1.Glass fiber post versus metal post in the restoration of endodontically treated teeth

BACKGROUND:The post and core system for residual roots and crowns can achieve satisfactory effects, when fil ing restoration, ful crowns or inlay has poor effects. OBJECTIVE:To evaluate the clinical use of glass fiber post and metal post in the restoration of endodontical y treated teeth. METHODS:A total of 106 patients with 160 endodontical y treated teeth were selected and restored with glass fiber post (n=80 teeth) and metal post (n=80 teeth) fol owed by porcelain-fused-mental crown. The restorations were evaluated clinical y for 9 to 33 months. RESULTS AND CONCLUSION:Glass fiber post treatment for 78 teeth was effective and that for two teeth were failed, with a successful rate of 98%. Metal post restoration was effective in 71 teeth and failed in nine teeth including one tooth of crown fracture, four of root fracture and four of post loss, with a successful rate of 89%. There was statistical y significant difference between two groups (P<0.05). Glass fiber post is better in convenient handling and good color and reduced probability leading to root broken for dental restoration compared with metal post.
2.Economic Operation of Xuzhou TCM Hospital under Medical Insurance Total Prepaid System in 2010-2013

Objective To analyze the relationship between measures and economic benefit under the total medical insurance prepaid system in Xuzhou TCM hospital.Methods The trend of financial data of Xuzhou TCM hospital from 2010 to 2013, its incomes, assets, and liabilities were analyzed to evaluate its profitability, operating capacity and solvency.Results Income:The annual average income was 439.81 million yuan, with an annual average growth rate of 12%;the annual average net profit was 24.36 million yuan, with an annual average growth rate of 0.004% (excluding the profit adjustment factors in 2010). Assets:The annual average total assets was 355.82 million yuan, in which 73.21 million yuan was the added value in 2010 compared with 2009 and the growth rate was 17.22%;247.71 million yuan has been reduced in 2011 compared with 2009;45.11 million yuan has increased in 2012 compared with 2011, while in 2013, 35.58 million yuan has increased compared with 2012. The state owned assets has increased significantly. Liabilities:The annual total liabilities were 114.86, 84.03, 112.35, and 120.52 million yuan respectively from 2010 to 2013;the average debt level was 105.61 million yuan. The rate of assets and liabilities were 23.04%, 33.52%, 37.98%, and 36.37% respectively from 2010 to 2013;the annual average debt rate was 29.68%. Profitability:The average net operating margin, net interest rate of total assets and net assets yield rate were 5.54%, 6.85% and 9.73% respectively, which showed the gradually increased profitability. Hospital operating capacity:The average total assets turnover rate was 124% from 2010 to 2013. The solvency of the hospital:asset liability ratio of the hospital was 29.68% from 2010 to 2013 on average, which was in the low level of debt.Conclusion Under the total prepaid system, the measures of the hospital are proper. Economic operation of the hospital is favorable, giving the consideration of combining both economic benefit and social benefit.
3.Study on the relationship between cyclin D1 G870A polymorphism and gastric cancer

Objective To assess whether the G870A polymorphism of cyclin D1 (CCND1)was associated with individual susceptibility to gastric cancer in China. Methods Case-control study of 214 individuals was conducted, including 108 controls (non ulcerative dyspepsia) and 106 cases of gastric cancer. The polymorphism analysis of CCND1 A/G was performed in blood samples by PCR-RFLP method. Results The frequency of GG genotype was 19.8% and 7.4% in patients with gastric cancer and controls, respectively. The risk for gastric cancer in patients homozygous (GG) for CCND1 genotypes was 3.5 times higher in comparison with patients carrying AA genotypes (P<0.05, OR =0.281). Furthermore, the stratification analyses showed that the male, elderly (≥60 years) or H. pylori infected gastric cancer patients who carrying GG genotype were more susceptible to gastric cancer compared with those carrying AA or GA genotype. Conclusion GG genotype is associated with increased susceptibility to gastric cancer in high risk area of Xi'an.
4.Clinical characteristics of fungemia in premature infants

