to that before treatment(P<0.05).Plaque thickness decreased compared to that before treatment(P<0.01)and the difference between before and after treatment in combination group was significantly greater than that in single medication group(P<0.05).Conclusion Cilostazol combined with pancreatic kininogenase has better clinical effects than cilostazol alone in treatment of patients with LEADDP.
3.Role of Serum Creatinine,Cystatin C,Abbreviated MDRD and Le Bricon Formula in the Evaluation of Renal Function after Renal Transplantation

Objective To investigate diagnostic value of the serum creatinine (Scr),Cystatin C (Cys C),MDRD and Le Bricon formula in the renal function after renal transplantation.Methods The total renal glomerular filtration rate (GFR)of 5 7 ca-ses of renal transplantation patients and 48 healthy control group were determined using 99mTc-DTPA from 2012 September to 2014 June.Scr,Cys C and calculate the estimated glomerular filtration rate of MDRD and Le Bricon,correlation analysis by Spearman,and use receiver operating characteristic curve (ROC)to evaluate the diagnosis value.Results DTPA,Scr, Cys C,MDRD and Le Bricon intransplantation group was 60.69±13.52 ml/min/1.73m2,1.34±0.28 mg/dl,1.42±0.15 mg/dl,66.97±15.85 ml/min/1.73m2 and 59.93±10.23 ml/min/1.73 m2 ,respectively.Control group were 90.12±12.61 ml/min/1.73m2 ,0.97±0.09 mg/dl,0.86±0.2 mg/dl,87.13±18.29 ml/min/1.73m2 and 94.80±18.73 ml/min/1.73m2 , respectively.There were statistically significant (t=0.237~1.35,P< 0.01)among groups.Scr,Cys C,MDRD and Le Bri-con between DTPA were different correlation (r=-0.76~0.82,P<0.01).The area under ROC curve were 0.856,0.973, 0.814 and 0.971,respectively.Conclusion Scr,Cys C,MDRD,Le Bricon have some predictive value to chronic kidney dis-ease in all patients after renal transplant recipients,but Cys C and Le Bricon were superior to the Scr and MDRD equation.
4.Observation of thyrohyoid muscle change after cutting the recurrent laryngeal nerve in dog

Objective.To observation the change of thyrohyoid muscle(THM)after recurrent laryngeal nerve(RLN)has been cutting off.The animal experimental results were reported.Method.The animal experimental model of RLN paralytic was completed in 9 dogs.The specimens of THM were treated by histochemiscal staining after 2,4,6,8,10,20 day operation respectively and the diameter and areas of three types muscle fibers were determined by microscope.Results.The diameters and areas about 3 types muscle fibers of THM descended after operation,the value of them reached the minimum simutaneously,then recovered step by step.Conclusion.The changes of THM was related to operative trauma and nutrition disorder probably,while it was not related to RLN cutting.
5.Expression of Matrix Metalloproteinase-9 and Tissue Inhibitor of Metalloproteinase-1 in Thyroid Carcinoma Tissue and Their Concentrations in Serum

Objective To explore the protein expressions of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1), and to investigate their relationships between their serum concentration before operation and the infiltration and metastasis of thyroid carcinoma. Methods The protein expressions of MMP-9 and TIMP-1 in 32 cases of thyroid carcinomas, 23 cases of adjacent tissues and 30 cases of benign hyperplastic lesions were measured by using immunohistochemistry. The preoperative serum concentrations of MMP-9 and TIMP-1 in 21 cases of thyroid carcinomas and 19 cases of benign hyperplastic lesions were determined by enzyme-linked immunosorbent assay. 6.Aplication effect of fiber bronchoscope examination in the treatment of respiratory internal medicine emer-gency and its effect on the quality of life of patients

Objective To study the aplication effect of fiber bronchoscope examination in the treatment of respiratory internal medicine emergency and its effect on the quality of life of patients.Methods 156 patients with emergency were randomly divided into the observation group and control group,each group in 78 cases.The control group was given routine treatment plan according to the primary disease type which was the respiratory treatment of the corresponding diseases,the observation group in the control group was added fiberoptic bronchoscopy based on the treatment.Compared blood clinical curative effect,heart rate and quality of life score changes of the two groups of patients after treatment.Results The total effective rate of the observation group was 94.87%,which was significantly higher than 81.01% of the control group(χ2 =6.28,P <0.05).The pH,PaO2 and SaO2 in the observation group after treatment were(7.35 ±0.04),(73.23 ±9.11)mmHg and(92.33 ±4.32)%,which were significantly higher than the corresponding index in the control group(t =3.97,6.08,4.96,all P <0.05);while PaCO2 and HR were (51.53 ±9.24)mmHg,(91.69 ±12.42)times/min,which were significantly lower than the corresponding index in the control group(t =6.73,5.13,all P <0.05);The quality of life score in the observation group was(54.04 ± 10.94)points,which was significantly lower than(77.23 ±12.32)points of the control group(t =5.86,P <0.05). Conclusion Fiberoptic bronchoscopy for respiratory internal medicine emergency treatment can significantly improve the clinical curative effect of treating disease and patient quality of life.
7.MAGI1 inhibits cancer cell migration and invasion of hepatocellular carcinoma via regulating PTEN

