3.Glass fiber post versus metal post in the restoration of endodontically treated teeth

BACKGROUND:The post and core system for residual roots and crowns can achieve satisfactory effects, when fil ing restoration, ful crowns or inlay has poor effects. OBJECTIVE:To evaluate the clinical use of glass fiber post and metal post in the restoration of endodontical y treated teeth. METHODS:A total of 106 patients with 160 endodontical y treated teeth were selected and restored with glass fiber post (n=80 teeth) and metal post (n=80 teeth) fol owed by porcelain-fused-mental crown. The restorations were evaluated clinical y for 9 to 33 months. RESULTS AND CONCLUSION:Glass fiber post treatment for 78 teeth was effective and that for two teeth were failed, with a successful rate of 98%. Metal post restoration was effective in 71 teeth and failed in nine teeth including one tooth of crown fracture, four of root fracture and four of post loss, with a successful rate of 89%. There was statistical y significant difference between two groups (P<0.05). Glass fiber post is better in convenient handling and good color and reduced probability leading to root broken for dental restoration compared with metal post.
4.Clinical study on cilostazol combined with pancreatic kininogenase in treatment of LEADDP

Objective To compare clinical effects between cilostazol(CLT)alone and cilostazol combined with pancreatic kininogenase(PK)on lower extremity arterial disease in diabetic patient(LEADDP).Methods Patients with LEADDP were randomly divided into 2 groups:single medication group or the control group(53 cases)and combination group or the treatment group(53 cases).Sugar-reducing medicines were given before and after treatment to keep a steady blood glucose level.The control group took CLT orally 100 mg each time,twice a day.Besides receiving the same dose of CLT,the treatment group took 120 tablet of PK on an empty stomach,three times a day,with the duration of 3 months.Clinical symptoms,ankle/brachial index(ABI),blood rheology,nailfold microcirculation,intimal medial thickness(IMT)and plaque thickness were compared between the 2 groups after treatment.Results Symptoms in both groups improved after treatment.ABI was elevated significantly compared to that before treatment(P<0.05)and the change was more obvious in combination group.The difference of ABI after treatment between the 2 groups had statistical significance(P<0.05).IMT of the right foot dorsal artery,and left and right posterior tibial artery increased significantly compared to that before treatment(P<0.05).Plaque thickness decreased compared to that before treatment(P<0.01)and the difference between before and after treatment in combination group was significantly greater than that in single medication group(P<0.05).Conclusion Cilostazol combined with pancreatic kininogenase has better clinical effects than cilostazol alone in treatment of patients with LEADDP.
5.Perioperative management of esophageal cancer patients concomitent with diabetes

Objective:To explore the perioperative treatment for esophageal cancer patients concomitent with diabetes.Methods:74 operation cases of esophageal and gastric cardia cancer associated with diabetes were analyzed retrospectively from 1976 to July 2003.Results:The occurrence of esophageal and gastric cardia cancer patients concomitent with diabetes was 6.1% among the esophageal cancer operative cases in the same period. The morbidity was 12.2% (including pulmonary infections in 2 cases,incision infection in 1,anastomosis fistula in 2,HHNC in 2 and hypoglycemia in 2 cases).The perioperative mortality was 1.4% with one patient dying of respiratory failure caused by pulmonary infection.Conclusion:Esophageal and gastric cardia cancer patients associated with diabetes are found to be common in thoracic operations (6.1%).It is crucial to control these patients for low morbidity and mortality through enough preparation,satisfactory aneathesial management and good nutrition support perioperatively.

7.The diagnosis of acute appendicitis with MSCT

Objective:To analyse the diagnostic of multislice CT(MSCT)for acute appendicitis.Methods:Sixty-seven patients with acute appendicitis confirmed by surgery and histopathology were analyzed retrospectively.The sizes of appendixes,appendolith,periappendiceal changes were observed and noted.Results:Sixty-four of the sixty-seven patients were correctly diagnosed as acute appendicitis.The direct CT signs of acute appendicitis were an enlarged appendix(82.08%).Indirect signs includedappeniceal perityphlitis(73.13%) and preiappendicular abscess(11.94%);and three cases were not observed with CT.Conclusion:Acute appendicitis has a variety of CT signs which is helpful to improve the diagnosis.

Metabolic syndrome(MS),as a clinical issue,has been proposed for over half a century. However,no consensus on its definition,pathogenesis and treatment has been reached so far.What is the entity of MS?Whether it is one kind of disease or constellation of several risk factors has been argued.Recent studies have shown that abdominal obesity is the most important feature of MS,which is linked with adipose tissue dysfunction and insulin resistance.Atherosclerotic cardiovascular disease is the main clinical outcome of MS.Thus, intervention of abdominal obesity is a key target for controlling MS and its complications.Although MS is composed of multiple subtypes,it is helpful for us to understand its entity and guide the clinical practice if we regard MS as an obesity-related cardiometabolic syndrome.
