1.Analysis of prognostic risk factors in elderly patients with acute coronary syndrome

Objective To explore short-term prognosis of patients with acute coronary syndrome (ACS) and prognostic risk factors, and to provide information for prognostic risk assessment. Methods A total of 156 patients with acute coronary syndrome were enrolled. The correlations of baseline characteristics with 30-day improvement, death, angina pectoris and heart failure were analyzed using unordered multivariate logistic regression. Results Logistic regression analysis showed that the independent risk factors for 30-day death included age (OR=1.20, 95%CI: 1.07-1.34), diabetes (OR=19.41, 95%CI: 3.02-124.70), leukocytosis (OR=11.36, 95%CI: 1.87-69.11) and increased platelet (OR=7.72, 95%CI: 1.29-46.15). The independent risk factors for 30-day angina pectoris included leukocytosis (OR=2.35, 95%CI: 0.89-6.17) and dyslipidemia (OR=6.25, 95%CI: 2.11-18.48). The independent risk factors for the occurrence of heart failure during 30-day post-ACS included age (OR=1.10, 95%CI: 1.03-1.17), male (OR=4.18, 95%CI: 0.81-21.51), leukocytosis (OR=2.97, 95%CI: 1.09-8.14) and dyslipidemia (OR=7.69, 95%CI: 2.39-24.76). Conclusions Age, diabetes, leukocytosis and increased platelet are independent risk factors associated with 30-day death; Leukocytosis and dyslipidemia are independent risk factors associated with 30-day angina pectoris; Age, male, leukocytosis and dyslipidemia are the independent risk factors associated with 30-day heart failure.
2.Mechanical properties of N48 type orthodontic NdFeB magnet

Objective: To test the mechanical properties o f N48 NdFeB magnet.Methods:Samples of N48 NdFeB magnet in the size of 2 mm?3 mm?4 mm were prepared, the residual flux density,coercive force and maximum energy product of the samples were measured, the magnetic density aroun d the magnets,the attractive and repulsive force between two magnets in 0~10 mm air gap were investegated.Results:The residual flux density of t he N48 NdFeB magnet was 1.373 T, coercive force 1 037 A/m and maximum energ y product 368 kJ/m 3.The magnetic density at magnet surface was 0.338 T,it decr eased to 0 at the point that leave magnet surface 20 mm away. The highest attrac tive force was 4.47 N between two magnets, the attractive force was 4.47~0.39 N in 0~5 mm air gap.The highest repulsive force was 2.82 N and the repulsive force was 2.82~0.39 N in 0~5 mm air gap.Conclusion:The magnetic and m echanical properties of the N48 NdFeB magnet can meet the standard of the orthod ontic force.
3.RANKL mRNA expression induced by Porphyromonas endodontalis in rat osteoblasts

Objective:To study the effects of receptor activator of nuclear factor-kB ligand-RANKL in osteoblasts (OBs) activiated by Porphyromonas endodontalis(Pe),and to investigate the bone resorptive pathogenesis induced by Pe.Methods:Primary rat calvarial osteoblasts were cultured, then were infected with Pe ATCC 35406 at the density of 107 and 109 CFU/ml respectively for 24 h.To determine the kinetic effect of Pe on OBs,107 CFU/ml of Pe was used to stimulate the cells for 0,6,12,18 and 24 h respectively, RT-PCR were used to examine the RANKL mRNA expression.Results:The grey level ratio of RANKL mRNA expression to ?-actin in untreated OBs was 0.32,that in the cells infected by 107 and 109 CFU/ml of Pe for 24 h was increased to 1.73 and 2.24 respectively.Infected by 107 CFU/ml of Pe for 6,12 and 18 h,the ratio was 0.93,2.01 and 1.97 respectively.Conclusion:Pe may stimulate RANKL expresson in osteoblasts.
4.Clinical study of the predicting values of brain natriuretic peptide on cardiovascular risks in patients with type 2 diabetes