Objective To explore the biological function and molecular mechanism of membrane associated guanylate kinase,WW and PDZ domain containing 1 (MAGI1) in hepatocellular carcinoma.Methods HepG2MAGI1 stable cell line was constructed by transfecting HepG2 cells with pcDNA3.1-MAGI1 plasmid.Wound healing and invasion assay were performed to compare the migration and invasion ability of HepG2MAGI1 and HepG2 cells.Furthermore,the expression of MAGI1 and phosphatase and tensin homolog deleted on chromosome ten (PTEN) was also examined by Western blot and the relationship was analyzed.Results The wound healing assay showed that the closure of HepG2MAGI1 cells was significantly slower than that of HepG2 cells [(90±10)% vs.(50±15)%,P<0.05],and the invasion assay showed that the number of HepG2MAGI1 cells that passed through the matrigel was fewer than HepG2 cells (68±18 vs.150±30,P<0.05).The protein expression level of PTEN was significantly elevated in HepG2MAGI1 cells compared with HepG2 cells (1.40±0.32 vs.0.28±0.15,P<0.05).MAGI1 and PTEN protein expression levels were positively correlated (r=0.913,P<0.01).Conclusion MAGI1 may inhibit the cancer cell migration and invasion of hepatocellular carcinoma via regulating PTEN.
8.Analysis on Government Investment on Special Funds Investment of Grassroot Maternal and Child Health

Objective: During the five years period from 2006 to 2010, the Chinese government had increased the special funding for primary maternal and child health enterprise to analyze the total amount and constitution of the special funding respectively. Methods: From the perspective of special funding investment in the eastern, central and western regions with different levels of economic development, to perform the comparative analysis. Results and Conclusion:The government’s overall investment on maternal and child special funds is in increasing trend, the average per capita amount of special funds invested in west region is higher than the eastern and central region; in the aspect of investment constitution , the central government special funds for women and children is the main source, which accounts for more than 85% of total investment, the reducing and eliminating items in eastern and western regions and the inpatient childbirth subsidy items constitute the major component of special funding for primary maternal and child health in corresponding region. Based on the analysis of investment on the condition of primary maternal and child health , policy recommendations for further development of the cause of women and children are provided.
9.Investigation and analysis of the present critical care medicine in Inner Mongolia

Objective To investigate the development of ICUs and critical care medicine at secondary and above hospitals in Inner Mongolia.Methods Questionnaire survey of ICUs at secondary and tertiary hospitals in Inner Mongolia Autonomous Region from October 2010 to June 2011.Results There are 76 ICUs at 26 hospitals,including 24 general ICUs and 52 special ICUs; tertiary hospitals average 2.47±0.63 special ICUs,while secondary hospital average 2.23±1.12 special ICUs; 35% of the ICUs are subject to close-off management; general ICUs at tertiary hospitals account for 1.21% ± 0.35 % of total beds of the hospital,and for 1.98% ± 0.63% of secondary hospitals.Regarding the ratio of physicians to ICUs beds,it is 0.75±0.15 at tertiary hospitals,and 0.62± 0.231 at secondary hospitals.As to the ratio of nurses to beds,it is 1.58 ± 0.72 at tertiary hospitals,and 1.36 ± 0.27 at secondary hospitals.Conclusion This survey showed that ICUs in Inner Mongolia Autonomous Region are acceptable in numbers,but are found with problems in insufficient beds,poor staff deployment,obsolete medical equipments and weak technical forces.Therefore,critical care medicine and ICU development should be enhanced during the second round of hospital accreditation in China.
10.A nationwide investigation and analysis of present status of independent clinical laboratories in China and suggestions

Objective To have a general picture the history and existing problems of independent clinical labs in China, for the purpose of building a supervision mechanism for such novel clinical institutions-clinical labs. Methods By way of the nationwide network of clinical labs, ICLs in China were surveyed with written questionnaires and field check. Results ICLs have grown into a greater and diversified scale thanks to the development of China's economy and laboratory technology. However,such vulnerabilities as unbalanced staff ratio, full-range quality control bugs, cutthroat competition,asymmetrical information disclosure, and bio-safety have loomed large in the meantime. Conclsion It is time to formulate a stricter industry access system and appropriate regulatory modes; ICLs should take a good care of the "Four relations" in their development, and health regulators should play a tighter role in the "Four supervisions". All these aim at better sharing medical resources, and building a win-win environment for the people, hospitals and ICLs.