Objective To realize the clinical characteristics of fungemia in premature infants.Methods Clinical characteristics of fungemia in premature infants in the intensive care unit of a children''s hospital between January 2011 and December 2015 were analyzed retrospectively, general condition of premature infants, laboratory-related indicators, and antimicrobial susceptibility testing results were compared.Results From January 2011 to December 2015, 42 premature infants with confirmed fungemia were treated in this hospital, 22 (52.38%) of whom were with fungemia caused by Candida albicans(C.albicans), 13 (30.95%) by Candida parapsilosis (C.parapsilosis), 3 by Candida krusei (C.krusei), and 4 by other fungi.Patients were grouped according to the main pathogens causing infection: C.parapsilosis group and C.albicans group.Maternal genitourinary tract infection rate and incidence of fungal meningitis in C.albicans group were both higher than C.parapsilosis group(27.27% vs 7.69%, 27.27% vs 0.00% respectively), peripherally inserted central catheter (PICC) rate in C.albicans group was lower than that in C.parapsilosis group(22.73% vs 69.23%), platelet count in C.parapsilosis group was lower than C.albicans group, differences were all statistically significant (all P<0.05).Conclusion The major fungi causing fungemia in premature infants were C.parapsilosis and C.albicans, maternal reproductive system infection during pregnancy can easily lead to candidemia, premature infants with candidemia are more vulnerable to developing fungal meningitis;PICC is more likely to lead to C.parapsilosis fungemia, and platelet decline is more obvious.
5.The expression of ICAM-1 and P-selectin in the healing process of incised wounds of rats

Objective Explore the expression of intercellular adhesion molecule-1(ICAM-1) and Pselectin in the healing process for possible application in timing of incised wounds. Method An immunohistochemical study on the expression of ICAM-1 and P-selectin was performed on rats vital skin incised wounds (5min~7d)using postmortem incisions(5~30min)as control.Results In the vital skin incisions,positive stain of ICAM-1 was observed on epidermis began from 1h and lasted till 3d, while the P-selectin stain was observed on vascular endothelium as early as 10min after incision and lasted 5h; in addition, the expression of ICAM-1 was also found in the inflammatory cells and fibroblasts,the lasting period varied from group to group (groupⅠ 5min~1h,groupⅡ 3~7h,group Ⅲ 9~12h,groupⅣ 1d,groupⅤ 3d,groupⅥ 5~7d.).The positive ratios of ICAM-1 among the cells were very low in group Ⅰ(0.41?0.73%),and increased considerably in group Ⅱ(9.79?3.74%) and group Ⅲ(23.33?1.10%),maximized in group Ⅳ(30.58?2.65%),then decreased in group Ⅴ and Ⅵ.There were no changes of ICAM-1 and P-selectin in the postmortem incision group. Conclusion ICAM-1 and P-selectin are considered potentially useful markers for wound age determination in forensic practice.
6.A new classification of extensions of the sphenoid sinus of Chinese adult by CT.

OBJECTIVE:
To examine various pneumatized extensions of the sphenoid sinus of Chinese people.
METHOD:
The sphenoid sinus and its surrounding structures were examined from 100 computed tomography images of the sinus. The type of the sphenoid sinus was classified according to the various extensions of the sinus.
RESULT:
The type of the sphenoid sinus was classified into the following 6 basic types based on the direction of pneumatization: sphenoid body, lateral, clival, lesser wing, anterior, and combined.
CONCLUSION
The variations in the extensions of pneumatization of the sphenoid sinus may facilitate entry into areas bordering the sphenoid sinus.

7.Generation of streptavidin-tagged human-granulocyte macrophage colony-stimulating factor fusion proteins.

OBJECTIVETo obtain streptavidin-tagged human granulocyte-macrophage colony-stimulating factor (SA/hGM-CSF) fusion protein and evaluate its bioactivity .
METHODSPET24a-6His-SA-L-hGM-CSF and PET24a-hGM-CSF-L-SA-6His plasmids were constructed and expressed in Rosetta (DE3) host bacteria to generate the fusion proteins. The two fusion proteins were refolded by gradient dialysis after Ni-NTA affinity chromatography and finally purified using DEAE-sepharose FF anion exchange chromatography. MTT method was used to evaluate the effect of SA/hGM-CSF fusion proteins in inducing the proliferation of human erythroleukemia cells (TF-1). The efficiency of the fusion proteins for surface modification of biotinylated MB49 tumor cells was evaluated by flow cytometry.
RESULTSThe recombinant fusion proteins SA-hGM-CSF and hGM-CSF-SA were highly expressed in Rosetta (DE3) at about 20% of the total bacterial proteins, with a purity of about 96% after purification. The two fusion proteins exhibited bifunctional activities, namely the pro-proliferation effect on human erythroleukemia cells (TF-1) and SA-mediated high-affinity binding to biotinylated cell surfaces (with an anchoring modified rate of about 99%).
CONCLUSIONSA/hGM-CSF bi-fusion proteins obtained in this study lays the groundwork for the development of cancer cell vaccines with surface modification by hGM-CSF.