Objective To explore the relationship of plasma level of B-type natriuretic peptide (BNP) with cardiovascular risk,the severity of coronary heart disease(CHD),and the short-term prognosis in patients with type 2 diabetes. Methods 154 patients with type 2 diabetes,of them 95 cases complicated with CHD and 65 with hyper-tension were selected in this study. The CHD patients were diveded into 3 groups: AMI(n=32), UAP(n=33) and SAP(n=30). The relationship of the plasma BNP levels with cardiovascular risks, with each coronary heart disea-ses,were observed. The patients were followed up for 6 months to study the predicting role of BNP on the death in pa-tients accompanied with CHD. Results The plasma BNP level was (397.34±217.79) ng/L, which was correlated with age, CRP, hypertension and CHD (r=0.631,0.672, 0.762,0.857, P<0.05 for each);the plasma BNP levels increased with age(r=0.896,P<0.01):(57.6±12.3) ng/L in patients <50 years old,(146.2±53.4)ng/L in patients 50≤and < 59 years old, (388.4±67.5) ng/in patients 60≤and < 69 years old, and (423.8±132.6) ng/L in patients≥70 years old (P<0.01 or P<0.05). The plasma BNP levels, was higher in patients with hyper-tension than that in patients without hypertension [(314.7±125.3) ng/L vs (136.8±98.7) ng/L, P<0.01];Higher in patients with CHD than that in patients without CHD [(425.03±200.80)ng/L vs (37.64±21.57) ng/L,P<0.01)]. The short-term prognosis of patients with CHD was correlated with the levels of BNP, and BNP levels≥485 ng/L may be an independent predicting factor for cardiac death within one month. Conclusions Plas-ma levels of BNP were associated with some cardiovascular risks,which may be one of biomarkers for cardiovascular risks in patient complicated with CHD.
5.Therapeutic effect of gamma knife on intracranial cavernous angioma

Objective: To evaluate the therapeutic effect of gamma knife on patients with intracranial cavernous angioma (CA). Methods: The medical records of 122 patients (134 lesions) who underwent radiosurgery were reviewed retrospectively. Results: hT e average follow-up period was 43 months. No patient died. One patient underwent CA resection. In patients with epilepsy, 83% patients showed alleviation of seizures. About 44% of the lesions shrank in size after treatment with gamma knife radiosurgery (59/134). Seven cases had hemorrhage again after radiosurgery, and the overall annual hemorrhage rate was 1.6%. Edema was found in 11.5% patients (14/122) and all patients showed improvement atf er treatment. Conclusion: Gamma knife is a safe treatment for CA, which could obviously improve the symptoms of epilepsy. Gamma knife radiosurgery is the if rst option for the treatment of cavernous sinus angiomas.
6.An analysis of the result of tumor screening for health examination participants

Objective To analysis the result of tumor screening for health examination participants.Methods Tumor screening was applied for 15 863 health examination participants without malignant tumor disease history.Clinical examination,laboratory tests and imaging studies were comprehensively analysed combining the results of other medical items.The cancer detection rate and asymptomatic rate were compared among groups of different age,gender and existence of relevant clinical symptoms,respectively.χ2 test and Fisher exact test were adopted for statistical analysis.Results Among 475 tumor patients with a definite diagnosis,116 were malignant tumor;The total detection rate of the group who were younger than 35 year-old(0.617%)was higher than other groups,the detection rate of intracranial neoplasms of the two groups who were younger than 45 year-old (0.206% and 0.132%,respectively)was higher than other groups,the total detection rate of 75-85 group (0.248%)was lower than other groups; The detection rate of lung cancer,liver cancer of the male(0.228% and 0.080%,respectively)was higher than the female,The detection rate of breast cancer and intracranial neoplasms of the female(0.366% and 0.100%,respectively)was higher than the male; There are 67 patients without correlated clinical symptoms,the asymptomatic rate was 57.8%.Conclusions Tumor screening was of importance in health management,which should be carried out actively in health examination.
7.Treatment of unstable thoracolumbar burst fractures: a comparison between anterior approach and posterior approach

Objective To retrospectively analyse the anterior and posterior surgical approaches in treatment of unstable burst thoracolumbar fractures and compare radiographic measurement parameters of beth surgical techniques so as to provide references for surgical treatment of such kind of fracture. Methods The study selected 41 patients with unstable thoracolumbar fracture treated with either anterior neurodecomprossion and fixation (n=19) or posterior reposition and internal fixation by pedicle screw (n=22) from January 2003 to December 2005. All patients were followed up for 24-48 months ( mean 38 months) and divided into anterior approach group and posterior approach group. Sagittal alignment was assessed by the Cobb angle depending on lateral radiographs. Results The Cobb angle of the anterior approach group was average 27.3°on admission but 3.1°postoperatively and 4.6° at follow-up; while the Cobb angle of posterior approach was average 26.1° on admission, 3.0°postoperatively and 12.5°at follow-up. There was no statistical difference between Cobb angle on admission and postoperative one (P>0.05) but showed significant differences between them at follow-up ( P<0.01). Conclusion The anterior surgical approach can consistently yield better maintenance of kyphotic correction compared with the posterior surgical approach.
8.The postoperative survival rate analysis of elderly patients with hip fracture

