1.Postoperative rectoanal function in lower rectal carcinoma patients undergoing intersphincteric resection

Objective To investigate the changes of rectoanal function after intersphincteric resection(ISR). Methods In this study,27 lower(3~5 cm from anal verge)rectal adenocarcinoma patients(T0-T2 determined by MRI and endoanal uhrasonography before surgery)undergoing intersphincteric resection between January2005 and January 2007 were followed up,their rectoanal function was assessed by rectoanal manometry 1 week preoperatively and 2 weeks,3 months,6 and 9 months postoperatively.The data were compared by using student t test and X2 test.Results For these 27 patients,within 3 months after surgery,the mean resting pressures(44±6)mm Hg to(19±3)mm Hg,maximum resting pressures(60±8)mm Hg to(33±4)mm Hg,squeezing pressures(180±19)mm Hg to(131±16)mm Hg and msximum tolerable volume(165±20)ml to(55±8)nd were decreased significantly(P<0.05),and the rectoanal inhibitory reflexes(BAIR)were all absent;6 months after surgery,the resting pressures,squeezing pressures and maximum tolerable volume began to increase obviously,and 6 patients presented RAIR;3 months later the mean resting pressures,maximum resting pressures and squeezing pressures were(41±5)mm Hg,(57±8)mm Hg and(173±19)mm Hg respectively,close to the normal range(P>0.05),though the maximum tolerable volume increased to(110±16)mi,still lower than the preoperative value(P=0.0153),11 patients restored BAIR.Conclusions The rectoanal function is injured obviously after intersphincteric resection within a short time period,but with time it was gradually restored.
2.Experimental study of Schwann cell growth and migration on biodegradable steroframe

Objective To observe the behavior of Schwann cell migration and wrapping up of fibres during co-culture of Schwann cells with steroframe of PLA and PLGA fibres. Methods Schwann cells were mixed into 30% ECM gel and co-cultured with PLA non-spinning fibre steroframes and seeded on one ends of PLGA fibres which were treated with collagen,polylysine or ECM. The attachment,growth and migration of Schwann cells were observed under phase contrast microscope and laser scan confocus microscope. Results (1)When Schwann cells were co-cultured with PLA steroframes,most of the Schwann cells kept in the fibre holes and attached on PLA fibres to form B?ngner band likd structures. (2)When co-cultured with PLGA fibers,Schwann cells could attach and migrate on PLGA fibres and the number of Schwann cells were obviously increased when PLGA fibres were treated by ECM. Conclusions (1)ECM gel colud promote Schwann cells attachment,growth and migration on PLA steroframes and PLGA fibers. (2)ECM gel is a good integrating substance for constructing tissue engineering bioartificial nerve.

5.Efficacy of plasma procalcitonin in evaluating severity of community-acquired pneumonia in elderly patients

Objective To analyze the plasma procalcitonin (PCT) as a predictor of the severity of community acquired pneumonia (CAP) in elderly patients.Methods Totally 90 elderly patients hospitalized with community acquired pneumonia from 2010 to 2011 were analyzed retrospectively for the relation between plasma procalcitonin and severity of pneumonia.All cases were divided into two groups,the severe group (n=36) and the non-severe group (n=54) according to diagnostic criteria.Results The level of plasma PCT was much lower in the severe group (median 2.44 μg/L) than that in the non-severe group (median 0.11 μg/L) (U=335.50,P=0.000).Among all patients,when PCT was lower than 0.5 μg/L,the incidence of non-severe CAP was 76%,however,when PCT was equal or above 2.0 μg/L,the incidence of non-severe CAP was reduced to 9%.In Binary logistic regression analysis,PCT was a risk factor of aged person with severe community acquired pneumonia independent of age and CRUB-65 scores [OR =1.328 (95 % confidential interval:1.072,1.645)].PCT had a positive correlation with CRUB-65 scores (U=10.162,P=0.006).In all cases,the patients who improved well had lower PCT value than the remaining (median 0.21 μg/L,17.0μg/L; U=10.000,P=0.000),which also happened in severe cases (median 1.47 μg/L,17.0 μg/L;U=8.000,P=0.000).The area under the receiver operating characteristic curve was 0.872 (95% confidential interval:0.741,0.914).At a PCT cut-off level of greater than or equal to 2.0 μg/L,the sensitivity and specificity to predict the severity of aged person with CAP was 55.6% and 98.9% respectively.Conclusions Plasma PCT may be a good predictor to evaluate the severity of CAP in elderly patients.

OBJECTIVE:To provide suggestions for China in dealing with the drug shortage problem by drawing experiences from USA.METHODS:The study was done by reiviewing literature and cases study.RESULTS:The FDA in USA has established procedures for the Center of Drug Evaluation and Organization of Regulation Affairs to resolve all kinds of drug shortage problems.CONCLUSIONS:A specific department should be set up to be responsible for resolving drug shortage problems in China with an emphasis on the shortage of medical necessary drugs,and it is urgent to get the information of drug shortage.
7.Expression of lncRNA TUSC7 in melanoma A375 cell line and its effect on proliferation and apoptosis of melanoma cells