8.Detection rate of pulmonary nodules by CT scan at different doses and application value of artificial intelligence assistance

Objective:To investigate the detection rate of pulmonary nodules (PN) by CT scan at different doses and the application value of artificial intelligence(AI) system assistance.Methods:From October 2019 to October 2021, 210 patients with PN in Beijing Hospital of Traditional Chinese Medicine, Capital Medical University, were retrospectively selected, and they were divided into the study group (106 cases) and the control group (104 cases) by CT scan at different doses. The control group used the conventional average dose (169 mAs) CT scan, the study group used an average low-dose (54 mAs) CT scan. The PN detection rate of different gender, age, body mass index (BMI) between the two groups were compared. The morphological characteristics, radiation dose, CT image quality between the two groups were compared. The diagnostic efficiency of radiologists and AI system was compared.Results:The detection rate of PN in the study group and the control group had no significant difference: 73.58% (78/106) vs. 80.77%(84/104), χ2 = 1.54, P>0.05. The detection rate of PN with different gender, age group and BMI in the two groups had no significant differences ( P>0.05). The diameter of nodules and the rates of calcification, cavitation, bronchial sign, lobar sign, burr sign and pleural adhesion sign in the two groups had no significant differences ( P>0.05). The mean effective tubular bulb dose, length product of radiation dose, total tubular bulb dose, radiation volume dose index in the study group were higher than those in the control group: (46.15 ± 7.38) mAs vs. (104.39 ± 10.53) mAs, (169.24 ± 19.77) mGy·cm vs. (427.17 ± 43.58) mGy·cm, (972.65 ± 58.34) mAs vs. (2 861.26 ± 181.37) mAs, (3.55 ± 1.16) mGy vs. (8.95 ± 2.07) mGy, there were statistical differences ( P<0.05). The excellent, good, acceptable, poor of 1.0 mm image quality in the study group were 26, 60, 18, 2, and in the control group were 32, 64, 8, 0, there was statistical difference ( u =1.71, P = 0.087). The excellent, good, acceptable, poor of maximum intensity projection (MIP) image quality in the study group were 58, 42, 6, 0 and in the control group were 70, 34, 0, 0, there was statistical difference ( u = 1.81, P = 0.070). The detection rate of PN by AI low-dose CT scan was higher than that of radiologists: 88.68%(94/106) vs. 73.58%(78/106), there was statistical difference ( χ2 = 7.89, P = 0.005). Conclusions:The low-dose CT chest scans for PN, the results of detection rate, morphological characteristics, CT image quality are basically the same as those of conventional-dose CT chest scans, and can greatly reduce the radiation dose, which is more suitable for PN screening, and combined with AI system can significantly improve the detection rate of PN.
9.Identification techniques of the recurrent laryngeal nerve in endoscopic thyroidectomy.

OBJECTIVE:
To investigate the application of identification techniques of the recurrent laryngeal nerve (RLN) in endothyroidectomies.
METHODS:
Routine identification of the RLN was performed in a series of 20 consecutive endothyroidectomies, and the clinical data were reviewed.
RESULTS:
Totally 20 RLNs were dissected. Neither transient nor permanent RLN injury occurred.
CONCLUSION
To expose RLN, both sharp and blunt dissection should be applied skillfully. Thorough liberation of the thyroid lobe is essential for the identification of RLN. It is safe and feasible to remove the thyroid and identify the RLN simultaneously.

10.Effect of prior statins use on the mortality rate among adult sepsis patients with type 2 diabetes

Objective:To evaluate the effect of prior statins use on the in-hospital mortality of adult sepsis patients with type 2 diabetes (T2DM).Methods:A total of 3 545 sepsis patients with T2DM were retrospectively collected from the Critical Care Medical Information Market (MIMIC Ⅳ) database, with in-hospital mortality rate as the outcome variable. According to whether they have taken statins in the past, they were divided into two groups and propensity score matching was used. Multivariate Cox regression analysis was performed to calculate the adjusted hazard ratio ( HR) and 95% CI, and the relationship between past statins use and in-hospital mortality in sepsis patients with T2DM was analyzed. Results:A total of 3 545 sepsis patients with T2DM were included between 2008 and 2016. 1 556 patients used statins before admission, and 1 989 patients did not use statins. After propensity score matching, the number of patients who had previously used statins and those who had not used statins were 1 230 and 1 298, respectively. After adjusting for potential confounding factors, it was found that previous use of statins was associated with a reduced in-hospital mortality rate ( HR=0.82, 95% CI: 0.61-0.99, P=0.038). Kaplan Meier curves showed that sepsis patients with T2DM who used statins before admission had a lower in-hospital mortality rate (Log rank test: P<0.001). Conclusions:Pre admission use of statins may be associated with a reduced mortality rate in sepsis patients with concomitant T2DM.