Objective To study the postoperative survival rate of elderly patients with hip fracture at different time and its related factors.Methods We conducted the follow-up studies of 255 elderly hip fracture patients from June 2008 to June 2011 in our hospital.The survival rates of patients at 6 months,1,2 and 3 years after operation and their related factors were studied.Results The survival rate was 96.5%,89.8%,78.0% and 69.4% at 6 months,1,2 and 3 years after operation respectively in 255 elderly hip fracture patients.Survival analysis showed that many kinds of preoperative comorbidities,low albumin level,old age and severe anemia were the risk factors for survival.Good activity of daily living before injury was a protective factor for survival.Gender,fracture type,the American Society of Anesthesiologists (ASA) classification,time from injury to surgery,anesthesia type,surgical approach,operative time,blood loss and hospital stay periods had no effect on survival after operation.Conclusions The survival rate in elderly patients with hip fracture after surgery is affected by many factors.Preoperative comorbidities,anemia and hypoalbuminemia should be actively treated,the activity of daily living should be improved by the directed therapy and rehabilitation,in order to improve the survival rate in elderly patients with hip fracture.
9.Intrauterine growth characteristics of twins and those twins discordant birthweight

Objective To investigate the intrauterine growth characteristics of twins and birthweight discordant twins (discordant twins ).MethodsTotal of 1010 twin pregnancies (2020 fetuses) with complete delivery records from the Department of Obstetrics and Gynecology,the First and Third Affiliated Hospital of SUN Yat-sen University between January 1,2000 and July 31,2010 were studied retrospectively.One handred and ninteen cases (238 fetuses) with intrapair birthweight difference ≥25% were determined as the discordant twins group,and the other 891 cases (1782 fetuses) with intrapair birthweight difference < 25% were identified as the concordant twins group.The singleton control group included 4042 singleton pregnancies in the same period.Results ( 1 ) Comparison of clinical data between the twins groups:the birthweight of larger-twin,smaller-twin and intrapair birthweight difference in the discordant twins group and the concordant twins group were ( 2090± 827 ) g,( 1392 ± 592 ) g,( 33.9 ±9.3 ) %,and ( 2408 ± 543 ) g,( 2191 ± 505 ) g,( 8.9 ± 6.5 ) %,respectively,with significant differences (P<0.01).The incidence of discordant twins was 11.78% (119/1010).Compared with the concordant twins group,the discordant twins group bad higher proportion of monochorionic twins,and higher prevalence of pregnancy complications such as late miscarriage,abnormal umbilical insertion,twin-twin transfusion syndrome and hypertensive disorders in pregnancy ( P < 0.05 ).( 2 ) The characteristics of twin birthweight distribution:① In all the 2020 twins,80.05% (1617/2020) fetuses had birthweight below the 50th percentile of the singleton control group,while 23.71% (479/2020) feeuses got birthweight below the 10th percentile of the singleton control group.② After 19th gestational week,the 50th and 90th percentile of all twins' birthweight were lower than those of singletons.After 38th gestational week,the birthweight of singletons kept increasing and reached its peak at 41 th week,while the birthweight of twins reached its peak at 38th week,followed by a decline at 39 weeks,which was even lower than the 10th percentile of the singleton control group.③ The distribution of birthweight of larger- and smaller-twin in the discordant twins group:65 (54.6%,65/119) larger-twins and one (0.8%,1/119) smaller-twin had birthweight above the 50th percentile of all twins,while 5 (4.2%,5/119) larger-twins and 97 ( 81.5%,97/119 ) smaller-twins got birthweight below the 10th percentile of all twins.Conclusions ( 1 ) The patterns of birthweight curves for each gestational week are different between twins and singletons.In order to evaluate the growth of twins, birthweight reference for twins shoull be employed.( 2 ) According to the reference of twins birthweight,the most discordant twins are complicated with fetal growth restriction at least in one twin.
10.Evaluation of Intracoronary Electrocardiogram on Myocardial Ischemia During Percutaneous Transluminal Coronary Angioplasty

The changes of intracoronary electrocardiogram (IC-ECG) and surface electro-cardiogram (S-ECG) were recorded simultaneously during percutaneous transluminal coronary angioplasty(PTCA) in 33 cases of coronary heart diseases with 41 vessel lesions. The results sug-gested that ST-segment change rate on IC-ECG(90. 2%) was higher than that on S-ECG(56%). ST-segment elevation on IC-ECG was earlier and more significant than that on S-ECG. The IC-ECG appeared to be more sensitive than S-ECG for detecting myocardial ischemia. The technique for recording IC-ECG is simple and it can help to perfome the PTCA successfully.