Purpose To investigate the expression level and the role of TUSC7 in human cutaneous malignant melanoma (CMM).Methods Quantitative real time-PCR (qRT-PCR) assay was performed to detect the expression of TUSC7 in 60 cases of CMM tissues,20 cases of benign nevus,4 CMM cell lines,and one normal human epidermal melanocytes.Then overexpression of TUSC7 was performed and its role in tumor progression was explored.Results TUSC7 expression was significantly downregulated in primary CMM tissues (n =60) compared to benign nevi (n =20),which were significantly downregulated in all the four melanoma cell lines,especially in A375 cells,compared with the normal melanocytes cells (all P < 0.05).In comparison with the A375 cells transfected with the empty plasmid,those transfected with pcDNA-TUSC7 showed an obvious decrease in the proliferation and colony formation activity,while increase in the apoptosis rate (all P < 0.05).Conclusion Our results suggested that the dysregulation of TUSC7 may play an important role in the CMM progression.
8.Relationship between daytime sleepiness and oxygen desaturation in the male elderly with obstructive sleep apnea/hyponea syndrome

Objective To analyze the relationship between daytime sleepiness and oxygen desaturation in the male eldcrly with obstructive sleep apnea /hyponea syndrome (OSAHS).Methods 147 male elderly (aged 60-80 years) with OSAHS in our sleep laboratory from March 2008 to February 2011 were divided into daytime sleepiness and daytime non-sleepiness groups. Epworth sleeping scale (ESS) and polysomnography (PSG) were employed to detect the degree of sleepiness.Oxygen desaturation were evaluated by oxygen desaturation index (ODI),average and minimum blood oxygen,time percentage of 90% oxygen desaturation (CT90). Resulls There were significant differences between the two groups in body mass index (BMI) (t=4.157),ODI (t=-10.063),average (t=5.800) and minimum (U=916) blood oxygen,CT90(U=887)and apnca/hypopneaindex (AHI) (t =-11.361) (all P<0.001).ESS scores were related with desaturation parameters of ODI,average and minimum blood oxygen and CT90 (r=0.683,-0.450,-0.583,0.507,all P<0.001).ODI was correlated with average and minimum blood oxygen and CT00 (r=-0.628,-0.763,0.689,all P<0.001).Among parameters of oxygen desaturation,only ODI had influence on ESS scores (odds ratio=1.11,95%CI:1.07 1.15),The sensitivity and specificity predicting daytime sleepiness at ODI ≥23 times/h were about 79.2% and 84.3%,respectively.Conclusions Daytime sleepiness is associated with oxygen desaturaion and may be predicted by ODI through which average and minimum blood oxygen and CT00 are related with daytime sleepiness in the male elderly with OSAHS.
9.The effect of different dose of butorphanol with epidural injections on neurological function for rats

Objective To observe the effect of epidural injection with different dose of butorphanol on the rats' neurological function.Methods A PE-530 catheter was inserted into the epidural space of all the SpragueDawley rats (male, weighting 180 ~210 g) at L1-2 level.After three days, a total of 32 rats without any motor dysfunction were randomly divided into 4 groups as follows saline(NS) group (group C, n= 8 )and butorphanol injection (B) group( B1∶ n=8;B2∶ n=8;B3∶ n=8).Rats in group C were epidurally injected NS 30 μl each ,and rats in group B1, B2 and B3 were respectively epidurally injected Butorphanol 60 μg/30μl, 120 μg/30 μl,240 μg/30 μl (all diluted with NS) ,and 1 time per day for5 days.The neurological function of rats was recorded before injection (T0) and 6h after injection on day 1 ~4(T1 ~T 4) and 6h,24h and 72h after injection on day 5 (T5 ~T7) by BBB (BASSO,BEATTIE and BRESNAHAN ) Score and the inclined plane test .Results Compared with group C ,the BBB score and the inclined plane test of group B1 showed no significant difference throughout the experimental period(P> 0.05 ).There was also no significant difference at T0 ~ T3 of group B2 and group B3 compared with group C (P > 0.05 ), while at T4, the BBB score ( ( 18.50 ± 2.00 ) points, ( 16.38 ± 2.33 ) points) and the inclined plane test( (58.75 ± 5.17 )°, (59.38 ± 3.20) ° ) of the two groups were both obviously decreased when compared with group C( (21.00 ±0.00) points, (65.00 ±3.78)°, P<0.05) ,and the same significant differences appeared at T5,T6 and T7 (P < 0.05 ).Conclusion Repeated epidural injection of butorphanol 60 μg have no effect on neurological function of rats,while repeated epidural injection of butorphanol 120 μg and 240 μg could impaire the neurological function.
10.Effects of Feiji Formula on lung cancer metastasis in mice

OBJECTIVE: To observe the effects of Feiji Formula, a compound traditional Chinese herbal medicine, on lung cancer metastasis in mice. METHODS: The lung cancer metastasis model of mice was established in this experiment study. Twenty-four mice were randomly divided into three groups: untreated group, cisplatin group and Feiji Formula group. Mice in the Feiji Formula group were treated with Feiji Formula decoction; in cisplatin group, with cisplatin by intraperitoneal injection; and in the untreated group, with normal saline (NS). After twenty-day treatment, the body and tumor weights as well as the number of metastatic tumors in both lungs of each mouse were measured. RESULTS: The body weight of mice in cisplatin group was significantly less than that of Feiji Formula group and untreated group (P<0.01); the tumor weight of mice in cisplatin group and Feiji Formula group was markedly lower than that of untreated group (P<0.01); and the number of metastatic tumors in cisplatin group and Feiji Formula group was markedly lower than that of the untreated group (P<0.01), no significant difference between the Feiji Formula group and cisplatin group in terms of the weights and the numbers of metastatic tumors in bilateral lungs. CONCLUSION: Feiji Formula can suppress tumor growth and decrease the number of lung metastatic tumors in the mice, and maintain the body weight of the mice.
